Abstract

Background: The implementation of Indonesia’s National Health Insurance (JKN) has expanded healthcare access. As of December 31, 2024, JKN coverage had reached 98.45%, yet disparities persist across socioeconomic groups and geographic regions. These disparities contribute to unequal service quality, low utilization, and financial burdens among low-income households. Objective: This study identifies determinants of disparities in healthcare utilization in Indonesia using Andersen’s Behavioral Model and provides an evidence base for policymakers. Methods: A systematic review was conducted following PRISMA guidelines. Searches of CrossRef, Scopus, and PubMed (2015–2025) identified studies on healthcare utilization and disparities in Indonesia. Eligible articles were screened in Covidence and thematically synthesized using Andersen’s Behavioral Model to classify determinants into predisposing, enabling, and need factors. Results: Enabling factors, such as socioeconomic status, insurance ownership, and geographic access, were the strongest determinants, compared with predisposing factors (age, sex, marital status, and educational background) and need factors (self-rated health, chronic conditions, perceived healthcare needs). High health needs did not always lead to service use, particularly among low-income and remote populations. Financial protection alone is insufficient without improvements in health literacy, equitable workforce distribution, and stronger primary care. Conclusion: Although JKN has expanded access, disparities remain. An integrated approach is needed to strengthen primary care, ensure equitable health worker distribution, improve health literacy, and enhance geographic access. These actions can shift JKN’s focus from coverage expansion to equitable service access, supporting Indonesia’s progress toward Universal Health Coverage (UHC) and the Sustainable Development Goals (SDGs).

Abstract

The Bed Occupancy Rate (BOR) of inpatient rooms at Mitra Sehat Mandiri Hospital Sidoarjo during 2019-2023 has not reached the ideal standard. The BOR values were 22%, 28%, 37%, 25%, and 20%, with an average of 26.4%, remaining below the ideal range of 60-85%. One of the most influential factors contributing to the low BOR is suboptimal service quality. This study aims to analyze the relationship between Patient-Perceived Hospital Service Quality, customer satisfaction , and behavioral intention among inpatients at Mitra Sehat Mandiri Hospital Sidoarjo. This study employed a quantitative method with an analytical approach and a cross-sectional design. The population included all inpatients, with a sample of 97 respondents determined using the Lemeshow formula. Multiple linear regression was conducted to examine the influence of variables. The results showed that the dimensions of Clinical Care Process, Relationship, and Administrative Procedures contribute significantly to customer satisfaction, with p-values of 0.006, 0.026, and 0.032, respectively. In addition, Trustworthiness, Clinical Care Process, and Administrative Procedures also significantly affected patients’ behavioral intention with a p-value < α (0.05). Furthermore, customer satisfaction has a significant effect on the behavioral intention of inpatients, as evidenced by a p-value < α (0.05). These findings indicate that improving service quality in various crucial aspects can encourage customer satisfaction and strengthen positive behavioral intentions in hospital services. Overall, the study concludes that improvements in key aspects of Patient-Perceived Hospital Service Quality directly enhance customer satisfaction, which in turn strengthens positive behavioral intention at Mitra Sehat Mandiri Hospital Sidoarjo.

Abstract

The main issue addressed in this study is the low level of physical activity and the increasing sedentary lifestyle among workers, which negatively impacts physical fitness and work productivity . This study aims to analyze the relationship between a healthy lifestyle and workers’ physical fitness based on recent scientific evidence. The method used is a literature review with a narrative-descriptive approach conducted through three stages: identification, screening, and article selection. Literature was searched in Scopus, PubMed, and Google Scholar databases, yielding 114 articles, which were reduced to 31 after screening, and finally 10 articles met the inclusion criteria and were analyzed using content analysis. The results show that workers with higher physical activity levels had lower absenteeism (36.3 days/year) compared to less active workers (64.4 days/year), along with increased productivity (r=0.46; p=0.005). Approximately 73.2% of workers adopted personal fitness programs after intervention, and digital-based interventions significantly reduced work-related stress. In conclusion, integrating healthy lifestyle practices with organizational support and digital technology is an effective strategy to improve physical fitness, mental health, and worker productivity sustainably.

Abstract

Background: The form of community empowerment that can support in tackling cases of forest and land fires is the Fire Care Community (MPA), namely communities in areas prone to forest and land fires who voluntarily care about controlling forest and land fires who have been trained or given supplies and can be empowered to assist forest and land fire control activities Objective: To know the general description of the empowerment of the Fire Care Community (MPA) in preparedness efforts in Kereng Bangkirai and Sabaru Villages, Palangka Raya City, Central Kalimantan Methods: This research was qualitative research with a case study design. Data collection techniques used are in-depth interviews and FGDs. This study used a case study approach to explore and obtain in-depth information to explore the perspectives of MPA members and related communities. Results: Kelurahan Kereng Bangkirai and Sabaru in carrying out their roles had met expectations. At the beginning, the performance of MPA Kereng Bangkirai was considered very good compared to MPA Sabaru because members of MPA Kereng Bangkirai were quite vocal and active. Still, over time there were internal problems in MPA Kereng Bangkirai which affected the performance of MPA Kereng Bangkirai team. The situation turned around, now MPA Sabaru was considered to have better performance because of the activeness and cohesiveness of the MPA Sabaru team. Conclusion: The empowerment of MPA in Kereng Bangkirai and Sabaru Villages has been quite good in carrying out their roles, but along the way, both MPA in Kereng Bangkirai and Sabaru Villages have had several obstacles, both internal and external.

Abstract

Background: Surabaya City is one of the cities in East Java Province with a high-risk zone in the spread of Covid-19 cases. One of them is in Tambaksari District, Pacarkeling Village, an area with confirmed cases of Covid-19. It is known that the community in the region has implemented the 5M health protocol, but the discipline for implementing this behavior is still not optimal. Objective: Therefore, the purpose of this study is to improve the knowledge of Capacity Building Cadres through Wani Ngandani socialization activities in Kampung Tangguh Wani Jogo Surabaya, Pacarkeling Village, Tambaksari District, Surabaya. Method: The research method used in this study is quantitative with design action research, namely training activities on knowledge of how to communicate and conduct surveillance on health cadres as problem solving conditions during the Covid-19 pandemic. The location of the activity is in Pacarkeling Village, Tambaksari District, Surabaya. The implementation of offline training by implementing strict 3M health protocols. The data obtained were analyzed descriptively. Results: The results of the study showed that increasing knowledge and skills in communication methods during the socialization of the 5M Health Protocol can increase the knowledge of health protocols for surveillance in 83% of Wani Ngandani cadres. Conclusion: Then it can be concluded that the results of the Wani Ngandani socialization activities in the Wani Jogo Surabaya Resilient Village environment can provide increased knowledge or Capacity Building for Cadres regarding health protocols. Furthermore, continued training is needed to increase the capacity of health cadres and PKK members, so that the knowledge gained is useful for community participation in health programs in the city of Surabaya.
