Abstract

We report our study to determine factors associated with increased nosocomial infections in neonates hospitalized in NJCU, from January until December, 1992. A total number of 116 infants were studied, 68 (58.6%) of them were male, and 101 (87 .1 %) were less than 6 days old. Forty six infants (39.8%) had a birth weight of< 1500 g, 27 (23.3%) between 1500-1999 g, 14 (12.1%) between 2000-2499 g, 27 (23.2%) >2500 g, and 2 (1.7%) were unknown. A total number of64 infants (55.2%) had more than 1 infections, yielding 103 episodes of infection, giving an incidence of 88.8% infection rate. The significant risk factor for nosocomial infections was length of hospital stay. Age, birth weight, gestational age, APGAR scores, use of nasogastric tube or intravenous line, were not statistically significant risk factors. Use of nasogastric tube was associated with increased nosocomial gastroenteritis, compared with use of intravenous lines.

Abstract

Tbe distribution of pathogens causing nosocomial gastroenteritis was E. coli (69.2%), Salmonella sp (23.1%), rotavirns (6.4%) and Shigella sp (1.3%) . Klebsiella pneumoniae seemed to be the pathogen most frequently associated with nosocomial skin infections (26. 7%), followed by Enterobacter aerogenes (20.0%), E. coli, Ps. Aeruginosa and S. aureus, each in 13.3% and others 03.4%). Salmonella sp was the pathogen most frequently associated with nosocomial bacteremia (20.2%), followed by Klebsiella sp 07.9%), S. a/bus 04.3%), Enterobacter sp 01.9%), Pseudomonas sp (9.5%), S. aureus (9.5%) and others (16. 7%). In nosocomial urinary tract infections, the result was E. coli (32.9%), Klebsiella pneumoniae (20.5%), Enterobacter sp 03. 7%), Citrobacter diversus (6.8%) and others (26.1%). In general, E. coli was the most frequently reported pathogens ( 42.0%), followed by Salmonella sp 07.1%), Klebsiella sp (10.2%), Citrobacter sp (7.2%), S. a/bus (4.5%), Pseudomonas sp (4.2%), S. aureus (3.6%) and others 01.2%), respectively.

Abstract

During a 9-month prospective study, from August, 1988 to April, 1989, a total number of 4328 hospitalized pediatric patients at the Department of Child Health, Padjadjaran University, Hasan Sadikin General Hospital Bandung, were observed to identify skin and soft tissue nosocomial infections (not included postoperative), especially catheter related injections. The gastroentestinal tract was the most frequent site of nosocomial infections (44.3%), then subsequently followed by skin infection (22.6%), bacteremia (16.3%) and urinary tract infection (14.1%). The most frequent cause of nosocomial skin infections like phlebitis was IVFD occurring in 82 out of 93 patients (88.2%). The overall phlebitis attact rate was 4.2% . When the duration of infusion is devided into 3 groups of 0-36 hours, 37-72 hours and ≥ 73 hours, then it becomes clear that the longer the duration of infusion, the higher the attack rate (x2=8.07, p<0.05). Klebsiella pneumonia seemed to be the pathogen most frequently associated with nosocomial skin infections (26. 7%), followed by Enterobacter aerogenes (20.0%), and then E. coli, Ps. aeruginosa and S. aureus 13.3% each. It could be concluded that the risk of contracting phlebitis from JVFD with a duration of ≥ 73 hours was. 1.9 times higher than that of less than 72 hours.

