Abstract

Diabetes mellitus has emerged as a prevalent public health issue, experiencing a steady rise across the globe, in both developed and developing nations. Among the chronic complications frequently observed in diabetic individuals is diabetic neuropathy, characterized by recurrent infections, non-healing ulcers, and the potential for finger or toe amputations. These neuropathic complications often prolong the treatment duration, consequently impacting the length of hospitalization for patients. Neuropathic conditions in Type 2 Diabetes Mellitus (DM) result in a prolonged healing process. This research aims to analyze the predictor factors contributing to the Length of Stay (LOS) in neuropathic DM patients. It employs a quantitative research approach utilizing a descriptive correlation methodology. The research was conducted from October 2023 to February 2024 at Indriati Solo Baru Hospital, with a questionnaire serving as the research instrument. The sample in this research comprised patients with Diabetic Neuropathy (DM Neuropathy) selected using the Purposive Sampling technique and totaled 127 respondents. Data analysis included univariate, bivariate, and multivariate analyses. The outcomes revealed that out of the seven factors examined, only three factors were significantly associated with the Length of Stay (LOS) among neuropathic DM patients: control of blood sugar levels, presence of complications in respondents, and presence of comorbid diseases in respondents. The predictor factor that exerted the greatest influence on the Length of Stay (LOS) level in neuropathic DM patients was the presence of comorbid diseases in respondents. The study primarily focuses on identifying predictor factors for length of stay (LOS) in neuropathic DM patients. However, it did not thoroughly control for potential confounding variables that could influence LOS, such as socioeconomic status, access to healthcare, severity of neuropathy, and presence of other comorbidities aside from diabetes. suggestion to the next research to focus on other variables such as healthcare, the severity of neuropathy, and the presence of other comorbidities aside from diabetes.

Abstract

One of the long-term complications of diabetes mellitus is diabetic ulcers (15%) and 85% of the cases are the cause of amputation on patients with diabetes mellitus. Diabetic foot ulcers increase apoptosis and decrease fibroblast proliferation of fibroblasts and inflammatory reactions is elongated, this is as evidenced by the presence of neutrophil granulocytes in large quantities in the wound. Neutrophil granulocytes secrete pro-inflammatory cytokines mainly TNF-α. TNF-α is causing wound healing becomes disconnected and uncoordinated. This paper discussed the role of TNF-α in diabetic foot ulcers.

Abstract

Background: Type 2 diabetes mellitus is a chronic metabolic disorder characterized by persistent hyperglycemia due to impaired insulin secretion and/or insulin resistance. One of the most common long-term complications is impaired peripheral tissue perfusion of the lower extremities, which can progress to diabetic foot ulcers and significantly increase the risk of amputation. Consequently, identifying effective and accessible non-pharmacological interventions to improve peripheral circulation is crucial. Objective: This literature review aimed to evaluate the effectiveness of Buerger Allen Exercise (BAE) in improving peripheral tissue perfusion among patients with type 2 diabetes mellitus. Methods: A literature review was conducted using Google Scholar, PubMed, ScienceDirect, ProQuest, and Elsevier databases. Articles published between 2016 and 2024 were identified using the keywords “Buerger Allen Exercise,” “BAE,” “Type 2 Diabetes Mellitus,” “Peripheral Perfusion,” and “Ankle Brachial Index,” combined with Boolean operators “AND” and “OR.” Inclusion criteria comprised intervention studies published in English or Indonesian that reported outcomes related to peripheral perfusion. Results: Ten studies met the inclusion criteria. Overall, BAE was associated with significant improvements in peripheral perfusion, evidenced by increased Ankle Brachial Index values, improved capillary refill time, enhanced skin temperature and color, strengthened peripheral pulses, and reduced lower extremity pain. Several studies also reported greater benefits when BAE was combined with adjunctive interventions such as vitamin C supplementation, foot spa therapy, or music therapy. Conclusion: Buerger Allen Exercise is a low-cost, feasible, and evidence-supported non-pharmacological intervention that effectively enhances peripheral tissue perfusion in patients with type 2 diabetes mellitus and may help prevent vascular complications and improve patient well-being.
