Abstract

Tinea Pedis merupakan salah satu infeksi jamur superfisial kulit yang paling umum di seluruh dunia yang bersifat menular dan berulang. Infeksi ini sering kali tertular secara kontak langsung dengan spora organisme penyebab atau bagian kulit yang terinfeksi. Prevalensi global tinea pedis diperkirakan sekitar 3% dengan risiko seumur hidup mencapai 70%. Prevalensi pada remaja dan dewasa lebih tinggi dibandingkan pada anak-anak. Kejadian tertinggi terjadi antara usia 16 dan 45 tahun saat aktivitas tinggi. Tujuan penelitian ini adalah untuk melihat perbandingan efektivitas antara obat anti jamur ketoconazole dengan Salep 2 - 4 yang mengandung salisilat dalam mengobati penyakit infeksi jamur di Tempat Pembuangan Akhir Sampah Terjun Kota Medan. Penelitian ini merupakan penelitian eksperimental uji klinis, yakni uji klinis acak terkontrol (Randomized Controlled Trial) yang dilakukan di Tempat Pembuangan Akhir Sampah Terjun Kota Medan sebanyak 32 pemulung dan petugas sampah, dibagi menjadi dua kelompok yaitu salep ketoconazole dan salep 2 - 4 dilakukan selama 14 hari dengan follow up pada hari ke - 4, 8, 14. Hasil pengobatan pada hari ke - 4 dengan angka kesembuhan salep ketoconazole (56.25%), salep 2-4 (18.75%) dengan nilai P = 0.028. Pada hari ke - 8 angka kesembuhan salep ketoconazole (68.75%) salep 2 - 4 (31.25%) dengan nilai P = 0.034. Pada hari ke – 14 angka kesembuhan salep ketoconazole (93.75%) salep 2-4 (56.25%) dengan nilai P = 0.037. Kesimpulan dari terdapat perbandingan efektivitas antara obat anti jamur ketoconazole dan salep 2-4 dalam mengobati infeksi jamur tinea pedis.

Abstract

Artritis reumatoid (AR) merupakan penyakit autoimun menahun yang ditandai peradangan sinovial berulang, sehingga berujung pada kerusakan sendi, keterbatasan fungsi, serta penurunan kualitas hidup. Tingginya aktivitas penyakit sering kali disertai masalah psikologis seperti kecemasan. Penelitian ini mengevaluasi hubungan antara tingkat aktivitas penyakit berdasarkan Disease Activity Score 28–C-reactive protein (DAS28-CRP) dengan gejala yang dinilai melalui Hospital Anxiety and Depression Scale (HADS-A) pada penderita AR di RS Royal Prima Msimpleedan menggunakan desain analitik observasional dengan pendekatan potong lintang. Sebanyak 58 pasien AR dipilih secara simple random sampling. Data dikumpulkan melalui pemeriksaan DAS28-CRP dan pengisian kuesioner HADS-A, lalu dianalisis menggunakan uji korelasi Spearman. Sebagian besar responden berjenis kelamin perempuan (91,4%) dengan aktivitas penyakit tinggi (41,4%) dan tingkat kecemasan normal (81,0%). Hasil analisis menunjukkan adanya korelasi signifikan antara aktivitas penyakit dan kecemasan (r = 0,304; p = 0,020 . Temuan ini menunjukkan bahwa semakin tinggi aktivitas penyakit, semakin besar tingkat kecemasan yang dialami pasien, sehingga aspek psikologis perlu diperhatikan dalam tatalaksana komprehensif AR. Penelitian ini memberikan kontribusi ilmiah dalam mendukung penerapan pendekatan biopsikososial sebagai strategi penting untuk meningkatkan kualitas hidup pasien melalui pengelolaan terpadu antara kondisi fisik dan psikologis. activity assessment an essential component of comprehensive care. This study aims to analyze the relationship between disease activity level based on the Disease Activity Score 28–C-Reactive Protein (DAS28-CRP) and anxiety and depression levels measured using the Hospital Anxiety and Depression Scale (HADS) among RA patients at Royal Prima Hospital Medan. The research employed an observational analytic design with a cross-sectional approach. The study sample consisted of 110 respondents who met the inclusion criteria. Data were analyzed using univariate tests to describe respondents’ characteristics, while the relationship between variables was assessed using bivariate correlation tests. The results revealed that most respondents had high disease activity, while the majority still fell into the normal category for anxiety and depression. Bivariate analysis showed a statistically significant relationship between DAS28-CRP scores and levels of anxiety and depression (p < 0.05). These findings suggest that higher disease activity is associated with an increased risk of anxiety and depression. The study concludes that a multidisciplinary approach is essential in the management of RA patients, focusing not only on physical symptom control but also on supporting patients’ mental health.

