Abstract

Anestesi spinal merupakan teknik umum yang efektif yang memiliki risiko komplikasi seperti hipotensi dan bradikardia. Pemberian cairan kristaloid secara coloading adalah salah satu cara untuk mengurangi risiko tersebut. Penelitian ini bertujuan untuk mengetahui pengaruh coloading cairan kristaloid terhadap perubahan tekanan darah dan heart rate pada pasien anestesi spinal di RS Emanuel Banjarnegara. Studi ini menggunakan desain pra-eksperimental dengan pendekatan pretest-posttest design. Penelitian ini menggunakan metode kuantitatif, dan didapatkan sampel sebanyak 111 sampel. Pengambilan data dilakukan pada bulan Maret – April. Karakteristik usia sebagian besar responden berusia 26-35 tahun kategori dewasa awal sebanyak 56 responden (50,5%) dengan jenis kelamin yang paling dominan yaitu jenis kelamin Perempuan sebanyak 67 responden (60,4%), status pasien dengan ASA 2 sebanyak 84 responden (75.7%). Hasil analisis menggunakan uji Wilcoxon menunjukkan bahwa pemberian coloading cairan kristaloid mencegah terjadinya penurunan tekanan darah (p value 0,000 < 0,05), terjadi kenaikan rata – rata diastolik sebesar 8,73 mmHg dan rata – rata kenaikan sistole sebesar 23,25 mmHg dan terhadap penurunan heart rate (p value 0,000 < 0,05), rata – rata kenaikan heart rate setelah pemberian coloading cairan kriataloid sebesar 11,96 kali per menit pada pasien spinal anestesi di RS Emanuel Banjarnegara. Pemberian coloading efektif dalam menurunkan insidensi kejadian hipotensi dan bradikardi pada pasien spinal anestesi.

Abstract

Sectio caesarea merupakan metode persalinan melalui pembedahan yang berisiko menimbulkan komplikasi salah satunya adalah Postoperative Nausea and Vomiting (PONV). Kondisi ini dapat mengganggu kenyamanan pasien dan memperpanjang waktu pemulihan. Sebagai alternatif penanganan, intervensi non-farmakologis seperti aromaterapi peppermint dan lavender dapat menjadi pilihan yang efektif. Penelitian ini bertujuan untuk mengetahui efektivitas sebelum dan sesudah pemberian aroma terapi peppermint dan lavender dalam mengurangi PONV pada pasien yang menjalani operasi Sectio Caesarea. Penelitian ini menggunakan desain kuasi-eksperimental dengan pre-test dan post-test pada 70 pasien. Sampel dibagi menjadi dua kelompok masing-masing 35: satu kelompok menerima aromaterapi peppermint (eksperimen) dan kelompok lainnya aromaterapi lavender (kontrol). Data PONV diukur menggunakan skala numerik sebelum dan sesudah intervensi, lalu dianalisis dengan uji Wilcoxon. Hasil penelitian menunjukkan bahwa nilai rata-rata sebelum dilakukan pemberian aromaterapi peppermint yaitu 4,428 dan sesudah di lakukan 3,60 nilai sedangkan rata-rata sebelum dilakukan pemberian aromaterapi lavender yaitu 5.06 dan sesudah di lakukan 4.60. Wilcoxon menunjukkan bahwa perbedaan nilai PONV setelah di lakukan antara kelompok eksperimen aromaterapi pappermint adalah 0,00 dan kontrol aromaterapi lavender 0,028 signifikan (p < 0,05), dua kelompok eksperimen ini menunjukkan bahwa terdapat penurunan tingkat PONV sebelum dan sesudah pemberian aromaterapi peppermint dan lavender. Penelitian ini mendukung penggunaan aromaterapi peppermint dan lavender sebagai metode yang efektif dalam manajemen PONV dan memberikan dasar untuk penerapan klinis yang lebih luas.

Abstract

Latar belakang : Bedah ortopedi adalah prosedur medis yang menangani kondisi muskuloskeletal, dan manajemeninyeri yang efektif sangat penting untuki kenyamanan pasien dan penyembuhan yang dipercepat pasca operasi. Penelitianiini bertujuan untukimengetahui pengaruh pendidikan pada pengetahuan manajemen nyeri di antara pasien operasi pasca bedah ortopedi di RSUD Dr. Mohammad Zyn Sampang. Penelitian ini menggunakanidesain one-groupipretest-posttest, yang melibatkan 77 responden dengan Teknik Purposive sampling. Media dalam penelitian ini menggunakan media leaflet terkait manajemen nyeri farmakologis dan non farmakologis serta menggunakan kuesioner untuk mengetahui tingkat pengetahuan manajemen nyeri sebelum dan sesudah edukasi. Temuan mengungkapkan bahwa mayoritas responden berusia >35 tahun sebanyak 38 reponden (49,4%) dan berjenis kelamin laki-laki sebanyak 52 responden (67,5%). Sebelum pendidikan pengetahuan manajemen nyeri sebagian besar responden menunjukkan pengetahuan cukup sebesar 37 responden (48,1%) dan sesudah diberikan pendidikan manajemen nyeri sebagian responden menunjukkan pengetahuan baik sebesar 68 responden (88,3%). Hasil penelitian ini mununjukkan adanya pengaruh edukasi terhadap pengetahuan manajemen nyeri pada pasien pasca bedah orthopedi dibuktikan dengan nilai P 0,000 (0,005). Ada pengaruhiedukasi terhadap pengetahuani manajemen nyerii pada pasien pasca bedah orthopedi di RSUD dr. Mohammad Zyn Sampang.

