Abstract

Maternal and child health is an important concern for all nations in the world including Indonesia. “Fun For Mom” is an innovation program at Sako Public Health Center in Palembang for increasing the knowledge of mothers regarding breastfeeding practice, complementary feeding, and maternal health during pregnancy and breastfeeding period.  This study describes the Fun for Mom program at Sako Public Health Center in Palembang.  This was a qualitative study that involved the analysis of data collected from in-depth interviews, documents review, and observations to describe the Fun for Mom program using the RE-AIM framework at Sako Public Health Center in Palembang. The purposive sampling technique has been applied and twelve interviews were conducted with a Head of Sako Public Health Center, a Program Manager, two Program Implementers, three Cadres, and five Program Participants. RE-AIM framework is a multi-attribute model with five dimensions: reach, effectiveness, adoption, implementation, and maintenance. According to this framework, the findings illustrated that around ten pregnant or breastfeeding mothers participated in each activity session. Exclusive breastfeeding increased after a year of its implementation from 2017 to 2018. Mothers easily understand the information given because of the use of game media in this program. All information is conveyed in simple language and can be applied in daily childcare. Health operational assistance (it is called BOK) is used to fund this program. Other financial supports are needed for the sustainability of the program. The Fun for Mom program is feasible as a community empowerment effort in a health center setting and can be developed further.

Abstract

One important aspect of the impact of decentralization policy on health sectors is that budget allocation is fully authorized by each province. Preventive program of contagious disease is one of program strategy in Indonesian health department to enhance community health status, particularly in order to eliminate contagious diseases in community. Financing is one essential factor; therefore, finding an appropriate policy model of budget allocation is urgent. As a result, budget allocation for each contagious disease is able to be balance. The research design was an observational study with combination of qualitative and quantitative methods. The population is including all health officials in South Sumatera. Data were obtained using a checklist of budget allocation model and deep interview regarding supports and factors that halted in preventive programs of contagious diseases. Univariate and content analysis were utilized. The results indicated that budget proportion of preventive program of contagious diseases in each district/region is various, it depended in program that has priorities over in each region. Furthermore, program financing did not depend on number of diseases’ cases in previous year. Consequently, there were some diseases that were not handled very well due to limited budget from the health officials in each region. It might also be because of limited support from local government that has priorities on free medical treatments that need more budget than prevention activities. In summary, decentralization has not showed positive impacts on implication of communicable diseases preventive program, particularly in budget allocation. We recommend that appropriate budget allocation for this program is urgent needed to decrease numbers of communicable diseases in all district health officials in South Sumatera.

Abstract

Community Health Insurance is the government's social assistance programs for the poor. Research from the Indonesian Corruption Watch (ICW) found out that the quality of health services for the poor through the program of community health insurance is still low. This problem can be seen from the report of implementation program JPKM ASKESKIN in 2007 and report on the implementation of public health insurance program in 2008 at Petaling Health Center that decreased the number of patient visits by 60,52%. Therefore, this study aims to determine patient satisfaction of community health insurance to quality health services at the General Out Patient Clinic Petaling Public Health Center of Bangka. This research is a quantitative study using importance-performance analysis. This sample amount of 73 patients of community health insurance taken by purposive sampling technique. Tools of data collection primary data derived from the results of questionnaires and secondary dataderived from health profiles Petaling Public Health Center. The results of this study indicated that the average value of fitness levels for all dimensions are 82.49% which the level of concordance 78.47% reliability dimension, responsiveness 80.70%, assurance 85.01%, empathy 82.68% and tangible 85, 63%. Based on the importance-performance analysis of 25 factors which are investigated there are 8 (eight) factors that are in quadrant I, 7 (seven) factors in quadrant II, 3 (three) factors in quadrant III and 7 (seven) factors in quadrant IV. Advice given to the Petaling Health Center of Bangka is to improve health worker performance in terms of reliability and responsiveness in a way to supervise the implementation of service provided and give awards to outstanding officers. In addition, health workers also need to maintain the performance of the services provided in terms of empathy, assurance and tangibles with means to evaluate the services routinely provided.

