Abstract

Children are a vulnerable group at risk of falling during hospitalization due to underdeveloped motor and cognitive functions. Falls can lead to serious injuries, thus requiring accurate assessment and preventive measures. Objektive study aims to describe the application of the Humpty Dumpty Falls Scale (HDFS) for fall risk assessment in pediatric patients at the Asoka Barat Ward of RSUP Dr. Sardjito Yogyakarta. Method used was a descriptive case study involving nine pediatric patients, divided into two groups: with and without medical interventions (BMP/LP). Data were collected through observation, interviews, and documentation review. Results showed that the majority of patients were classified as high fall risk (score ≥12), especially in the group with BMP/LP interventions, where an increase in HDFS scores was noted after the procedures. Conclusion that HDFS is effective in identifying fall risks in children and can serve as a reference for appropriate nursing interventions

Abstract

Background : Hospitalization is an unpleasant experience for children, often causing fear and anxiety. Anxiety in children must be addressed immediately as it can interfere with growth and development. Children need activities to help divert their fear and anxiety so they can manage these feelings effectively. Puzzle play is a beneficial form of play therapy for preschool-aged children. It is a simple associative game that involves assembling and disassembling puzzle pieces. When children engage in puzzle play, their focus shifts away from fear. Objective : To describe the application of puzzle skill play in reducing anxiety in hospitalized children aged 3–6 years in the Aster Barat Ward of Dr. Sardjito General Hospital Yogyakarta. Method : This study used a descriptive method to obtain a factual and accurate picture of the characteristics, facts, and relationships among the phenomena studied. Results : After four interventions over two consecutive days, each lasting 20 minutes, the anxiety scale decreased from an initial score of 41 (moderate) to a final score of 25 (mild). Conclusion : Puzzle skill play therapy is an effective non-pharmacological intervention to reduce anxiety in hospitalized children when applied four times over two consecutive days for 20 minutes per session.

Abstract

Background : Surgery is a medical procedure performed by cutting or opening certain body parts. Sleep disturbance is a common problem often experienced by preoperative patients due to anxiety, pain, and discomfort before undergoing a surgical procedure. Non-pharmacological therapy such as classical music therapy can be an effective alternative to improve sleep quality. Objective : To describe the application of classical music therapy in improving sleep quality in preoperative patients in the Padmanaba Barat Ward of Dr. Sardjito General Hospital, Yogyakarta. Method : The research used is a descriptive case study of one preoperative patient who experienced sleep disturbance. The intervention was carried out by providing classical music therapy for 15-30 minutes before going to bed for five consecutive days. Sleep quality was measured using the Pittsburgh Sleep Quality Indeks (PSQI) before and after the intervention. Results : Shows a gradual increase in sleep quality from a PSQI score of 13 to 5, indicating an improvement from moderate to good sleep quality. Conclusion : from this study is that classical music therapy has been proven effective in improving the sleep quality of preoperative patients and can be used as one of the non-pharmacological interventions in nursing services.

Abstract

Background: Pain is a subjective experience that is difficult to express, especially in children who are not able to express their complaints verbally. Tools such as the Wong–Baker Faces Pain Rating Scale (WBFPRS) can help children communicate pain through facial expressions. Objective: To describe the application of pain assessment using WBFPRS in children in the East Daisy Room of Dr. Sardjito Hospital Yogyakarta. Methods: The study used a descriptive case study design with subjects of 10 children who were hospitalized and experienced pain. Data were obtained through direct observation, semi-structured interviews with parents and medical personnel, and documentation of medical records. The analysis was carried out descriptively. Results: Most children showed moderate pain (60%), severe pain (30%), and severe pain (10%) due to invasive measures such as injection (60%) and IV insertion (10%). WBFPRS makes it easier for nurses to quickly identify pain levels, especially in children who are not able to express complaints verbally. Conclusions: WBFPRS is effective and practical in the assessment of pain in children, as well as improving the accuracy of documentation and nursing interventions. Suggestion: Nurses are advised to use WBFPRS consistently to improve the accuracy of child pain assessments

Abstract

Background : School-age children often experience anxiety and pain from invasive procedures such as injections. Poorly managed pain can cause physical and emotional trauma. Deep breathing relaxation combined with playful techniques like blowing a pinwheel has been shown to divert attention and reduce perceived pain. Objective : To determine the effectiveness of deep breathing relaxation therapy with pinwheel blowing in reducing pain caused by intravenous injection in school-age children. Method : A descriptive case study involving a 12-year-old child experiencing pain from intravenous injection. The intervention was applied twice daily over 3 days. Pain was assessed using the Wong-Baker Faces Pain Scale. Data were collected through observation, interviews, and documentation. Results : The patient's pain scale decreased from an initial score of 8 (very painful) to 0 (no pain) after five therapy sessions. The child showed signs of relaxation and did not cry or resist the procedure by the final day. Conclusion : Deep breathing relaxation therapy with pinwheel blowing is effective in reducing pain from intravenous injection in school-age children. It can serve as a simple, non- pharmacological alternative that is both effective and child-friendly.

