Abstract

Tuberculosis (TB) disease intervention is a national program and being a target of MDGs. Therefore, a government determined minimum service standards of a hospital that had to be implemented in all health service units and hospitals in Indonesia. A strategy of DOTS at Aisyiyah Hospital in Muntilan had been available particularly in terms of case finding. Notwithstanding, a process of patient diagnosis had not been implemented in accordance with a standard of human resource. Number of medical officers at a TB DOTS unit was not sufficient. In addition, quality of information like completeness, accurateness in diagnosis, and timeliness in reporting TB cases had not achieved a target. The aim of this study was to develop decision support system of TB as an effort to systematically diagnose and manage data of TB based on the standard. This was qualitative-quantitative research. A qualitative method was used to identify each step of information system development. Meanwhile, a quantitative method was used to assess quality of information between before and after developing information system using pre-experimental design (the one group pre and posttest design). Qualitative data were collected by conducting indepth interview, whereas quantitative data were collected using checklist. Data were analysed using T test. Research object was decision support system before and after applying at TB unit at Aisyiyah Hospital in Muntilan. Research subjects consisted of four TB officers as main informants. Informants for triangulation purpose consisted of head of SIMRS and head of medical services. The results of T test showed that variables of completeness, accurateness, and preciseness after developing the decision support system were better than that of before developing the system (p-value = 0.000). As suggestions, head of SIMRS needs to conduct further development of decision support system of TB in another unit particularly in inpatient unit. The decision support system needs to be evaluated and adjusted with the standard.

Abstract

   Medical Record Information System Service at Prof. Dr. V.L. Ratumbuysang Mental Hospital North Sulawesi Province had not run as expected. Indicated by the differences in patient data in the recording and the medical records. Factors that allegedly affect are human, organization and technology. This study aims to evaluate medical record service information system with the HOT FIT Model approach and its benefits.     This was a qualitative study. Data collected through in depth interviews and analyzed by content analysis method. Research subjects were 1 admin officer and 3 archival officers as main informants and the chief of medical record service, the head medical services division and an assistant director of medical services and nursing as the triangulation informants.    The results showed that the medical record service information system had facilitated the work but the knowledge and skills of the officers were still lacking. Organizational factors were supported but the budget was still lacking in system development. Technological factors had not run optimally because the LAN network did not work.  The quality of the information had not been met because the data of the visits should be checked again before used for decision making. Quality of service was not maximal yet, because the connectivity between medical record and polyclinic was impaired, then the officer used manual form.     It can be concluded that the hospital need to increase the staff knowledge and allocate funds for improvement and development of information systems or cooperate with other parties in system maintenance.

