Abstract

Background: Cancer pain remains a significant clinical challenge, with a high prevalence across various disease stages. Opioids are the mainstay for moderate to severe pain management but carry risks of dependence, tolerance, and serious side effects, necessitating careful supervision. Purpose: To review the effectiveness of pharmacist interventions in managing opioid use among cancer pain patients. Method: A literature search was conducted using two approaches: keyword-based searches through electronic databases (Google Scholar and Scopus) and the snowballing technique. Keywords included "cancer pain," "opioid," "pharmacist intervention," and "breakthrough pain," combined with Boolean operators. After identifying primary articles, backward and forward snowballing was performed to find additional relevant studies. All articles were screened based on predefined inclusion and exclusion criteria. Results: The review demonstrates that pharmacist interventions can improve pain control by up to 40%, reduce opioid-related side effects such as constipation by up to 50%, and enhance patient adherence to therapy. In Indonesia, however, barriers such as limited access to strong opioids, low patient education levels, and minimal pharmacist involvement in multidisciplinary teams remain significant challenges. Conclusion: Pharmacists play a strategic role in cancer pain management, particularly in optimizing pain control, minimizing opioid dependence, and managing adverse effects. Their active involvement in multidisciplinary teams enhances patients' quality of life and reduces inappropriate opioid use. Strengthening policies, providing continuous professional development, and conducting context-specific research are essential to optimize pharmacist contributions in Indonesia.   Keywords: Cancer Pain; Opioids; Pain Management; Pharmacist Intervention.   Pendahuluan: Nyeri kanker merupakan tantangan klinis utama dengan prevalensi tinggi pada berbagai tahap penyakit kanker, dan opioid menjadi terapi lini utama untuk nyeri sedang hingga berat. Namun, penggunaannya memiliki risiko ketergantungan, toleransi, serta efek samping serius, sehingga diperlukan pengawasan ketat. Tujuan: Untuk meninjau literatur terkait efektivitas intervensi apoteker dalam pengelolaan penggunaan opioid pada pasien nyeri kanker. Metode: Pencarian literatur dilakukan dengan dua pendekatan, yaitu pencarian menggunakan kata kunci melalui basis data elektronik (Google scholar dan Scopus) serta teknik snowballing. Kata kunci yang digunakan meliputi: "cancer pain", "opioid", "pharmacist intervention", dan "breakthrough pain", yang dikombinasikan menggunakan operator Boolean. Setelah diperoleh artikel utama, dilakukan teknik snowballing secara backward dan forward untuk menemukan studi relevan. Semua artikel yang diperoleh diseleksi berdasarkan kriteria inklusi dan eksklusi. Hasil: Tinjauan menunjukkan bahwa intervensi apoteker dapat meningkatkan efektivitas terapi nyeri hingga 40%, menurunkan kejadian konstipasi sebagai efek samping hingga 50%, serta meningkatkan kepatuhan pasien terhadap regimen terapi. Namun, di Indonesia, implementasi peran apoteker masih menghadapi hambatan seperti keterbatasan akses opioid kuat, kurangnya edukasi pasien, dan rendahnya keterlibatan apoteker dalam tim multidisiplin. Simpulan: Dari tinjauan ini menunjukan bahwa intervensi apoteker terbukti memiliki kontribusi strategis dalam pengelolaan nyeri kanker, khususnya melalui pengendalian nyeri yang lebih efektif, pencegahan ketergantungan opioid, dan manajemen efek samping yang lebih terstruktur. Keterlibatan apoteker dalam tim multidisiplin dapat meningkatkan kualitas hidup pasien dan menurunkan risiko penggunaan opioid yang tidak tepat. Namun, untuk mengoptimalkan peran ini, diperlukan dukungan kebijakan, pelatihan berkelanjutan, serta riset lebih lanjut dalam konteks lokal untuk menjawab tantangan implementasi di Indonesia.   Kata Kunci: Intervensi Apoteker; Nyeri Kanker; Opioid; Pengelolaan Nyeri.

