Abstract

Tuberculosis (TB) is an infectious disease caused by Mycobacterium tuberculosis (MTB) that lives intracellularly. MTB can inhibit lysosomal enzymes and phagolysosomal fusion, which is challenging to eliminate. These are due to ESAT-6/CFP-10 originating from the RD1 region genome that expresses the Esx-1 type VII secretion system. Esx-1 encodes Esx-A (ESAT-6) and Esx-B (CFP-10), potential vaccine candidates still under research and development. ESAT-6/CFP-10 is predicted to affect macrophage phagocytic activity, IFN-γ, ROS/NO, and IL-10 levels. Several studies have begun to focus on the ESAT-6 antigen due to the high levels of ESAT-6 antibody found in pleural effusion and granuloma fluid. They can last up to 1 year compared to CFP-10 in experimental animals. This study aimed to analyze the effect of ESAT-6 on the phagocytic activity of macrophages, ROS/NO, IFN-γ, and IL-10 in peripheral blood mononuclear cells (PBMCs) cultures of pulmonary TB patients. It is experimental laboratory research with a post-test-only control group design with PBMCs from October 2019 to December 2020 in the Aretha Laboratory Bandung. There were two groups: the negative group (without ESAT-6) and the positive group (with ESAT-6). Six subjects were sampled at the Pindad Hospital in Bandung, and the research was carried out at the Aretha Laboratory in Bandung. Statistical analysis using paired t test. There was a significant difference between the negative and positive groups (p<0.05). ESAT-6 can decrease macrophage phagocytic activity, intracellular ROS/NO, and IFN-γ but increase IL-10 levels.

Abstract

Spiritual health is an aspect that can provide motivation and purpose for someone to achieve their goals even in a difficult situation. One of the factors supporting the success of breastfeeding is the mother’s self-efficacy. Self-efficacy gives mothers confidence and strong motivation even in a difficult situation. This study aimed to determine the correlation between spiritual health and breastfeeding self-efficacy. This analytical correlational study used a cross-sectional approach. The study was conducted in October 2019 in Soreang Sub-district, Bandung Regency, one of the rural areas in Bandung Regency. The subjects of the study were chosen by cluster random sampling method and selected 52 subjects from 13 Posyandu in Soreang sub-districts. The primary data were obtained using instruments in the form of questionnaires. The level of the spiritual health of the subjects was obtained using Spiritual Health Inventory questionnaire. The level of breastfeeding self-efficacy of the subjects was obtained using the Breastfeeding Self-Efficacy Scale. Spearman rank correlation analysis indicated there is a correlation between spiritual health with breastfeeding self-efficacy, (p=0.029 r=0.303). According to the studied data, breastfeeding self-efficacy and spiritual health are correlated among breastfeeding mothers in Soreang Sub-district.

Abstract

Background: Chronic kidney disease (CKD) in children has a long-lived impact, such as an increased risk of bacterial infection. Infection may accelerate disease progression, making early detection crucial. Inflammatory markers typically used for bacterial infection are C-Reactive Protein (CRP) and Procalcitonin (PCT). This study aimed to determine the correlation between levels of neutrophil-to-lymphocyte ratio (NLR) and immature-to-total-neutrophil ratio (I/T ratio) to bacterial infection in children with CKD as indicated by the serum levels of CRP and PCT.Methods: Observational analysis with a cross-sectional design was conducted from January 2019 to November 2021 in children from 3 months to 18 years old with CKD and bacterial infection.  Retrospective data were obtained from medical records at Dr. Hasan Sadikin General Hospital, Bandung. Correlation analysis was performed (SPSS program) at a 95% confidence level, and results were considered significant if the p-value <0.05.Results: There were 42 children, and 57% were female; with a median age of 13 years (range 1–17 years). Most patients had normal nutritional status (55%) although 40% were malnourished. Correlation analysis between I/T ratio and NLR with PCT was positive, with r=0.284 (p<0.05) and r=0.265 (p<0.05), respectively, whereas there was no significant correlation of I/T ratio (r=0,154; p>0.05) and NLR (r=0,188; p>0.05) to CRP.Conclusions: NLR and I/T ratios have a significant positive correlation with PCT levels but not with CRP levels. NRL and I/T ratios can be considered as alternative markers for diagnosing CKD in children with a bacterial infection.

