Abstract

Community Health Care Insurance (JPKM) has been developed since 1992 and it is the embryo of National Social Security System based on Acts No.40 year 2004. Purbalingga is a district in Indonesia has developed JPKM since 2002 and has commitment to reach universal coverage by the year 2012. This research aims to analyze several factors that influence the informal community participation on JPKM Kejobong, Purbalingga. Survey method used cross sectional approach. Sample making through random proportional technique with amount 100 respondents. Data processing attained through quantitative univariat, bivariat and multivariat method. While data analyze used logistic regression with chi square test. The research results show that JPKM coverage among informal community in Kejobong still low (43%) and 57% respondents not enrolled. The reasons not enrolled respondents are inability (42.1%), knowledge (21.1%), poor public health service (22.8%) and others (14%). Factors proved influence are amount of family, knowledge, attitude and cadre role.

Abstract

Background: Pregnant women with HIV positive is potential risk to transmit the virus to their infants. Prevention Mother to Child Transmittion (PMTCT) program is a solution for HIV women if they want to have children. However, most pregnant women detected HIV positive when they have been pregnant for more than 6 months through Provider initiated test and counselling (PITC). On the other hand, the data shows that the increasing number of HIV positive women who have been willing to have children.  This research aims to identify and examine the factors influence the plan of having children among HIV-positive women who access HIV treatment in Kariadi Hospital Semarang. Method: This study employs a quantitative research design with cross sectional approach, followed by qualitative study to explore the feeling and attitudes of women with HIV positive to plan of having children. There were 31 women involved in this study, selected with the criteria of fertile women HIV positive who have been taking antiretroviral therapy in Kariadi Hospital and willing to participate in this study. Qualitative data collected from two women who has been planning to have children. Quantitative data were analyzed by univariate, Chi-Square and multiple logistic regression. Whilst, qualitative data employ content analysis.Results: There were 16% of women with HIV positive who have been intending to have children in this study. The rest have not planned it because they worried if their pregnancy and delivery could transmit HIV to their infants. CD4 cell count, ARV access and women’s attitudes have significant correlation to the intention of having children among women with HIV positive. Comprehensive information about PMTCT and ARV were needed to deliver to fertile women with HIV positive.

Abstract

Stigmatisasi karena HIV dan AIDS telah diakui sebagai salah satu tantangan terbesar untuk meningkatkan perawatan HIV dan AIDS di seluruh dunia. Stigmatisasi bidan pada ibu hamil dengan HIV dan AIDS merupakan hasil keterwakilan dari tiga komponen dalam proses kognitif bidan, yaitu pelabelan ibu hamil HIV dan AIDS, penilaian ringkas mengenai ibu hamil HIV dan AIDS serta budaya dan pengalaman bidan yang didapat dari lingkungan bidan. Tujuan dari penelitian ini adalah untuk mengetahui stigmatisasi bidan pada ibu hamil dengan HIV dan AIDS di Kota Semarang. Penelitian ini menggunakan metode penelitian kualitatif, dengan mengaplikasikan teori sosial kognitif. Subjek penelitian terdiri dari 4 kelompok diskusi, yaitu kelompok diskusi bidan rumah sakit, bidan puskesmas, bidan praktik swasta dan bidan PMTCT. Pengambilan data dilakukan dengan teknik focus group discussion (FGD). Analisis data dengan analisis kualitatif dan diolah dengan cara deskriptif isi. Penelitian ini menunjukkan bahwa stigmatisasi ibu hamil HIV dan AIDS lebih banyak ditemukan di kelompok diskusi bidan praktik swasta dibanding kelompok diskusi lainnya. Sebagian besar bidan praktik swasta menganggap ibu hamil HIV dan AIDS adalah wanita pekerja seks dan seseorang yang memiliki perilaku menyimpang, menganggap ibu hamil HIV dan AIDS memiliki virus mematikan dan membahayakan sehingga bidan akan membedakan pelayanan pada ibu hamil HIV dan AIDS dengan pasien lainnya. Kata Kunci: stigmatisasi, ibu hamil HIV dan AIDS, bidan ABSTRACT Stigmatization By Midwives Toward Pregnant Women With HIV and AIDS In Semarang City; Stigmatization because of HIV and AIDS has been recognized as one of the biggest challenges to improve the care of HIV and AIDS patients worldwide. Stigmatization by midwives toward pregnant women with HIV and AIDS is result of representation of the three components in cognitive process,namely: labeling of pregnant women with HIV and AIDS, evaluation summary of pregnant women with HIV and AIDS ,and supportive knowledge structure about pregnant women with HIV and AIDS. The aim of this study was to know stigmatization by midwives toward pregnant women with HIV and AIDS in Semarang City. This research used qualitative method, with application of social cognitive theory. Subjects consisted of four discussion groups, namely midwives who work in hospital, primary health center, private midwives and PMTCT service . Data collection with focus group discussion (FGD). Data analysis with qualitative analysis and processed by content analysis. This study showed that stigmatization by midwives toward pregnant women with HIV and AIDS were more common in private midwives compared to other discussion groups. Most of the midwives in private practice considered pregnant women with HIV and AIDS is a female sex worker and a person who has deviant behavior, considered pregnant women with HIV and AIDS has deadly and dangerous virus therefore they will differentiate the service in pregnant women with HIV and AIDS with other patients. Abstract