Abstract

Supervisi fasilitatif bidan koordinator (bikor) puskesmas terhadap bidan praktek mandiri (BPM)merupakan pembinaan klinis dan manajemen yang dilakukan secara berkesinambungan sertatepat sasaran. Pelaksanaan supervisi fasilitatif yang dilakukan selama ini belum optimal,sehingga perlu upaya pengkajian lebih lanjut. Penelitian ini bertujuan untuk menganalisispelaksanaan supervisi fasilitatif bidan koordinator puskesmas terhadap BPM terkait pelayanankehamilan dan persalinan di Kota Semarang. Penelitian ini menggunakan pendekatan kualitatif.Analisis data menggunakan metode analisis isi (content analysis). Variabel dalam penelitian inimeliputi aspek yang dibina (aspek klinis, non klinis, logistik dan manajemen), cara penilaian dan langkah pelaksanaan. Hasil penelitian menunjukkan bahwa supervisi meliputi semua aspek yang dibina. Belum semua bikor menggunakan daftar tilik asuhan persalinan. Supervisi aspek klinis tidak dilakukan dengan pengamatan langsung. Langkah pelaksanaan pra supervisi : belum ada pelatihan supervisi fasilitatif bagi bikor puskesmas dan buku acuan supervisi fasilitatif tidak diberikan. Langkah pelaksanaan supervisi belum sesuai acuan, karena sifatnya inspeksi mendadak. Disarankan kepada Dinas Kesehatan untuk mengadakan pelatihan supervisi fasilitatif bagi semua bidan koordinator puskesmas. Bagi puskesmas membuat kesepakatan dengan BPM terkait jadual supervisi fasilitatif.Facilitative supervision of primary healthcare center (puskesmas) coordinator midwife (bikor) to self-employed practice midwives (BPM) constituted an ongoing and on-target clinical andmanagerial supervision. Current implementation of facilitative supervision was not optimum;therefore, further study was required. Objective of this study was to analyze the implementation of facilitative supervision of primary healthcare coordinator midwives to BPM concerning pregnancy and delivery services in Semarang city. A qualitative approach was applied in this study. Data were analyzed using content analysis method. Study variables were supervised aspects (clinical, non-clinical, logistic, and managerial aspects), assessment method and implementation stages. Results of the study showed that supervision had included all supervised aspects. Not all bikor used delivery care list. Supervision on clinical aspect did not include direct observation. Pre supervision implementation steps: facilitative supervision training for bikor of the primary healthcare centers had not been done, reference book for facilitative supervision had not been distributed. Steps of the implementation of supervision were not performed according to the guidelines. This was caused by on the spot implementation of supervision.Suggestion for District Health Office is to conduct facilitative supervision for all primary healthcare centers coordinator midwives. Suggestion for primary healthcare center is to make agreement with BPM concerning facilitative supervision schedule

Abstract

Indicator of posyandu work performance in West Aceh was still far below the target determined and there was a decrease during 2011-2012. The results of the preliminary study showed that the motivation of village midwives were still low in posyandu cadre coaching. The aim of this research was to identify the factors associated village midwives motivation in posyandu cadre coaching. The study was conducted quantitavely and qualitatively. The quantitative research used survey method with a cross sectional design. The subjects were 146 village midwives selected by clustered random sampling at public healthcare center level. Data colection was conducted by interviews using structured questionnaire. The bivariate analysis used Chi Square tests, and the multivariate analysis used logistic regression. The qualitative data was collected by in-depth interviews to the village midwives and analyzed by content analysis. The results showed that 64.4% of the respondents had low performance, 54.8% had less responsibility, 62.3% had insufficient potential development, 57.5% felt inadequate compensation, and 52.7% had less supervision. There were associations between responsibility (C= 0.237, p=0.006), potential development (C= 0.268, p=0.001), compensation (C=0.518, p=0.0001), working condition (C=0.237, p=0.006), supervision (C=0.261, p=0.002,) and the village midwife performance in posyandu cadres’s coaching. Compensation had the most significant effect on the village midwive motivation in posyandu cadres coaching (Exp-B=17.383, p=0.0001). The obstacles experienced by the village midwives in posyandu cadres coaching were insufficient fund, unavailable special training for the posyandu cadre coaching and unavailable health facilities. Compensation has the most significant effect on village midwive motivation in posyandu cadres coaching. It is suggested to public healthcare centers and Health office to implement refreshing cadre coaching and improving village midwives performance in by giving appreciation to village midwives who conducted posyandu cadre.