Abstract

Effect of digital food ordering system on timeliness of feeding and patient satisfactionBackground: Computer-based information systems simplify food ordering, speed up the calculation of order data, and reduce operational costs for patient food ordering processes to reduce the time lag between ordering food and serving it to consumers. Only a few studies have examined the effect of digital food ordering applications on feeding accuracy and patient satisfaction.Objective: This study aims to determine the relationship between the digital food ordering system and the timeliness of feeding, and the relationship between timeliness of feeding, taste, menu variety, staff appearance, and food service with patient satisfaction.Methods: This study uses a mixed design design with a parallel concurrent mixed method. Quantitative research uses the one-group pretest-posttest pre-experimental design without a control group design and qualitative research uses the social constructivism–interpretation paradigm. The research was conducted in July 2022 at the Bayu Asih Hospital in Purwakarta. The study population consisted of 91 inpatients using a sampling technique, namely total sampling, while the number of samples for qualitative data was five persons using purposive sampling. Bivariate analysis used the Wilcoxon and Chi-Square tests, while the qualitative analysis used a deductive approach.Results: The average time for serving food with manual ordering tends to be greater, namely 181.5 seconds, which decreases to 156.62 seconds after using a digital food ordering system. Most of patients (73.6%) were satisfied with the timeliness of feeding, the taste of the food provided, the varied menu, and the neat appearance of the servers. The results of the analysis showed a significant relationship between the timeliness of food serving (p=0.006); food taste (p=0.001); menu variety (p=0.001); and the appearance of the waitress (p=0.001) with patient satisfaction.Conclusion: The timeliness of feeding increases after using a digital food ordering system. Timeliness of feeding, food taste, variety of food menus, appearance of food serving staff, and food service are significantly related to patient satisfaction.

Abstract

Permasalahan gizi merupakan salah satu masalah utama pada negara berkembang, termasuk Indonesia. Pendek (stunting) merupakan masalah gizi kronis yang dapat memberikan gambaran kegagalan pertumbuhan yang terakumulasi sejak sebelum dan sesudah kelahiran yang diakibatkan oleh tidak tercukupinya zat gizi. Tingginya angka prevalensi stunting di Kabupaten Tanggamus yaitu 26,8% yang melebihi standar yang ditetapkan oleh WHO yaitu 20% yang artinya stunting baduta di Kabupaten Tanggamus masih diatas batas toleransi yang ditetapkan oleh Badan Kesehatan Nasional. Tujuan penelitian ini untuk mengetahui hubungan praktik Pemberian Makan Bayi dan Anak (PMBA) pada baduta usia 6-24 bulan dengan kejadian stunting. Penelitian ini merupakan penelitian observasional analitik, dengan desain Cross Sectional, dengan jumlah sampel penelitian 100 orang ibu yang memiliki baduta usia 6-24 bulan di Posyandu Desa Purwodadi, Gisting Bawah, dan Banjar Manis wilayah kerja Puskesmas Gisting yang diambil secara consecutive sampling. Pengumpulan data diperoleh melalui pengukuran panjang badan untuk kejadian stunting, wawancara untuk data IMD, pemberian ASI Ekslusif, praktik PMBA, riwayat penyakit ISPA, riwayat penyakit Diare, serta survey konsumsi makan dengan menggunakan food recall 24 hours untuk data riwayat asupan energy, protein dan kalsium, kemudian data diolah dan dianalisis secara deskriptif analitik menggunakan uji chi-square dan uji regresi logistik ganda. Hasil penelitian di tempat penelitian menunjukkan prevalensi kejadian stunting sebesat 26%. Faktor yang terbukti berhubungan dengan stunting adalah praktik pemberian makan bayi dan anak (PMBA) p=0,032, sedangkan IMD p=0,211 dan ASI Ekslusif p=0,283 yang tidak berhubungan dengan kejadian stunting. Dari hasil regresi logistic ganda, faktor yang paling dominan yang berhubungan dengan kejadian stunting adalah PMBA dengan OR 3,27 (1,22-8,57) dengan nilai p=0,018. Praktik PMBA merupakan faktor dominan yang berhubungan dengan kejadian stunting. Promosi dan pendidikan kesehatan tentang PMBA sebaiknya diberikn sejak masa kehamilan untuk mencegah kegagalan pertumbuhan, promosi PMBA yang lebih dekat dengan masyarakat dengan memnfaatkan kader yang telah mengikuti pelatihan konseling PMBA dan membuat panduan PMBA yang dilengkapi resep makanan dan tips yang disesuaikan dengan umur anak.

Abstract

WHO and UNICEF data state that the level of exclusive breastfeeding in the world is below the target of 40%. Breastfeeding self-efficacy was identified as one of the strongest modifiable predictors. This scoping review aimed to analyze the effectiveness of breastfeeding education models to increase breastfeeding self-efficacy and exclusive breastfeeding. The design of this article is a systematic review using the Preferred Reporting Items for Literature Review and Meta-Analyses (PRISMA) method. Article searches used electronic databases from Scopus, Science Direct, PubMed and Sage. The keywords in the article search are "Exclusive Breastfeeding", "Breastfeeding Education", and "Self-Efficacy". The inclusion criteria in this literature review were studies that have samples of pregnant women ≥ 30 weeks, normal pregnancies without complications. Criteria for babies to be born at term and have normal weight. The results of the scoping review show that the most effective breastfeeding education and support models are prenatal counselling, integrated breastfeeding education program and breastfeeding education with grandmothers' attention. The main characteristic is education carried out from antenatal to breastfeeding. Education is provided involving family members. Education is carried out in a multicomponent manner through breastfeeding training, home visits and post-natal telephone follow-up. The theoretical framework uses self-efficacy theory. The minimum educational frequency consists of 2-4 sessions with a minimum length of 40 minutes. It is hoped that the results of this review will serve as a guide for designing an effective exclusive breastfeeding education model by applying the five main characteristics to increase breastfeeding self-efficacy and exclusive breastfeeding.