Background: In 2016 Kopeta Health Center was ranked the 4th highest number of cough disease not pneumonia and pneumonia cases from 23 Puskesmas in Sikka District. Health education on pneumonia in this health center has often been conducted, but it seems that the results have not been effective in improving the knowledge of mothers. This is due to the fact that it is only prioritized for mothers who come as their children’s illness and information provision has not been improving the knowledge of mothers on child pneumonia prevention. This study aims to compare the effectiveness of booklets provision and buzz group methods to increase knowledge, attitudes and practices of mothers in the prevention of pneumonia in children.Method: This study employed a quasi experimental design with non-equivalent control group. The sample of this study is the mothers who have children 0-5 years old, in Nangalimang and Kabor villages and 90 mothers were selected by using systematic random sampling technique.  Data was collected using face to face interview by trained enumerators guiding with close-ended questionnaire that has been validated. Data were then analyzed by Chi-Square test and Manova Post Hoc Tukey.Results: Pneumonia lectures followed by giving booklets and buzz group discussion were effective in increasing maternal knowledge and attitudes in the prevention of children pneumonia. Whereas, buzz group were more effective in improving the practice of mothers in the prevention of pneumonia than the provision of booklets. This is because the buzz method makes mothers more actively participate and creative in solving pneumonia problems by looking at other mothers' experiences. It is recommended that using buzz group lectures as an alternative method in efforts to health education for mothers, especially in the prevention of children pneumonia.

Abstract

Background: Semarang City had a highest cases of HIV and AIDS in Central Java. In order to provide health care service for People with HIV and AIDS (PLWHA), three from the five designated hospitals were actively providing Care, Support and Treatment (CST) as a referral hospital for PLWHA in Semarang City. HIV-related discrimination is one of the barrier in the response to HIV and AIDS that influenced PLWHA to seek HIV treatment and lost of HIV treatment because they were afraid of getting stigma and discrimination in health care facilities. Some research have indicated that there were stigma and discrimination by health workers, including doctors. This study aims to find out doctors practices in providing health care to patients with HIV and AIDS in a referral hospital in Semarang City.Method: This study used qualitative methods. The data collecting used indepth interview. The data was collected from 13 doctors as a primary informants and triangulation with 3 HIV team leader.Results: The results showed that the majority of doctors practice was good, they did not discriminate when providing care or treatment to patients with HIV and AIDS. However, there were still a some doctors who discriminate when providing care for patients with HIV and AIDS such as the use of Personal Protective Equipment (PPE) was more strictly when dealing with patients of PLWHA like using double gloves. Most doctors have a good knowledge and positive attitudes towards HIV and AIDS. However, there were some doctors who has less understanding of HIV and AIDS.Keywords : doctor practice, HIV and AIDS patients, hospitals