Abstract

A 9 month prospective study , from August 1988 to April 1989, was performed at the Department of Child Health, Medical School, Padjadjaran University, Hasan Sadikin Hospital Bandung. The aim of the study was to identify the pattern of sensitivity of bacteria causing nosocomial infections. It was revealed that among 4328 hospitalized pediatric patients, a total number of 342 patients had one or more nosocomial infections, yielding 411 episodes. Twenty-five per cent of isolated S. aureus was resistant to either ampicillin or oxacyllin. Higher percentages of resistance were observed in S. albus, i.e., to penicillin (53.3%) and oxacyllin (46.7%), and to cefuroxime, erythromycin and clindamycin (each in 40.0% of isolate). E. coli was In 86.3% resistant-to ampicillin, 78.4% to chloramphenicol (and thiamphenicol), 56.8% to tobramycin and 48.2% to gentamicin. The isolated Salmonella sp was almost 100% resistant to ampicillin and chloramphenicol (and thiamphenicol) but was highly sensitive to amikacin, cefotaxime and netilmicin, arround 100%. Pseudomonas sp was 92.9% resistant to ampicillin, 85.7% to chloramphenicol (and thiamphenicol) and 78.6% to cefuroxime; on the contrary it was 92.9% sensitive to amikacin. It can be concluded that gram-negative microorganisms were in general showed highly resistant to ampicillin, chloramphenicol (and thiamphenicol), tobramycin, and gentamicin; on the other hand more than 90% were sensitive to netilmicin, cefotaxime and amikacin.

Abstract

A 9 month prospective study on nosocomial infections was held from August 1988 till April 1989 at the Department of Child Health, Medical School, Padjadjaran University, Hasan Sadikin Hospital Bandung. The purpose of the study was to compare the duration of hospitalization in patients with and without nosocomial infections. The Department of Child Health has 4 main wards, A2 (for low sosioeconomic families), A3 (for middle and high socioeconomic families), R-17 (for neonates) anda NICU. There were 4328 hospitalized pediatrica patients observed consisting of 293 children (29 9%) in A2, 485 (11.2%) in A3, 2487 (57.5%) in R-17, and 41 episodes in NICU, showing rates of 9.9% in A2, 4.5% in A3, 8.8% in R-17 and 65.1% in NICU. The means hospital stay of patients with nosocomial infecitons were 26.77 and 22.44 days in A2 and NICU, while in A3 and R-17 17.61 and 6.75 days. The means hospital stay of patients without nosocomial infecitons were 13.11 and 9.24 days in A2 and NICU, and 1 0.48 and 3.10 days In A3 and R-1 7. Length of stay specific nosocomial infeciton rate rose with the duration of hospitalization, with 3 2% of patients staying upt to 6 days, 16.7% of patients staying 7-13 days, 19.7% of patients staying 14 to 20 days, and 48,8% of patients staying 35 days or more. It can be concluded that the longer the length of stay, the higher the number of nosocomial infections. The overall mean hospital stay of patients with nosocomial infection was 2.4 times higher than that without nosocomial infection.

Abstract

Chemokine receptors CCR5 and CXCR4 which lied on lymphocyte cell surface play important role in HIV infection and pathogenesis.The expression of these chemokine receptors will affect progressively the disease. The objectives of the study are to find the distributionof lymphocyte T cell subset and monocyte among the peripheral blood mononuclear cells and to know the determination of CCR5 andCXCR4 co receptors expression on T lymphocyte cells subset and monocyte. This study is a preliminary study to explore the distributionof co receptors CCR5 and CXCR4 expression in healthy people. The sample taken is peripheral blood mononuclear cells (PBMC) fromhealthy subjects. The identification of T lymphocyte cells subsets and monocyte, and the expression of CCR5 and CXCR4 co receptorswere determined using flowcytometry. The memory T cell (CD4+CD45RO) is found to be the largest proportion among T lymphocyte cell(66.2%), whereas the other T lymphocyte cell subset, regulatory T cell, which identified by CD25+ high expression was found between2.0-5.3% from the whole T lymphocyte cell. The proportion of CXCR4 co receptors was found higher compare to CCR5 co receptorson all T lymphocyte subsets and monocyte. Only small proportion of monocyte expresses both co receptors (2.85%), but most of the Tlymphocyte cell expressed both CCR5 and CXCR4. The expression of the CXCR4 on regulatory T cell (18.18%) is the lowest compared toother cells, but the fluorescence intensity of both co receptors was very high (CCR5 53.53 and CXCR4 49.33). The different distributionof CCR5 and CXCR4 co receptors among T lymphocyte cell subsets and monocyte will influence the vulnerability and the pathogenicityof HIV infection.

