Abstract

This study aims to analyze the enforcement of nursing diagnoses according to the Nursing Care Standards of X Hospital Jakarta. The research method uses a case study approach with interviews, document review and observation, and performs problem analysis using fishbone analysis. The results showed that the enforcement of nursing diagnoses in accordance with the Standards of Nursing Care was not optimal, so that it had an impact on the quality of care and its documentation. Nursing diagnoses that are in accordance with Nursing Care Standards are 48.6%. Lack of supervision from managers, assessments using personal assumptions and the standard of writing diagnoses in the local Hospital Information System (SIRS) has not been facilitated. Through strengthening the directing function with SAK media innovation, there was an increase in writing nursing diagnoses according to SAK to 62.3%. In conclusion, optimizing the enforcement of nursing diagnoses in accordance with Nursing Care Standards can be implemented with SAK media innovations (Softfiles on the Google Form Platform and flipcharts) which are given to implementing nurses at X Hospital. Abstract

This research aims to identify the optimization of the implementation of tiered supervision for the completeness of nursing documentation in inpatient rooms. The method used is a case study approach with interviews, document review, observation, and problem analysis using fishbone analysis. The research results show that all nursing managers can optimize the implementation of tiered supervision for the completeness of nursing documentation. Nursing managers can carry out routine maintenance of staff under them regarding the suitability of the diagnoses raised with subjective and objective data collected from the results of assessments carried out by nurses, proven by trials of implementing tiered supervision in three inpatient rooms with observational evaluations, and data obtained that there is an increase in the percentage of conformity of diagnoses. First nursing with subjective and objective data from 43.4% to 82.6%, and second diagnosis from 52.1% to 95.65%. The room is in the refreezing change stage where, after implementation or trial, the Nursing Services Department and the head of the room, along with the PPJA and PA, are trying to maintain its continuity and become an organizational culture in the hospital. The conclusion of the activities carried out during the socialization and mentoring process has provided changes in behavior and mindset for nurses in implementing tiered supervision for completeness of nursing documentation on a routine and scheduled schedule. Abstract

Berpikir kritis merupakan komponen penting bagi perawat dalam proses pemecahan masalah dan pengambilan keputusan. Perawat pendidik memegang peran penting dalam pengembangan keterampilan berpikir kritis perawat sehingga menghasilkan praktik yang aman, berbasis bukti dan optimal. Studi ini bertujuan untuk menginisiasi pengembangan perawat pendidik melalui suatu panduan yang memberi arah dan strategi pembelajaran dalam pencapaian kompetensi perawat. Metode yang digunakan adalah case report dengan pendekatan action research. Temuan menunjukkan bahwa peran clinical care manager sebagai perawat pendidik, mampu memahami kebutuhan pengetahuan, keterampilan dan sikap yang diperlukan oleh perawat, mampu merancang pembelajaran dan menerapkan temuan penelitian untuk meningkatkan asuhan keperawatan. 80% perawat merasa nyaman dengan budaya belajar, lingkungan kerja yang kondusif untuk belajar serta tingginya motivasi belajar perawat. 84% perawat menyatakan puas terhadap metode pembelajaran klinik yang diberikan oleh perawat pendidik seperti ronde asuhan keperawatan, laporan kasus, journal reading, bed side teaching, one day one clinical question, preceptorship-mentorship-coaching yang dilakukan secara rutin minimal satu bulan sekali. Namun saat ini belum ada panduan pembelajaran klinik keperawatan sebagai acuan bagi perawat pendidik untuk dikembangkan secara optimal. Dukungan dari bidang keperawatan, komite keperawatan maupun manajemen rumah sakit diperlukan untuk merancang panduan pembelajaran klinik keperawatan untuk mengembangkan sepenuhnya peran perawat pendidik dalam pendidikan keperawatan di rumah sakit.

