Abstract

Dharmais Cancer Hospital is one of 237 hospital appointed by the government of Indonesia to give treatment, support and ARV therapy for HIV/AIDS patients. Ev-ery year, there is a significant increasing number of HIV/AIDS patients in Dharmais Cancer Hospital, therefore successfully of therapy is needed to be carried out regularly for the optimum result to the patients. One of methods to evaluating therapy is by reviewing efficacy of ARV combinations toward escalation of immunity respond (es-calation of CD4). The ARV combinations give a good efficacy if increasing CD4 > 50 cell/mm3. The objective of this study was to know the efficacy of four ARV combina-tion (each type consists of two Nucleosides Reverse Transcriptase Inhibitor and one Non-Nucleosides Reverse Transcriptase Inhibitor) base on increasing CD4 mean HIV/AIDS patients after 6 – 12 months treatment in Dharmais Cancer Hospital from 2005 – 2006, and to compare the efficacy of four ARV combinations. The four ARV combinations are combination I (Lamivudin + Zidovudin + Efavirenz), combina-tions II (Lamivudin + Zidovudin + Nevirapin), combination III (Lamivudin + Stavudin + Efavirenz), and combination IV (Lamivudin + Stavudin + Nevirapin). This study was analytical, cross-sectional design. Samples for this study were taken by total sampling using all data of HIV/AIDS patients in Dharmais Cancer Hospital from the year 2005 – 2006. The inclusion criteria were patients of fifteen years of age or more, baseline count CD4 < 200 cell/mm3, received ARV treatment for 6 – 12 months, received treatment of either one of the four ARV combination, and had data of CD4 from laboratory result before and after the treatment. Data were taken from patients’ medical record and analyzed with ANOVA-test. The result of this study from 151 patients showed that all the four combinations gave good efficacy based on the increasing CD4 mean. There was a significant difference increasing CD4 mean to HIV/AIDS patients between those received ARV combination II and those received ARV combination III (p value = 0,032). And there was not a significant difference for the other combinations. This study was from the four ARV combinations gave two the best efficacy are combination II and combination III.

Abstract

Objectives. To determine the prevalence of adverse drug reaction related hospital ad-missions in geriatric patients, to describe the most frequent clinical manifestations and the drugs responsible to adverse drug reaction related hospital admissions. Design. Observational cross-sectional study. Methods. Naranjo algorithm used to assess the adverse drug reaction causality. Subjects and setting. Geriatric patients admitted to geriatric inpatient installation of Cipto Mangunkusumo general hospital over one month period and assessed for cause of admissions. Results. 14,7% of 102 admissions were identified to be adverse drug reaction related hospital admissions. One adverse drug reaction was categorized as definite and 14 were probable causality. Gastrointestinal bleeding and hypoglicemia were the most common clinical manifestation found. The drugs most frequent responsible for these adverse drug reactions were nonsteroidal antiinflamatory drugs and oral antidiabetic drugs. Conclusion. Adverse drug reactions are an important cause of hospital admission in geriatric patients.

Abstract

The administration of antibiotic at RSUD dr. H. Abdul Moeloek Lampung is based on sensitivity test result and empirical therapy on diabetic ulcer. Therefore, the objective of the research is to analyze the effectivity of various antibiotics adminis-tration based on the sensitivity test result on clinical outcome reached by consider-ing influenced factors. On the other hand, total evaluation of the resistances of empirical antibiotic used by considering germ and sensitivity test feature was done. The research was conducted at dr. H. Abdul Moeloek Regional Hospital from April - July 2008 by applying cross sectional design retrospectively. Data was taken by the diabetic ulcer patients medical record which were treated from January 1st 2005 to Mei 30, 2009. Requirement of patient inclution are patient with diabetic ulcer infection, receiving antibiotic recipe, antibiotic given is based on sensitivity test result, patient was cured until getting the doctor approval to go home. The research which involved 98 patient inclution showed that clinical outcome of diabetic ulcer patient who was administered antibiotic based on sensitivity test result was influ-enced by chronic level of patient and age.

