Abstract

Pendahuluan: Tindakan implan merupakan salah satu upaya untuk mengganti gigi yang hilang. Pemasangan implan yang baik, mampu memberikan kenyamanan dan aspek estetis yang baik. Pemasangan implan yang penuh dengan resiko dan ketidak hati-hatian pada pemasangan berakibat tidak baik bagi pasien. Tujuan dari penulisan laporan kasus ini adalah untuk melaporkan kasus ketidaknyamanan yang disebabkan timbulnya reaksi inflamasi disertai supurasi pada sinus maksilaris paska pemasangan implan, dan juga untuk melihat kemampuan dari Cone Beam Computed Tomography (CBCT) dalam menganalisa hal tersebut. Laporan  kasus: Perempuan berusia 40 tahun, mengeluhkan adanya rasa tidak nyaman berupa bau mulut dan hidung disertai hidung tersumbat dan pusing kepala. Anamnesa diketahui bahwa pasien telah melakukan pemasangan implan 3 bulan sebelumnya. Pemeriksaan intraoral menemukan adanya implan pada regio posterior, tanpa rasa sakit dan tanda peradangan. Manajemen kasus dilakukan dengan meminta pasien melakukan pemeriksaan CBCT, karena dicurigai rasa tidak nyaman, pusing dan bau disebabkan oleh implan yang saat ini telah terpasang. Setelah dilakukan pemeriksaan CBCT ternyata ditemukan sinus aproksimasi pada ujung implan. Ujung implan masuk ke dalam sinus dengan panjang lebih dari 2 mm. Hal ini menyebabkan infeksi pada dinding sinus dan berkumpulnya nanah pada daerah sinus. Hal ini membuktikan bahwa implan menyebabkan infeksi pada sinus sehingga kasus ini terjadi. Pasien kemudian dirujuk ke bagian bedah untuk dilakukan perbaikan pada implan. Simpulan: Inflamasi sinus disertai supurasi pada sinus maksilaris paska pemasangan dapat terjadi, hal ini terjadi kemungkinan karena respon tubuh terhadap implan yang masuk ke rongga sinus. Analisa dapat dilakukan dengan memanfaatkan radiografi CBCT.Kata kunci: Implan, CBCT, infeksi sinus maksilaris. ABSTRACTIntroduction: Dental implant placement is an attempt to replace missing teeth. Installing the right implant can provide comfort and good aesthetic aspects. However, the installation of implants with full risks and caution will hurt the patient; thus proper planning is needed for implant placement. The purpose of this case report was to report cases of discomfort caused by an inflammatory reaction accompanied by suppuration in the maxillary sinus after implant placement and also to see the ability of Cone Beam Computed Tomography (CBCT) in analysing this. Case report: A 40-years-old woman complains of discomfort in the form of bad breath, nasal congestion, and headache. Anamnesa found that the patient had implant placement 3 months earlier. An intraoral examination found an implant in the posterior region, with no signs of pain and inflammation. Case management was performed by asking the patient to do a CBCT examination due to suspected discomfort, dizziness and bad breath caused by implants that are currently installed. After a CBCT examination found a sinus approximation at the tip of the implant. The tip of the implant goes into the sinus with a length of more than 2 mm. This caused an infection of the sinus wall and the gathering of pus in the sinus area. This proves that the implant caused an infection of the sinuses. The patient was then referred to the surgical section for the implant repairment. Conclusion: Sinus inflammation accompanied by suppuration of the maxillary sinus after installation can be occurred likely due to the body’s response towards the implants entering the sinus cavity. Analysis can be performed using CBCT radiography.Keywords: Implant, CBCT, maxillary sinus infection.

Abstract

Objectives: This report is aimed to compare diagnostic accuracy of impacted maxillary canine and dentigerous cyst in determining its dimension and position using panoramic and CBCT (cone beam computed tomography) imaging. Case Report: A 30-year-old woman was referred to Department of Dental and Maxillofacial Radiology with the suspect of upper right impacted canine. She had chief complaint of swelling pain in the palate for ± 3 months. Panoramic showed 13 impacted superiorly to the maxillary sinus and surrounding with a radiolucent lesion attached on cementoenamel junction. However, panoramic has limitations in assessing the labio-palatal position of impacted canine and lesion extension. CBCT revealed the tooth located closer to palate and no sign of direct contact with sinus. The radiolucent lesion has destructed anteroposterior aspect of cortical plate which explain the reason of swelling and pain. Conclusion: Precise dimension and position are important to avoid mistakes since impacted maxillary canine accompanied with dentigerous cyst required invasive management. In our case, panoramic and CBCT contributed to guide the diagnosis and help the referrer determining the exact site of surgery.

Abstract

Objectives: The study is aimed to give further explanation and understanding of radiograph quality assurance based on scientific literatures. Literature Review: A radiograph’s quality can be assessed based on the produced image itself. The radiograph must meet the standard values to be interpreted by practitioners and radiologists. There are various kinds of standard value of a radiograph that should be met before doing an interpretation, therefore a review is needed to assess the quality of a radiograph. This literature will discuss how to assess the quality of a radiograph. Based on the review of previous literatures, differences have been found between one quality assessment and another, so that the quality of radiograph is fulfilled. Conclusion: A radiograph has many aspects to be assessed to determine its quality.

