Abstract

Latar Belakang: Dalam proses pembangunan kesehatan untuk meningkatkan kualitas hidup masyarakat, Kementerian Kesehatan memutuskan untuk mengeluarkan kebijakan dana BOK (Bantuan Operasional Kesehatan). Dimana anggaran yang digunakan untuk membiayai pelayanan kesehatan masyarakat. Akan tetapi tersedianya dana BOK tersebut dirasakan belum efektif, sehingga belum menekan angka kematian ibu dan anak. Tujuan dari penelitian ini adalah mengetahui implementasi Bantuan Operasional Kesehatan (BOK) dengan cakupan penanganan komplikasi kebidanan di Puskesmas Kota Bekasi tahun 2019. Metode: Penelitian ini merupakan penelitian kualitatif. Pengambilan data dilakukan dengan wawancara mendalam dan telaah dokumen kepada informan yang terlibat pengelolaan dana BOK tahun 2019. Sampel yang terlibat dalam pelaksanaan program bantuan operasional kesehatan di Puskesmas Kota Bekasi terdiri dari sembilan responden. Hasil: penelitian mengungkapkan bahwa perencanaan anggaran sudah sesuai dengan juknis, kebijakan sumber daya manusia belum memadai, jumlah dana sudah mencukupi dan proses akuntabilitasnya memerlukan pengawasan dan penyederhanaan pelaporan. Pada tingkat capaian program penanganan komplikasi kebidanan, cakupan layanan ada yang telah meningkat, tetapi ada beberapa layanan yang tidak memenuhi target Standar Pelayanan Minimal (SPM). Saran: Diharapkan kepada Dinas Kesehatan Kota Bekasi dapat meningkatkan capaian SPM bidang kesehatan di Kota Bekasi dengan mengoptimalkan dana bantuan operasional kesehatan menuju masyarakat kota bekasi yang lebih sehat.

Abstract

Nurse training at Ilaga Regional Hospital aims to improve nurses' quality, but its implementation does not have a significant impact. This must be addressed immediately, because the failure of ineffective, inefficient and irrelevant training to the needs of nurses at Ilaga Regional Hospital can hinder efforts to improve the quality of service at Ilaga Regional Hospital. This study aims to identify training needs at Ilaga Regional Hospital, using quantitative methods of survey data from the Hennesy-Hick’s questionnaire (n = 29, purposive sampling), consisting of 25 questions (five categories). Qualitative methods use semi-structured interview data (n = 14) consisting of 42 questions (organizational analysis, task analysis, personal analysis). Training needs according to education level, agency of origin, age and length of service, namely administration and clinical skills. However, the competency gap is smaller in respondents S1 + Profession, the highest in IRNA, tends to decrease with age. and there is a variation in the gap in each category of length of service. The results of the organizational, task and personal analysis, there is a need for RME training, clinical skills, SOP training, and motivational training.

Abstract

Merokok memengaruhi moralitas anak, berdasarkan penelitian Hakimi et all tahun 2024 menyatakan bahwa dampak negatif merokok antara lain cabut, bolos sekolah, mencuri, melawan orang tua dan guru (Hakimi et al., 2024). Penulisan ini bertujuan untuk menganalisis bagaimana Implementasi Kawasan Tanpa Rokok pada tatanan sekolah di Indonesia telah dilaksanakan. Penelitian ini menggunakan metode literature review, yakni metode yang mencakup tahapan seperti membaca artikel, menelaah, dan menilai literatur yang kemudian dievaluasi serta disesuaikan dengan kebutuhan penelitian untuk mencapai tujuan yang ditetapkan (Snyder, 2023). Adapun hasil penelitian menunjukkan beberapa aspek yang mendukung terlaksananya implementasi KTR yaitu aspek komunikasi, sumber daya, sarana dan prasarana, struktur birokrasi.

Abstract

The COVID-19 pandemic has significantly impacted community activities, including social and economic aspects, as well as health behavior and healthcare services. Diabetes Mellitus (DM) is a chronic disease that requires continuous management to prevent chronic complications. Ngoerah Hospital (RS Ngoerah), Bali, Indonesia, is a type A hospital where DM patients generally present with complications. This study analyzes the impact of the COVID-19 pandemic on DM patient visits to the Diabetes Center Polyclinic at RS Ngoerah by considering economic aspects. This research is a retrospective study of patient visit data to the Diabetes Center Polyclinic at RS Ngoerah from 2019 to 2024, covering the time before, during, and after the COVID-19 pandemic. A descriptive analysis was carried out to observe changes in patient visits during each period. Furthermore, an aggregate analysis was conducted to examine the correlation between the number of patient visits and the number of detected COVID-19 cases, the COVID-19 pandemic timeline along with government regulations during the pandemic, and the economic condition reflected by the Gross Regional Domestic Product (GRDP) of Bali Province in the same year. The COVID-19 pandemic led to a decrease of up to 58.65% in DM patient visits to the Diabetes Center Polyclinic at Ngoerah Hospital. Directly, there was no significant correlation between the number of reported COVID-19 cases in Bali Province and the number of DM patient visits to the Diabetes Center Polyclinic at RS Ngoerah (r=0.262, p=0.155); however, other factors influencing the number of patient visits included government policies during the COVID-19 pandemic and the GRDP