Abstract

Introduction: Suicide rates continue to rise globally, with self-poisoning being a common method in some developing countries. This report reported a case of hydrogen peroxide ingestion leading to aspiration pneumonitis, a less-discussed complication compared to gastrointestinal toxicity. Case Illustration: A 41-year-old woman presented to the emergency unit 8 hours post-ingestion of a hydrogen peroxide-based toilet cleaner. Symptoms included vomiting, hoarseness, faintness, and frothing at the mouth. The physical examination revealed dyspnea, irritability, a cleaning solution odor in the oral cavity, rhonchi in the middle and lower lung fields, epigastric tenderness, and increased bowel sounds. Gastric lavage with 200 mL of saline was performed, and activated charcoal and sucralfate syrup were administered via a nasogastric tube. The patient had previously received ranitidine at another facility and was also treated with omeprazole and ondansetron. Laboratory results showed leukocytosis, and a chest x-ray indicated bilateral lung infiltration and increased broncho-vascular pattern. Discussion: Early and proper intervention by trained healthcare professionals, combined with preventive measures such as positioning the patient in a semi-upright position and avoiding emetogenic liquids, is essential. Conclusion: This case showed the importance of prompt management involving gastric lavage, mucosal protection, acid suppression, and absorption inhibition. Nonetheless, respiratory complications, such as aspiration pneumonitis, remain challenging to prevent.

Abstract

Lemon pepper does not only have a unique taste but also has some active compounds, which has anti-inflammatory activity. This study intended to investigate the anti-inflammatory effect of lemon pepper nanoemulsion on carrageenan-induced Wistar rats. This experimental study used Twenty-five male Wistar rats that were grouped into five different groups, including control, standard, lemon pepper nanoemulsion dose of 25, 50, and 75 mg/ kgBW. Initially, all rats received air injection in the back area for 5 days to form an air pouch. After that, all rats were injected with 1% carrageenan solution, and received treatment based on the groups for 48 hours via oral route. Then, it counted the number of white blood cells by Giemsa Stain that was expressed as a number of cells per High Power Field (HPF). The increasing of Lemon pepper nanoemulsion dose significantly decreased the number of leucocytes in the carrageenan aspiration after 48 hours of treatment (P-Value < 0.001). The lowest number of leucocytes was found in the Lemon Pepper Nanoemulsion dose of 75 mg/kgBW (2.40 leukocytes per HPF). Hence, it can be deducted that the lemon pepper nanoemulsion has an anti-inflammatory effect, especially at a dose of 50-75 mg/kgBW.

Abstract

The development of natural-based antibacterial agents is essential to reduce dependency on antibiotics and the risk of resistance. Sweet orange peel and fig fruit contain bioactive compounds with potential antibacterial effects. This study aimed to develop tea bag formulations made from sweet orange peel with the addition of fig fruit as an additive and to evaluate their in vitro antibacterial activity against Staphylococcus aureus. This experimental study employed two approaches, including disc diffusion and microdilution on several combination formulations of sweet orange peel and fig tea bags. The formulations consisted of F1 (1 g sweet orange peel and 1 g fig), F2 (1.5 g sweet orange peel and 0.5 g fig), and F3 (1.75 g sweet orange peel and 0.25 g fig). The disc diffusion assay showed that all formulations exhibited weak antibacterial activity against Staphylococcus aureus, producing inhibition zones of approximately 6–7 mm, except for F3 infusion, which demonstrated the widest inhibition zone with a mean diameter of about 8.21 mm (P-Value = 0.688). In the microdilution test, the MIC of F3 infusion was found to be 40 mg/mL, while the MBC was not detected at the tested concentrations. Overall, it can be concluded that the infusion of the tea bag formulation containing 1.75 g sweet orange peel and 0.25 g fig was the most promising combination; however, its antibacterial activity remained weak and was bacteriostatic rather than bactericidal.