Abstract

Objectives: The purpose of this study was to determine alveolar bone density in patients with chronic periodontitis and aggressive periodontitis through panoramic radiography . Material and Methods: The method used in this study was descriptive. This study was conducted on populations that met all criteria. The sampling method used was purposive sampling technique, so that 31 panoramic radiographs were diagnosed with chronic periodontitis and 7 panoramic radiographs were diagnosed with aggressive periodontitis. Results: The study showed that bone density in chronic periodontitis had an average of bony trabeculae 28.6% and marrow spaces 71.4% while bone density in aggressive periodontitis had an average bony trabeculae 20.2% and marrow spaces 79.8% Conclusion: Based on the results of the study it can be concluded that bone density in patients with chronic periodontitis and aggressive periodontitis has a low bone density value. The average bone density in patients with chronic periodontitis and aggressive periodontitis has different values, where the bone density in chronic periodontitis is greater than aggressive periodontitis.

Abstract

Objectives: The purpose of this research was to describe radiographic density of mandibular bone in calcium channel blocker anti-hypertensive drug users. Bone density in the mandible is assessed from the trabecular. Panoramic radiograph is a routine examination that is often done in dentistry that can be used to assess changes in quality in the form of changes in bone density in users of anti-hypertensive calcium channel blockers Material and Methods: This research is a descriptive study of 21 panoramic radiographs of calcium channel blocker anti-hypertensive drug users aged 40-75 years. Panoramic radiograph archive density checks in the distal region of the foramen mentale and the mandibular angular region using software image j, with the final result was the percentage between bone and marrow. Results: This research showed the average radiographic density in male using calcium channel blocker antihypertensive drugs was 18.81% and the average radiographic density in female was 20.92%. Conclusion: Based on the results of the study found that the average radiographic density of female patients taking antihypertensive drugs calcium channel blockers was higher than male.

Abstract

Objectives: Understanding the differences of general signs and typical characteristics of a lesion in radiograph is constantly important for dentists to determine the nature of a lesion. Some signs that can be observed more specifically were the presence of periosteal reaction and the border of existing lesions. These differences may be taken into consideration to define the diagnosis and type of the lesion. The purpose of this article is to provide an overview of the radiographic features of periosteal reaction and border in jaw lesions. Literature Review: This article is a literature review which discussed several articles relating to the radiographic features of border and periosteal reaction in various jaw lesions. Based on this review, there were different features of border, where each lesion had its own borderline that differs between cystic, benign and malignant properties. While the picture of periosteal reaction indicates the extent to which the lesion involves cortical tissue in the bone. Conclusion: The conclusion is that the border image and periosteal reaction can be one of the typical markers in determining jaw lesions.

Abstract

Objective: This research was aimed to analyze the mandibular cortical width based on a mental index (MI) and gonial Index (GI) in hypertension patients correlated with serum calcium and cortisol levels. Materials and Methods: This study was an analytic-observational study with 31 hypertension patients aged 41-79. All of the patients have checked their serum calcium and cortisol level. The panoramic radiograph was taken and analyzed using Image-J Fiji software. The mandibular cortical width was then measured with Mental Index (MI) and Gonial Index (GI) and correlated with the serum calcium and cortisol level. Results: The data obtained from examining serum calcium and cortisol levels and the measurement of Mental Index (MI) and Gonial Index (GI) showed varying results but tended towards normal values. The results of the correlation test between mental index and gonial index with cortisol and serum calcium showed no correlation with p>0.05. Conclusion: There was no correlation between serum calcium and cortisol level in the value of MI and GI in hypertension patients.

Abstract

Objectives: This scientific paper discusses aspects of biological bone and radiograph examination in helping diagnose systemic diseases with a decrease in bone quality more accurately. Literature Review: Osteoporosis often occurs in postmenopausal women because of reduced estrogen. Sign analysis is related to four important factors to assess bone quality, namely bone density, bone turnover, bone size and bone architecture. Mineral Bone Examination Density is a gold standard examination by the World Health Organization for the diagnosis of osteoporosis and bone biomarkers can provide an overview of the renovation process being carried out. Conclusion: Panoramic radiographs are expected to be a potential checkpoint for early detection of systemic diseases that manifest in the maxillofacial region with bone conversations characterized by bone enlargement, changes in bone microstructure and trabeculae that indicate changes in bone quality.

Abstract

Objectives: This study is aimed to introduce an overview of the anatomy of the neck region as well as an overview of some pathological conditions that can be seen through Ultrasound. Literature Review: There was a characteristic in the anatomy of the neck by Ultrasound. Anatomy of the neck on Ultrasound, divided into several areas with its characteristics. Ultrasound can thoroughly assess pathological conditions related to anatomy. Conclusion: Ultrasound was a modality that can be used to see the condition of the anatomy, including the neck area. Pathological conditions were also able to be appropriately seen through Ultrasound.

Abstract

Objectives: This case report is aimed to present a finding of a benign tumor at the Temporomandibular Joint (TMJ) area involving the condylar head of the mandible that radiographically showed the typical features of osteochondroma using and emphasizing on the application of Cone Beam CT (CBCT) imaging. Case Report: A 24-year-old female patient came to the Radiology Department of Unpad Dental Hospital as referred from her previous dental surgeon to get CBCT examination of her entire right side of mandible with a provisional diagnosis of mandibular hyperplasia. Conclusion: Osteochondroma and condylar hyperplasia are often clinically difficult to differentiate, CBCT imaging can easily distinguish the enlargement of condylar head in condylar hyperplasia with irregular condylar mass and altered trabecular pattern in osteochondroma. CBCT may be helpful in establishing the diagnosis of condylar tumors originating from bone.