Abstract

Background: In Pati District KTD cases in adolescents aged <17 years as much as 43.8%, which affectsthe marriage at an early age is increasing. This research aims to describe the response of parents and teens junior who experience unwanted pregnancy, and the effects of marriage at an early age.Method: This research method was qualitative case study design. With a purposive sampling and data collection using in-depth interviews. The main informants were adolescent who experienced unwanted pregnancy 5 people. Informants triangulation parents 5 people.Results: The results showed the phenomenon of early marriage makes parents and teens are not afraid of an unwanted pregnancy, as a result people who previously regarded premarital sex behavior that violates norms, are now more likely to be accepted by society. Parents and teenagers who live rural areas provide a response to resolve the problem by marrying unwanted pregnancy informant. While living urban areas, asking informants to have an abortion, but after failing informant eventually wed. Marriage was a solution unwanted pregnancy experienced by adolescents because of the permissive society towards marriage early age and culture in the community who think if the domestic sector only female duties, so that the nature of women is to be a housewife. Suggested provide information to adolescents and parents about the effects of early marriage. The government particularly Health Provider can further provide youthfriendly health services since the junior level.Keywords: unwanted pregnancy, early marriage, adolescents of junior high school

Abstract

Background: Female Sex Workers (FSWs) is one of the high risk groups of HIV and other Sexual Transmitted Infections. They are engaged in unsafe sex practices with multi partner. Therefore, they should conduct HIV test regularly to prevent them from HIV transmission. This study aims to analyze the factors associated with FSWs’ VCT repeat practices at SunanKuning Brothel.Method: It was a cross sectional study, involved 90 FSWs from total 635 populations.Result: Based on 95% confidence interval and 50% assumed proportion, it was found 71.1% of FSWs had a good belief about VCT and 75.6% had a good value of knowing their HIV status. Most of them (97.8%) were having good support to be re-tested and 86.7% complied with others’ support. The vast majority of FSWs (88.9%) perceived the VCT clinic was wellorganized, only few of them did not satisfy with VCT clinic’s service.Bivariate analysis employed chi-square test showed that belief on VCT (p=0.000), value of knowing their HIV status (p=0.000) and motivation to comply other’s support (p=0.000) were significantly associated with FSWs’ VCT repeat practices. Multivariate analysis found that belief about VCT contributed more than other variables in predicting FSWs’ VCT repeatpractices.Keywords : Female Sex Worker, VCT Repeat Practice, Sunan Kuning

Abstract

Information and Counseling Center for Reproductive Health Youth (PIK KKR) Adolescent Care and Health Services (PKPR) are not fully utilized by adolescents. This study aimed to find out the description of adolescent reproductive health service utilization in Batang Regency. Data were collected through Focus Group Discussion and in-depth interviews involving 42 people in FGD and 12 insider in-depth interview. Utilization of reproductive health services is still low rod. Most teens have not learned of adolescent reproductive health services in Batang. Some teens perceive the existing services are not innovative, boring and not attractive because the use of inappropriate delivery of media, youth service officer for the clinic is not friendly, discriminate-distinguish and not concern for teenagers. Adolescent reproductive health services needed by adolescents is a service which is located in strategic places, special places, the official collaborationbetween physicians, psychologists, substitute teachers, adolescent, with care official, could keep a secret, do not discriminate and competent. The case of unwanted pregnancy requires special services for psychological therapy and involves active participation of service personnel to conduct home visits.