Abstract

Artritis Reumatoid (AR) merupakan penyakit inflamasi kronik yang berdampak pada fungsi fisik, kondisi mental, dan kualitas hidup pasien. Hubungan antara tingkat aktivitas penyakit dan penanda inflamasi seperti C-Reactive Protein  (CRP) dengan kualitas hidup masih menunjukkan hasil yang bervariasi. Oleh karena itu, penelitian ini dilakukan untuk mengetahui hubungan antara C-Reactive Protein  dan DAS 28 terhadap SF 36 pada pasien Artritis Reumatoid (AR) di Rumah Sakit Royal Prima. Penelitian observasional ini menggunakan rancangan cross-sectional pada 60 orang pasien Artritis Reumatoid (AR) di Rumah Sakit Umum Royal Prima yang dipilih melalui teknik pengambilan sampel convenience sampling. Data yang dikumpulkan meliputi karakteristik pasien, skor DAS28, kadar CRP, dan skor kualitas hidup (SF36). Data dianalisis secara deskriptif. Korelasi Spearman digunakan untuk menguji hubungan skor DAS28 dan kadar CRP dengan skor SF-36, sedangkan korelasi Kendall tau digunakan untuk menilai hubungan tingkat progresivitas penyakit dan kelompok kadar CRP dengan kualitas hidup. Hasil penelitian ini menunjukkan bahwa terdapat korelasi lemah antara skor DAS28 dan kualitas hidup (p = 0,008; r = –0,339) serta komponen fisik (p = 0,008; r = –0,342), tetapi tidak dengan komponen mental (p = 0,073). Kadar CRP tidak berkorelasi dengan seluruh aspek kualitas hidup, dan hasil tetap tidak signifikan setelah pengelompokan. Maka, dapat disimpulkan bahwa aktivitas penyakit berhubungan dengan kualitas hidup total dan fisik pada pasien AR, sedangkan kadar CRP tidak menunjukkan hubungan bermakna. Faktor yang memengaruhi kualitas hidup pasien AR lebih terkait dengan tingkat aktivitas penyakit dibandingkan marker inflamasi

Abstract

Pulmonary tuberculosis (TB) remains a global and national health challenge. Peripheral blood inflammatory indices—NLR, PLR, PNR, MLR, dNLR, SIRI, SII, and AISI—have been proposed as markers of systemic inflammation and prognosis, yet local data are scarce and findings vary. Thus, this study was aimed to describe these indices in pulmonary TB patients at Dr. Pirngadi Regional General Hospital, Medan (2019–2020). This cross-sectional study was analysed 105 inpatient medical record via total sampling technique. Demographics and eight inflammatory indices were analyzed using descriptive statistics. Median values ​​of NLR, PLR, PNR, MLR, dNLR, SIRI, SII, and AISI in pulmonary tuberculosis patients are quite varied and tend to be higher than several previous studies. High NLR and PLR values ​​can reflect a severe degree of inflammation, while PNR and dNLR values ​​show less consistent results. Indices such as SIRI, SII, and AISI show potential as indicators of systemic inflammation, but still require further study. These indices have the potential to be used as markers of inflammation and prognosis in pulmonary tuberculosis, but cannot yet be used as a single reference

Abstract

Pulmonary Tuberculosis (TB) is an infection of the lung parenchyma caused by Mycobacterium tuberculosis and is transmitted via droplets. A routine hematological examination is crucial for evaluating disease progression, complications, and therapeutic responses. Thus, this study aimed to To identify routine hematological characteristics in pulmonary tuberculosis patients in RSUD Dr. Pirngadi Kota Medan. This observational study with cross-sectional design included 105 pulmonary tuberculosis patients, that selected via total sampling technique from 2019 to 2020. Data on demographic characteristics (age, sex, and occupation) and hematological profiles were analyzed. The majority of patients were male (67.6%), aged 46–55 years (24.8%), and worked as self-employed (47.6%). Hematological profiles showed elevated neutrophils (72.4%) and erythrocyte sedimentation rate (53.3%), alongside reduced hemoglobin (58.1%), hematocrit (61%), and lymphocytes (83.8%). Most patients had normal number of leukocytes (52.4%), platelets (63.8%), monocytes (47.6%), eosinophils (87.6%), and basophils (99%). he most characteristic routine hematological changes in pulmonary tuberculosis patients include decreased hemoglobin, hematocrit, and lymphocytes, as well as increased neutrophils and erythrocyte sedimentation rate. These findings emphasize the importance of hematological examination in the diagnosis and monitoring of pulmonary tuberculosis.

Abstract

Diabetes mellitus is a metabolic disorder marked by high blood glucose levels (hyperglycemia) that is caused by insulin deficiency. It also affects male infertility. Pakkat, young rattan, contains various phytochemicals, which can affect blood glucose levels, spermatogenesis, and testosterone production. Objective: To assess the effectiveness of Pakkat extract in regulating blood glucose levels, testosterone hormone, and spermatogenesis in alloxan-induced diabetic rats. This experimental study used 25 diabetic male Wistar rats and was grouped into five groups including negative control (distilled water), positive control (metformin), Treatment 1 (125 mg/KgBW Pakkat extract), 2 (250 mg/KgBW Pakkat extract), and 3 (500 mg/KgBW Pakkat extract) for 14 days. All data were analyzed by One-way ANOVA followed by a Post-Hoc Test. Pakkat extract at 125 mg/kgBW dose effectively reduced glucose levels (P-Value < 0.001) after 14 days. It also significantly increased spermatogenesis (P-Value < 0.001) and testosterone levels (P-Value < 0.001) in alloxan-induced diabetic rats. Thus, it can be concluded that any doses of Pakkat (Calamus L. Blume) extract significantly improve blood glucose levels after 14 days of administration. However, a higher dosage is required to increase spermatogenesis and testosterone hormone in diabetic conditions.
