Abstract

Background: Maternal and neonatal mortality remain significant challenges in low- and middle-income countries, particularly in rural Indonesia, where village midwives serve as the first responders to obstetric emergencies. Despite numerous digital health initiatives, fragmented systems and poor usability often hinder timely emergency management. This study aimed to explore the lived experiences, challenges, and expectations of community midwives in designing an effective maternal–neonatal emergency digital application in Tanah Laut District, South Kalimantan, Indonesia. Methods: This qualitative study employed a Husserlian descriptive phenomenological approach. Ten active village midwives were recruited through purposive sampling; inclusion required at least one year of experience, while those unable to attend or refusing participation were excluded. Data were collected in August 2024 through a focus group discussion (FGD) lasting 120 minutes. Data were analyzed using Colai zzi’s phenomenological analysis method, and reporting followed the COREQ guideline. Results: Five interrelated themes emerged: (1) Maternal emergencies as multidimensional crises, (2) Informal communication as the real emergency network, (3) Fragmented and burdensome reporting systems, (4) Digital and infrastructural barriers in rural practice, and (5) Expectations for an integrated, user-friendly digital solution. These themes progress conceptually from the initial crisis experience, through the mobilization of informal networks due to systemic barriers, culminating in user-driven expectations for digital design. Conclusion: The study highlights midwives’ resilience amid digital fragmentation. Policymakers and developers should adopt participatory co-design approaches involving midwives to ensure the usability and sustainability of maternal emergency digital systems

Abstract

Maternal mortality rate (MMR) and neonatal mortality rate (NMR) in Indonesia are still a challenge in health services. Technological innovations such as the SIGNAL application (Maternal Neonatal Emergency Information System) were developed to improve early detection of high risk in pregnant women and neonates. Objective to determine the impact of using the SIGNAL application for midwives in improving the speed and accuracy of maternal neonatal emergency services in Bantul Regency. Descriptive quantitative research with a cross-sectional approach was conducted on 50 midwives using SIGNAL at the Bantul Regency Health Center. The sampling technique used purposive sampling with a total of 50 respondents who actively used the SIGNAL application. Data were collected through questionnaires and analyzed descriptively. The results showed that 82% of midwives felt helped in early identification of high risk, 76% stated that referrals were faster, and 70% felt that the application increased confidence in making clinical decisions. The main obstacles were internet network and device limitations. The use of the SIGNAL application has a positive impact on improving the quality of maternal neonatal emergency services by midwives in Bantul Regency, although improvements in supporting infrastructure are still needed.

Abstract

Pediatric cancer patients undergoing chemotherapy often experience a range of adverse effects, include discomfort, fatigue, pain, and poor sleep quality. These symptoms can have a detrimental effect on the patients’ physical and psychological well-being. Semi-immersive virtual reality (SVR) has emerged as a promising non-pharmacological approach to enhance comfort and reduce treatment-related distress. To examine the effectiveness of SVR in improving comfort, reducing fatigue and pain, and enhancing sleep quality among pediatrics cancer patients undergoing chemotherapy. A quasi-experimental pretest-posttest study was conducted with 52 children aged 6–14 years receiving outpatient chemotherapy. The sampling technique used is using purposive sampling.  Instruments included the Pittsburgh Sleep Quality Index (PSQI), Shortened General Comfort Questionnaire (SGCQ), Child Fatigue Scale 24 Hours (CFS-24), and Numeric Rating Scale (NRS) for pain. Data were analyzed descriptively to compare pre-intervention and post-intervention scores. The validity test used was KMO–Bartlett and Factor Loading (EFA), while the reliability test used Cronbach's Alpha. Following the SVR intervention, improvements were observed across all variables. The Pittsburgh Sleep Quality Index (PSQI) improved, with “very poor” decreasing from 31% to 0%, “poor” from 37% to 25%, “fair” increasing from 33% to 63%, and “good” from 0% to 12%. Pain intensity (NRS) decreased, with “moderate” pain dropping from 67% to 0% and “mild” pain increasing from 33% to 100%. Fatigue (CFS) improved: “very high” decreased from 25% to 0%, “high” from 48% to 6%, “moderate” from 27% to 0%, and “low” increased from 0% to 94%. Comfort (SGCQ) increased, with “low” decreasing from 31% to 0%, “medium” from 69% to 50%, and “high” increasing from 0% to 50%. Semi-immersive virtual reality effectively enhances comfort, reduces fatigue and pain, and improves sleep quality among pediatrics cancer patients undergoing chemotherapy. SVR can serve as a complementary, non-pharmacological intervention to support children’s well-being the course of cancer treatment.