Abstract

Pada tahun 2011-2013 terjadi penurunan cakupan K4 di Kabupaten Sumba Timur dan hanya mencapai 44,7%, jauh dari target 85%. Hasil studi pendahuluan menunjukkan bahwa ibu hamil tidak mengetahui manfaat pelayanan K4 dan mempunyai kesulitan dalam akses pelayanan kesehatan. Penelitian ini bertujuan untuk menganalisis faktor-faktor yang berhubungan dengan pemanfaatan pelayanan K4 oleh ibu hamil. Penelitianobservasional analitik ini menggunakan metode survey dengan desain cross sectional. Subjek penelitian adalah 96 ibu hamil yang dipilih secara multistage random sampling pada tingkat Puskesmas dan Desa. Pengambilan data dilakukan dengan wawancara menggunakan kuesioner terstruktur. Analisis bivariat dengan uji Chi Square dan multivariat dengan regresi logistik. Hasil penelitian menunjukkan 71,8% responden tidak memanfaatkan pelayanan K4, 66,7% pengetahuan kurang, 66,7% persepsi kurang terhadap pelayanan bidan, 52,1% tersedia sarana prasarana, 75,0% sulit untuk mencapai Puskesmas, 56,3% tidak tersedia biaya, 65,6% persepsi terhadap kebutuhan pelayanan K4 masih rendah. Ada hubungan positif antara pengetahuan (p=0,004), persepsi terhadap pelayanan bidan (p=0,004), sarana prasarana (p=0,444), kemudahan mencapai Puskesmas (p=0,001), ketersediaan biaya (p=0,006), persepsi terhadap kebutuhan (p=0,001) dengan pemanfaatan pelayanan K4. Tidak ada hubungan antara ketersediaan sarana prasarana Puskemas dengan pemanfaatan pelayanan K4. Faktor-faktor yang berpengaruh bersama-sama terhadap pemanfaatan pelayanan K4 adalah kemudahan mencapai Puskesmas (Exp-B=4,728, p=0,001) dan persepsi terhadap kebutuhan (Exp-B=4,177 p=0,001). Akses dan persepsi kebutuhan merupakan dua faktor utama dalam pemanfaatan pelayanan K4 di Kabupaten Sumba Timur. Disarankan Dinas Kesehatan untuk membangun Poskesdes di setiap Desa, agar pelayanan K4 lebih dapat dijangkau oleh ibu hamil.

Abstract

Angka Kematian Bayi di Kabupaten Klaten pada tahun 2011 sebesar 13,346/ 1000 kelahiran hidup dan tahun 2012 AKB sebesar 10,36 per 1000 kelahiran hidup. Kematian bayi tersebut terjadi pada bayi umur 0 – 28 hari atau periode neonatal. Cakupan pelayanan kunjungan neonatus oleh bidan di Kabupaten Klaten dari tahun 2009 sampai dengan 2012 mengalami menurunan dari kunjungan murni dan kunjungan lengkap. Dalam pelayanan neonatus bidan desa hampir tidak semua melalukan sesuai dengan standar pelayanan minimal. Tujuan penelitian ini adalah untuk mengetahui hubungan karakteristik bidan desa dengan pelayanan neonatus di wilayah Puskesmas Kabupaten Klaten tahun 2013. Metodologi penelitian observasional survey ini dilaksanakan dengan pendekatan cross sectional. Sampel dalam penelitian ini adalah 82 bidan desa di Kabupaten Klaten yang diambil secara Cluster Random Sampling. Instrumen penelitian menggunakan kuesioner dan lembar observasi. Data primer maupun sekunder diolah dan dianalisis secara univariat dengan distribusi frekuensi dan persentase, bivariat dengan Korelasi Chi Square. Hasil penelitian menunjukkan bahwa hubungan karakteristik dengan perilaku bidan desa dalam pelayanan neonatus di Kabupaten Klaten adalah ada hubungan karakteristik dengan pengetahuan yaitu: umur (p = 0.002), pendidikan (p = 0.000), pelatihan (p = 0,011) dan lama kerja (p = 0,000). Ada hubungan karakteristik dengan sikap yaitu: umur (p = 0,005), pendidikan (p = 0,000), pelatihan (p = 0,003), lama kerja (p = 0,000) dan pengetahuan (p = 0,000). Tidak ada hubungan karakteristik dengan pelayanan neonatus yaitu: umur (p = 0.354), pendidikan (p = 0,751), pelatihan (p = 0.336) dan lama kerja (p = 0.950), pengetahuan (p = 0,739) dan sikap ( p = 0,896). Disarankan bagi Dinas Kesehatan Klaten untuk meningkatkan pengetahuan Bidan Desa terutama tentang pelayanan neonatus melalui pelatihan dan sosialisasi buku KIA serta MTBM bagi semua Bidan Desa. Melakukan supervisi ke bidan desa dalam pelayanan KIA.  Infant Mortality Rate (IMR) in District of Klaten in 2011 and in 2012 respectively was 13.346/1,000 live births and 10.36/1,000 live births. The aged of infant mortalities ranged from 0 to 28 days or during a neonatal period. The coverage of neonatal services provided by village midwives in District of Klaten from year of 2009 to year of 2012 decreased in terms of pure and complete visits. Almost all village midwives did not provide the neonatal services which were in accordance with minimum services standards. This research aimed to find out about the relationship between characteristics of village midwives and neonatal services at health centers in District of Klaten in 2013. This was observational-survey research using cross-sectional approach. Number of samples was 82 village midwives in District of Klaten selected by a technique of Cluster Random Sampling. Research instruments used a questionnaire and an observation sheet. Primary and secondary data were analyzed using methods of univariate analysis (frequency distribution and percentage) and bivariate analysis (chi-square test). The result of this research revealed that variables of age (p=0.002), education (p=0.000), training (p=0.011), and work period (p=0.000) significantly related to a variable of knowledge. Furthermore, variables of age (p=0.005), education (p=0.000), training (p=0.003), work period (p=0.000), and knowledge (p=0.000) significantly related to a variable of attitude. On the other hand, variables of age (p=0.354), education (p=0.751), training (p=0.336), work period (p=0.950), knowledge (p=0.739), and attitude (p=0.896) did not relate to the neonatal services. As a suggestion, Klaten District Health Office needs to improve the knowledge of village midwives especially about neonatal services by conducting training and socialization of a Maternal and Child Health (MCH) book and Integrated Management of Young Infants for them. In addition, supervision needs to be done to village midwives in MCH services.