Abstract

Background: The elderly population is a vulnerable group due to physiological changes, including decreased organ function. In Depok City, the number of elderly people is expected to increase to 202,194 by 2023. Potentially inappropriate medication use (PDR) in the elderly is associated with an increased risk of adverse drug reactions (ADRs). STOPP_INA is a tool for identifying PDRs and can help reduce the incidence of PDRs. Purpose: To analyze potentially inappropriate medication use associated with adverse drug reactions (ADRs) in elderly patients. Method: This was a descriptive observational study with a cross-sectional design. Data were collected retrospectively from electronic prescriptions and electronic medical records of elderly inpatients at RSUI. The minimum sample size was 52. Results: Of 528 medications prescribed to 52 patients, 90 PDRs (17.04%) were identified. The most common medications included in the STOPP_INA criteria were aspirin, ramipril, clopidogrel, spironolactone, clonidine, and amlodipine. Twenty-two patients (42.3%) experienced ADRs, with the most common adverse effects being hypotension (12 cases), followed by cognitive impairment, gastric ulcer recurrence, bleeding, and decreased renal function (3 cases each). Conclusion: There was a very strong correlation between the number of PIMs and ADRs (r = 0.758; p < 0.01) in patients with polypharmacy.   Keywords: Adverse Drug Reactions (ADRs); Elderly; Potentially Inappropriate Medications.   Pendahuluan: Populasi lanjut usia merupakan kelompok rentan karena mengalami perubahan fisiologis, termasuk penurunan fungsi organ. Di Kota Depok, jumlah lansia meningkat mencapai 202,194 jiwa pada tahun 2023. Penggunaan obat yang berpotensi tidak tepat atau potentially inappropriate medications (PIMs) pada lansia dikaitkan dengan peningkatan risiko reaksi obat yang tidak diinginkan (ROTD). STOPP_INA merupakan salah satu instrumen untuk mengidentifikasi PIMs dan dapat membantu mengurangi kejadian ROTD. Tujuan: Untuk menganalisis obat yang berpotensi tidak tepat terhadap reaksi obat yang tidak diinginkan (ROTD) pada pasien lansia. Metode: Studi observasional deskriptif dengan desain cross-sectional. Data dikumpulkan secara retrospektif dari resep elektronik dan catatan rekam medis elektronik pasien lansia rawat inap di RSUI. Jumlah sampel minimal berdasarkan perhitungan adalah 52 sampel. Hasil: Dari total 528 obat yang diresepkan kepada 52 pasien, ditemukan 90 PIMs (17.04%). Obat yang paling sering termasuk dalam kriteria STOPP_INA adalah aspirin, ramipril, klopidogrel, spironolakton, klonidin, dan amlodipin. Sebanyak 22 pasien (42.3%) mengalami kejadian ROTD, dengan efek samping terbanyak berupa hipotensi (12 kasus), diikuti gangguan kognitif, kekambuhan tukak peptik, perdarahan, dan penurunan fungsi ginjal (masing-masing 3 kasus). Simpulan: Terdapat korelasi yang sangat kuat antara jumlah PIMs dan ROTD (r = 0.758; p < 0.01) pada pasien dengan polifarmasi.   Kata Kunci: Lansia; Obat yang Berpotensi Tidak Tepat; Reaksi Obat yang tidak Diinginkan (ROTD).

Abstract

Hypertension is a chronic disease that requires long-term management. The use of medicinal plants as complementary therapy is increasingly recognized, although scientific evidence remains limited. This study aimed to evaluate the effectiveness of amlodipine with concomitant herbal therapy (bay leaves, celery, and/or cucumber) compared with amlodipine monotherapy in hypertensive patients. A retrospective cohort design was employed using medical record data and patient questionnaires from three community health centers in Surakarta. Treatment effectiveness was measured by the achievement of the target blood pressure over three months. Data were analyzed using bivariate tests and multivariate logistic regression. The results showed that clinical effectiveness was higher in the amlodipine group with concomitant herbal therapy (bay leaves, celery, and/or cucumber) at 78.4% compared to amlodipine monotherapy at 68.8%, although the difference was not statistically significant (p=0.160). Factors significantly associated with treatment effectiveness were medication adherence (p<0.001), baseline systolic blood pressure (p<0,001), and educational level (p=0,047). Concomitant herbal therapy with amlodipine showed potential blood pressure–lowering effects, though not statistically significant. Prospective studies or clinical trials are warranted to further assess the benefits and safety of this therapeutic approach.

