Abstract

Self-efficacy and low adherence was significant problem on using antihypertension drugs. The study purpose was to evaluate the effectiveness of counseling and provision of leaflets againts self-efficacy and adherence as well as the blood pressure of hypertensive patients at two community health centers in Depok City. The study desain is a quasi-experimental with 37 patients in counseling group and 36 patients in the leaflets group during March to June 2013. The instrument determine self-efficacy is MUSE scale, MMAS-8 scale for adherence and the tensimeter for blood pressure. Data were analyzed uses the Wilcoxon and Kolmogorov-Smirnov test. The results showed that the largest percentage of hypertension patients in community health centers have mild hypertension, has been suffering from hypertension between 1-5 years, received a single drug, mostly captopril, and did not feel any side effects. The counseling and provision of leaflets can increase self-efficacy and medication adherence, and lower systolic blood pressure significantly. Provision of leaflets to patients can increase self-efficacy and adherence, and lowering the systolic and diastolic pressure significantly. There is no significant difference between the effects of counseling and the provision of leaflets to increase self-efficacy and adherence, as well as a decrease in blood pressure in hypertensive patients in community health centers in Depok City

Abstract

Injectable contraceptives raise health problems risk than oral or implan contraceptive. This study aimed to compare the incidence of adverse drug reactions (ADRs) single injectable contraceptive use (Depot medroxyprogesterone acetat/DMPA) with a combination of injectable contraceptive (CICs) use (DMPA and Estradiol cypionate/E2C). The study design was a cross-sectional two population comparison. The sample consisted of 88 acceptors in each group. The ADRs were analyzed using Chi Square and logistic regression multivariate. The largest side effect in single injectable contraceptive group were menstrual disorders (86,4%) and mood dischange, while the side effects in CICs group were mood (64,8%) and bone pain (51,1%). Menstrual disorders in single injectable contraceptives usage was 10.0 folds than CICs. Headache in injectable contraceptives usage was 1.9 folds than in the use of CICs. Menstrual disorders in overweight acceptor 3,8 folds than not overweight.As the ADR observation, the use of CICs is relatively safer than single injectable contraceptive

Abstract

The use of angiotensin converting enzyme inhibitors (ACEi) as an antihypertensive agent can cause side effects such as dry cough. The aim of this study is to evaluate risk of ACEi administration on dry cough incidence with captopril as the standard against lisinopril and ramipril in hypertensive patients at RSUD Cengkareng and RSUD Tarakan Jakarta. The design of this study is prospective cohort. The inclusion criteria were patients who received ACEi as hypertension therapy for ≤ 3 months gathered from outpatient and willing to participate as sample in this study at RSUD Cengkareng and RSUD Tarakan. Subject of this study consist of 54 patients who received captopril and 54 patients received non captopril ACEi, taken by consecutive sampling from January-July 2014. Data was collected using structured interviews and medical record. Dry cough incidence due to ACEi was evaluated using Naranjo Algorithm and analyzed using Chi Square test. Dry cough incidence was found in 19,44% of sample. No significant relationship of age, gender, ethnic, comorbidity, body mass index (BMI), dosage, and duration of use with the dry cough incidence due to the use of ACEi. There is no difference on dry cough incidence between the use of captopril, lisinopril and ramipril.

Abstract

This study aimed to evaluate the effect of diabetes medication adjustment during Ramadan fasting according to the recommendation of International Diabetes Federation-Diabetes and Ramadan International Alliance (IDF-DAR) to HbA1c values in type-2 diabetes mellitus patients. The study was conducted at a hospital in Jakarta, Indonesia with a cross-sectional study design involving 80 ambulatory patients with type-2 DM who fasted during Ramadan in 2016. About 60% of patients used oral antidiabetes drug (OAD), mostly the combination of biguanide + sulfonylurea drugs (27.5%). The adjustments were made in which 56.2% was in accordance with IDF-DAR recommendations. The study showed that the HbA1c values decreased insignificantly (p = 0.082) from 8.75 ± 1.90 to 8.63 ± 1.82 after the adjustment. The appropriate drug adjustment based on IDF-DAR had a significant effect on the HbA1c value of DM patient with p-value 0.030 (p < 0.05).The discrepancy of drug use based on IDF-DAR 3.222 times greater caused uncontrolled HbA1c. The most influential variable to HbA1c value was drug type with p-value 0.006 (p < 0.05). Improper adjustment of Insulin or combination insulin-OAD caused HbA1c value 5 times greater to be uncontrolled than OAD.