Abstract

Pendahuluan: Shivering merupakan komplikasi umum pasca spinal anestesi yang dapat mengganggu kenyamanan pasien dan meningkatkan kebutuhan oksigen tubuh. Faktor-faktor yang diduga berhubungan dengan kejadian shivering antara lain usia dan lama operasi. Penelitian ini bertujuan untuk mengetahui hubungan usia dan lama operasi dengan derajat shivering pada pasien post spinal anestesi tindakan sectio caesarea. Metode: Penelitian ini menggunakan desain kuantitatif analitik dengan pendekatan cross-sectional. Sampel berjumlah 50 responden yang diambil dengan teknik purposive sampling di RS Emanuel Banjarnegara. Data dikumpulkan melalui lembar observasi derajat shivering menurut kriteria Alfonsi dan dianalisis menggunakan uji Spearman Rank. Hasil: Sebagian besar responden berusia 26–35 tahun (60%) dan menjalani operasi ≤ 60 menit (66%). Derajat shivering terbanyak adalah derajat 1 (40%). Hasil uji Spearman menunjukkan adanya hubungan yang signifikan antara usia dan derajat shivering (ρ = 0,470; p = 0,001), serta antara lama operasi dan derajat shivering (ρ = 0,546; p = 0,000). Kesimpulan: Terdapat hubungan yang signifikan antara usia dan lama operasi dengan derajat shivering pada pasien pasca spinal anestesi sectio caesarea. Pasien usia produktif dan dengan durasi operasi singkat cenderung mengalami shivering ringan atau tidak sama sekali.

Abstract

As people age, physiological changes significantly impact the cardiovascular, respiratory, renal, and central nervous systems, as well as drug elimination processes. These changes make elderly individuals more vulnerable to the side effects of general anesthesia, potentially prolonging recovery time and increasing the risk of postoperative complications. This study aims to assess the effect of early passive Range of Motion (ROM) mobilization on the recovery time of consciousness in elderly patients post-general anesthesia at RSD Gunung Jati, Cirebon. The study used a quasi-experimental design with a posttest-only non-equivalent control group. A total of 42 elderly patients participated, divided into two groups: 21 patients in the intervention group who received 10 minutes of passive ROM mobilization, and 21 patients in the control group who received standard postoperative care. Recovery of consciousness was assessed using the Modified Aldrete Score, monitored every 5 minutes until a score of ?9 was achieved. Data were analyzed using the Shapiro-Wilk test for normality and the Mann-Whitney test to examine the effect of ROM mobilization on recovery time. The results showed the intervention group had a mean recovery time of 22.86 minutes (SD 2.28), compared to 27.14 minutes (SD 6.90) in the control group. Statistical analysis revealed a significant effect (p < 0.000). These findings support the claim that early passive ROM mobilization effectively accelerates recovery of consciousness in elderly patients after general anesthesia and should be considered as an adjunct strategy in the Post Anesthesia Care Unit (PACU).

Abstract

Pre-surgical information for patients is important for patients to know so that they are better prepared for the surgical process. Inadequate pre-surgical preparation can be a cause of anesthetic accidents. This study aims to describe the knowledge of preoperative patients about the importance of fasting before surgery at Pidie Jaya Hospital. This research method is quantitative with a descriptive approach. Sampling technique with nonprobability sampling by means of accidental sampling as many as 104 respondents who perform operations. Data analysis was performed univariately. The results showed that the majority of patients aged > 35 years were 59 respondents (56.7%), the majority were male, 63 respondents (60.6%), the majority had junior high school education, 61 respondents (58.7%), the majority 37 respondents (35.6%) worked as farmers, the majority had never undergone surgery, namely 92 respondents (88.5%). The majority of preoperative patients' knowledge about the importance of fasting is in the less category, namely as many as 43 respondents (41.3%). This study concluded that the knowledge of preoperative patients about the importance of fasting at Pidie Jaya General Hospital was mostly in the less category