Abstract

Maternal and neonatal mortality remain significant public health challenges in Indonesia, particularly in regions with limited access to health services. This study presents the MONITA (Monitoring Ibu dan Bayi Tanggap Darurat) application, an Android-based platform designed to enhance early detection of maternal and neonatal complications and strengthen emergency response coordination. MONITA integrates a rule-based screening module following WHO standards, real-time GPS tracking, automatic emergency notifications, and a spatial dashboard for monitoring critical cases. The development process involved system architecture design, UI/UX implementation, prototype testing, and integration of local health data. Preliminary deployment demonstrated that MONITA enables faster identification of high-risk cases, improves communication between field health workers and referral facilities, and supports timely decision-making in emergency situations. The application has the potential to contribute to reducing maternal and neonatal mortality rates and optimizing emergency care in remote or resource-limited areas.

Abstract

Background: Childhood cancer remains a major global health challenge, with leukemia being the most common malignancy among children. Chemotherapy often causes substantial physical and psychological distress that negatively affects children's treatment experiences. Although previous studies have reported that Semi-Immersive Virtual Reality (SIVR) may reduce pain, anxiety, and procedural distress, little qualitative evidence has explored children's treatment experiences by integrating children's own perspectives with parents' narratives. Therefore, this study explored children's treatment experiences during chemotherapy using SIVR across different developmental stages. Methods: A qualitative descriptive study following the Consolidated Criteria for Reporting Qualitative Research (COREQ) guideline employed purposive sampling. Seventy children undergoing chemotherapy participated in the SIVR intervention. Children aged >6 years participated in individual semi-structured interviews, whereas parents of children aged ≤6 years participated in semi-structured Focus Group Discussions (FGDs) as proxy informants. Interviews and FGDs were audio-recorded, transcribed verbatim, and analyzed using Braun and Clarke's thematic analysis. Trustworthiness was established through member checking, data triangulation, and audit trails. Results: Five interconnected themes with several categories described children's emotional, physical, and behavioral treatment experiences: (1) treatment burden and emotional strain, (2) SIVR as emotional escape and comfort, (3) physical and ergonomic limitations, (4) treatment engagement and coping, and (5) acceptability and future expectations. Integrating children's interviews with parents' narratives provided complementary insights across developmental stages, with participants describing greater emotional comfort and coping despite ergonomic challenges. Conclusion: The findings suggest that SIVR may serve as a promising supportive intervention for enhancing children's treatment experiences during chemotherapy. Integrating children's interviews with parents' narratives provides comprehensive evidence to support child-centered pediatric oncology care. Future multicenter qualitative or mixed-methods studies should evaluate ergonomically optimized SIVR to strengthen evidence for broader clinical implementation.

Abstract

Diabetes mellitus is a non-communicable disease that often leads to chronic complications, particularly foot complications that may reduce independence and quality of life among the elderly. One preventive strategy to reduce diabetic foot complications is diabetic foot exercise, which aims to improve blood circulation, joint flexibility, and foot muscle strength. This community service activity aimed to improve the knowledge and skills of elderly people in performing diabetic foot exercises independently in Village Mangir Lor. A descriptive-participatory approach was applied through initial observation, education, demonstration, repeated practice, and evaluation. The participants consisted of six elderly individuals with diabetes mellitus. Diabetic foot exercise training was conducted three times per week for 20–30 minutes using simple media in the form of newspapers. Evaluation was carried out qualitatively by observing participants’ practical skills, participation, and enthusiasm. The results indicated an improvement in participants’ knowledge and ability to perform diabetic foot exercises independently, along with high levels of participation. This activity has the potential to serve as a preventive effort to reduce diabetic foot complications and is recommended for sustainable implementation in community settings.
