Abstract

Background: Cervical cancer plays a role as the leading cause of cancer death for women in developing countries. Radiation therapy kills cancer cells through double strand breaks and the apoptotic process. Bax protein is one of the regulators of apoptosis. This study analyzed the role of Bax expression as a prognostic factor in radiation therapy response in reducing tumor size in patients with advanced cervical cancer (IIB-IIIB). Subjects and Method: This research is a retrospective cohort observational analytic study from January 2021 to April 2021 in the Department of Obstetrics and Gynecology in collaboration with the Department of Anatomical Pathology, Dr. Moewardi Hospital, Surakarta, Indonesia. This study involved 30 cervical cancer stage IIB-IIIB patients. The level of Bax expression was determined by immunohistochemical examination. ROC curve analysis was used to find cut-off points and evaluate the sensitivity and specificity of Bax in the prognosis of radiotherapy in patients with advanced cervical cancer (IIB-IIIB). Chi square test was used to determine the relationship between Bax expression and changes in tumor size in cervical cancer patients. Results: The ROC curve analysis showed that the AUC Bax score in prognosis of radiation therapy was 0.575 (CI 95%= 0.32 to 0.83) and the cutoff point was 62.5%. Based on the analysis of the ROC curve, the Bax expression of 62.5% shows a sensitivity of 56.5% and a specificity of 71.4%. Analysis of the relationship between Bax protein expression on changes in tumor size using the Chi Square test showed a P value of 0.390 (p> 0.05). Conclusion: The expression of Bax protein cannot play a role as a prognostic factor in the response of radiation therapy to the reduction in tumor size in stage IIB-IIIB uterine cervical cancer.

Abstract

Background: Low birth weight (LBW) is defined as a newborn baby weighing less than 2500 grams. Low birth weight is a serious condition in developing countries. The impact of cigarette smoke on pregnant women who are exposed to cigarette smoke is at a higher risk of complications, such as miscarriage, premature birth, or babies born with low weight. This study aims to analyze the relationship between exposure to secondhand smoke and low birth weight. Subjects and Method: This research is a systematic review and meta-analysis by systematically reviewing primary articles from Pubmed, Google Scholar, Scient Direct and Springerlink by selecting articles published in 2012-2022. The articles searched using PICO were P: Pregnant women, I: exposed to cigarette smoke, C: not exposed to cigarette smoke, O: low birth weight. Keywords "Exposure to smoking pregnant women" OR "Expectant mother" OR "Pregnancy women" OR "Exposed to cigarettes" OR "Secondhand smoke exposure" OR "Cigarette smoke expo- sure" AND "Low birth weight" OR "Low weight babies" AND “Case control” AND “Adjusted odds ratio”. Articles used PRISMA flow diagrams and analyzed using Review Manager 5.3. Results: There were nine articles analyzed consisting of Nepal, Morocco, Ethiopia, Sierra Leone, India, Indonesia and China. Meta-analysis showed that cigarette smoke exposure increased the risk of low birth weight and was statistically significant (aOR = 2.19; 95% CI = 1.58 to 3.03; p < 0.001). Conclusion: Exposure to cigarette smoke increases the risk of weight gain in LBW infants by 2.19 times compared to not exposed to cigarette smoke. Abstract

Stunting is the result of an interaction between lack of nutritional intake and increased nutritional needs, which has an impact on physical growth and cognitive development, as well as increasing the risk of chronic diseases. In Indonesia, the prevalence of stunting in 2023 will reach 20.8%, still far from the target of 14% in 2024. Adolescence is an important phase to build awareness of nutritional status, because adolescents who understand nutrition can prepare themselves for a healthy pregnancy and reduce the risk of stunting in the next generation. Objective: this study aims to explore the understanding and needs of adolescents related to stunting prevention. This study uses a phenomenological approach to explore adolescents' understanding of stunting prevention programs in Gunungkidul Regency, Yogyakarta. The subjects of the study were 12 adolescent girls aged 12-15 years, selected by purposive sampling based on their experiences. Data collection methods include in-depth interviews and Focus Group Discussions (FGD). The validity of the data was obtained through triangulation and checking with informants. The results of FGD and interviews with adolescents identified three categories of needs: knowledge, behavior, and health conditions of adolescents. Limited access to nutritional information and unhealthy lifestyles increase the risk of stunting. The importance of balanced knowledge and clean living behaviors to support adolescent health is emphasized, especially in preventing future health problems. The social and cultural environment plays an important role in shaping adolescent behavior. The provision of easily accessible nutritional information, as well as support from families and communities, is urgently needed to support the role of adolescents in preventing stunting and ensuring their health and that of future generations.