Abstract

This study aims to develop learning media thorugh a game referred to as edugame, in this case is using quartet cards regarding snacks for school children. This was a development research (R&D) which referred to the ADDIE development model including 4 stages, namely: Analysis to analyze module development needs, Design of the product in the form of module, Development of the module which was further assessed for feasibility by material and media expert for improvement, Implementation of the module as learning and teaching material, and Evaluation as the result which indicated that edugame (quartet cards) was feasible to use. In fact, this study only reached the development process due to time and manpower limitations. The edugame development procedures included the stages of needs analysis, literature study, media selection, preparation of teaching materials, media design, product validation, and final product improvement. The study instruments used in this study were material validation questionnaire, media validation questionnaire and parental acceptance response questionnaire. It was found that according to material experts, the edugame (quartet cards) regarding healthy snacks for students (PJAS) was included in the “very good” category, both in the aspect of material quality and the aspect of material benefit. Furtehrmore, according to media expert, such edugane was included in the “good” category in terms of media quality, language use and media layout aspects. In addition, the acceptance response variable was included in the “Very good” category in terms of ease of use, attractiveness, and efficiency aspects. Thus, based on the results of the assessment results, it can be concluded that the edugame (quartet cards) regarding healthy snacks for students was feasible to use.

Abstract

Stunting is a global health issue that affects physical growth, cognitive development, and future productivity. WHO (2022) reported that 22.3% of children under five years old experience stunting worldwide, with a prevalence of 26.4% in Southeast Asia. In Indonesia, the stunting prevalence is 21.7%, while in Bandung City, it reaches 16.3% (Kemenkes, 2023). One of the efforts to prevent stunting is educating community health cadres on Infant and IYCF as agents of change in society. This study employed a descriptive method with a cross-sectional approach involving 400 community health cadres in Bandung City. The training was conducted using a blended learning approach, combining online and offline sessions over four stages. Evaluation was performed through pretest and posttest assessments to measure knowledge improvement and attitude changes among cadres. The results showed an increase in cadre knowledge from 49.7% before the training to 67.8% after the training. However, negative attitudes toward IYCF increased from 46.2% to 56.4%, likely due to implementation challenges in the field. These findings highlight that while training effectively enhances cadre knowledge, practical application in communities still faces obstacles. Therefore, policy support, cross-sectoral involvement, and more effective communication strategies are needed to enable cadres to optimize their role in stunting prevention.

Abstract

Introduction: Atopy is a genetic tendency of allergic condition with specific clinical manifestations such as atopic dermatitis (AD), allergic rhinitis (AR), and bronchial asthma (BA). Atopic individuals might experience other health problems that coincide with atopic manifestations, including the appearance of recurrent aphthous stomatitis (RAS) which is an oral mucosal disease that can also be influenced by allergies. This study was aimed to be the preliminary study regarding the level of IL-6 serum in atopic patients who were also having RAS. Methods: Thirty subjects were recruited and grouped into three groups with ten subjects each were group of atopic (dermatitis, allergic rhinitis, or bronchial asthma) with RAS, atopic without RAS, and control. Blood specimens were obtained with an approved institutional board review protocol. This research was cross-sectional research with consecutive sampling method conducted at Dr Hasan Sadikin Hospital Bandung. Results: There were significant differences in IL-6 log levels in all three groups (p < 0.001). The higher IL-6 levels in the group of atopic with RAS might be due to the influence of atopy because this condition can increase the acute inflammation of RAS. IL-6 is a pro-inflammatory marker found in the circulation of atopic and RAS patients. Conclusion: Serum IL-6 levels in the group of atopic patients with RAS is higher than in other groups. Keywords: Serum, IL-6, atopic, recurrent aphthous stomatitis

Abstract

Health education in primary healthcare facilities serves as a key factor in facilitating the success of exclusive breastfeeding. Primary healthcare facilities, such as community health centers and clinics, serve as initial points for mothers to understand that exclusive breastfeeding, along with using the right breastfeeding techniques, greatly benefits their baby’s health and growth. Through effective health education programs, healthcare providers can enhance mothers' knowledge and skills in breastfeeding, which in turn can reduce failures in exclusive breastfeeding practices. Research shows that mothers who receive health education on breastfeeding techniques those educated on the topic tend to achieve exclusive breastfeeding success more often than those without such knowledge. A scoping A review approach adhering to the PRISMA-ScR criteria was utilized, with literature searches conducted through scientific literature platforms, for example PubMed, Google Scholar, and Scopus. The results of effective health education in primary healthcare facilities can help achieve national targets for exclusive breastfeeding and improve the health of mothers and children.