Abstract

ABSTRAKSelama 4 tahun terakhir, di Rumah Sakit Islam Nahdlatul Ulama Demak (RSI NU Demak) telah terjadi penurunan BOR dari 71% pada tahun 2009 menjadi 49% pada tahun 2012. Penurunan terjadi akibat rujukan ke RS lain (76,5% pada tahun 2012) karena tidak tersedianya ICU. Saat ini di Kabupaten Demak baru tersedia 10 TT ICU dari kebutuhan ideal minimal 22 TT ICU. Tujuan penelitian ini adalah melakukan studi kelayakan pendiran ICU RSI NU Demak ditinjau dari aspek manajemen SDM dan aspek pasar. Metode kualitatif dengan indepth interview digunakan untuk menjelaskan aspek manajemen SDM dan aspek pasar terkait rujukan. Informan utama aspek manajemen SDM adalah Ketua Yayasan, manajemen RS dan informan triangulasi dokter spesialis anasthesi dan dokter spesialis penyakit dalam.Informan utama aspek pasar terkait rujukan medis adalah 10 dokter umum yang berpraktek di sekitar RSI NU Demak . Metode kuantitatif melalui wawancara kepada 30 pasien dan atau keluarga di Poliklinik Penyakit Dalam dan Bedah RSI NU Demak dengan kuesioner terstruktur digunakan untuk menghitung ATP dan WTP. Studi banding dilakukan di RSUD Sunan Kalijaga dan RS Pelita Anugerah Demak. Pengolahan data kualitatif dilakukan dengan analsis isi (content analysis) dan kuantitatif secara deskriptif. Hasil penelitian aspek manajemen SDM menunjukkan jenis tenaga kesehatan sudah memenuhi, jumlahnya memerlukan penambahan 13 perawat. Semua tenaga kesehatan belum memiliki sertifikat pelatihan bantuan hidup lanjut dan sertifikat ICU. Pada aspek pasar, pendirian ICU mendapat dukungan dari dokter di sekitar RSI NU Demak dalam hal rujukan medis. Hasil ATP 1(kemampuan membayar terendah) sebesar Rp. 93.225,00 dan ATP 2 (kemampuan membayar tertinggi) sebesar Rp. 723.000,00 dan WTP sebesar Rp. 300.000,00. Disimpulkan bahwa dari aspek manajemen SDM, ICU di RSI NU Demak belum layak didirikan. Dari aspek pasar, ICU di RSI NU Demak layak didirikan.Kata kunci : Studi Kelayakan, ICU, Rumah SakitABSTRACTIn the past four years, bed occupancy ratio (BOR) in Nahdlatul Ulama Islamic hospital (RSI NU) Demak decreased from 71% in 2009 to 49% in 2012. The decrease was because of referral to other hospitals (76.5% in 2012) due to no intensive care unit (ICU) facility. Currently, the availability of number of beds in the ICU in Demak district was 10; the minimal ideal requirement of ICU beds was 22. Objective of this study was to conduct feasibility study for the establishment of an intensive care unit of RSI NU Demak viewed from human resource and market aspects. Qualitative method with in-depth interview data collection technique was used to explain human resource management aspect and market aspect related to referral. Main informants for human resource management aspect were a director of the foundation and management of the hospital. Triangulation informants were anesthesiologists and internists. Main informants for market aspect related to medical referral were 10 general practitioners who conducted their private practice in surrounding RSI NU Demak. Quantitative method was implemented through in-depth interview to 30 patients and/or family member who visited internal disease and surgery polyclinics of RSI NU Demak. Structured questionnaire was used to calculate