Abstract

The demand for palliative care in Indonesia continues to rise as more patients live with chronic and life-limiting conditions. Yet, discharge planning—which should serve as a bridge between hospital care and home care remains limited, particularly when it is not integrated with Advance Care Planning (ACP). This study aimed to design an ACP-based discharge planning model that supports smoother transitions and strengthens continuity of care for palliative patients after hospital discharge.  A descriptive design with a developmental action research approach was carried out at the Hospital. Thirty nurses participated through surveys, complemented by in-depth interviews with the head nurse, nursing committee, and nursing division, as well as direct observation of clinical practice. This study used a purposive sampling technique, with data collected through observations, in-depth interviews, and structured surveys, and analyzed using thematic analysis for qualitative data and descriptive analysis for quantitative data to explore nurses’ knowledge, attitudes, and readiness in implementing ACP-based discharge planning. Data were analyzed descriptively and further explored using SWOT analysis and a fishbone diagram to map strengths, weaknesses, opportunities, threats, and root causes. Findings showed that most nurses understood the concept of ACP (80%) and expressed readiness to be involved, although the majority (73.3%) had never received formal training. Key barriers included limited empathic communication skills, the absence of standard operating procedures (SOPs), and the lack of standardized ACP forms. As an initial step, the study produced draft SOPs, technical guidelines, and ACP forms, which were considered useful but still require refinement and pilot testing. Overall, ACP-based discharge planning has strong potential to improve continuity of care and enhance the quality of life for palliative patients at home. Its success, however, will depend heavily on sustained managerial support, continuous training for nurses, and the active involvement of patients and their families in the care process.

Abstract

Medical equipment control is a crucial aspect in supporting the continuity and effectiveness of nursing services in inpatient wards. This study aimed to evaluate internal and external factors influencing the effectiveness of medical equipment control and to formulate the need for developing a structured, documentation-based handover tool. This preliminary study was conducted in hospital inpatient wards involving Nurse Unit Managers (NUMs) and staff nurses. Data were collected through in-depth interviews, field observations, and questionnaires. Medical equipment control processes were examined using the POSAC management framework. Questionnaire data were analyzed descriptively, while overall data were analyzed using POSAC and SWOT analyses, followed by IFE and EFE matrices to formulate improvement strategies. Data validity was ensured through source and method triangulation. The findings indicated that medical equipment control had not been optimally implemented due to the absence of standard operating procedures, inconsistent documentation, unequal distribution of equipment, and the lack of routine evaluation. Analysis of external factors revealed significant opportunities, including government regulations, accreditation requirements, technological advances, and increasing public expectations for fast and accurate services. A standardized, user-friendly, and integrated medical equipment handover system is required to improve efficiency, accuracy, and the overall quality of nursing services.

Abstract

Obesity in nurses is a growing occupational health problem and has implications for the quality of service and patient safety. Organizational support is seen as an important resource within the nursing management framework that can shape a health-promoting work environment and lower the risk of obesity. To analyze the relationship between organizational support and the incidence of obesity in nurses in hospitals. A quantitative study with a cross-sectional design was conducted on 227 nurses in a teaching hospital by 2025 nurses were selected as the sample using a purposive sampling technique. Obesity status is determined based on Body Mass Index (BMI). Organizational support was measured using a structured questionnaire that has been tested for validity and reliability. The analysis was carried out in univariate, bivariate, and multivariate logistic regression with a significance level of p < 0.005. The prevalence of obesity in nurses was 57.3%. Bivariate analysis showed that organizational support was significantly related to obesity incidence (p < 0.005). In a multivariate analysis, nurses with low organizational support were 5.582 times more likely to be obese than nurses with good organizational support (OR = 5.582; 95% CI: 2.064–15.094; p = 0.001), after controlling for confounding variables. Organizational support is a strong independent determinant of obesity in nurses.

Abstract

Napping is an evidence-based method used to reduce fatigue and increase alertness during night shift work. Objective: This study aims to identify the relationship between napping in night shift nurses and the work safety of nurses in hospitals. Method of This research uses a quantitative research design using a cross sectional method. Data was collected from 213 nurses using the Napping Nurse Questionnaire and the Occupational Safety questionnaire. Data collection was carried out from June to December 2024. Data analysis included univariate analysis in the form of a frequency distribution table, bivariate analysis using the chi-square test, and multivariate analysis using the multiple logistic regression test (p>0.05). Results There is a significant relationship between the implementation of night shift nurse napping and work safety at Khidmat Sehat Afiat Regional Hospital, Depok City and Graha Permata Ibu Hospital (p=0.001). Nurses with good napping will have the opportunity to have good work safety. The implementation of napping is the factor that most influences nurses' work safety.
