Abstract

Hypertension is one of cardiovascular diseases in geriatrics. Hypertensive geriatric patients require a combination of more antihypertensives to achieve blood pressure targets, it can cause high risk of polypharmacy. Polypharmacy in hypertensive geriatric patients is at high risk of drug interactions that can affect their blood pressure. This study aims to identify the interactions of antihypertensives based on mechanism and severity and also to determine whether there is a change in blood pressure caused by drug interactions in geriatric hypertensive inpatients at RSUD Dr Soedarso on January – June 2019. Data were collected retrospectively from 38 patients’ medical records, using total sampling technique. Data were analyzed descriptively using E-book Stockley’s Drug Interaction and Drug Interaction Checker on medscape.com, drugs.com. There were 28 patients (73%) who may experience drug interactions with a total of 85 cases. Drug interactions with moderate severity and pharmacodynamic interaction mechanism often occur in a row were 61 cases (72%) and 79 cases (93%). There were a decrease and increase blood pressure, which in a row were 23 and 4 patients. The decrease of patients’ blood pressure estimated caused by synergistic drug interactions of antihypertensives and adjustment of each drug dose that has potential to increase blood pressure.

Abstract

Background: In addition to the financial burden that Diabetes Mellitus (DM) places on diabetics and their families due to the complications of Diabetic Foot Ulcers (DFU), treatment costs are escalating. Along with the costs associated with diabetic impairments, direct medical costs may contribute significantly to the financial burden of diabetes.Objectives: This study aimed to determine expenditures associated with DFU, identify cost-influencing factors, and compare the costs of patients with DFU and those without ulcers.Methods: To achieve this objective, this study employed a cross-sectional design and included 198 medical records of DM patients who met the following criteria: age range of 18 to 60 years; type 1 or type 2 DM; receipt of anti-diabetics; payment information; absence of autoimmune or end-stage chronic disease; and absence of corticosteroid. The significance of the cost difference between DFU and non-ulcer patients was evaluated using parametric and non-parametric tests as well as linear regression analysis to determine the cost-influencing factors.Results: This study included 188 patients with type 2 diabetes and ten patients with type 1 diabetes. A total of 131 people with diabetes did not have ulcers, while 67 had a history of DFU. The average direct medical expenses for patients with DFU are IDR 760,146.32, compared to IDR 542,51.24 for patients without ulcer.Conclusion: The presence of ulcers and glycemic conditions had an effect on the direct costs; insulin use also affected the direct costs. The direct costs of diabetic ulcer were significantly higher than non-ulcer, and they were predominantly affected by insulin prescribing.

Abstract

Matriculation is a series of activities to welcome new students and institutional service for new students whereas they are accepted not only from alumni of Tanjungpura University with diverse backgrounds, especially academic and social abilities. This activity is needed to uniform and to harmonize academic skills and support students readiness in Pharmacist Professional Education Study Program at University of Tanjungpura, so the students are expected to pass the educational process and to pass the competency test. In addition, to refresh and to increase the knowledge and understanding of new students when they attend lectures, providing to new students already have a basic concept of knowledge that will be studied in pharmacist professional education. The method used was one group pretest-posttest design. The results of the activity showed that matriculation can increase and affect the value of student learning outcomes significantly with the pretest average score indicator at 46.35933 (scale 0 until 100) to 65.89633 (scale 0 until 100) of post-test average score. The results of filling out the questionnaire indicated that this activity was considered very useful and hoped that the activity would be continuing.

Abstract

Hemodynamic medicines are important to recover patients with a critical period in the ICCU. Hemodynamic medicines have a small range of dose so that it has a large effect on the therapy. Therefore, a study on the use of hemodynamic medicines and doses is necessarily conducted. This study aims to investigate the use and doses of hemodynamic medicines administered to patients in the ICCU of dr. Soedarso Hospital Pontianak. This study is observational research with a descriptive cross-sectional design. The research sample was 49 patients who had met the inclusion criteria. The data were analyzed descriptively to describe the frequency and percentage. The results show that a single hemodynamic medicine is administered to 65% of the patients and a combination of hemodynamic medicines is administered to 35% of the patients. The single medicine refers to dobutamine administered to 84.4% of the patients. Meanwhile, the combination of hemodynamic medicines consists of dobutamine and norepinephrine that are administered to 52.9% of the patients. The doses of hemodynamic medicines frequently administered to patients are 3-5 μg/kg/min dobutamine, 3-10 μg/kg/min dopamine, 0.01-2-2 μg/kg/min norepinephrine, and 1 mg/ml epinephrine. This study concludes that dobutamine (inotropic) is the most frequently used single hemodynamic medicine, and norepinephrine (vasopressor) is the most frequently used combined hemodynamic medicine for the ICCU patients at dr. Soedarso Regional Public Hospital, Pontianak.

Abstract

Self-care practices and appropriate glycemic management may prevent diabetic ulcer, a complication that has a detrimental effect on patients and their families. Through mobile-health applications, educational initiatives can increase patient awareness and promote the early preventative measures required to minimize complications from diabetes. The purpose of this review of the literature is to investigate research on the development of mobile-health applications for people with diabetes. The articles reviewed were obtained through keyword searches in the PubMed and Google Scholar databases within the last 10 years. According to seven published articles, six applications were created as self-care resources for people with diabetes mellitus. In order to prevent diabetic ulcer, mobile-health applications might assist people with diabetes maintain their independence in self-care routines.

Abstract

Tanaman lidah buaya merupakan tanaman khas Kalimantan Barat yang memiliki sifat antibakteri, sehingga pada penelitian ini menjadi bahan baku utama pada sediaan sabun cair. Pada penelitian sebelumnya sediaan sabun cair antiseptik lidah buaya ini berpotensi sebagai antiseptik pada penderita ulkus diabetik, oleh karena itu diperlukan uji iritasi sebelum diberikan kepada penderita. Tujuan penelitian adalah untuk menguji sifat fisikokimia dan menentukan ada atau tidaknya efek iritasi akibat pemberian sabun cair lidah buaya pada partisipan sehat. Desain penelitian untuk uji iritasi adalah penelitian eksperimental one group pre-test and post test design yang melibatkan 12 orang partisipan. Formulasi sabun cair terdiri dari infus kulit daun lidah buaya, minyak jarak, KOH, HPMC, asam stearat, gliserin, BHT, dan akuades. Sabun cair diujikan sifat fisikokimia sebelum dilakukan uji iritasi. Uji iritasi menggunakan metode open patch test dengan mengoleskan satu kali sehari sebanyak 2 mL sabun cair ke daerah tengkuk selama 3 hari berturut-turut. Pengamatan efek iritasi pada 30 menit, 1 hari dan 3 hari setelah pengolesan. Hasil uji iritasi yang diperoleh bahwa tidak terdapat gejala iritasi berupa rasa gatal, kemerahan, kulit bengkak, dan rasa perih pada semua partisipan. Berdasarkan hasil uji tersebut dapat disimpulkan bahwa sabun cair lidah buaya tidak memiliki efek iritasi pada kulit partisipan sehat.