ATP and WTP. Benchmarking to Sunan Kalijaga Demak district general hospital (RSUD) and Pelita Anugerah hospital Demak wasconducted. Content analysis was applied in the qualitative data analysis; quantitative data was analyzed descriptively. Results of the study showed that viewed from human resource management aspect: health worker type had fulfilled the expectation; however, additional 13 nurses were needed. All health workers did not have training certificate of life saving for elders and ICU certificate. In the market aspect: the development of ICU of RSI NU Demak obtained medical referral supports from physician surrounding RSI NU Demak. ATP1 (the lowest ability to pay the fee) was Rp. 93,225.00, and ATP2 (the highest ability to pay the payment) was Rp. 723, 000,00, WTP was Rp. 300,000.00. In conclusion, viewed from human resource management, ICU of RSI NU Demak is not feasible to be built. Viewed from market aspect, ICU of RSI NU Demak is feasible to be built.Keywords : feasibility study, ICU, hospital

Abstract

Latar Belakang: Upaya meningkatkan cakupan pemberian ASI eksklusif sudah lama dilakukan namun keber-hasilan cakupan program masih rendah. Cakupan ASI eksklusif wilayah DKK Kabupaten Semarang tahun 2011 sebesar 20% masih dibawah target sebesar 80%. Pada tahun 2012 telah terbit Peraturan Pemerintah (PP) nomor 33 tentang Pemberian ASI Eksklusif yang diikuti dengan terbitnya Peraturan Menteri Kesehatan yang mengatur tentang donor ASI, penyediaan Ruang Laktasi di tempat kerja, sarana umum, tata cara penggunaan susu formula, dan tatacara pengenaan sanksi administratif bagi tenaga kesehatan dan penyelenggara fasilitas pelayanan kes-ehatan. Sanksi itu diberikan bertahap dari teguran lisan, tertulis, hingga pencabutan ijin.4 Tujuan: penelitian ini adalah untuk menganalisis system pengelolaan ruang laktasi di perusahaan swasta ditinjau dari aspek input, process dan output diwilayah kerja Dinas Kesehatan Kabupaten Semarang. Metode: Jenis penelitian adalah kualitatif yang disajikan secara deskriptif eksploratif melalui wawancara menda-lam. Pemilihan informan dengan cara purposive, pengambilan data melalui wawancara (indepth interview) pada in-forman utama adalah 5 penanggung jawab ruang laktasi di perusahaan swasta, dan informan triangulasinya yaitu 5 orang HRD perusahaan swasta, 5 orang karyawan bekerja yang menyusui, 1orang dari Ukesja Dinas Kesehatan Kabupaten Semarang. Data dikumpulkan dengan teknik wawancara mendalam. Analisis data menggunakan me-tode analisis isi (content analysis) Hasil: Hasil penelitian dari variabel input yaitu mengenai SDM, sudah memenuhi syarat minimal yaitu pendidikan terakhir Sarjana Kedokteran dan Diploma Keperawatan maupun Kebidanan. Sumber dana utama adalah murni dari masing-masing perusahaan swasta. Sarana prasarana disediakan murni dari perusahaan swasta. Pada pros-es ditemukan belum semua ruang laktasi membuat rencana tahunan. Simpulan: Pengorganisasian ruang laktasi perusahaan swasta belum semua mempunyai struktur organisasi yang jelas. Pelaksanaan kegiatan ruang laktasi belum semuanya berjalan dengan baik. Evaluasi dan supervisi sudah di-laksanakan secara rutin dari instansi terkait. Kegiatan integrasi belum semua ruang laktasi berjalan dengan baik.