Abstract

Latar Belakang: Asuhan keperawatan merupakan salah satu indikator dalam menentukan kualitas dalam pelayanan. Metode asuhan keperawatan primer merupakan asuhan profesional yang komprehensif, dalam praktek keperawatan di rumah sakit pelaksanaan metode keperawatan primer belum optimal. Tujuan: laporan kasus bertujuan merancang pelaksanaan perubahan berencana dalam inisiasi pengembangan metode asuhan keperawatan primer di ruang rawat inap. Metode: dengan menggunakan Case Report, pengumpulan data melalui kuesioner, wawancara, dan observasi. kemudian dilakukan analisis melalui pendekatan strength, weakness, opportunity, threats (SWOT) dan Fishbone, selanjutkan akan dirumuskan Plan of Action (POA) sehingga dapat di implementasi dan evaluasi Hasil: Berdasarkan  hasil pengumpulan data dapat diperoleh bahwa belum optimalnya pelaksanaan metode Asuhan Keperawatan Primer di ruang rawat inap, sehingga dalam fungsi perencanaan adanya pembuatan panduan metode asuhan keperawatan primer yang akan menggunakan pendekatan perubahan berencana teori Lewin’s three step model. Rekomendasi: Pendekatan perubahan berencana Lewin’s three step model dapat digunakan serta dilanjutkan oleh rawat inap rumah sakit dengan pengembangan metode asuhan keperawatan primer.

Abstract

This study aims to optimize the communication of the SBAR method in nurse handover that encourages a culture of patient safety and effective communication. The method used is a pilot study starting from the process of identification, analysis, prioritization of problems, plan of action, implementation, evaluation, and follow-up plan. The results showed that the application of guidelines and Standard Operating Procedures (SPO) with socialization and handover assistance with SBAR method communication had a good effect, as evidenced by the trial application of the guidelines obtained data there was an increase in the percentage of vital sign mentions of 100%, the results of the latest nursing assessment increased to 40%, the delivery of nursing diagnoses and recommendations for further nursing actions by 60%. The hospital organization is in a stage of movement change where after implementation, the Nursing Services Department and the head of the room try to maintain its sustainability and make the organizational culture in the hospital. In conclusion, the activities that have been carried out during the socialization and mentoring process provide a change in behavior and a good mindset for nurses in implementing and cultivating the implementation of handover with SBAR communication in order to realize quality nursing services. Abstract

This research aims to identify the relationship between the application of nursing ethical principles and nurse performance. The method used was quantitative with a cross-sectional approach to 129 nurses on duty in the Inpatient Room at RSAL Dr. Mintohardjo. Data analysis was carried out using a linear regression test. The research results show that factors related to performance include rank or class (p=0.037), respect for others (r=0.621), compassion (r=0.594), advocacy (r=0.610), intimacy (r=0.608). The regression test shows that 7 variables have a joint influence with the equation Y: 31.864+1.127*Respect to Others + 1.689*Education + 1.179 Length of Work - 1.919 *class rank + 0.755 Intimacy – 1.975*career level + 1.054 Advocacy. Respect to others partially has a positive effect on nurses' performance in nursing care (t=2.433, p=0.016), while the other variables are confounding variables (p>0.05, t<1.98). Variables namely education, rank/class, length of service, career level, advocacy, respect for others and intimacy influence nurse performance with a determined influence of 39.3%. Conclusions from the 4 sub-variables of ethical implementation where these four variables have a significant relationship independently to nurse performance. Abstract