Abstract

During the luteal phase of the menstrual cycle, which typically begins 7-10 days before menstruation and ends with menstruation, women of childbearing age often experience a collection of physical, behavioral, and emotional symptoms known as premenstrual syndrome (PMS). Stress levels are among the factors that can contribute to PMS. The objective of this study is to investigate the relationship between stress levels and PMS. This research employs a quantitative approach, using descriptive correlation methods and a cross-sectional design. The sample consisted of 62 respondents, selected through total sampling. Data were collected using a shortened Premenstrual Assessment Form (SPAF) questionnaire and the Perceived Stress Scale (PSS). Statistical analysis involved the Spearman Rank Test, which revealed a significant relationship between stress levels and PMS among Nursing Study Program students at Harapan Bangsa University Undergraduate Program. The p-value was 0.000, which is less than the significance level (α = 0.05), leading to the rejection of the null hypothesis (H0).The correlation coefficient (r) was calculated to be 0.456, indicating a moderate strength of correlation. This suggests that higher stress levels are associated with a greater perceived severity of PMS symptoms. Female students demonstrated an ability to both avoid stressors and manage stress, which can help reduce the occurrence of PMS symptoms

Abstract

The patient’s care body temperature below 36°C is usually diagnosed as hypothermia. The human thermoregulatory system maintains metabolic activity by keeping body temperature about 37ºC. Hypothermia is linked to anesthesia and surgery. This study examines the risk factors for hypothermia in spinal anesthesia patients at the Central Surgical Installation of RSI Fatimah Cilacap. Age, gender, BMI, and length are variables. This research method is using a quantitative analytical observational approach with total sampling technique. The results of this study showed that the most dominant age of respondents was adults 25-46 years (45.9%) with mild hypothermia (29.7%). The study also found 59.5% of participants were female and 29.7% had mild or moderate hypothermia. Most respondents (64.9%) had an average BMI 37.8% had mild hypoyhermia. Finally, 56.8% of respondents had 1-2-hour surgery, with 32.4% experiencing mild hypothermia. The conclusion of this study found that Body Mass Index (BMI) is the main risk factor for hypothermia with 64.9% of people having a normal BMI.

Abstract

Hypertension is a prevalent health problem that increases the risk of cardiovascular complications. In addition to pharmacological therapy, non-pharmacological interventions such as brisk walking exercise play an important role in helping control blood pressure. This study aimed to analyze the effect of brisk walking exercise on blood pressure reduction among patients with hypertension in the working area of Puskesmas Kembaran 1, Banyumas Regency. This research employed a quasi-experimental design with a one-group pretest–posttest approach. A total of 32 respondents were selected using purposive sampling, consisting of hypertensive patients without complications and with stable conditions. The intervention consisted of brisk walking exercise performed for three consecutive days, 15 minutes per session. Blood pressure was measured 30 minutes before and after each intervention using a sphygmomanometer and stethoscope. Data analysis used the paired t-test for systolic pressure and the Wilcoxon test for diastolic pressure. The results showed that most respondents were late elderly (56.3%), female (62.5%), and had a family history of hypertension (65.6%). The mean blood pressure before the intervention was 149.47 mmHg (systolic) and 89.97 mmHg (diastolic). After the intervention, it decreased to 142.09 mmHg and 84.38 mmHg, respectively. Statistical tests indicated a significant reduction in systolic (7.38 mmHg) and diastolic pressure (5.59 mmHg) with p = 0.0001. The study concludes that brisk walking exercise has a significant effect on reducing blood pressure among hypertensive patients. This intervention is recommended as a simple, safe, and affordable strategy for hypertension management in primary health care settings

Abstract

Delayed recovery of motor function following spinal anesthesia may prolong recovery room stay and delay early mobilization. Passive Range of Motion (ROM) is a simple non-pharmacological intervention that may accelerate motor recovery. To describe the implementation of passive ROM in accelerating Bromage Score recovery among patients after spinal anesthesia at Rumah Sakit Khusus Bedah Jatiwinangun Purwokerto. This community service program involved 30 postoperative spinal anesthesia patients who met the inclusion criteria. Activities included patient education, assessment of Bromage Score before intervention, passive ROM implementation for 5–10 minutes, and post-intervention evaluation. Data were analyzed descriptively using frequency distribution. Most participants were 17–45 years old (76.7%), male (60%), classified as ASA I (66.7%), received 10 mg hyperbaric bupivacaine (46.6%), and had no previous surgical history (66.7%). Before intervention, all participants had Bromage Score 3 (100%). Following passive ROM, 60% achieved Bromage Score 1, with an average recovery time of 41 minutes. Passive ROM is a practical, safe, and feasible intervention that may accelerate motor recovery after spinal anesthesia and support early postoperative mobilization.