Abstract

Medication adherence is important in controlling blood sugar levels (HbA1c) in patients with Diabetes Mellitus (DM). Studies on the relationship between medication adherence and blood sugar control on the quality of life (QoL) in Type 2 DM patients are limited in Indonesia. This study aims to assess the relation between medication adherence with blood sugar control and the quality of life of Type 2 DM patients. This study used a cross-sectional design conducted at Universitas Indonesia Hospital. It used Medication Adherence questionnaires and EuroQoL 5D-5L questionnaires to assess patients’ medication adherence and quality of life. This study involved a total of 74 Type 2 DM patients with an average age of 57 ± 10.74 years. Medication adherence with good blood sugar control (HbA1c <7%) showed a significant relationship (p-value <0.05) with COR values of 3.74 (1.21 - 11.6) compared to non-adherent patients. But not on the QoL Index and VAS (Visual Analog Scale) values, where the value is lower in patients with high adherence and good blood sugar control. Factor asscociated with blood sugar controlled were comorbidites and medication adhrence while factor that associated with quality of life were comorbidities, ages, education and employment status

Abstract

Diabetes mellitus is a chronic metabolic condition frequently associated with complications and comorbidities that often require hospitalization for effective management. Such patients are often prescribed multiple medications, which elevate the risk of potential drug-drug interactions (pDDIs). Hence, this study aims to determine the prevalence, severity, common interacting pairs, and factors associated with pDDIs among hospitalized diabetes patients. The study used a retrospective cross-sectional study design conducted at Universitas Indonesia Hospital. Lexicomp® Lexi-InteractTM software was used to analyze and classify possible drug interactions. Logistic regression analysis was used to identify different factors associated with potential DDIs. Of the 200 patients, 89.0% were observed to have pDDIs. A total of 966 interactions were found, of which 75.6% were moderate, 16.2% were minor, and 8.1% were of major severity. Meanwhile, in the risk rating C category, 71.0% were predominant, followed by B and D, 15.0% and 11.0%, respectively. Multivariate regression analysis showed a statistically significant association of pDDIs with 5–8 prescribed medicines (OR=22.8; 95% CI=5.5-94.7; p<0.001), >8 prescribed medicines (OR=64.4; 95% CI=11.3-336.5; p<0.001). The findings of this study revealed that pDDIs are highly prevalent in adult inpatients with diabetes. This emphasizes the critical need for appropriate monitoring and management strategies to reduce pDDIs and their adverse consequences.

Abstract

Abstract: Tocilizumab is an anti-IL-6 which is recommended as cytokine release syndrome therapy in the treatment of Covid-19. One of the side effects of tocilizumab that can occur is hepatotoxicity, from mild elevated transaminases to severe drug-induced liver injury (DILI). Currently, research related to tocilizumab hepatotoxicity and the risk factors affecting Covid-19 patients is limited and still needs to conduct. This research is an observational study with cross-sectional design. Data collection was carried out retrospectively using secondary data obtained from medical records department, patients with Covid-19 at Universitas Indonesia Hospital in 2020-2021. Patients confirmed with positive Covid-19 receiving Tocilizumab therapy in medical record were included in this study. Patient demographics, clinical diagnoses, laboratory examinations, history of others medication were also reviewed. Patients with less than 18 years old, incomplete medical record data, and referred to other hospitals were excluded. The sampling technique used was total sampling, namely all patients who met the inclusion criteria were taken as subject in this research. Chi-square test was used to analyze the relationship between each risk factors and the incidence of elevated transaminases. The results of Chi-square test showed that several risk factors significantly increased the incidence of elevated transaminases in patients given tocilizumab therapy, including age, obesity, diabetes mellitus, and ceftriaxone as antibiotic therapy.

