Abstract

Precision medicine, particularly pharmacogenetic-guided approaches, has emerged as a promising tool to optimize treatment strategies for cardiovascular disease. However, the economic evaluation of these approaches in Asian populations remains underexplored. This review study aimed to synthesize evidence on the implementation, methodology, research gaps, and limitations of pharmacogenetic-guided precision medicine in cardiovascular disease among the Asian population. A comprehensive search of electronic databases (Pubmed, Embase, Google Scholar and Cochrane Library) was conducted to identify relevant studies. The data were extracted and synthesized to address the study objectives A total of 12 studies were included in the analysis. Economic evaluation studies of pharmacogenetic-guided precision medicine in cardiovascular disease management in Asian countries are still limited, with precision medicine interventions often requiring high resources. The sampling process remains a challenge in conducting economic evaluation studies, and there are limitations in obtaining clinical outcome descriptions from real-world evidence, as most studies rely on literature reviews or modeling-based approaches. Furthermore, most studies use a provider or healthcare cost perspective, limiting the comprehensiveness of cost information. This evidence synthesis highlights the potential for pharmacogenetic-guided precision medicine in cardiovascular disease management among the Asian population, as well as the current challenges and limitations in conducting economic evaluations. Further research is needed to address these issues, develop more robust sampling strategies, obtain real-world clinical outcome data, and provide comprehensive cost information to inform clinical practice and healthcare policy in the region.

Abstract

. Pregnant women are one of the risk groups that should be considered to receive particular attention. Maternal mortality is caused by direct and indirect causes, which generally can be identified in early pregnancy. The Ministry of Health has launched various programs to reduce maternal mortality. At the community level, since 2007, there has been a birth planning and prevention of complications (P4K) program as a screening tool for pregnant women at risk by using stickers. The main point of this program is the active participation of families and communities to care for pregnant women in their environment. Purwakarta Regency, a region with uncomplicated geographical conditions, is supported by Health Human Resources (SDMKes) and Health facilities to do various programs in reducing the number of maternal deaths; one of them is P4K with stickers. In 2017, there were still 21 maternal deaths that should have been identified and referred to the hospital if the program was running well. The study aims to analyze the implementation of the P4K Sticker at the District Level. This research is qualitative research with in-depth interviewing techniques involving 12 informants and related documents, according to the theory of Van Meter and Van Horn policy implementation based on 6 (six) variables. This study found that the standard and policy objectives have not been fully achieved. There is still insufficient funding to support the program optimally. In terms of communication, there still a different understanding between the Health Office and Primary Healthcare Center. Despite there are still constraints on the number of human resources, the program's attitude is supportive. Economic and political situations are good, but social conditions are not supportive. The conclusion is that the program's implementation still has constraints in each variable. Existing program barriers can be overcome by optimizing inter-organizational communication, in this case, the District Health Office with Primary Healthcare Center, to have a common understanding regarding the implementation of Sticker P4K.

The research respondents were all people who were registered as JKN participants in the Maluku and Papua Islands regions as many as 77.307 people. This study uses the econometric approach with the Logit regression model. The results showed that JKN was positively associated with outpatient visits, with coefficients of PBI (0,713) and Non PBI (0,807). The results also showed that age had a positive effect on outpatient visits with a coeficent value (0,406), gender had a positive effect on outpatient visits with a coefficient (0,146), employment status negatively affected outpatient visits with a coefficient value (-0,198) likewise the health status has a positive effect on outpatient visits with a coefficient (3,914). JKN ownership has an important role for the community in fulfilling the utilization of health facilities when suffering from health complaints, one of which is the use of outpatient visits.

Abstract

One of the endeavors that the government has done to prevent and control non-communicable diseases (NCD) by increasing community participation in the early detection of NCDs is the NCD Integrated Guidance Post (Posbindu). However, in 2017, only 12.96% of the residents in Bogor City accessed it. This meant that the 30% target was not reached. The in this study, we researched the implementation of the NCD Posbindu at Bogor City. Data was gathered through in-depth interviews, Focus Group Discussion (FGD), documentary research, and observations. Eleven informants was interviewed and 24 informants was involved in the FGD. Two NCD Posbindus was observed at Mekarwangi and Cipaku Public Health Centers (PHC). We discovered that the standards and policies have supported the program, but not all the informants were aware of the targets of the program; there were also problems in the number and abilities of the human resources involved, ineffective communication between the cadres, in the coordination between the cadres, and lack of support from local public figures. The program had adequate funding from the regional budget, the Operational Health Aid Funds, and donations from the community. Although more funds are necessary for the promotion and prevention part of the program. We recommend that new cadres are recruited and trained, and that a reward system is used to provide motivation for the cadres.

Abstract

Chronic kidney disease (CKD) has become a serious global health problem with significant socio-economic implications. This study aims to analyze the determinants of the utilization of hemodialysis services for National Health Insurance (JKN) participants in the Special Region of Yogyakarta (DIY) in 2023. The study employs a cross-sectional design to analyze secondary data from the BPJS Kesehatan claim database in 2020, focusing on JKN participants receiving dialysis services at Advanced Referral Health Facilities (FKRTL). Data extraction is followed by analysis using STATA software, which includes univariate and bivariate analyses, multicollinearity and overdispersion tests, model comparison via Akaike Information Criterion (AIC), and multivariate analysis through Poisson regression and Binomial Negative methods. A total of 3,220 participants (0.089%) were enrolled in the study. The highest distribution of participants was in Sleman Regency (36.49%), followed by Bantul (29.53%), and Yogya City (25.96%). From the results of the binomial regression analysis, it was found that the age factor had a significant negative influence (p=0.000) with a coefficient of -0.0046 and an IRR of 0.9954, which means that every increase in age by one year will decrease the frequency of visits by 0.46%. The prevalence of diabetes (3.1%) and hypertension (10.2%) which exceed the national average indicates a high risk factor for CKD in this region is high. The age factor and location of health facilities in SLEMAN Regency significantly affected the frequency, while the segment group, gender, and nursing class rights did not show significant influence.

Abstract

Diabetes Mellitus (DM) is a chronic disease and a global public health challenge. The aim of this study is to review the most commonly used complementary therapies to control glycemic in patients with type 2 DM. The research contributes to the field of diabetes management by systematically reviewing and evaluating complementary therapies that are frequently employed for diabetes management. The literature searches are structured through online databases including Pubmed, Cochrane, and Science Direct. The population in this study was individuals with uncomplicated type 2 diabetes mellitus who were >18 years of age, the interventions were complementary and alternative therapies, the study types were RCT and Quasi Experimental, and the result was controlled lipid levels. A total of 12 articles were obtained that met the inclusion criteria. The results of the literature review through PRISMA showed that the majority of the articles were published in English and English. The majority of articles were randomized controlled trials (RCTs) that evaluated the efficacy of complementary therapies in reducing blood glucose, insulin resistance, and oxidative status in type 2 diabetic patients. In addition, this review can highlight gaps in the current literature, paving the way for future research initiatives aimed at exploring the effectiveness, safety, and mechanisms of action of various complementary approaches in diabetes management, thereby facilitating informed decision-making in patient care.