Abstract

Infeksi saluran napas menduduki peringkat pertama dalam kelompok penyakit penyebab rawat jalan. Salah satu penyebabnya tingkat polusi udara. World Health Organization (WHO) menyatakan polusi udara bertanggung jawab atas 200 ribu kematian di daerah perkotaan dan 87% kasus kematian yang terjadi di negara berkembang pada tahun 2012. Peningkatan jumlah kendaraan bermotor merupakan penyebab polusi udara di dunia maupun Indonesia. Badan Statistik (BPS) menyatakan kendaraan bermotor di Indonesia akan terus meningkat dan tercatat pada tahun 2016 terdapat 129.281.079 kendaraan. Populasi yang sangat rentan terhadap penurunan fungsi organ paru adalah pekerja outdoor, salah satunya adalah supir angkot akibat terpapar langsung oleh polusi udara dan tidak menggunakan masker sebagai alat pelindung diri. Tujuan penelitian ini mengetahui pengaruh lama kerja terhadap fungsi paru pada supir angkot jurusan Cimahi di Terminal Leuwi Panjang Bandung periode Maret–Mei tahun 2018. Pengukuran fungsi paru menggunakan spirometer. Penelitian ini menggunakan desain analitik observasional dengan rancangan cross sectional terhadap 40 responden. Analisis statistika menggunakan Pearson correlation test. Hasil penelitian ini 16 responden (40%) memiliki lama kerja £5 tahun dan 24 responden (60%) lama kerja >5-15 tahun, memiliki rerata usia 31,27 tahun, berat badan 61,8 kg, tinggi badan 168,35 cm, dan IMT 21,79 kg/m2. Terdapat 17 responden (42%) memiliki fungsi paru normal, 23 responden (58%) mengalami gangguan restriktif fungsi paru dan sebagian besar responden dengan fungsi paru restriktif memiliki lama kerja >5-15 tahun p=0,00. Simpulan terdapat pengaruh lama kerja terhadap fungsi paru supir angkot jurusan Cimahi. THE EFFECT OF LENGTH OF WORKING ON LUNG FUNCTION AT DRIVER OF CIMAHI DESTINATION AT TERMINAL LEUWI PANJANG BANDUNGRespiratory infections ranked first in the outpatient disease category. One of the main factors is high level of air pollution. World Health Organization (WHO) states that air pollution is responsible for 200,000 thousands of deaths in urban area and 87% of the total deaths in developing countries in 2012. The increase in the number of vehicles is the cause of air pollution in the world including in Indonesia. Statistical agency claimed that the number of vehicles in Indonesia would grow exponentially and it has been recorded that there was 129,281,079 vehicles in 2016. Populations which are highly susceptible to decline in lung function is field workers, one of them is drivers of public transportation due to direct exposure of air pollution and this is exacerbated by the disuse of mask as self-protection. The aim of this research was to discover the relationship between the long working hours and lung function on the drivers of public transportation to Cimahi as the destination at Terminal Lauwi Panjang Bandung period on March–May in 2018. Lung function measurement using spirometer. The research design was analytic observasional using cross sectional design by 40 respondents and statistics analysis with Pearson Correlation test. The results of this study indicated 16 respondents (40%) with length working £5 years dan 24 respondents (60%) with length working >5-15 years had mean of age 31.27 years, body weight 61.8 kg, height 168.35 cm, and IMT 21.79 kg/m2. 17 respondents (42%) had normal function of lung and 23 respondents (58%) decrease the function of lung and most of the respondents who experienced restrictive had length working >5-15 years (p=0.00). In conclusion, there is effect of length of working on lung function at driver of Cimahi destination.