Abstract

In the previous study the analgesic effect of Cinnamommum zeylanicum Breyn had been investigating. The aim of this study was to investigate analgesic effect of the 70% ethanol extract of cinnamon bark (Cinnamommum zeylanicum Breyn.). This study used Tail Flick method at 25 male mice which have passed sensitivity test, divided into five groupes. Group I as negative control was administered 0,5% CMC, group Il as positive control was administered tramadol HCI, group Ill, IV and V was administered extract of cinnamon bark at 8; 16 and 32 mg/20 g BW. Drugs were orally administered to mice. The reaction of time exhibited by each animal was counted for two hour with interval thirty minutes. The Result revealed that the prepared extract in at the all dose, had analgesic effect significantly in all groups which have been studied (p< 0,05) in comparison to the negative control, to the highest analgesic effect by dose (32 mg/ 20 g BW). Our results showed that the analgesic effect of extract (32 mg/20 g BW) the same with tramadol hidroclorida.

Abstract

Sepsis still becomes a major health problem worldwide, with a fairly high mortality rate ranging from 20 to 50%. Antibiotic therapy with rational use can reduce the mortality rate. In contrast, the irrational use of antibiotic therapy will increase the occurrence of resistance, which impacts the increase of morbidity, mortality, and health costs. The objective of this study was to evaluate the quality in the use of antibiotics using the Gyssens method in patients with sepsis. This study was an observational study with a cross-sectional method conducted at Fatmawati Central General Hospital Jakarta from January to December 2020. The research subjects were 110 patients with sepsis who met the inclusion criteria. In this study, 49.09% of patients used rational antibiotics and 50.91% of patients used irrational antibiotics that were found in category VI (0.91%), V (17.28%), IVa (3.63%), IVb (0.91%), IVc (0.91%), IIIa (3.63%), IIIb (20%), IIa (0.91%), and IIb (2.73%). The duration of antibiotic therapy was the only factor that affected the quality of antibiotic use (p = 0.012). There was no difference in mortality between patients with rational and irrational use of antibiotics (p = 0.333, OR = 1.654, 95% CI 0.714-3.829). However, after adjusting for the duration of therapy, irrational use of antibiotics significantly affected mortality (p = 0.017, OR = 3.5, 95% CI 1.255-9.761). In conclusion, the antibiotic use in half of the sepsis patients were irrational. Since irrational use was associated with mortality, efforts need to be taken to improve the quality of antibiotic use.

Abstract

Background: Drug-resistant tuberculosis (TB-DR) is a deadly disease caused by infectious agents. Indonesia is one of the countries with the highest drug-resistant tuberculosis (TB-DR) rates in the world. The prevalence of tuberculosis cases in Depok City between 2019-2022 showed an increase in cases every year. This study aims to measure the level of adherence of drug-resistant tuberculosis (TB-DR) patients to their treatment using the Eight-Item Morisky Medication Adherence Scale (MMAS-8) questionnaire.Methods: This study is an observational study using a cross-sectional study design, conducted from February 2024 to March 2024 at the University of Indonesia Hospital. Eighty-seven respondents participated in this study. Respondents completed the validated Indonesian version of the MMAS-8 questionnaire after signing informed consent forms.Results: The results of the study showed that 50.6% of respondents had low adherence levels, 47.1% had moderate adherence levels, and 2.3% had high adherence levels. The data was then statistically analyzed using the SPSS version 29 statistical package with Chi-Square analysis, which resulted in a significant correlation (P<0.05) between adherence levels and gender.Conclusion: There was a significant correlation between gender and respondent adherence. More than 50% of drug-resistant tuberculosis (TB-DR) patients at the University of Indonesia Hospital still have low adherence levels to their treatment.

