Abstract

Background: This study focuses on Cesarean section (CS) procedures and the associated risk of surgical site infections (SSIs). The global CS rate is currently at 21%, higher than the recommended 15%, leading to an increased risk of SSIs. These infections are a significant burden on economies, healthcare systems, patients, and their families, despite being preventable. The research aims to address the issue in Uganda, where the burden of SSIs stands at 15.5%. While previous studies have examined various risk factors, such as age, parity, surgeon expertise, and antiseptic solutions, there is a lack of research on pre- and post-CS antibiotic practices and their connection to SSIs. This study seeks to establish the prevalence of SSIs and evaluate the antibiotic practices before and after CS procedures at Mbale Regional Referral Hospital from January 1, 2020, to December 31, 2022.Methods: The design was retrospective cross-sectional. Consecutively selected 104 patient files that developed SSIs and randomly an equal number of patient files that did not develop SSIs to make up a sample size of 208 files that were reviewed at Mbale regional referral hospital (MRRH) from 1st –January -2020 to 31st -December-2022.Result: The prevalence of surgical site infections (SSIs) was 4.3%. Among the 208 patient files reviewed, 71.2% received antibiotic prophylaxis, with intravenous ceftriaxone being the most common (47.1%). However, only 14.9% received prophylaxis within the recommended 30-60 minutes before incision. Post-cesarean section, 92.8% received intravenous ceftriaxone and metronidazole, and the mean hospital stay was 2.74 days. Factors significantly associated with reduced SSI risk were the timing of antibiotic prophylaxis, administration of intravenous antibiotics post-cesarean, oral antibiotics post-cesarean, duration of hospital stay, and the cadre of the surgeonConclusion: The prevalence of surgical site infections (SSIs) at Mbale Regional Referral Hospital (MRRH) was found to be 4.3%. This study highlighted several factors that were associated with a reduced risk of SSIs in women undergoing Cesarean section (CS), including timely administration of antibiotic prophylaxis within 30-60 minutes before the incision, the use of post-CS intravenous (IV) ceftriaxone and metronidazole, hospital stays of 3 days, oral cefixime upon discharge, and being operated on by a medical officerRecommendation: The study emphasizes the importance of medical professionals following guidelines and acquiring knowledge on appropriate antibiotic use to combat surgical site infections (SSIs) following cesarean sections. Adhering to guidelines and continuous education is crucial in preventing SSIs and addressing antibiotic resistance

Abstract

Background: VIA test is one of the early detection programs for cervical cancer in Semarang City. The usage of VIA is only 1.8% and only 42 WUS participated in the Kedungmundu Health Center in 2018.Objective: Objective of the study is to analyze the factors associated with the utilization of IVA examination aged 30-49th.Method: This study uses a quantitative method with a case control approach with 40 women of childbearing age participated IVA as cases and 80 women who have not participated IVA as controls.Results: factors associated to VIA test: fear (p = 0,0001; OR = 8,7; 95% CI = 3.67-20 87), Feeling Embarrassment (p = 0,0001; OR = 6.67; 95% CI = 2.87-15.48), family support (p = 0,0001; OR = 1.05; 95% CI = 0.01-1.38).Conclusion: fear, feeling embarrasement, family support are factors associated with participation in VIA test.

Abstract

Urinary tract infection (UTI) is a bacterial infection that is often found in pediatric patients. The prevalence of infections due to ESBL-producing Enterobacterales is increasing worldwide. This study aims to identify and analyze the factors that influence Extended Spectrum Beta Lactamase (ESBL) producing bacteria in UTI patients at RSUD dr. R. Soetijono Blora. A cross-sectional study has been carried out at RSUD dr. R. Soetijono Blora in January-March 2023. The results of this study showed that 58 respondents with urine culture results found Enterobacterales pathogenic bacteria. Based on the sensitivity test for the Extended Spectrum Beta Lactamase (ESBL) enzyme producer, 39 people (67.2%) were categorized as ESBLs and 19 people (32.8%) Non-ESBLs. The study variables included age, gender, history of catheter insertion, history of stones or urinary tract surgery and history of taking antibiotics in the last 6 months. The chi-square test to determine the relationship between variables was used to determine significant risk factors. Bivariate logistic regression showed that history of catheter insertion and history of use of antibiotics in the last 6 months were significant risk factors. Prevalence of UTIs due to ESBL-producing Enterobacterales in RSUD dr. R. Soetijono Blora is high and the wise use of antibiotics, especially the third generation cephalosporins as therapy, is needed for preventive strategies to reduce antibiotic resistance

