Abstract

Hubungan karakteristik responden, perilaku kesehatan gigi dan mulut dengan status kebersihan rongga mulut lansia di kawasan pertambangan kapur: studi cross-sectionalABSTRAK Pendahuluan: Lansia merupakan kelompok berisiko dengan masalah kesehatan. Masalah kesehatan gigi dan mulut yang sering ditemukan pada lansia berkaitan dengan kebersihan rongga mulut yang buruk, yang dapat menimbulkan penyakit periodontal. Masalah periodontal berdampak pada kualitas hidup lansia. Kawasan pertambangan kapur merupakan wilayah yang kaya mineral, yang dapat memengaruhi kandungan mineral saliva dan berkaitan dengan pembentukan plak dan kalkulus.. Penelitian bertujuan menganalisis hubungan karakteristik responden, perilaku kesehatan gigi dan mulut dengan status kebersihan rongga mulut pada lansia di lingkungan sekitar pertambangan kapur. Metode: Jenis penelitian cross-sectional. Pengambilan sampel pada penelitian menggunakan Slovin, jumlah sampel minimal 96 orang. Sampel diambil secara purposive sampling dan didapatkan 125 lansia. teknik pengambilan data menggunakan kuesioner karakteristik responden dan perilaku kesehatan gigi dan mulut serta status kebersihan rongga mulut (indeks OHI-S). Data analisis menggunakan Uji chi-square. Hasil: Status kebersihan rongga mulut lansia tergolong buruk yaitu 110 orang (88,0%), Uji chi-square didapatkan hasil signifikan dengan karakteristik responden adalah pendidikan terakhir, dan faktor perilaku kesehatan gigi dan mulut dengan status kebersihan rongga mulut yang signifikan adalah frekuensi, ketepatan waktu menyikat gigi, kunjungan ke dokter gigi, kebiasaan merokok setiap hari, dan sumber air (p<0,05). Simpulan: Terdapat hubungan antara status kebersihan rongga mulut lansia di kawasan pertambangan kapur dengan karakteristik responden di tingkat pendidikan dan  terdapat hubungan dengan perilaku kesehatan gigi dan mulut pada perilaku frekuensi, ketepatan waktu menyikat gigi, kunjungan ke dokter gigi, kebiasaan merokok setiap hari, dan sumber air. Kata kunci lansia, pertambangan kapur, kebersihan rongga mulut The relationship of the respondent characteristics, oral health behaviors and oral hygiene of the elderly in the limestone mining area: cross-sectional study ABSTRACTIntroduction: The elderly are a risk group with health problems. Oral health problems often found in the elderly are related to oral hygiene. Poor oral hygiene can lead to periodontal disease. Periodontal problems can result in premature tooth loss, which impacts the quality of life of the elderly. Limestone mining areas are rich in minerals, which can affect the mineral content of saliva and are related to plaque and calculus formation in the oral cavity. This study aims to analyze the relationship between respondent characteristics, oral health behaviors, and oral hygiene of the elderly in the limestone mining area. Methods: This study is a cross-sectional study. Based on the Slovin formula, a minimum sample size of 96 people was obtained. Samples were taken using purposive sampling, and 125 elderly people were involved in this research. Questionnaires on respondent characteristics and oral health behavior were given to the respondents, and an examination of oral hygiene status using the OHI-S index was carried out on respondents. The chi-square test was carried out in this study. Results: The majority of older people's oral hygiene status was classified as poor, namely 110 people (88.0%). The Chi-Square test showed a significant relationship between respondent characteristics, namely levels of education, oral health behaviors, including frequency of brushing teeth, timeliness of brushing teeth, visits to the doctor, smoking, and water sources, and oral hygiene of the elderly (p<0.05). Abstract

Background: The increasing number of elderly people in Indonesia demands improvements in health services. Elderly individuals, with their vulnerability to diseases, require quality services that meet their expectations. This study describes how elderly patients evaluate the quality of services they receive at Hospital X in Jember Regency. Method: This study uses a descriptive quantitative approach with a survey method to collect data. A total of 269 patients participated in this study, selected through a simple random sampling process. A modified SERVQUAL questionnaire was used to measure patient perceptions. The data obtained were analyzed using the SPSS application. Results: The perception of geriatric patients assessed the quality of service at Hospital X in Jember Regency in the high category (96.3%) which included the dimensions of physical evidence (97%), reliability (94.8%), responsiveness (94.4%), assurance dimension (98.5%), empathy (99.3%) and accessibility and affordability (91.8%). Conclusion: Positive perceptions of geriatric patients are an indicator of a hospital's success in providing quality healthcare services that meet the standards for elderly patients.