Abstract

Background: Pain is a very individual subjective experience that can affect adults and children of all ages. Pain can come from disease processes, injuries, procedures and interventions, routine actions are invasive procedures, namely infusion and blood draws. The intravenous therapy procedure causes acute pain for children. According to data from the World Health Organization (WHO), the incidence of infusion in hospitals is quite high, namely 85% per year. Based on survey data of 120 million people out of 190 million patients treated in hospitals using IVs. Purpose : To determine the description of the application of squishy play therapy to reduce pain in preschool children due to invasive procedures. Method : This case study uses a descriptive method, data collection using this method uses interviews and observations. Result : The results obtained after being given squishy play therapy for 3 consecutive days were able to help reduce the pain scale with a decrease in the pain scale value of 2 every day and the mean value was 2. Conclusion : Squishy play therapy can reduce pain due to invasive procedures in preschool children.

Abstract

Background: Hospitalization in preschoolers causes anxiety in children, if left unchecked, can worsen the child's health condition. One of the therapies that can be done to reduce anxiety is to play therapy with a busy book. Objective: to find out an overview of the application of play therapy with busy book media in reducing anxiety in preschool-age children due to hospitalization in the West Aster Ward of Sardjito Hospital. Methods: Using a descriptive case study of one child who met the inclusion criteria. The intervention was carried out for three days with a total of three times of therapy, and anxiety level measurement using the Face Image Scale (FIS). Results: the application of play therapy with busy book media which was carried out for three days with a total of three therapies was obtained before and after therapy with an initial score of 5 (severe anxiety) with the child's facial expression showing the corners of the lips were very bent under the chin until crying, and after the therapy was carried out play therapy busy book To be a score of 2 (not anxious) with the child's waah expression showing the corner of the lips slightly raised upwards towards the eyes. Conclusion: Play therapy with Busy Book media has been shown to be effective in reducing anxiety levels in preschoolers aged 3-6 years due to hospitalization.

Abstract

Background: Childhood cancer is an abnormal and uncontrolled growth of cells that attacks growth and development in children. Chemotherapy is one of the cancer treatments that causes side effects, such as nausea, vomiting, and fatigue. Monitoring at the chemotherapy stage (pre, intra, and post) is carried out to ensure safety and comfort during therapy. Objective: To find out the results of chemotherapy monitoring in children with cancer in the West Asoka Room of Dr. Sardjito Hospital. Methods: using a descriptive case study method of four children undergoing chemotherapy with the drug Etoposid. Monitoring was carried out in three stages, namely pre, intra, and post chemotherapy. Data was collected through observation, interviews, and documentation studies using monitoring sheets set by the hospital. Results: Of the four respondents who underwent Etoposid chemotherapy, three children (75%) experienced nausea, while one child (25%) experienced no side effects. Side effects such as shock, allergies, or seizures were not found. Conclusion: From the monitoring results, three out of four children experienced nausea. Monitoring provides benefits in maintaining safety during chemotherapy.

Abstract

Background: Postoperative pain is a common problem that can affect patient comfort, healing, and psychological well-being. Effective pain management should not rely solely on pharmacological therapy but also incorporate non-pharmacological techniques such as finger hold relaxation, which has been proven to reduce pain intensity. Objective: This study aims to describe the application of the finger hold relaxation technique in reducing postoperative pain in patients at Alamanda 2 Ward, Sleman Regional General Hospital. Methods: A descriptive case study design was used involving one postoperative appendectomy patient who reported a pain score of 5 according to the Numeric Rating Scale (NRS). The intervention was performed for 15 minutes, twice daily, over three consecutive days. Pain was measured before and after each session. Results: The results showed a gradual reduction in pain from 5 (moderate pain) to 2 (mild pain), accompanied by stable vital signs throughout the intervention period. Conclusion: The finger hold relaxation technique is effective, simple, and safe in reducing postoperative pain and can be considered a holistic, non-pharmacological approach to support patient recovery.

Abstract

Background: Surgery is a medical procedure performed by dissecting or opening a body part. Patients undergoing surgery often experience nursing problems such as anxiety. Preoperative patient anxiety requires nursing intervention. One of the non-pharmacological therapies is guided imagery which aims to reduce anxiety before surgery. Based on 2023 data, services in the operating room of Dr. Sardjito General Hospital reached 131.4% or 24,461 surgical procedures, showing a significant increase compared to the previous year. Objective: Describe the implementation of guided imagery therapy in reducing the anxiety scale in preoperative patients in the West Padmanaba ward of Dr. Sardjito General Hospital Yogyakarta. Method: Descriptive case study on one patient with preoperative anxiety. Guided imagery therapy was carried out for 10-15 minutes, three times until the patient was about to undergo surgery. Data were obtained through measuring the anxiety scale with the Hamilton Anxiety Rating Scale (HARS) questionnaire before and after implementation. Results: After three sessions of therapy, the patient's anxiety scale decreased from 25 to 15. Conclusion: It was found that guided imagery therapy was able to reduce the anxiety scale in preoperative patients. Suggestion: Guided imagery therapy can be used as a non-pharmacological alternative to reduce anxiety in preoperative patients.