Abstract

Stunting adalah kondisi gizi kronik yang diakibatkan oleh pemberian nutrisi yang tidak seimbang dengan kebutuhan berdasar atas indeks panjang badan menurut usia dengan nilai melampaui <–2 Standar Deviasi (SD). Efek jangka pendek stunting salah satunya dapat memengaruhi perkembangan motorik. Stunting dan perkembangan motorik erat kaitannya dengan kuantitas dan kualitas nutrisi yang masih terbatas sehingga proses perkembangan saraf yang dibutuhkan untuk fungsi motorik menjadi terganggu. Penelitian ini bertujuan mengetahui hubungan stunting dengan perkembangan motorik kasar dan halus pada anak usia 2–5 tahun di Desa Panyirapan, Kec. Soreang, Kab. Bandung periode Agustus–September 2019. Penelitian ini merupakan analitik observasional menggunakan desain kasus kontrol dengan prosedur matching. Subjek dipilih menggunakan teknik pemilihan purposive sampling didapat sampel 37 anak untuk tiap kelompok kasus dan kontrol. Penilaian perkembangan motorik kasar dan halus menggunakan kuesioner praskrining perkembangan aspek motorik kasar dan halus. Data status stunting didapat berdasar atas skor-z indeks tinggi badan menurut usia kurang dari –2 SD. Analisis data menggunakan Uji McNemar. Hasil uji diperoleh status gizi stunting kemungkinan 5,02 kali (IK95%: 1,46–17,21) mengalami suspek gangguan perkembangan motorik kasar (p=0,013) serta kemungkinan 6,28 kali (IK95%: 1,85–21,39) mengalami suspek gangguan perkembangan motorik halus dibanding dengan status gizi tidak stunting (p=0,012). Simpulan, terdapat hubungan stunting dengan perkembangan motorik kasar dan halus pada anak usia 2–5 tahun. CORRELATION BETWEEN STUNTING WITH GROSS AND FINE MOTOR DEVELOPMENT OF CHILDREN AGED 2–5 YEARS OLD IN PANYIRAPAN SUBDISTRICT SOREANG BANDUNGStunting is a chronic nutritional condition caused by the intake of nutrients that are not balanced with the needs based indicator of body length for age with values exceeding <-2 Standard Deviation (SD). One of the short-term effects of stunting can affect motor development. Stunting and motor development are closely related to the quantity and quality of nutrients that are still limited so that the process of nerve development needed for motor function is disrupted. This study aims to determine the relationship of stunting with gross and fine motor development in children aged 2–5 years in Panyirapan subdistrict, Soreang, Bandung. This study was an observational analytic using a case control design with matching procedures. Subjects selected by purposive sampling technique obtained 37 samples of children for each case and control group. Gross and fine motor development was measure using kuesioner pra-skrining perkembangan (KPSP). Stunting was measured by indicator of body length for age (z-score) less than –2SD. Data analysis using McNemar test. The test results obtained that stunting has a possibility of 5.02 times (95%CI: 1.46–17.11) suspected of impaired gross motor development (p=0.013), and a 6.28 times probability (95%CI: 1.85–21.39) had suspected impairment of fine motor development compared with non-stunting (p=0.012). Conclusion, there is an correlation between stunting with gross and fine motor development in children aged 2–5 years.

