Abstract

Disease index is an arrangement of summary lists, data on types of diseases and conditions of illness based on a classification system or numbering code as a means of communication. Good information can be displayed in a display, with that information will look more efficient and effective. One of the sheets contained in the Medical Record is the disease index. The method used is a qualitative research method with a descriptive approach, with data collection techniques based on observations, interviews, and literature studies. Problems found in this study: Data processing has not been carried out optimally so that officers take a long time, there is no special system for computerized disease indexes, disease index activity reports are still done manually. From the results of this study, Hospital X is expected to immediately make an outpatient disease module so that the physical activity proof of the Outpatient Disease Index display can be printed directly

Abstract

Penelitian ini bertujuan untuk merancang sistem informasi peminjaman dan pengembalian berkas rekam medis rawat jalan pada internal unit rawat jalan di Rumah Sakit X Bandung. Dalam penelitian ini metode yang digunakan yaitu metode kualitatif dengan pendekatan deskriptif. Teknik pengumpulan data yang digunakan adalah dengan cara observasi, wawancara, serta studi pustaka yang terkait dengan penelitian. Metode pengembangan perangkat lunak ini menggunakan Metode Waterfall Metode ini dilakukan dengan pendekatan yang sistematis, mulai dari tahap kebutuhan sistem lalu menuju ke tahap analisis, desain, coding, testing/verification, dan maintenance. Langkah demi langkah yang dilalui harus diselesaikan secara satu per satu tidak dapat langsung lanjut ke tahap berikutnya dan harus berjalan secara berurutan, oleh karena itu di sebut waterfall ada juga tahapan yang harus di lakukan yaitu Requirement definition, system software design, implementation, unit testing integration dan unit testing yang di tetapkan menggunakan Microsoft Visual Studio 2010 dengan Bahasa pemograman MySQL Sebagai Database nya. Masalah dari hasil penelitian yang dilakukan yaitu: pengolahan peminjaman dan pengembalian rekam medis rawat jalan masih menggunakan pencatatan secara manual yaitu dengan dicatat dalam buku ekspedisi yang tersedia dan masih adanya keterlambatan dalam mengembalikan dokumen rekam medis rawat jalan. Adapun saran-saran yang diberikan yaitu: 1) merancang sistem Informasi baru yang membantu lebih memudahkan pekerjaan dengan tepat, efektif dan efisien, dan 2) perlu adanya sosialisasi kembali mengenai maksimal lamanya waktu dokumen rekam medis rawat jalan yang dipinjam untuk segera di kembalikan ke unit Rekam Medis.

Abstract

This study aims to develop a medical record information system that supports faster and more accurate reporting of newborn immunization data at Gunung Sembung Public Health Center (Puskesmas). The identified issue involves the mixing of newborn vaccination data with that of older children, alongside a manual reporting process that hampers timely submissions to the Health Office. A descriptive qualitative approach was employed, with data collected through observation, interviews, and literature review. The system development followed the V-Model methodology within the Software Development Life Cycle (SDLC) framework. The results show that the developed system improves efficiency and accuracy in newborn immunization reporting. It is recommended that the system be implemented sustainably and further enhanced to support broader health data integration.

Abstract

The preliminary study revealed that the outpatient reporting system at Hospital X is still fragmented across individual polyclinics, with the reporting process conducted manually using Microsoft Excel. This condition results in a time-consuming report compilation process. The aim of this study is to develop a centralized outpatient reporting system integrated within a single information system. This centralization is expected to enhance the effectiveness of medical record staff in compiling outpatient reports. The research employed a research and development (R\&D) method with a systematic approach to improve the existing system. The information system was designed using the waterfall method and developed with Visual Studio Code software. The outcome of this study is an outpatient information system equipped with various features to support the compilation of outpatient service reports. Based on the study’s findings, Hospital X has the potential to further develop this reporting system to be more optimal and fully integrated with the existing Hospital Management Information System (HMIS).

Abstract

The purpose of this research is to implement a website as an information system for the sale of agricultural production facilities, which can be used by the community to find suitable agricultural medicines for farmers in Kelubir Village. This website is designed to facilitate farmers in purchasing or conducting agricultural transactions and to provide information related to the required production facilities. The research method used involves an approach to farmers by collecting data through interviews, observations, and documentation studies. The results of this study indicate that the implementation of the website can facilitate and improve the efficiency of selling agricultural production facilities and accelerate access to information about agricultural production facilities. This website is expected to be an effective solution in supporting the development of the agricultural sector in Kelubir Village and strengthening the local economy in the village.