Abstract

Background: Dengue fever is an environment-based disease that is endemic in Bandung City. Bandung City with an average temperature of 25.86°C has a humid and cool climate which is a favorable environment for Aedes aegypti. The average amount of rain per day is 21.3 days per month with an average rainfall of 200.4 mm. These environmental conditions favor the breeding of Aedes aegypti. Objective: The study aimed to obtain the dominant variables in the 3M Plus PSN.Penelitian ini merupakan penelitian kuantitatif dengan desain cross sectional. Method: The population in this study were all elementary school students in Bandung City. Through the Cluster Sampling calculation, 973 minimum samples from 13 elementary schools were obtained. With a minimum sample for each elementary school totaling 76 6th-grade students. Results: The results of the study showed that a Knowledge P-value of 0.023, Attitude P-value 0.036, Perception P-value of 0.000, and motivation P-value of 0.018 were associated with DHF Prevention through 3M Plus PSN. Exposure to information media on dengue prevention through the 3M Plus PSN is not associated with dengue prevention through the 3M Plus PSN. The variable that most influenced the prevention of Dengue Fever through the 3M Plus PSN was perception with a P-value of 0.000 CI of 2.118 (1.639-2.737). Conclusion: Knowledge, perception, attitude, and motivation are related to dengue fever prevention through 3M Plus PSN. Perception is the most dominant factor associated with the prevention of dengue fever through the 3M Plus PSN.

Abstract

Based on data from the Semarang City Health Office, the highest rat population in Tembalang District in 2019 was in Tandang Village, with 164 rats caught. In addition, in 2018 there were 4 cases of Leptospirosis in Tandang Village and Leptospirosis was the highest infectious disease in Tandang Village, Tembalang District, Semarang City. This study aims to determine the relationship between the behavioral factors of leptospirosis disease prevention on rat density in Tandang Village, Semarang City. This study is a cross sectional study with a total sample of 64 respondents (RW 03 Tandang Village, Semarang City). The results of the analysis showed that the relative density of rats (trap success) in Tandang village was 28.1% and it was categorized as dense (> 7%) and showed that there was no relationship between knowledge of rat density (α = 0.05. P = 0.068), community attitudes. In the prevention of leptospirosis on rat density (α = 0.05. p = 0.07), there was a relationship between leptospirosis prevention behavior and rat density (α = p = 0.00). Conclusion: There is no relationship between knowledge and attitude in the prevention of leptospirosis on rat density. There is a relationship between leptospirosis prevention practices and rat density in Tandang Village, Semarang City.

Abstract

The increase of COVID-19 cases in Indonesia is in line with the increasing duties of health workers at the point of entry of the country. The task of handling COVID-19 provides psychological pressure and potential to cause depression, stress, and anxiety problems for health workers. This type of research was analytic observational with a cross-sectional design. Data collection was collected by online survey on May to August 2021. The Number of sample in this study was 350 health workers at Health Quarantine Offices in Indonesia. The mental health data collection instrument used DASS-21 questionnaire to check the level of depression, stress, and anxiety. The result showed that there was health workers with moderate to very severe depression in Health Quarantine Offices were 104 respondents (29,7%). The level of anxiety of health workers with moderate to very severe category was 146 respondents (41.7%). Meanwhile, the stress level of health workers with moderate to very severe category was 79 respondents (22,6%). The researcher concludes that the identification of data about the level of depression, anxiety, and stress in health workers at the Quarantine Health Office was needed as an effort to prevent of mental health disorders in health workers. Joint collaborative actions can be pursued by various parties, especially in providing interventions to deal with mental health problems for health workers during the COVID-19 pandemic.