Abstract

Latar belakang: Pandemi COVID-19 menyebabkan perubahan kegiatan dan inovasi mutu pada pelayanan kesehatan. Di Indonesia, sektor kesehatan memiliki dampak akibat tingginya penyebaran COVID-19, mulai dari tenaga kesehatan hingga ke tingkat manajemen rumah sakit karena organisasi tidak mempunyai waktu untuk merespon krisis tersebut. Tujuan: Penelitian ini untuk mengetahui upaya pelayanan kesehatan dalam menghadapi pandemi COVID-19 menggunakan metode Lean Six Sigma (LSS). Metode: Tinjauan ditentukan berdasarkan PICOS. Pencarian dilakukan pada basis data Google Scholar, PubMed, dan Science Direct menggunakan PRISMA. Artikel dibatasi dari Januari 2020-Desember 2022, di pelayanan kesehatan saat atau setelah pandemi COVID-19 serta menggunakan metode LSS. Hasil: Berdasarkan hasil pencarian ditemukan 144 artikel dengan 1 duplikasi. Hasil screening pada judul dan abstrak didapatkan 7 artikel namun 1 artikel tidak lengkap sehingga 6 artikel yang akan ditinjau oleh peneliti. Dari 6 penelitian, 5 artikel menggunakan metode DMAIC (Define, Measure, Analysis, Improve, and Control) dan 1 artikel dengan jenis penelitian retrospektif. Metode DMAIC dimulai dari mendefinisikan masalah yang dihadapi (Define) dengan menggunakan beberapa teknik pengukuran (Measure). Selanjutnya, capaian kinerja dan tujuan yang ingin dicapai dianalisis untuk mempersiapkan rencana perbaikan yang akan dilakukan (Analysis). Perbaikan (Improvement) dan kontrol (Control) dilakukan untuk pencapaian tujuan jangka panjang. Kesimpulan: Penerapan LSS pada masa pandemi COVID-19 dapat membantu pelayanan kesehatan di beberapa negara untuk peningkatan kualitas manajemen, tenaga medis, efisiensi, dan mengurangi waktu tunggu pelayanan.

Abstract

Pendahuluan : Pendekatan kedokteran keluarga dalam pelayanan kesehatan primer. Tujuan : untuk meningkatkan akses, efektivitas, dan kualitas layanan kesehatan. Studi ini merupakan tinjauan literatur yang mengeksplorasi peran kedokteran keluarga dalam meningkatkan kualitas pelayanan kesehatan primer dan pengelolaan penyakit kronis. Metode yang digunakan adalah tinjauan sistematis dengan mengikuti pedoman PRISMA. Pencarian literatur dilakukan melalui berbagai basis data ilmiah, dengan kriteria inklusi mencakup studi yang membahas peran kedokteran keluarga dalam layanan primer dan manajemen penyakit tidak menular. Hasil analisis menunjukkan bahwa pendekatan kedokteran keluarga berkontribusi terhadap peningkatan akses layanan kesehatan, efektivitas pengelolaan penyakit kronis, serta koordinasi perawatan multidisiplin. Kedokteran keluarga berperan dalam deteksi dini, edukasi pasien, dan pemantauan kesehatan jangka panjang. Selain itu, pendekatan berbasis komunitas dan teknologi kesehatan, seperti e-health, mendukung keberlanjutan layanan. Meskipun demikian, tantangan seperti keterbatasan sumber daya manusia, kurangnya kebijakan pendukung, dan beban kerja tinggi masih menjadi kendala dalam implementasi yang optimal. Kesimpulan:  pendekatan kedokteran keluarga memiliki potensi besar dalam memperkuat sistem pelayanan kesehatan primer. Diperlukan kebijakan yang mendukung penguatan layanan primer, peningkatan jumlah tenaga medis, serta pemanfaatan teknologi untuk meningkatkan akses dan efektivitas layanan kesehatan. Kata Kunci: Kedokteran keluarga, layanan kesehatan primer, penyakit tidak menular, manajemen penyakit kronis.