Abstract

Doksorubisin masih sering digunakan dalam pengobatan kanker. Efek samping doksorubisin terhadap jantung perlu mendapatkan perhatian khusus. Penelitian ini bertujuan untuk memperoleh gambaran penurunan fraksi ejeksi ventrikel kiri dan faktor risiko yang mempengaruhinya pada pasien kanker yang mendapatkan kemoterapi doksorubisin di Rumah Sakit Kanker Dharmais (RSKD). Desain penelitian yang digunakan adalah cross sectional. Data diperoleh dari rekam medis pasien. Populasi adalah pasien kanker yang mendapatkan kemoterapi doksorubisin minimal 4 siklus periode Oktober 2011-Oktober 2013. Penilaian fungsi jantung didapatkan dari data Echocardiography jantung yaitu fraksi ejeksi ventrikel kiri sebelum kemoterapi dan setelah siklus terakhir kemoterapi. Penurunan fraksi ejeksi ventrikel kiri ≥10% pada penelitian dinilai sebagai penurunan yang bermakna. Analisis statistik menggunakan software Statistical Products Social Science (SPSS) versi 18. Sebanyak 77 pasien memenuhi kriteria inklusi dan eklusi. Sebanyak 37 pasien (48,05%)  mengalami penurunan fraksi ejeksi ventrikel kiri <10% dan sebanyak  28  pasien  (36,36%) mengalami penurunan fraksi ejeksi ventrikel kiri ≥10%. Hipertensi berpengaruh terhadap penurunan fraksi ejeksi ventrikel kiri ≥10% secara bermakna (p=0,032). Jenis kelamin laki-laki dan radiasi dada kiri menunjukkan adanya kecenderungan berhubungan dengan penurunan fraksi ejeksi ventrikel kiri ≥10% (p=0,095 dan p=0,051). Doksorubisin dapat menyebabkan penurunan fraksi ejeksi ventrikel kiri ≥10% pada lebih dari sepertiga pasien kanker di RSKD dan faktor yang paling mempengaruhinya adalah komorbiditas hipertensi. Kata Kunci: doksorubisin; faktor risiko; penurunan fraksi ejeksi ventrikel kiri

Abstract

Tuberculosis remains a leading cause of global mortality and morbidity. Current TB elimination programs have not yet met their targets. Drug resistance poses a significant challenge, necessitating effective alternative treatments for drug-resistant TB patients. Bedaquiline and Linezolid are primary drugs (Category A) used to combat drug-resistant TB. However, they come with side effects and toxicity, have narrow therapeutic window, and exhibit pharmacokinetic variability. Therefore, therapeutic drug monitoring (TDM) is recommended to achieve desired treatment outcomes and prevent or minimize potential side effects or toxicity. The objective of this study is to assess the urgency of TDM implementation to address these issues. This research is a literature review involving the synthesis of data from six studies that met inclusion and exclusion criteria. Data sources include Science Direct, PubMed, and Nature. The results indicate that Bedaquiline and Linezolid exhibit pharmacokinetic variability and the occurrence of side effects. Therefore, TDM is recommended for Bedaquiline and Linezolid to achieve desired therapeutic effects and avoid the onset of side effects or toxicity

Abstract

Medication adherence is crucial for the successful treatment of tuberculosis (TB), a persistent global health issue that demands prolonged and rigorous therapy to achieve cure, prevent drug resistance, and reduce transmission rates within communities. Recent studies highlight predictors of effective treatment outcomes such as being a new TB case and being HIV-negative. There is a significant relationship between various psychosocial factors that influence treatment adherence, particularly among elderly TB patients, where adherence is characterized by complexity and multidimensionality. Factors like depression, anxiety, income levels, social support, patient status, and TB classification play critical roles in adherence. Understanding these factors is essential for optimizing treatment strategies and improving outcomes in TB management.