Preventive measures must be taken to improve oral hygiene for the elderly starting from the pre-elderly group so that later it will form healthy and productive elderly. One of the efforts to improve dental hygiene and health is through mechanical tooth cleaning with a toothbrush. However, currently, there is no toothbrush design that is suitable for the condition of the pre-elderly oral cavity. Therefore, it is necessary to analyze the design of the number of toothbrush bristles as initial research to obtain an appropriate toothbrush design for the elderly. Method:The study was conducted on 30 participants using a toothbrush with a number of toothbrush bristles paths: with 4 paths, 3 paths, and 2 paths. Abstract

Background: Diabetes melitus is a chronic metabolic disease due to the inability of the pancreas to produce insulin or the body cannot use insulin effectively.Periodontitis is one of the complications of microvascular disorders that ranks second in oral cavity diseases. One of the aerobic bacteria thought to play a role in the severity of diabetes melitus is Staphylococcus aureus, while the pathogenic anaerobic bacteria in periodontal disease are Treponema, Bacteroides, Porphyromonas, Prevotella, Capnocytophaga, Peptostreptococcus, Fusobacterium, Actinobacillus, and Eikenella. The study aimed to determine the microbiological profile in oral cavity infections of rats with diabetes melitus and periodontitisMethod: This study was experimental laboratories on male wistar rats induced by diabetes melitus and periodontitis using wire ligature on mandibular molars, which were divided into 3 treatment groups: group I (control), group II (diabetes melitus) and group III (diabetes melitus and periodontitis). Periodontitis (wire) rats were treated for 7 days and saliva was collected to identify the microbiological profile of the oral cavity. Result: Identification indicated the presence of genus and species of bacteria in the saliva of rats in 3 groups, gram positive: Staphylococcus intermedius, Staphylococcus cohnii subsp.urelyticus. Gram negative: Chromobacterium violaceum, Kleibseilla pneumoniae, Eikenella corrodens, Enterobacter sakazaki and Chryseobacterium meningosepticum. Conclusion: The number of bacterial colonies in the treatment group of diabetes melitus and periodontitis rats was greater than the treatment group of diabetes melitus rats and the control group. Gram staining results found groups of gram positive and gram negative bacteria.

Abstract

Background: Temporomandibular disorder (TMD) is a collection of symptoms that causes pain and disturbs a person's life quality. One of the trigger factors is mechanical overloading. Mechanical overloading in occlusal disharmony conditions will lead to an inflammatory reaction in the temporomandibular joint (TMJ). This condition will induce nuclear factor Kappa Beta (NFkB) activation to transcribe proinflammatory cytokines and matrix metalloproteinase-13 (MMP13) and will also degrade condylar cartilage as a major factor in strengthening the TMJ. Purpose: The aim of this study was to analyze the expression of NFkB and MMP13 in the condylar cartilage of TMJ with occlusal disharmony. Methods: This research was an experimental study with post-test-only control group design. Twenty Rats (Rattus norvegicus) were divided into four groups: One control group without any intervention and three experimental groups. Occlusal-reducing intervention was due on the right molar of the experimental groups, which were divided into the 7th, 14th, and 21st days. Immunohistochemical staining was performed to determine the expression of NFkB and MMP13 in the condylar cartilage. The data were analyzed by using the Welch test and independent t-test. Results: There were significant differences in NFkB and MMP13 expression between the control and experiment experimental groups (p<0.05). NFkB expression increased on the 7th, 14th, and 21st days of observation. The MMP13 expression showed a significant difference between the control and experimental groups (p<0.05). Conclusion: Occlusal disharmony increases NFkB and MMP13 expression and could affect TMJ integrity and induce TMD. These findings are important for describing the mechanism of TMJ damage and developing potential alternative therapies to prevent further TMD.
