Abstract

At this time food safety problems often occur in the management of the production process from internal and external factors, this occurs because the quality of the production is not good enough to affect the quality of the product. Aims to improve quality in the production process of Kremes Noodles to determine critical to quality, determine the quality level of noodle products, and develop strategies to improve the quality of Kremes Noodle products. So by using the six sigma method which has the steps of Defining, Measuring, Analyzing, Improving, and Controlling. To strengthen the strategy in the improve stage, this is done using a SWOT analysis. In this study, there are 5 types of errors that have an impact on product quality and one of the strategies used is to carry out inspections before the process starts and monitoring during the process.

Abstract

UD. Tiga Putra is an industry engaged in cracker processing. There are product defects in the UD cracker production process. Tiga Putra so that it has an impact on the order rate from 50% to 30%. The purpose of the study is to determine the level of product defects with the highest risk level of failure that occurs and an improvement is made. The method used is the six sigma method and FMEA. The results of this study indicate an average DPMO value of 10848.08, a sigma value of 3.80 with the highest defect value of 41.1% in the type of crumbling defect. To get the best quality results, improvements can be made by performing maintenance and supervision on every aspect or factor that allows failure. Improvements can minimize defects and can increase company productivity so that it can be said that the company has achieved the desired target. Highlights : Utilizing Six Sigma Method and FMEA: Implementing these methodologies ensures a systematic approach to identifying and addressing product defects. Focus on High-Risk Areas: Prioritize improvement efforts based on the identified highest-risk failure types, such as crumbling defects, to maximize quality enhancement. Continuous Improvement Culture: By integrating maintenance and supervision across all factors contributing to failures, the company fosters a culture of ongoing improvement, ultimately enhancing productivity and achieving set targets. Keywords: Six Sigma, FMEA, Product Defects, Improvement, Quality Results

Abstract

PT. Bumi Lestari Karya Perkasa experiences 25 workplace accidents in a year. The purpose of this study is to estimate the amount of potential work accident risks in the field of roll forming machines and to control risks with appropriate Personal Protective Equipment (PPE) to reduce occupational risks. The Hazard Identification Risk Assessment and Risk Control (HIRARC) method was used in this study. This research is a qualitative research. Field observations, literature study of company documents, and in-depth interviews with the SMK3 division at PT. Bumi Lestari Karya Perkasa is used to collect data. The risk score is calculated first in the analysis. This research resulted in identification of companies in an effort to improve the safety of all work in the roll forming machine area to avoid unsafe actions and unsafe conditions, as well as the use of Personal Protective Equipment (PPE) by adjusting the type of work being carried out and the lack of awareness about the importance of tools. Personal Protection (PPE). Highlights : Quantifying Risk: Using HIRARC method to assess and quantify potential work accident risks provides a structured approach to understanding and addressing safety concerns. Qualitative Research Approach: Employing field observations, literature study, and in-depth interviews allows for a comprehensive understanding of the factors contributing to workplace accidents. Emphasis on Prevention: Identifying unsafe actions and conditions underscores the importance of proactive measures, such as proper PPE usage and raising awareness among employees, to mitigate occupational risks. Abstract

General Background: Product quality control is essential to ensure consistent quality from the beginning to the end of the production process. Specific Background: PT. FGH faces significant quality challenges, specifically during the health plaster coating stage, which has resulted in numerous product defects. Knowledge Gap: Although previous research has applied Six Sigma for quality improvement, detailed studies on health plaster manufacturing using combined Six Sigma and 5S Kaizen methodologies remain limited. Aims: This study aims to assess the product defect rate at PT. FGH using the Six Sigma methodology, measure defect levels via the sigma level through Defect Per Million Opportunities (DPMO) conversion, and propose improvement strategies through 5S Kaizen. Results: Findings reveal a DPMO of 7,039.4959, with a sigma level of 3.96, indicating that while the process capability values for Cp and Cpk are 1.00, suggesting production aligns with specification limits, tighter monitoring is required. Notably, product defects are predominantly folding, perforated edges, and asymmetry, comprising 80% of defects and leading to financial losses of IDR 340,524,744 over a one-year period. Novelty: This study uniquely combines Six Sigma and 5S Kaizen in addressing specific defect types and sources, including mechanical and human factors, within the health plaster industry. Implications: By implementing targeted process improvements, this research contributes a practical framework for defect reduction and cost mitigation in production, supporting broader quality control efforts in similar manufacturing sectors.

Abstract

General Background: Ensuring consistent product quality is crucial for maintaining competitive advantage, especially in manufacturing. PT. Starr Panel Industry, a company in the refrigeration sector, focuses on high-quality sandwich panels but experiences frequent defects in its production processes. Specific Background: Despite prioritizing premium raw materials, the company faces recurring product failures, particularly in the finishing phase, which affect production flow and on-time delivery. Knowledge Gap: Current quality assurance measures at PT. Starr Panel Industry lack a systematic approach to identifying and prioritizing the root causes of defects, hindering the development of targeted improvement strategies. Aims: This study aims to identify, analyze, and address potential failure modes and their root causes within the production processes, employing Failure Modes and Effects Analysis (FMEA) and Fault Tree Analysis (FTA). Results: The findings reveal that the finishing process has the highest Risk Priority Number (RPN) of 112, primarily due to insufficient adherence to standard operating procedures (SOPs) by the workforce. Novelty: The integration of FMEA and FTA provides a comprehensive failure analysis model, with FMEA ranking failure risks and FTA tracing them to foundational causes, offering a new dual-method approach to improve operational standards effectively. Implications: Implementing the proposed FMEA-FTA framework can significantly reduce product defects, improve quality consistency, and minimize delivery disruptions, especially in the critical finishing stage. Emphasizing SOP compliance across divisions, particularly in finishing, can lead to sustained quality improvements and enhanced operational efficiency in sandwich panel production.