Abstract

Swamedikasi antibiotik dapat meningkatkan terjadinya resistensi dan risiko penggunaan antibiotik yang tidak tepat. Salah satunya disebabkan kurangnya pengetahuan masyarakat terhadap bahaya menyimpan antibiotik dirumah. Penelitian ini bertujuan untuk menilai pengaruh penyuluhan dan pemberian leaflet pada ibu terhadap pengetahuan, persepsi dan penyimpanan antibiotik. Penelitian ini menggunakan rancangan penelitian Quasi Experiment non randomized pre post control group. Intervensi berupa penyuluhan dan pembagian leaflet pada ibu. Penelitian dilakukan di Kota Depok pada bulan Januari-Oktober 2014. Wilayah kerja kelurahan Pancoran Mas sebagai lokasi intervensi dan kelurahan Beji sebagai kontrol. Subjek penelitian adalah ibu rumah tangga usia 18-65 tahun. Jumlah subjek untuk kelompok intervensi 52 dan kontrol 52 ibu rumah tangga. Analisis data dilakukan dengan menggunakan uji beda dan analisis multivariat. Hasil penelitian menunjukkan pengetahuan awal ibu didapatkan informasi sebesar 81,7% tidak tahu bahwa membeli antibiotik harus menggunakan resep, persepsi awal ibu didapatkan informasi sebesar 42,3 % menganggap antibiotik bukan obat keras dan sebesar 51% diketahui ibu menyimpan antibiotik dirumah. Perbedaan bermakna pengetahuan dan persepsi ibu pada kelompok penyuluhan dan pemberian leaflet lebih tinggi dibanding sebelum penyuluhan. Sedangkan pada kelompok kontrol tidak bermakna (p>0,05). Penyuluhan dan pemberian leaflet memberikan pengaruh secara bermakna terhadap peningkatan pengetahuan dan persepsi. Ada hubungan yang bermakna antara pengetahuan, persepsi dan penyimpanan antibiotik ditunjukan dengan nilai p < 0,05.

Abstract

Telepharmacy is a telemedicine health service in the pharmaceutical sector that uses telecommunications to assist patients located far away during the Covid-19 pandemic. Furthermore, professional organizations are responsible for continuously providing information to pharmacists, ensuring they stay updated with the latest developments in the field. Mobile applications have emerged as the primary medium through which information can be accessed. Therefore, this research aimed to determine the implementation of telepharmacy in the Bangka Belitung Islands Province using observational research with a qualitative approach. The participants comprised a total of 11 individuals and the technique employed was purposive sampling. Subsequently, the collected data were subjected to analysis using the Miles and Huberman model. The results showed that the utilization of telepharmacy commenced within Bangka Belitung Islands Province and the community pharmacy service implemented the concept in the form of Drug Information Services, drug consultation or patient counseling, and prescription services through e-prescribing. Additionally, field observations highlighted the presence of services such as home delivery of medicines, patient education, and online non-cash payments. The implementation of telepharmacy within hospitals remains absent, despite the widespread adoption of digital services facilitated by the SIMRAS application. Pharmacists also ensured the administrative compliance of their colleagues through the utilization of the SIAP application. The preliminary stages of the implementation commenced in the Bangka Belitung Islands Province.

Abstract

Background: Drug-resistant tuberculosis (TB-DR) is a deadly disease caused by infectious agents. Indonesia is one of the countries with the highest drug-resistant tuberculosis (TB-DR) rates in the world. The prevalence of tuberculosis cases in Depok City between 2019-2022 showed an increase in cases every year. This study aims to measure the level of adherence of drug-resistant tuberculosis (TB-DR) patients to their treatment using the Eight-Item Morisky Medication Adherence Scale (MMAS-8) questionnaire.Methods: This study is an observational study using a cross-sectional study design, conducted from February 2024 to March 2024 at the University of Indonesia Hospital. Eighty-seven respondents participated in this study. Respondents completed the validated Indonesian version of the MMAS-8 questionnaire after signing informed consent forms.Results: The results of the study showed that 50.6% of respondents had low adherence levels, 47.1% had moderate adherence levels, and 2.3% had high adherence levels. The data was then statistically analyzed using the SPSS version 29 statistical package with Chi-Square analysis, which resulted in a significant correlation (P<0.05) between adherence levels and gender.Conclusion: There was a significant correlation between gender and respondent adherence. More than 50% of drug-resistant tuberculosis (TB-DR) patients at the University of Indonesia Hospital still have low adherence levels to their treatment.