Abstract

The provision of Visum et Repertum services at Hospital X remains constrained because the process is carried out manually using Microsoft Word and Excel, resulting in duplicated tasks for staff. The objective of this study is to design an information system that supports the Visum et Repertum service process in hospitals in a structured, efficient, and well-documented manner. The system was developed using the Agile Development methodology, which includes planning, design, implementation, testing, documentation, and deployment. Microsoft Visual Studio 2012 was employed as the development platform, with Microsoft Access serving as the database. In this study, the author presents a more fully integrated visum information system encompassing features for data management, document generation, reporting, and archiving. The findings indicate that the developed information system is capable of supporting the visum service process within an integrated workflow, thereby enhancing staff productivity. Black-Box testing demonstrated that all features functioned according to user requirements without functional errors. Consequently, the system has been proven to reduce redundant workloads, minimize the risk of recording errors, and contribute to improving the efficiency and quality of visum services, while also supporting the sustainable integration of medical information.

Abstract

Chronological records of events in the Emergency Department (IGD) are often unstructured, resulting in incomplete data and External Cause coding errors (ICD-10). This research aims to develop a prototype of a web-based information system to facilitate the chronological recording of events in a standardized manner, thereby improving the accuracy of codification. The development method uses the Waterfall model with the stages of needs analysis, design (context diagram, DFD, ERD), implementation (HTML, Bootstrap, Flask, SQLite), and Blackbox testing. As a result, the system provides a structured interface for the input of incident data (accidents, violence) and generates automated PDF reports that are ready for codefinition. Functional testing shows all modules (data inputs, filters, generated reports) are operating as expected. The system has the potential to reduce coding errors based on related literature studies, while supporting integration with Electronic Medical Record (EMR). Recommendations for advanced research include clinical trials and the development of artificial intelligence features for automated code recommendations.

Abstract

Information systems are needed in every company to facilitate fast and accurate data processing. One of them is a sales information system currently used at PT. MARKTEL still has processes that have not been computerized. To meet these information needs, especially to deal with data processing problems, report generation and systems that have not worked as expected, companies need a computer-based information system. The system development method in this study uses the waterfall method which has a systematic and clear flow so that the system development is more detailed. The waterfall method is a structured modeling-based method consisting of Flowmaps, Context Diagrams, Level 0 Data Flow Diagrams (DFD) and Entity Relationship Diagrams (ERD). In designing and implementing the program, the author uses the PHP programming language with MySQL database. The existence of a web-based sales information system at PT.MARKTEL is expected to simplify and speed up data processing and report generation and can minimize errors so that company goals can be achieved optimally.

Abstract

Doctor Practice Schedule is one of the important components needed in every health care facility. Scheduling that is fairly conventional by still using Ms.Excel, as well as the lack of accurate information about the schedule of which doctors are practicing or doctors whose services are full become obstacles for patients who will seek treatment. Therefore, it is proposed to design an information system to improve the quality of service more effectively and efficiently. The design of this web-based doctor schedule information system aims to make it easier for patients to see information on doctor practice schedules and appointments digitally. This research uses qualitative research with a descriptive approach. The development method used is the waterfall model with data collection techniques through observation and literature study, which is designed using the PHP programming language with the Laravel framework and MySQL as a storage database. The results of this study, with a website-based system, patients and prospective patients can see the doctor's practice schedule and appointment online anytime, anywhere and by anyone through a browser such as google chrome connected via the internet network. So that patients do not need to come to the hospital if they only want to see the doctor's schedule.

Abstract

Technology is currently very rapidly developing in all fields including the health sector, one of which is a hospital in a medical record agency, namely the claiming of BPJS internal medicine polyclinic patients who have not been computerized. The results of research conducted by the author found several problems, namely incomplete internal medicine polyclinic patient data, unfilled entry and exit dates, vacant diagnoses and anamnesis. So that it can affect the claiming of BPJS polyclinic internal medicine patient files, therefore, an information system is needed to support the completeness of BPJS patient medical record files so that claims are made quickly and accurately. Methods for collecting research data used include literature studies, interviews, and observations. The design of the information system uses the V model with the stages of Verification, Validation, Coding. System development consists of Flowmap, DFD, and ERD. then implemented using microsoft visual studio 2010 and database using microsoft access 2021. The results of designing an information system using the methods that have been chosen can help the performance of officers facilitate claiming and support the completeness of the medical record files of BPJS internal medicine polyclinic patients for accurate claiming.