Abstract

Di Kabupaten Magelang kasus HIV positif pada remaja mulai muncul tahun 2008 dan selalu muncul pada tahun berikutnya, demikian juga untuk perkawinan dibawah umur pada tahun 2011 meningkat 3x lipat dari 2009 yang mayoritas disebabkan kehamilan tidak diinginkan. Remaja sangat membutuhkan informasi mengenai seksualitas dan ibu sangatlah penting. Tujuan penelitian ini adalah untuk mengetahui faktor-faktor yang mempengaruhi perilaku ibu dalam memberikan pendidikan seksualitas. Jenis penelitian ini adalah analitik korelasional dengan pendekatan cross sectional. Penelitian ini dilakukan pada bulan Juni s.d. Juli tahun 2012. Sampel sebanyak 92 ibu yang memiliki anak remaja berusia 10-14 tahun dan tergabung dalam BKR Percontohan di Kabupaten Magelang yang dipilih secara cluster sampling. Pengumpulan data dengan wawancara menggunakan kuesioner. Analisis data meliputi univariat, bivariat dan multivariat. Hasil penelitian menunjukkan hanya 44,6% ibu yang berperilaku baik dalam memberikan pendidikan seksualitas. Ada hubungan antara pendidikan ibu (p=0.001), keikutsertaan ibu dalam organisasi kemasyarakatan (p=0.001), tingkat pengetahuan ibu tentang KRR (p=0.000), tingkat pengetahuan ibu tentang pendidikan seksualitas (p=0.000), persepsi ibu akan kemampuan dirinya dalam memberikan pendidikan seksualitas (p=0.000), sikap ibu mengenai pendidikan seksualitas (p=0.002) dengan perilaku ibu dalam memberikan pendidikan seksualitas. Sedangkan variabel yang paling berpengaruh adalah persepsi ibu akan kemampuan dirinya dalam pendidikan seksualitas (p=0.001, OR=6.72).Kata kunci: perilaku, ibu, pendidikan seksualitasABSTRACTIn Magelang HIV positive cases in adolescents began to emerge in 2008 and always comes up in the next year, also in 2011 the marriage under age 3x increase from 2009, the majority due to unwanted pregnancies. Sample of 92 mothers of adolescents aged 10-14 years belonging to the BKR Pilot Magelang selected by cluster sampling. Data collection by interview using a questionnaire. Data analysis includes univariate, bivariate and multivariate analyzes. The results showed only 44.6% of mothers who behaved well in giving sexuality education. There are relationship between mother’s education (p=0.001), the participation of mother in society organizations (p=0.001), mother’s level of knowledge about the ARH (p=0.000), mother’s level of knowledge about sexuality education (p=0.000), mother’s perception of her ability to provide sexuality education (p=0.000), mother’s attitudes toward sexuality education (p=0.002) and mother’s behavior in providing sexuality education. While from the above variables are most influential is mother’s perception of the ability in giving sexuality education (p=0.001, OR=6.72).Keywords: behavior, mother, sexuality education

Abstract

Background : Indonesian young people have been facing significant challenges to their health and well-being. This situation has causing an increased vulnerability to health hazards of various kinds, specially related to reproductive and sexual health, including the growing threat of HIV/AIDS. These adolescent health issues, most of which are preventable, can lead to significant morbidity and mortality.Method : This paper reports on findings from a recent study, which seeks to identify predictor factors of youth sexual behaviour and their need for services, in order to derive practical policy for enhancing youth sexual and reproductive health services. Involving a total 2000 sample derived from youth population aged 18-24 years old. 1000 samples were each randomly selected from factory employers and university students.Result : The findings show that the overall pattern of sexual and reproductive youth health risk is relatively low in comparison to that in many other countries, this is partly related to distinctive and positive characteristics of the culture of Central Java. The findings also showthat Self-efficacy is the strongest predictor of youth sexual behaviour.This study recommends that future policy and program should address how to protect oneself if engaging in sex, rather than only focusing on how to abstain from sex. Such policies and program development should be addressed to the ways to maintaining young people’s positivenorms and values in line with existing culture and religion by enhancing their self efficacy and life-skill through school-based sexual and reproductive health education and services.Keywords : Health services, reproductive and sexual health, sexual behaviour, javaneseyouth.
