Abstract

Cigarette smoking is a major cause of cardiovascular disease due to elevation of free radical substances as well impairment in lipid metabolism. Smoking cesation has been linked with reduced risk of mortality. However, data regarding cardiovascular disease (CVD) risk factor in former smoker is limited. The aim of the study was to evaluate the CVD risk factor among smoker, non-smoker and former smoker in Yogyakarta Special Region, Indonesia. A cross-sectional design from 86 apparently healthy male, aged between 25-50 years old was performed for this study. From total subjects, 45 subjectsare smokers, 26 are non-smokers and 15 are former smokers. Body weight, height, and blood pressure were also measured from the subjects. Blood was drawn for assessment of total cholesterol (TC), triglyceride, low density lipoprotein (LDL), high density lipoprotein(HDL) and C-reactive protein (CRP) concentrations. We found significant difference in the level of HDL, cardiovascular risk index (LDL/HDL ratio), atherogenic index (TC/HDL ratio) and also in systolic blood pressure among groups (p<0.05). Smoking person hadsignificantly low HDL level with high CVD risk index and atherogenic index (p<0.05). The CVD risk index was not different between former smoker and smoker groups. Serum CRP level was not different among the groups. In conclusion, smoking is associated with increased risk of CVD. Smoking cessation slightly decrease the CVD risk.

Abstract

ABSTRACTIntroduction. Chronic stress disturbs the equilibrium of oxidant-antioxidant redox in the human body which accelerates cellular aging. This condition promotes earlier onset of age-related diseases. Human body needs exogenous antioxidant to fight stress. Syzigium cumini has high antioxidant activity because it’s rich in anthocyanin. It has potential to become natural antioxidant source thus it needs to be developed.Aim. This study aims to investigate the effect of Syzigium cumini (Pulp) extract on the stress biomarkers which are Malondialdehyde and blood pressure in rat induced by restraint stress.Method. Chronic stress phase is conducted with male Wistar rats (n=20) which were divided into 4 groups (normal control, negative control, and two treatment groups with the ethanol extract of Syzygium cumini (Pulp) at 20 mg/200 gram and 40 mg/200 gram body weight). Restraint stress was carried out by placing the animal in narrow tube for 30 minutes daily (7 days). Blood pressure was evaluated before and after the treatment while Malondialdehyde was evaluated after the treatment. The One-way ANOVA was used to analyze the parameters.Results. Based on chronic stress phase, treatment groups show significant result in preventing the increase in blood pressure (P < 0,001). The increased levels of Malondialdehyde were prevented significantly in all treatment groups (P < 0,001)Conclusion. In chronic stress phase, Syzygium cumini (Pulp) extract can become an antihypertensive agent. Syzygium cumini (Pulp) extract shows an effect in decreasing lipid peroxidation products. This result shows that the Syzygium cumini (Pulp) extract has potent antioxidant function.Keywords. Syzigium cumini, restraint stress, malondialdehyde, MDA, blood pressure

Abstract

Cardiovascular disease is number one killer disease in the world. The most common etiology is atherosclerosis. LDL and HDL-cholesterol are often associated with atherosclerosis which are affected by diet. Quercetin and curcumin considered have influenced to LDL and HDL. They are antioxidant that protect against LDL oxidation and raised HDL-cholesterol. In this study, we invest about comparison effect of prevention in LDL and HDL level change, after giving quercetin and curcumin. 20 males Rattus novergicus rats in 200-300 grams divided into 4 groups. Group B, C, D were given high-fat diet for 4 weeks, while group A (negative control) was given standard woof. Group B was positive control and no treatment, C was given quercetin, D was given curcumin. The treatment and giving high-fat diet was done simultaneously to detect prevention effect. HDL and LDL-cholesterol evaluated two times, pre and post-treatment. Result showed the increase prevention level of LDL-cholesterol and decrease prevention level of HDL-cholesterol in group C and D significantly. The best prevention effect is quercetin, p=0.000. Furthermore, we showed there was no significant correlation between DLDL and DHDL-cholesterol level. In conclusion, quercetin and curcumin have a good effect in preventing hypercholesterolemia, however quercetin have better effect than curcumin.

Abstract

This study aims to analyze body type, dietary intake, and cardiorespiratory function of college soccer players. A total of 32 players (20 players from Yogyakarta State University and 12 players from Gadjah Mada University) were involved in this study. The study was carried out in April - June 2014. Body type, or somatotype, was calculated based on anthropometric measurements (weight, height, epicondylar humerus and epicondylar femur width, arm and calf circumference, and subcutaneous fat thickness). 24-hour food recall was used to measure the athlete's dietary intake. VO2max was used to describe the athlete's cardiorespiratory function, and it was measured by yo-yo intermittent recovery test level 2. The data were analyzed descriptively using statistical software. More than three-quarter of the players had normal nutritional status with body mass index=21.54±1.84. The majority player's body type was balanced mesomorph (2.6-4.4-2.7). Balanced mesomorph players and ectomorphic mesomorph players had adequate energy and protein intake (2,623.83 kcal energy and 75.82 g protein for balanced mesomorph players; 2.724.68 kcal energy and 84.83 g protein for ectomorphic mesomorph players). Ectomorphic mesomorph players had the highest VO2max level (52.37±2.61 ml/kg/min). Soccer players with a mesomorph component supported by good energy and protein intake have the most optimal VO2max level.