Abstract

Salah satu kegiatan mahasiswa untuk dapat menyelesaikan studinya adalah membuat tugas akhir. Pandemi corona virus-19 (COVID-19) telah mengubah banyak kegiatan masyarakat dan munculnya kebijakan yang dilakukan pemerintah di berbagai negara, termasuk di bidang pendidikan baik pembelajaran maupun tugas akhir. Sebanyak 82,4% mahasiswa  menganggap model pembelajaran tersebut lebih menyulitkan dibandingkan dengan pembelajaran seperti biasa. Penelitian ini merupakan jenis penelitian survei, dengan pendekatan deskriptif analitik, dilakukan pada tahun 2020. Sebagai responden adalah   mahasiswa yang sedang mengerjakan tugas akhir sebanyak 242 dipilih menggunakan teknik accidental sampling. Hasil penelitian menunjukkan secara umum terdapat 30,2% menyatakan tidak puas untuk bimbingan tugas akhir dilakukan secara daring. Menggunakan uji Chi Square terdapat hubungan beberapa variable seperti usia (p=0,001 < 0,05), status pernikahan  (p=0,001 < 0,05), aktifitas pekerjaan dengan kepuasan dalam mengikuti bimbingan tugas akhir secara daring dengan kekuatan asosiasi yang lemah. Disarankan perlu adanya strategi khusus dalam bimbingan tugas akhir secara daring sehingga mahasiswa tidak merasa bosan untuk mengikuti  bimbingan

Abstract

Coverage of exclusive breastfeeding in the District of Sewon still low, either because they are less optimal role of KP motivator. They lack of confi dence in dealing with problems in breastfeeding. The research objective was to analyze the effect of the use of a handbook by the motivator of the knowledge and skills motivator in dealing with problems in exclusive breastfeeding in the District of Sewon Bantul. This research is pre experiment with the design of nonequivalent one group pre test and post test design. The population was Motivator KP Capital in District Sewon many as 50 people. Collecting data through questionnaires and observations then performed univariate analysis (descriptive), bivariate (Paired T test). The results of research showed that the respondent’s average age was 46.74. Education respondents mostly high school was 25 people, most of the respondents did not work that 31 people (62%). Motivator knowledge prior to intervention use a pocket book value of the average was 64,5 while the value after the intervention mean value is 72.1. Motivator skills during pretest score was 57.6 at, while the second posttest mean value is 64.65. There was the effect of using a pocket book support group of mothers about breastfeeding, the problem of skills variable (p-value=0.0001) means the use of a paperback book motivators influencing skills in breastfeeding issues. Suggested for Health Department and Community Health Center in order to develop a motivator as refresher training for, to improve the knowledge and skills motivator in breastfeeding issues so as to increase the coverage of exclusive breastfeeding.