Abstract

Nutrisi merupakan kebutuhan yang sangat penting dalam proses pertumbuhan dan perkembangan bayi. Untuk meningkatkan status gizi agar menurunkan angka kematian anak, United Nations Children’s Fund (UNICEF) dan World Health Organization (WHO) merekomendasikan sebaiknya anak diberi air susu ibu (ASI) eksklusif selama 6 bulan, lalu diberi makanan pendamping ASI setelah 6 bulan, dan ASI dilanjutkan sampai usia 2 tahun. Menurut WHO, makanan pendamping air susu ibu (MP-ASI) merupakan sebuah proses penting yang mengedepankan kesiapan bayi dalam menyambut makanan yang akan dikonsumsinya. Tujuan penelitian ini mengetahui pengaruh ASI eksklusif+MP-ASI terhadap status gizi bayi usia 6̶̶−9 bulan. Penelitian ini menggunakan metode observasional analitik dengan pendekatan cohort  menggunakan teknik pemilihan sampel cluster random sampling periode Maret−Juni 2018. Pengumpulan data diambil dari hasil pengukuran antropometri  berat badan/usia untuk mengetahui status gizi bayi 4 bulan ke depan (bulan ke-6, -7, -8, dan -9). Hasil penelitian menggunakan analisis Uji Fisher Exact dan Kruskal Wallis diperoleh dari 52 sampel. Jumlah kelompok yang diberi ASI eksklusif 27 bayi, sedangkan jumlah kelompok yang diberi ASI noneksklusif 25 bayi. Hasil analisis didapatkan terdapat pengaruh ASI eksklusif+MP-ASI terhadap kenaikan status gizi pada kelompok ASI eksklusif maupun noneksklusif (p=0,047). Faktor pendidikan ibu, pekerjaan ibu dan sosio ekonomi keluarga terhadap status gizi kelompok ASI eksklusif dan noneksklusif tidak terdapat pengaruh (p=0,19; p=0,25; dan p=0,54). Kenaikan status gizi kedua kelompok tersebut tiap bulannya mengalami kenaikan yang signifikan. Simpulan, terdapat pengaruh ASI-eksklusif+MP-ASI terhadap status gizi bayi usia 6̶̶−9 bulan di Desa Sukawening Kecamatan Ciwidey Kabupaten Bandung. EFFECT 0F EXCLUSIVE BREASTFEEDING+COMPLEMENTARY FOOD FOR BREAST MILK TO NUTRITIONAL STATUS BABY AGE 6−9 MONTH IN SUKAWENING VILLAGE, DISTRICT CIWIDEY DISTRICT BANDUNGNutrition is a very important requirement in the process of baby growth and development. Improving nutritional status in order to reduce child mortality, the United Nations Children’s Fund (UNICEF) and the World Health Organization (WHO) recommend that children be given exclusive breast milk for 6 months, then given complementary breastfeeding after 6 months, and breast milk continued until the age of 2 years. According to WHO, complementary food for breast milk is an important process that prioritizes the readiness of the baby in welcoming the food he will consume. The purpose of this study was to determine the effect of exclusive breastfeeding+complementary food for breast milk on the nutritional status of infants aged 6−9 months. This research used an analytic observational method with a cohort approach using cluster random sampling technique for the period March−June 2018. Data collection was taken from the results of weight / age anthropometry measurements to determine the nutritional status of infants 4 months ahead (-6th -7th, , -8th, and -9th month). The results of the study using Fisher exact and Kruskal Wallis analysis were obtained from 52 samples. The number of groups given exclusive breastfeeding was 27 babies, while the number of groups that were given non-exclusive breastfeeding was 25 babies. The results of the analysis showed that there was an effect of exclusive breastfeeding + complementary food for breast milk on the increase in nutritional status in exclusive and non-exclusive breastfeeding groups (p = 0.047). The factors of maternal education, maternal work and family socioeconomic on the nutritional status of exclusive and non-exclusive breastfeeding groups were not affected (p=0.19, p=0.25 and p=0.54). The increase in nutritional status of the two groups each month experienced a significant increase. Conclusions there are the effect of exclusive breastfeeding+complementary food for breast milk on the nutritional status of infants aged 6−9 months in Sukawening Village, Ciwidey District, Bandung Regency.