Abstract

Background: Medical students in Indonesia are required to pass a national licensing examination (e.g., UKMPPD) as a requirement of graduation and to obtain their certificate of competence. Medical schools prepared their students based on their needs and capacity. The preparation might use different teaching approaches. Undergraduate medicine program in Universitas Gadjah Mada conducts a national licensing examination preparation program facilitated by both faculty members and peer tutors. The peer tutors are newly graduated medical doctors who were recruited and trained. The use of trained peer tutors in a structured program to prepare high-stake examination such as a national examination is considered novel. This paper aims to describe a peer-assisted learning (PAL) approach used to prepare medical students in facing a national licensing examination.Case Discussion: A preparation program using faculty and peer tutors was deployed to prepare final-year medical students for facing the Indonesian national licensing examination. The ratio of sessions facilitated by faculty to peer tutors was 1:2. The preparation included CBT and OSCE training. Sessions with faculty used a medium to large class approach while sessions with peer tutors used a small to medium group approach. Through February 2020, 147 students have participated in the program. A questionnaire was developed to measure agreement that the program was successful and if the tutees were satisfied with various aspects of the program. The tutees responded with high rates of satisfaction to most items on the questionnaire  (responses for all but one item ranged between 72.1% and 90.5% in favorable agreement or satisfaction). The one exception was CBT preparation where the level of satisfaction was lower (54.4% rated CBT favorably). Participants highly valued the involvement and the performance of peer tutors.Conclusion: The use of near-peer assisted learning for national licensing examination preparation has strong potential. Further study is required to investigate the impact of the peer-assisted national licensing examination preparation program on students’ achievement and clinical practice.

Abstract

HbA1c is a parameter that may be used in predicting and diagnosing diabetes. Since diabetes is predicted to increase in Indonesia, it is necessary to understand the distribution and associated factors of HbA1c in the general Indonesian population. However, those data are still limited. This study aims to determine the distribution of HbA1c and its associated factors in Indonesian adults. We conducted a cross-sectional study analyzing data from Indonesia Family Life Survey (IFLS) 2014-2015. HbA1c was analyzed using dried blood spot (DBS) specimen with Bio-Rad D10 HPLC. We performed Student’s t test and ANOVA to show the results of the bivariate analyses and multiple linear regression to determine the association between variables. The study included 4.101 subjects (20-59 years) without diabetes and provided an overview of the distribution of HbA1c levels based on socio-demographic factors and smoking behavior described in percentiles. The mean of HbA1c was 5.35% with a standard error of 0.01. The threshold value of HbA1c for prediabetes (5.7%) corresponded approximately to the 75th percentile. Although there were differences in HbA1c distribution, socio-demographic factors such as education levels, employment, and settlement region as well as smoking were not independently associated with HbA1c levels in Indonesian adults. Furthermore, age and sex were associated with HbA1c.

Abstract

Background: Iron deficiency anemia in pregnancy remains an important health problem in Indonesia. Ferroportin (FPN1) is one of important gene in iron metabolism encodes ferroportin for iron absorption, release, and recycle inside the body. The polymorphism of FPN1 Q248H alters negative charge of the amino acid sequence positively, hence influenced its ubiquitination and degradation. Beside of that, the high prevalence of anemia not only caused by genetics, nutrition, physiology of each pregant women, but also demographic factor and sosio-economic burden of family. This research will study the polymorphism of ferroportin Q248H and demographic character in pregnant women in Surakarta.Methods: The study using descriptive analytic case-control approach. The data used were from questionaire and blood sampling whose counted for screening anemia in private laboratorium. Pregnant women who had intention and had 10-25 week in pregnant were selected to be research subject.  FPN1 Q248H polymorphism were determined using PCR-RFLP method. Analysis Chi-square dan Independent t-test used to conclude the relationship between each variable to anemic state of pregnant women in Surakarta. A p<0.05 was considered as significant. The study using decriptive analytic design method. Results: Prevalence of anemia in Surakarta is 25.7% with 80.7% have mild anemia. There was no varian in FPN1 Q248H. Majority of pregnant women in group of anaemia have demographic character like 20-25 old, multigravid, last education in High School, housewife, their income range from Rp 500.000,00 to Rp 1.000.000,00, and consume Fe tablet. The bivariat analysis show no statistical significancy of parity, maternal age, education, income, and comsumption of iron tablet to state of anaemia (p>0.05).Conclusions: The FPN1 Q248H polymorphism was not a risk factor for iron deficiency anemia in pregnant women, likewise socio-economic factors had no significant role to incidence of anemia in Surakarta. Keywords: Anemia in pregnancy, Ferroportin (FPN1) Q248H polymorphism, Socio-Economic