Abstract

Abstract: Cilacap City is an endemic area of dengue fever and in 2020 Tambakreja Village was one of the contributors to high dengue cases with an IR of 150,4/100,000 population. Ovitrap filled with straw attractants can monitor well the density of Aedes sp. because it emits carbon dioxide (CO2) and a characteristic odor emanating from ammonia that is attractive to mosquitoes. The purpose of this study was to determine the population density of Aedes sp. by the ovitrap method of modification of straw soaking water attractants. Methods: Field experimental research using a descriptive approach. Samples of 20 housing units using purposive sampling techniques. Data were obtained from the calculation of eggs trapped in ovitrap and the identification of larvae. Result: The number of eggs obtained is 1458 eggs, with an index of 31.25% (medium category). Aedes aegypti larvae were found more with a percentage of 66.1% than Aedes albopictus larvae with a percentage of 33.9%. Conclusion: The population of Aedes aegypti is more found in ovitrap inside the house with a percentage of 94.3%, while Aedes albopictus in ovitrap outside the home with a percentage of 53.4%. The Health Office and local communities should immediately carry out control by eliminating potential breeding sites and controlling using larvicides.Keywords: Straw Soaking Water, Ovitrap, Aedes aegypti, Aedes albopictus, Cilacap

Abstract

Background: Immunization is a medical process where a person is given a certain vaccine to increase the body's immune response to a particular disease because viruses and bacteria cause many deaths throughout the world. A total of 1.7 million deaths, which contributed 5% to the Indonesian toddler group, are estimated to result in diseases that can be prevented from immunization (PD3I). In 2021, acceptance of complete basic immunization for children under five in Central Java will reach 99% for all types of antigens. However, Temanggung is included in 10 districts that have not achieved the targets set by the Strategic Plan, supported by the occurrence of extraordinary incidents related to measles, diphtheria and hepatitis. The incidence of diseases that can be prevented by immunization in an area is caused by the existence of community groups who have not carried out basic immunization. According to data from the Temanggung District Health Service, there are people who refuse to carry out basic immunizations for babies and toddlers with the main factor being trust. Therefore, further research needs to be carried out regarding the relationship between trust and refusal of immunization in the Tembarak Community Health Center working area. Method: The method used in this research is descriptive observational cross sectional using primary and secondary data Result: The result was that respondents with basic immunization status received more respondents who had supportive beliefs (92.2%) compared to respondents who had unsupportive beliefs, 31.4%) with the chi-square statistical test which showed a p-value <0.001. Conclusion: There is a significant relationship between trust and basic immunization status in children, so solutions such as cross-sector collaboration are needed.

Abstract

Dengue Hemorrhagic Fever (DHF) is still becoming health problem  appeared in Semarang. Ngaliyan district is endemic area of DHF. People's behavior plays a role in the availability of container and the existence of larvae. The purpose of this research is to analize the relation between the knowledge and PSN practice with dengue vector bionomic in Ngaliyan. This research used cross sectional design. Sample was taken using purposive sampling technic based on the highest DHF case. The research sample 300 houses in total includes 100 houses in RW II, 100 houses in RW V, 100 houses in RW VI. The measuring of knowledge variable  and PSN practice used quissioner meanwhile vector bionomic variable used observation paper. Statistic analyse was done using chi square test. The result showed that Ngaliyan district was the area which has high possibility of DHF transmission with HI 30%, Cl 10,7% and BI 39,67. The identification of 593 mosquito in Ngaliyan district, 479 mosquito is Aedes aegypti. A total of 1.112 containers were inspected and 119 larvae positive container. Containers are the most abundant larvae are located in the bathtub in the house.  There was the relation between knowledge about dengue vector and PSN with larva’s existence (p=0,0001). There was the relation between PSN practice with larva’s existence (p=0,0001). The suggestion for all to do PSN practice regularly with assistance so that the existence of larvae in Ngaliyan district progress can be monitored.

Abstract

Diabetes mellitus is a degenerative disease that occurs due to the unbalanced amount of insulin in the body. Diabetes Mellitus is a disease that is hidden before symptoms appear such as hunger, thirst, and frequent urination. Based on data from the Cirebon District Health Office, the number of diabetes mellitus cases in the inpatient hospital in 2015 ranked 10th as much as 1.5%. And in 2018 Pertamina Hospital Cirebon had a total of 250 cases of Diabetes Mellitus patients. The research objective was to describe the characteristics of type 2 diabetes mellitus patients hospitalized at Pertamina Cirebon Hospital. This type of research is a descriptive quantitative approach with a cross-sectional design. The sample size in this study used the Slovin formula and obtained 37 respondents. The results showed 94.6% of the patients were more than 39 years old, 54.1% were male, 43.2% had a high school education background, and 29.7% worked as housewives. As many as 51.4% of the patient's knowledge level was high, 78.3% had a bad patient behavior level, and 81.1% had high patient severity. There is a relationship between age, level of education, occupation, descent, and inpatient DM patients at Pertamina Hospital Cirebon.