Abstract

Pharmaceutical care based on WHO patient care indicators in various developing countries, including Indonesia, have not met the requirements. Puskesmas accreditation is expected to improve pharmaceutical care that has an impact on rational drug use. The purpose of the study was to analyze pharmacy care based on WHO patient care indicators in all sub-district health centers (9 health centers not yet accredited and 2 health centers already accredited) in Depok City. This study used a cross-sectional design and was carried out in December 2016-February 2017. The sample was adult patients from general poly and accompanying patients from integrated management of sick toddlers who met the inclusion criteria, as well as all prescription drugs taken. There were 30 respondents and 30 prescriptions for each sub-district health center. Data collection was done through interviews and observation of respondents and analysis of the recipe sheets. Data analysis used Mann-Whitney and Chi-square tests. The results show that the sub-district health centers have not been accredited which meets WHO recommendations for preparation time and delivery of drugs (> 3 minutes) of 77.8% and those who are 100% accredited. None of the sub-district health centers in Depok City meet WHO recommendations for the suitability of drug delivery, adequate drug labeling and patients with correct drug knowledge (= 100%). Abstract

Drug reconciliation is needed to get rational treatment therapy. The purpose of this study was to analyze the relationship between the completeness of filling out the form of drug reconciliation and rational drug use at Bekasi X Hospital. The study design used a cross-sectional comparative study to compare between 56 completed drug reconciliation forms and 133 incomplete drug reconciliation forms. Sampling is done in total sampling. The research sample that fulfilled the inclusion criteria was 189 forms (40.6%) of all (466) drug reconciliation forms in November 2015-end April 2016 at X Hospital Bekasi. Data was analyzed using Chi-square test. The results showed that the largest percentage of filling out drug reconciliation forms was incomplete (70.4%), carried out by the pharmacist (56.1%) and not signed by a doctor (63.5%). Percentage of rational drug use by 7.9%, with details: correct diagnosis 53.4%, correct indication 53.4%, correct selection of medicine 33.9%, correct dosage 8.5%, exactly how to administer 37.6%, on time giving 13.8% and correct time giving 14.8%. There is no significant relationship between the completeness of filling out the drug reconciliation form, the types of health workers, the presence/absence of a doctor's signature and rational drug use in inpatients (p> 0.05).Tthere is no significant relationship between the completeness of filling out the reconciliation form and rational drug use in inpatients at Bekasi X Hospital (p> 0.05)

Abstract

Procurement and distribution of drugs to community health centers generally performed by health office districts, such as Depok City, but in the province of Jakarta conducted by the community health centers in sub district. The purpose of the study was to compare the rational drug use among community health centers in the district of Depok City and South Jakarta. This study used a cross-sectional design conducted in treatment and non- treatment community health centers in District X Depok City and district Y South Jakarta period MarchOctober 2011. Rational drug use data was collected in accordance with WHO procedure. Data was analyzed by Kruskal Walis test. Conclusion of the study shows that rational drug use based on prescribing indicators in community health centers in Depok City relatively better than South Jakarta, but no statistically significant (p> 0.05). Abstract

Treatments in elderly patients are very complex and usually multiphatology thus causing an increase in number of polypharmacy for every clinical condition. The presence of hypertension, diabetes, and/or dyslipidemic will increase the risk of Adverse Drug Reaction (ADR) because of polypharmacy and long term of treatments. This study aimed to assess the effect of elderly age on the incidence of ADR in patients with hypertension, diabetes, and/or dyslipidemia at Puskesmas Pancoran Mas, Beji, and Tanah Baru in Depok. The design of the study is cohort study. The study was conducted at January-June 2014. Sixty two elderly patients were collected as cohort group and 62 non-elderly patients as control group. Subjects were monitored every week for a month. Clinical Manifestation of ADR event was an evaluation result of the recording complaints experienced by the subject using Naranjo scale. Clinical manifestation of ADR events obtained from both group were analyzed using Chi-square and Logistic Regression Test. This study found that 30,6% patients experienced ADR events with 39 times of accurrences. The most common clinical manifestation was dry cough related to the use of captopril (56,3%). The risk factor of ADR event in elderly age is 3,577 times greater than non-elderly age.