ABSTRAKPrevalensi pressure ulcer yang tinggi menimbulkan kerugian bagi banyak pihak dalam pelayanan perawatan akut dan jangka panjang. Tujuan penelitian: Penelitian ini bertujuan mengidentifikasi hubungan sikap dan pengetahuan dengan perilaku pencegahan pressure ulcer pada mahasiswa profesi keperawatan. Metode: Penelitian ini berdesain cross sectional, melibatkan 227 sampel dengan teknik consecutive sampling.  Penelitian menggunakan The Pressure Ulcer Knowledge Assessment Tool (PUKAT) 2.0, Attitude towards Pressure Ulcer Prevention (APuP), dan kuesioner perilaku yang valid dan reliabel. Analisis univariat menggunakan distribusi frekuensi menggambarkan karakteristik responden, pengetahuan, sikap, dan perilaku. Analisis bivariat dengan uji Fisher exact dan chi square. Hasil: Hasil penelitian menunjukkan bahwa sebagian besar mahasiswa profesi memiliki pengetahuan masih kurang (53,7%), memiliki sikap yang baik (59%), dan sebagian besar memiliki perilaku melakukan tindakan pencegahan pressure ulcer (55,9%). Tidak ada hubungan signifikan antara pengetahuan dengan perilaku (p value = 0,071; α = 0,05), tetapi ada hubungan signifikan antara sikap dengan perilaku (p value= 0,010; α = 0,05). Diskusi: Sikap yang baik dipandang dapat menghindari terjadinya pressure ulcer. Pengetahuan tidak berpengaruh terhadap perilaku yang menandakan pengetahuan masih bersifat recall memory tentang materi yang dipelajari ketika tahap akademik.  Kesimpulan: Penelitian ini merekomendasikan agar institusi pendidikan merekonstruksi kurikulum, ujian teori preklinik, dan evaluasi berkala yang dilakukan pada tahap profesi.Kata Kunci: pengetahuan, perilaku pencegahan, pressure ulcer, sikap  Effects of Attitude and Knowledge on Pressure Ulcer Prevention Behavior ABSTRACTThe significant prevalence of pressure ulcers imposes detrimental consequences on various stakeholders within acute and long-term care services. Objective: This study aims to identify the correlation between attitude and knowledge with regard to pressure ulcer prevention behavior among nursing profession students. Methods: The research design employed in this study was cross-sectional, involving a sample size of 227 participants selected through the consecutive sampling technique. The data collection instruments included The Pressure Ulcer Knowledge Assessment Tool (PUKAT) 2.0, Attitude towards Pressure Ulcer Prevention (APuP) questionnaire, and validated and reliable behavior questionnaires. The univariate analysis entailed the use of frequency distribution to delineate the respondents' characteristics, knowledge levels, attitudes, and behaviors. The bivariate analysis consisted of Fisher's Exact test and Chi-square test. Results: The findings indicated that a majority of nursing profession students possess inadequate knowledge (53.7%), demonstrate positive attitudes (59%), and engage in pressure ulcer prevention behaviors to a considerable extent (55.9%). Notably, no statistically significant correlation was observed between knowledge and behavior (p-value=0.071; α=0.05); however, a significant correlation was found between attitude and behavior (p-value=0.010; α=0.05).  Discussion: This study highlights the pivotal role of positive attitudes in preventing pressure ulcers. Moreover, it is suggested that knowledge has limited influence on behavior, as it primarily relies on the recollection of information acquired during the academic phase. Conclusion: This study recommends educational institutions to reconstruct their curriculum, refine pre-clinical theory examinations, and implement regular evaluations during the professional stage.Keywords: knowledge, prevention behavior, pressure ulcer, attitude

Abstract

This study aims to identify the factors that influence interest in becoming an intensive nurse. The design of this study used a cross-sectional approach. The results showed self-efficacy, appreciation, work unit, age, family support and supervisor support (p=<0.001), years of service (p=0.005), gender (p=0.002), career level (p=0.004), and experience. Training (p=0.003) affected the interest of intensive nurses. On the other hand, knowledge and level of education were not influenced by the pull of intensive nurses. The results of multiple linear regression analysis found that the most influential interest factors in becoming an intensive nurse were self-efficacy, followed by rewards, work units, and gender. These four factors contributed 57.4% in influencing the interest in becoming an intensive nurse. In conclusion, what most determines interest in becoming an intensive nurse is self-efficacy. Abstract

This study aims to analyze career paths and interest in being an intensive nurse and develop problem-solving solutions by conducting interactive discussion activities to disseminate intensive unit services to attract interest in becoming an intensive nurse. The method used is a case study and analysis of results and implementation gaps with a discussion based on the literature review. The results of the career path analysis research, respondents have a D3 nursing education level (90%), 5-8 years of service (87.5%), occupy the last PK level 1-5 years (93.4%) and attend BHD, PPI training, PCNT, BHL/PALS, and basic KMB training (53.3%). Interest in being an intensive nurse (63.3%). Interest in carrying out their respective roles as caregivers, educators, and researchers (60%). In conclusion, some nurses are interested in becoming intensive nurses with the appropriate level of education, tenure, and career level and have attended basic training to become intensive nurses. However, when internal recruitment is carried out to become intensive nurses, few nurses are willing to become intensive nurses, so that still needs proper implementation. Abstract

This study aims to determine the effect of Case Reflection Discussion (DRK) on improving nurses' ability to carry out nursing documentation according to 3S standards (IDHS, SIKI, SLKI) Hospital X Jakarta. The method used in this research is using a pilot project starting from problem identification, problem analysis and problem priority setting, preparation of a Plan of Action (POA), implementation, and evaluation. The results obtained are that there is an increase in the ability of nurses in preparing nursing diagnoses according to the IDHS standards by 40% after the Case Reflection Discussion (DRK) and an increase in the knowledge of nurses in preparing interventions and outcomes according to the SIKI and SLKI standards by 24% after carrying out the Case Reflection Discussion ( DRK). In conclusion, Case Reflection Discussion (DRK) is very efficient in improving the ability of nurses to carry out nursing documentation according to 3S standards (IDHS, SLKI, SIKI).