Abstract

Indonesia has been one of the three countries in Asia that contributes to the highest case of Tuberculosis (TB) in the world. Tuberculosis itself counted as the top ten cause of death globally. Two of the challenges in controlling TB cases are the occurrence of drug-resistance strain and patients’ adherence. Drug-resistant TB has to be treated with the second line drugs of TB with higher risk of adverse events. Linezolid as one of the suggested drugs by WHO in the treatment may increase patients’ risk of cytopenia events. This study was conducted to represent the profile of drug-resistant TB obtained from Rumah Sakit Universitas Indonesia (RSUI) Depok period October 2024. Design of this study is a quantitative-descriptive where the data of patients obtained from the patients’ medical records in the period of October 2024. The findings of this study showed the total number of drug-resistant TB was 104 with 3 cases of HIV and 70 patients having Linezolid in their regimen. Most of the patients are male with mean of age 41.3 years. New cases and RR/MDR TB level are dominated while on the other hand there’s 9 cases of After Failure and 4 others are After Loss to Follow-up. Median values obtained for patients’ leukocytes, hemoglobin, and platelets are 7818.0, 11.9, 279.5 respectively.

Abstract

One of the adverse drug reactions that occur in patients who receive treatment therapy is drug side effects. Statins are the first choice in cholesterol-lowering therapy. Although statins are safe and well tolerated, they have the potential for side effects. The purpose of this study is to determine the prevalence of adverse drug reactions, the incidence of adverse drug reactions that occur, and the probability score of adverse drug reactions based on the Naranjo Algorithm instrument in patients with inpatient atherosclerosis cardiovascular disease who use statin therapy at RSUI. The method used in this study is a prospective observational descriptive method with observation through patient interviews and medical record data collection as well as causality analysis using the Naranjo Algorithm. The results of this study were obtained from 116 patients who used statin therapy in the RSUI inpatient room in accordance with the inclusion and exclusion criteria. Of the 116 patients, as many as 14 patients (12.07%) experienced adverse drug reactions statins. Adverse drug reactions felt included shoulder pain (3.40%), nausea (2.60%), muscle pain (1.70%), blurred vision, difficulty sleeping, chest palpitations, joint pain, dizziness and body aches as much as 0.9% each. The total probability score of the Naranjo algorithm is a probable score of 5-8 in 7 patients (50%) and a possible score of 1-4 (50%).

Abstract

Surgical procedures involve interventions that may compromise tissue integrity, necessitating prophylactic antibiotics to prevent surgical site infections (SSIs). Proper timing, selection, and administration of antibiotics are crucial to achieving optimal therapeutic outcomes. This study aimed to evaluate the prescribing patterns of prophylactic antibiotics in patients undergoing surgical procedures at the University of Indonesia Hospital and assess adherence to established guidelines. A retrospective observational study analyzed medical records of patients who underwent surgery. Data collected included the type of antibiotics prescribed, timing of administration, dosage, duration, and adherence to national and international guidelines. Descriptive statistics were used to summarize the findings. The study analyzed 213 surgical patients at the University of Indonesia Hospital in September 2024. Prophylactic antibiotics were prescribed in 90.61% of cases, predominantly Cefazolin (86.48%), with doses of 2 g (76.12%) and 1 g (10.36%). Most patients (97.93%) received antibiotics 30–60 minutes pre-incision. All antibiotics were administered intravenously with research results, 4.29% were affected by IDO. These findings highlight overall adherence to prophylactic antibiotic guidelines, with minor deviations in timing and dosage observed in a few cases. Optimal prescribing practices were achieved in most surgical patients to reduce the risk of SSI.
