Abstract

Background: Hypertension management in rural Java currently focuses more on physical treatment and traditional herbal remedies, whereas a holistic care approach that integrates physical, psychological, social, cultural, and spiritual aspects is needed to be more effective, relevant, and sustainable. The lack of understanding of holistic care among cadres and older women has led to limited community involvement, so it is necessary to describe their perceptions to develop practical hypertension management appropriate to rural communities' needs. Purpose: To describe the perception of holistic care in hypertension management among female community health workers (locally known as cadres)  and older women in rural Java. Method: The research was conducted during integrated health development post activities and through home visits from July 2025 to February 2026 in a village in Semarang Regency, Central Java. The instruments used were Focus Group Discussion guidelines, semi-structured interview lists, and observation sheets. There were 22 Respondents, including 11 cadres and 11 older women, who met the inclusion criteria: being older adults cadres or older women with hypertension aged >60 years, being Javanese, and having a history of hypertension for >1 year. The exclusion criteria were elderly individuals with disabilities, bedridden individuals, and individuals unable to communicate. Data validity was ensured through triangulation of sources with community health Center nurses, midwives, and village heads. Results: The study revealed four themes: knowledge of the older women and cadres, health practices, dissemination of health information, and the roles of village midwives, elderly cadres, and families. Conclusion: This study found a limited understanding of holistic care among cadres and rural older women who still focus on the physical complaints of hypertension. Comprehensive education through midwives, cadres, families, and nurses is needed for optimal holistic management.

Abstract

Background: Consistent self-care is key for effective hypertension management, but is difficult for older adults in rural settings. Understanding how individual and contextual factors interact helps develop targeted nursing interventions. This study examined the determinants of self-care adherence among older adults with hypertension in rural Central Java by analyzing the direct and indirect links between hypertension knowledge, self-care profiles, and health care empowerment. Methods: A quantitative study with path analysis included 60 older adults (aged >60 years) with hypertension in Getasan District, Semarang Regency, Central Java. Data were collected from August to December 2025 via validated questionnaires and analyzed using SmartPLS and IBM SPSS version 24. Result: Health service empowerment had the strongest positive effect on self- care adherence (β = 0.402, p < 0.001). Hypertension knowledge did not directly influence adherence, but it did so through health care empowerment (β = 0.171, p = 0.004). The model explained 23% of the variance in self-care adherence (R2 = 0.230). Conclusion: Health care empowerment, rooted in patient motivation and perception, is the main factor in adherence and mediates health knowledge. Nursing interventions should shift from education to empowerment-based strategies to enhance independence in hypertension management among rural populations.

Abstract

Background: The rising prevalence of non-communicable diseases (NCDs) necessitates the development of innovative digital healthcare solutions to improve prevention and self-management. This study developed and evaluated an Android-based application for NCD prevention and management, focusing on feasibility, validity, and reliability. Methods: A research and development (R&D) approach was utilized, comprising four stages: define, design, develop, and disseminate. The application underwent expert feasibility testing with nursing experts, community specialist physicians, contextual experts, and program managers. Construct validity was determined using item-total correlation (Pearson), and reliability was assessed with Cronbach’s alpha. Additionally, a small-scale user trial with 40 participants evaluated ease of use, appearance, satisfaction, and overall usability using a 17-item Likert-scale instrument. The application achieved high feasibility, with an overall score of 86.4%. Results: All items met established validity and reliability criteria. User testing further indicated strong acceptance, favorable usability, high user satisfaction, and a positive interface evaluation. Conclusion: In summary, the Android-based NCD application is feasible, valid, and reliable for supporting the prevention and management of non- communicable diseases. Larger-scale studies are warranted to assess its effectiveness in improving health behaviors and outcomes.

Abstract

Abstract

Hypertension is a major health problem among older adults because it increases the risk of chronic disease complications. In Ngrawan Village, 70 older adults were identified as having hypertension during activities at the elderly integrated health post (Posyandu Lansia), while 87.14% of the foods consumed by older adults were classified as high in salt. This condition highlights the need to strengthen the role of community health cadres in promoting hypertension-preventive behaviors in the community. This community service program aimed to enhance the capacity of Posyandu cadres to provide hypertension prevention education based on local food resources. The activity was conducted using a service-learning approach involving 25 health cadres in Ngrawan Village, Central Java, on September 15, 2024. The educational media used included a pocketbook entitled Ngadi Salira Atasi Hipertensi and a DASH diet video entitled Diet Ingkang Atasi Saben Hipertensi, which were developed using local language, attractive visuals, and examples of local foods. Evaluation was carried out using a questionnaire assessing language clarity, visual presentation, and overall media quality. The results showed that all cadres (100%) received the media very positively. This positive reception was followed by increased readiness among cadres to deliver education in a more active, communicative, and contextual manner, as well as to encourage the adoption of healthy behaviors among older adults, particularly reducing salt intake, choosing healthier local foods, and monitoring blood pressure regularly.