Abstract

Background: Hypertension is a worldwide public health problem, mainly due to its high frequency and risks leading to cardiovascular diseases. The prevalence of hypertension in the Indonesian population aged > 18 years in 2018 was 34.11%. The unmet need for healthcare has generally been explored in most empirical studies concerning people with hypertension. Aims: This study investigated the determinants of unmet needs for healthcare among people with hypertension. Methods: The design of this study was cross-sectional on data from the Indonesian Family Life Survey wave 5 (IFLS-5). The survey sample was 6,302 adults aged > 40 years stratified by rural/urban residence status. A three-level multilevel analysis was performed to estimate the individual, household, and community-level determinants of unmet needs for hypertension care. Results: As many as 78.4% of the respondents with hypertension reported unmet needs for healthcare. Age, female, single, income, having insurance, urban residence, and the number of health-integrated posts for the elderly (Posyandu Lansia) were significantly associated with umeet needs for healthcare utilization among people with hypertension, while education and employment status showed no association with these variables. Conclusion: Improvement in access to healthcare and reduction in health inequality is required to address this problem.

Abstract

The Infant Mortality Rate (IMR) is an indicator to assess the degree of public health in a country. One of the efforts to reduce infant mortality is through the promotion of breastfeeding practices. The purpose of this study is to analyze the scope and determinants of IMD implementation in Nusa Tenggara. The design of this study uses a cross-sectional approach. This study analyzed secondary data from the 2017 Indonesian Health Demographic Survey (IDHS). The sample used in this study was women of childbearing age (15-49 years) who have given birth in the last 2 years, have their last child under 6 months old who is still alive, and live with their child with with a total of 603 respondents. Binary logistic regression was used to analyze the data. The results of this study show that 70.8% of respondents carried out the Early Breastfeeding Initiation. Normal delivery has a relationship with the implementation of early breastfeeding initiation in Nusa Tenggara. Socialization and education regarding normal childbirth need to be optimized as an effort to increase early initiation of breastfeeding. Abstract

AbstractCurrently, health problems in Indonesia are referred to as a double burden of disease because it is dominated by the development of Non-Communicable Diseases (NCDs) and on the other hand the incidence of infectious diseases that still occur. One of the strategic issues in the 2030 SDGs that must be a development priority in every country that requires cooperation in handling cross-sectors is NCDs, so a community-based NCDs control model is developed through Posbindu as a form of community participation as an effort to control risk factors independently and continuously. The role and function of Posbindu cadres are important as executors of NCDs risk factor control for the community. Therefore, to maximize the role of cadres in Posbindu, the Faculty of Public Health, University of Sriwijaya, empowers the Posbindu community through Posbindu cadres so that they can become self-reliant for the community to live healthily by increasing the capacity of Posbindu cadres. The community service team wanted to determine the effect of providing education and training to cadres on the knowledge and skills of Posbindu cadres in Bangun Jaya Village, Ogan Ilir Regency. The method used in this service is the education and training of 15 health cadres. Measurement of knowledge is done by using a questionnaire (pretest and posttest). The results of the activity showed that there was an increase in the knowledge and skills scores of cadres regarding Posbindu after education and intervention. However, there were also those with a fixed score. Continuous training is needed to improve cadres skills and optimize the community's active participation through Posbindu cadres.Keywords: Cadres, Posbindu, NCDs, Knowledge, Skills

Abstract

Internet access has become an important part of people's lives. Various digital platforms have been used to deliver health information to large populations. This study aimed to describe the health status based on digital access and sociodemographic factors of people in Southeast Asia, especially Indonesia, Malaysia, and Thailand. This study was a secondary data analysis of the World Value Survey Wave 7 (2017–2022) with a cross-sectional design. The sample in this study was the population aged 18 years and over, with a total of 2,917 respondents in Indonesia, 1,313 respondents in Malaysia, and 1,446 respondents in Thailand. The data were analyzed descriptively. Respondents in Thailand had the best health status (98.5%). Meanwhile, respondents who utilized digital access were most prevalent in Malaysia (95.4%). In all three countries, namely Indonesia, Malaysia, and Thailand, most respondents who utilize digital access have a good health status. Good health status in all three countries is dominated by male respondents, productive age, having ≤4 family members, working, being married, and having a high education level.