Abstract

Background: According to WHO data, prevalence of anemia pregnancy in Indonesia is 44.3%, it’s higher than world prevalence (41.8%). Ferroportin (FPN1) is one of important iron exsporter for iron absorption, release, and recycle inside the body. The varian of FPN1-1355 G/C in promoter region, leads to increased of ferroportin expression and iron export, increased cellular iron needs, overexpression of soluble transferrin receptor (sTfR), decrease hemoglobin (Hb) and erythrocyte indices that manifest to iron deficiency anemia (IDA).Objective: This research will study the frequency of FPN1-1355 G/C polymorphism as a risk factor of IDA in pregnant women in Indonesia.Method: The research design was a case and control study. Blood samples were taken from 26 pregnant women with anemia and 48 pregnant women without anemia. FPN1-1355G/C polymorphism were determined using PCR-RFLP method. sTfR and ferritin level were measured with ELISA. Hemoglobin, erythrocyte indices, and sTfR level were compared among genotype group, then statistically analyzed using independent sample t-test and one way ANOVA. Bivariat analysis of Pearson test was conducted to analyze correlation between level of blood Hb and ferritin in pregnant women (p<0.05).Results: FPN1-1355 G/C polymorphism with frequency in pregnant women with IDA and in pregnant women with anemia non IDA were 100% and 95.2%, respectively (p=0.710; OR=1.600; 95%CI: 0.296-8.653). The mean of Hb level and erythrocyte indices in subjects carrying C allele were lower than subjects carrying only G allele although Hb level is not significantly different (p>0.05). The sTfR and hepcidin level in subjects carrying C allele were higher than subjects carrying only G allele (p<0.05). Conclusion: In this study the FPN1 gene promoter -1355 G/C polymorphism was not a risk factor for anemia, but it was a risk factor for iron deficiency anemia in pregnant women.

Abstract

Backgrounds: The immune system changing affects the fitness. Exercise induces the immune responses, oxidative stress and tissue damage that alters leukocyte counts. Maltodextrin supplies higher energy reserves, that can improve the immune system profiles. Vitamin C as an antioxidant against tissue damage.Objective: This study aims to assess the effectiveness of a maltodextrin and vitamin C combination drink on leukocyte differential count after exercise.Method: This study was quasi-experimental with the within-subject design. Subjects were 14 collegiate football players of the State University of Yogyakarta and fulfilled the inclusion and exclusion criteria. Subjects were selected with a purposive sampling. Subjects received a combination of 15% maltodextrin and 250 mg of vitamin C beverage, 300 mL. Subjects did wash out for 6 days, then received placebo beverage (300 mL plain water). Subjects drank 30 minutes before and 5 minutes after physical exercise. Exercise loads were yoyo intermittent test and run gradually until 80 HRmaks. Blood samples were taken immediately and 30 minutes after exercise.Results: In the administration of intervention beverage, immediately to 30 minutes after exercise neutrophils increased (p=0,006); lymphocytes decreased (p=0.015); monocytes decreased (p=0.000); and eosinophils decreased (p=0.613). In the administration of placebo beverage, immediately to 30 minutes after exercise, neutrophils increased (p=0.359); lymphocytes decreased (p=0.257); monocytes decreased (p=0.146); and eosinophils increased (p=0.549). Neutrophils (p=0.003) and monocyte (p=0.003) had significant difference between both administration drink at 30 minutes after exercise. Lymphocyte and monocyte significantly decrease (p<0.05) at intervention than placebo.Conclusion: Maltodextrin and vitamin C combination drink effect on the decreasing of lymphocyte and monocyte 30 minutes after exercise.

Abstract

Background: Lifestyle changes with high-fat food consumption is one of the factors the risks of cardiovascular diseases like of coronary heart disease and atherosclerosis. A healthy diet and a balanced diet and consume foods that contain lots of antioxidants is one of the effective ways to prevent hyperlipidemia. Mung bean sprouts have properties that neutralize free radicals cause Hyperlipidemia and cardiovascular diseases because it is an antioxidant compound.Objective: The aim of this study was to determinate the effect of mung bean sprouts (Phaseolus radiatus (L)) to blood pressure and histopathology aorta of Sprague-Dawley male rats.Method: The type of study was experimental research using pre-post test controlled group design for blood pressure variable and post test only controlled group design histopathology aorta. The thirty-five of Sprague-Dawley male rats was eight weeks divided into 5 groups. The first group was given standard diet, group 2 was given a hight fat diet, the third group was given a high-fat diet and mung bean sprout 0,67 gram, group 4 was given a high-fat diet and mung bean 1,34 gram, and group 5 was given a high-fat diet and vitamin E doses of 23 IU.Results: Result of this study showed that after 4 weeks of treatment, increased in blood pressure systole in the given of  high fat diet higher than group who were given a high fat diet and mung bean sprout and also on group who were given high fat diet and vitamin E, but there is no difference effect a decrease in blood pressure between the provision of mung bean sprouts and vitamin E (p>0,05). Statistical analysis to thick the wall the aorta show the similarity meaningful in all the treatment group, it can be said that overall thick the wall the aorta in this research is not different.Conclusion: A dose of mung bean sprout 0,67 g is optimal doses in preventing a rise in blood pressure and prevent alterations histopathology Sprague-Dawley male rats.