Abstract

Traditional medicine in Indonesia, rooted in local wisdom, is recognized nationally and internationally to support public health preventively, promotively, and curatively, backed by government commitments through regulations such as jamu certification and the Germas program, as well as the abundant biodiversity of thousands of medicinal plant species serving as raw materials for the significant national traditional medicine industry, with ongoing scientification efforts to ensure safety and efficacy. This study aimed to identify trends in the utilization and expenditure of phytopharmaceutical drugs and to provide alternative strategies for enhancing the competitiveness of phytopharmaceuticals as traditional medicine. A cross-sectional study with a quantitative descriptive approach utilized secondary data on domestic phytopharmaceutical drug expenditures from 2023–2024 sourced from the Ministry of Health's LKPP e-purchasing system, which was processed using Microsoft Excel and analyzed descriptively for frequency distribution, percentages, patterns, and trends. Total phytopharmaceutical expenditure reaches Rp25.94 billion, with an increase from Rp11.08 billion in 2023 to Rp14.86 billion in 2024; provinces with the highest spending include Central Java (Rp3.88 billion), followed by West Java (Rp3.02 billion) and East Java (Rp2.38 billion), which contribute more than one-third of the national total, while immunomodulator products like Stimuno dominate with Rp15.01 billion, but utilization for chronic diseases remains low, such as hypertension (Tensigard: Rp1.08 billion) and diabetes (Diabadex: Rp0.295 billion). This study concludes that although phytopharmaceutical expenditure and utilization show positive trends, their utilization is not yet optimal due to limitations in integration into the National Health Insurance, regional distribution disparities, high research costs, as well as low promotion and acceptance among healthcare professionals and the public.

Abstract

Health workers are the spearhead of services in health facilities. WHO (Workforce 2030) states that many countries experience a concentration of health workers in urban areas and shortages in remote areas. Data from the Indonesian Ministry of Health 2023 shows that DKI Jakarta (urban) does not experience a shortage of doctors, dentists, nurses, and midwives (0%), while Papua Mountains (remote) experiences a significant shortage of doctors at 62.8%, dentists at 90.2%, nurses at 45.1%, and midwives at 72.6%. This indicates the need for policy analysis of the distribution of health workers between urban and remote areas. This study is a scoping review by analyzing articles from search results using PRISMA guidelines in the Scopus, PubMed, DOAJ, and Google Scholar databases. The results of this study indicate that the distribution of health workers in Indonesia is still uneven, with a concentration in urban areas and a significant shortage in remote areas. Efforts to achieve equality through various government policies such as the Healthy Archipelago and special assignments have been carried out, but their implementation has not been optimal in addressing inter-regional disparities, so data-based policies and sustainable strategies are needed to strengthen the distribution and retention of health workers in remote areas.Keywords: Health Workforce Distribution, Inequality, Policy, Remote, Urban.

Abstract

Strengthening primary care services in Indonesia requires that community health centers, namely Pusat Kesehatan Masyarakat (Puskesmas), involve medical personnel with family medicine competence. However, the fulfillment of physicians with this competence, especially Primary Care Family Medicine Specialists or Spesialis Kedokteran Keluarga Layanan Primer (Sp.KKLP), still face significant obstacles. This qualitative research uses the integrated Easton's System Model and Buse's Policy Framework to analyze the dynamics of this policy. The findings reveal a gap between the strong legal foundation and weak implementation mechanisms on the ground. First, there is ambiguity in terminology and lack of harmonization among regulations, especially concerning the Sp.KKLP nomenclature, which creates confusion in terms of legal status and professional recognition. Second, an implementation gap (policy–practice gap) exists because the central government has not yet issued technical derivative regulations (technical guidelines, compensation mechanisms). Furthermore, the decentralization of authority and limited regional budgets impede the equitable placement of the Sp.KKLP. The success of the policy relies heavily on the synergy among the Ministry of Health, Health Council, Family Medicine Collegium, educational institutions such as Program Pendidikan Dokter Spesialis (PPDS) and Recognition of Prior Learning (RPL), and professional organizations such as Pengurus Besar Ikatan Dokter Indonesia (PB IDI). Recommendations include the development of technical implementation regulations, clarification of the Sp.KKLP definition, and establishment of inclusive pathways for competence enhancement such as PPDS and RPL.

Abstract

Background: Abundance studies revealed the significant contribution of health volunteers or lay health workers in supporting health program, including for Tuberculosis program. This article aims to describe the cascade of volunteer involvement in community-based TB programs and to map out forms of rewards for volunteers. Methods: The searching was done from Pubmed, Scopus, Science Direct and Google Scholar. The three authors performed all steps of review started from searching, screening, and selecting the and conducted consensus to determine articles that are relevant and worthy of further analysis. We analyzed the manuscript with steps including generating the findings, coding the subthemes and doing thematic analysis. Result: As many as 13 studies were selected, of which were published after 2014. Most of the literature indicates that training (including transportation and meal costs) serves as both a reward and a means of sustaining volunteer efforts. Some studies also describe non-material rewards, such as recognition from health service providers, special access to health services, and symbolic items like uniforms or pins. Only a few studies mention specific amounts of material incentives. Nevertheless, this study supports the view that providing material rewards helps sustain field programs and enhances the commitment of health volunteers to TB prevention and control efforts at the community level.