Abstract

Imunisasi merupakan upaya memberikan kekebalan pada anak dengan memasukkan vaksin ke dalam tubuh agar tubuh membuat zat antibodi untuk mencegah penyakit tertentu. Kelengkapan imunisasi dipengaruhi oleh banyak faktor antara lain usia ibu, pekerjaan ibu, pendidikan terakhir ibu, pendapatan keluarga, ketakutan akan efek samping imunisasi, tradisi keluarga dalam pemberian imunisasi, larangan agama, pengetahuan tentang jadwal imunisasi, jarak tempat pelayanan, biaya imunisasi, dan rumor buruk tentang kandungan vaksin. Tujuan penelitian ini mengetahui faktor-faktor yang memengaruhi cakupan imunisasi dasar di Puskesmas Cijagra Lama Kota Bandung periode Agustus−November 2019. Desain penelitian adalah deskriptif dengan pendekatan cross sectional. Data hasil penelitian dianalisis univariat untuk menghitung distribusi, frekuensi, karakteristik responden, dan karakteristik setiap variabel penelitian. Sampel penelitian adalah ibu yang datang ke Puskesmas Cijagra lama Kota Bandung dengan membawa bayi berusia 0−24 bulan yang akan dilakukan imunisasi dasar. Pengambilan sampel dengan cara total sampling. Besar sampel sebanyak 139 responden. Hasil penelitian ibu yang membawa anaknya untuk imunisasi dasar lengkap didominasi oleh ibu yang berusia <30 tahun (57,1%), ibu rumah tangga (84,2%), pendidikan terakhir SMA dan perguruan tinggi (86,5%), sedangkan pendapatan keluarga hampir sama antara di bawah Upah Minimum Regional (UMR) dan di atas UMR. Responden yang anaknya diimunisasi dasar lengkap mayoritas tidak takut akan efek samping (p=0,006), sudah tradisi untuk diimunisasi (p=0,214), tidak ada larangan agama (p=0,02), mengetahui  jadwal  imunisasi  (p=0,023),  jarak  tempat  pelayanan imunisasi tidak terlalu jauh (0,004), biaya masih terjangkau (p=0,603), serta   tidak   perduli   akan   rumor   buruk   tentang   kandungan   vaksin (p=0,877). Responden yang tidak melakukan imunisasi dasar lengkap, mayoritas usia ibu ≥30 tahun, ibu rumah tangga, pendidikan SMA dan perguruan tinggi, dan pendapatan keluarga   di bawah UMR. Simpulan, faktor yang memengaruhi cakupan status imunisasi dasar adalah ketakutan akan efek samping, larangan agama, pengetahuan jadwal imunisasi, dan jarak tempat pelayanan imunisasi. FACTORS AFFECTING BASIC IMMUNIZATION COVERAGE IN PUSKESMAS CIJAGRA LAMA BANDUNG CITYImmunization is an attempt to provide immunity to children by introducing vaccines into the body so that the body makes antibodies to prevent certain diseases. Completeness of immunization is affected by many factors including mother’s age, mother’s occupation, mother’s last education, family income, fear of immunization side effects, family traditions in immunization, religious restrictions, knowledge of immunization schedules, the distance of services, immunization fees, and bad rumors about vaccine content. The purpose of this study was to determine the factors that affecting the basic immunization coverage in the Cijagra Lama Health Center in the City of Bandung in the August– November 2019 period. The study design was descriptive with a cross- sectional approach. Data were analyzed univariate to calculate the distribution, frequency, characteristics of respondents, and characteristics of each study variable. The sample of this research was mothers who come to the old Cijagra Health Center in Bandung City with babies aged 0−24 months who will be given basic immunizations. Sampling by total sampling. The sample size was 139 respondents. The results of the study of mothers who bring their children to complete basic immunizations were dominated by mothers aged <30 years (57.1%), housewives (84.2%), high school education and college (86.5%), while income almost the same family between below regional minimum wage and above regional minimum wage. Respondents whose children were fully immunized basic majority were not afraid of side effects, it was tradition to be immunized, there was no religious prohibition, knowing the immunization schedule, distance between immunization services was not far, the cost was still affordable, and no matter the bad rumors about vaccine content. Respondents who did not complete basic immunization, the majority of mothers aged ≥30 years, housewives, high school and college education, and family income below regional minimum wage. In conclusion, that most of the coverage of basic immunization status at the Cijagra Lama Health Center in Bandung are complete, and the small part that is incomplete is influenced by maternal age ≥ 30 years, family income below regional minimum wage, less knowledge about immunization.