Abstract

Stunting is a chronic nutritional problem caused by multiple factors and is intergenerational. The incidence of stunting shows the problem of chronic malnutrition and occurs at the age of toddlers. Monitoring the growth of children under five is very important to detect growth faltering at an early stage. To determine this growth, weighing toddlers every month is very necessary. Anthropometric measurements are usually carried out by cadres at the Posyandu. However, the cadres cannot monitor the growth of toddlers either through anthropometric measurements (weight and height) or by filling out the KMS. This is because most of the cadres have never received training on anthropometric measurements and filling in the KMS. The results of this activity showed that most of the cadres experienced an increase in scores before and after the training, namely an increase in nutritional knowledge (54.3%), anthropometric measurements (78.2%), and KMS filling (58.7%). Based on the results of the Wilcoxon test showing a p value of 0.000 < 0.005, there is a difference in knowledge scores before and after the intervention, both knowledge of nutrition, anthropometric measurements of toddlers and filling KMS increases. It can be concluded that this cadre training activity was quite successful, seen from the results of the frequency distribution obtained and the results of statistical tests. It is necessary to carry out regular training related to monitoring the growth and reading of KMS, especially if there is a change of cadres.

Abstract

Implementation of Balanced Nutrition is critical to dealing with the COVID-19 pandemic. It is hoped that adolescents can apply balanced nutrition in the current situation so that the body remains healthy and increases body immunity. The purpose of this activity is to increase adolescent knowledge about the Application of Balanced Nutrition to support immunity during the Covid 19 pandemic through peer educator empowerment. The target of this activity is the youth of SMPIT Bina Ilmi Palembang which consists of 5 peer educators and 38 adolescent participants. This activity consists of two stages, namely peer educator training and nutrition counseling to adolescents by peer educators. Peer educators were selected by the school and then given training on balanced nutrition and its relationship to body immunity through Zoom meetings. Then peer educators provide counseling using the lecture method with PowerPoint tools to adolescents simultaneously through the breakout room at the Zoom meeting and conduct pretest and postest using Google form through WhatsApp group. The result of this activity is an increase in knowledge of both nutrition ambassadors and adolescents after receiving training. It is necessary to carry out regular training on balanced nutrition and body immunity so that adolescent knowledge is getting better so that it can improve the adolescent's immune system.

Abstract

Students, as agents of change, are expected to assist in reaching the success of the Indonesian National Health Insurance/Jaminan Kesehatan Nasional (JKN) program. Therefore, this study aimed to analyze the JKN knowledge among public health students and the associated factors in Palembang City, Indonesia. A cross-sectional online survey was conducted to collect data from 279 students at Universitas Sriwijaya in Palembang City, Indonesia. The analyzed variables included gender, age, marital status, residence, family size, income, study level and period, as well as source of JKN information. Additionally, statistical analysis using binary logistic regression, presenting odds ratio and 95% CI, was conducted to identify the determinants of JKN knowledge with significance set at a p-value <0.05. The results showed that 92.47% of respondents had low JKN knowledge with the following characteristics; female (93.83%), aged >20 years (92.93%), not married (93.75%), living in urban areas (90.24%), having family members >4 people (91.39%), income ≤UMR (93.38%), undergraduate level (93.70%), study period ≤2 years (93.89%), and receiving information from the Social Security Administration Agency for Health/Badan Penyelenggara Jaminan Sosial Kesehatan (BPJSK) (94.32%). The most associated variable with JKN knowledge was residence after being controlled by gender, study period, and information source variables. Specifically, respondents residing in urban areas (aOR = 10.73; 95% CI: 1.32–86.89) showed a higher likelihood of having good JKN knowledge than those in rural areas.