Abstract

Cervical cancer is the most common cause of death among gynaecological neoplasms. Management of advanced cervical cancer currently has not been able to improve the prognosis. Standard intervention therapy, chemotherapy and radiotherapy, as well as the emerging pain could lead to psychological stress till depression, thus reduce patients' quality of life. CBT for psychological stress and depression is expected to improve the fruitfulness of standard therapy. Serotonin is a neurotransmitter which plays a role in the patophysiology of depression. The aim of this study is to analyze the effect of cognitive behavioral therapy on serotonin level, depression score and quality of life in advanced cervical cancer patient with experimental quasi pre and post test design methods. The samples are 15 subjects in both intervention and control groups. Intervention group was given CBT and standard therapy, while control group was given standard therapy only. Study was held in Department of Obstetric and Gynecologic Dr. Moewardi Hospital Surakarta and Prodia Laboratory, in January to March 2015. Independent variable was advanced cervical cancer patients underwent CBT intervention and dependent variables were serotonin level, depression score and quality of life score. The result, serotonin level and quality of life scores were higher after intervention compared with control, (219.43±33,42 vs 89.57±23.23) and (85.13±14.62 vs 41.86±7.24), respectively. Depression score was lower after intervention than without CBT intervention (11.20±4.94 vs 17.00±4.86) and statistically significant (p < 0.05). So, there were effects of cognitive behavioural therapy on serotonin level, depression score and quality of life score in cervical cancer patients and were statistically significant.

Abstract

Delivery, induction, stimulation, cesarean section with the aid of a vacuum extraction, for example, can reduce maternal confidence on the smooth delivery process, as well as improve postpartum stress. These stressors stimulate the HPA axis (hypothalamus-pituitary-adrenal), so that the adrenal cortex produces more cortisol hormone, it can increase postpartum blues. The objective of this study was to analyze the effect of delivery stimulation on protracted labor on cortisol levels and the occurrence of postpartum baby blues. This was an experimental study of non-randomized post-test control group. The subject of the study 30 patients in labor taken consecutive sampling, divided into 2 groups (normal delivery and stimulation) each group consist of 15 patients. In stimulation delivery group, it is examined of cortisol serum levels after five days and then continued for postpartum blues occurrence measurement. Statistical analysis using t-test for differences in levels of cortisol and chi square for analyzing the effect on the occurrence of post-partum blues (a=0.05). Mean  level of cortisol in delivery stimulation group is 40.29 ± 5.58, in normal delivery is 33.59 ± 11.17, with p=0.047, meaning there are significant differences both study groups. Stimulation delivery increases the occurrence of post-partum blues 5.50 times compared to normal delivery (OR=5.50 and p=0.028). Mean cortisol levels on post-partum blues higher at 42.90 ± 6.97 compared to no post-partum blues 30.14 ± 6.66, p=0:00, which means there are significant differences both groups. In conclusion, there was significant relationship between stimulation in protrated labor that increases cortisol serum level and post partum blues incidence.

Abstract

Background: Syphilis is believed to be one of the oldest diseases, first discovered in the late 15th century. Indonesian Ministry of Health reported a total of 76,923 new cases in 2020. WHO has set an ambitious target to reduce the incidence by 90% before 2030, in spite of slow global  response.  This number is lower than the number obtained in 2019, which was 4,169 people. Syphilis can be transmitted through sexual contact, blood transfusion, and vertical mother-to-child transmission (MTCT). It is estimated that the risk of transplacental infection ranges from 20-80%, depending on various factors, such as the stage of infection in the mother. This study aimed to assess the effect of counseling in increasing patient knowledge of syphilis cases in pregnancy. Subjects and Method: This was a quasi experiment conducted at obstetrics and gynecology department in Dr. Moewardi Hospital, Surakarta, Central Java, Indonesia, in June 2025. A sample of 84 pregnant women was selected for this study. The independent variable was counselling. The dependent variable was knowledge about Syphilis in pregnancy. Knowledge was tested before and after counselling. The data were examined using independent t test.             Results: The knowledge score of pregnant women about syphilis after the intervention (Mean= 77.14; SD= 13.85) was higher than before (Mean= 56.43; SD= 15.88), and it was statistically significant (p <0.001). Conclusion: The results of this study indicate that this recovery is effective in improving understanding of syphilis cases in pregnancy in obgyn polyclinic patients at Dr. Moewardi Hospital.
