Abstract

Objective: To describe the prompt and successful treatment of an extrauterine pregnancy case in a tertiary-level hospital in Indonesia. Methods: We report a case of prompt and successful management of unruptured tubal pregnancy in the dr. Soetomo General Hospital Surabaya. Results: A 32-years-old woman presented with lower abdominal pain and vaginal bleeding one days before admission. She was sexually active, used no contraceptives, and had a history of one miscarriage. On examination, she was hemodynamically stable. A bimanual exam revealed cervical motion tenderness and pain. A high human chorionic gonadotropin (hCG) level (1,725 IU/L) and a left-sided mass, highly suspected as an extrauterine gestational sac confirming a 6 week, 1 days age of pregnancy without sign of free fluid in the abdomen nor fetal heart rate on ultrasound, prompted diagnostic laparoscopy. We found a tubal pregnancy located on the ampullae of the left fallopian tube with minimal hemoperitoneum (50 ml). A chromopertubation test was done to ensure a patent right fallopian tube, so we did a salpingectomy. The patient recovered well and was discharged home on day 2 post-procedure. Conclusion: Early diagnosis is vital and feasible to prevent morbidity and mortality in women with ectopic pregnancy. All sexually active women complaining painful abdomen or vaginal bleeding must be examined for an ectopic pregnancy to enable early diagnosis and prompt treatment. A laparoscopic surgery done by a trained individual provided a safe and minimally invasive intervention to this case. Abstract

High-risk pregnancy is a pregnancy that can cause harm and complications to the mother and fetus during pregnancy, childbirth or after delivery compared to normal pregnancy and childbirth. The aim of this research is to determine "Factors Associated with Husband's Involvement in Antenatal Care Visits for High Risk Pregnant Women". The design in this research is cross-sectional. The sample in the study was 187 people. The validated questionnaire was used in collecting data for this research. and analyzed univariately using frequency distribution, bivariate using chi-square test, and multivariate using logistic regression test. The research results showed that age (p=0.875), education (p=0.004), income (p=0.015). Based on the multivariate results, it was found that the most dominant factor related to husband's involvement in antenatal care visits was education with the highest OR value of 0.139.

Abstract

The Long Term Contraceptive Method (MKJP) is an effective contraceptive method used to plan, postpone, and terminate pregnancy. MKJP is considered effective because the failure rate is low compared to non-MKJP contraception. The factor of couples of childbearing age in using contraception cannot be separated from the behavioral factors of each individual. This research aims to determine the relationship between age and the behavior of using MKJP among active family planning acceptors in the family planning village program in Sungai Penuhcity. The design of this research is a cross-sectional study. The sample in this study was 354 people. Data was collected using a questionnaire and analyzed using chi-square. The research results showed a relationship between age (p= 0.003) and MKJP usage behavior.

Abstract

Background: Endometrioid endometrial carcinoma (EEC) is a prevalent gynecological malignancy whose prognosis is influenced by factors including histopathological grade and lymphovascular invasion (LVI). Angiogenesis, crucial for tumor growth and metastasis, is significantly mediated by vascular endothelial growth factor (VEGF). This study aimed to investigate the expression of VEGF in EEC and its correlation with histopathological differentiation and LVI. Methods: This observational analytical study employed a cross-sectional design using 36 archival paraffin block samples of EEC diagnosed between January 2022 and December 2024 at the Anatomical Pathology Laboratory of Dr. M. Djamil General Hospital Padang. Cases were selected via simple random sampling from a population of 59. Histopathological grade (Grade 1, 2, or 3 based on FIGO architectural and nuclear criteria) and LVI (negative, focal, or substantial) were re-evaluated from Hematoxylin-Eosin (H&E) stained slides. VEGF expression was assessed by immunohistochemistry, scored semiquantitatively based on the percentage of positive tumor cells and staining intensity, and categorized as low or high. Data were analyzed using Chi-square tests, with p<0.05 considered statistically significant. Results: The mean age of patients was 54.36 years, with the highest prevalence in the 51-60 age group (41.7%). Grade 3 tumors were most common (38.9%), followed by Grade 2 (33.3%) and Grade 1 (27.8%). LVI was present in 47.2% of cases, predominantly focal (38.9%). High VEGF expression was observed in 58.3% of EEC cases. A statistically significant association was found between high VEGF expression and higher histopathological grade (p=0.000), with 66.7% of Grade 3 tumors showing high VEGF expression. No significant association was found between VEGF expression and LVI (p=0.080). Conclusion: High VEGF expression significantly correlated with higher histopathological grades in EEC, suggesting its role in tumor aggressiveness and dedifferentiation. However, a significant association with LVI was not established in this cohort. VEGF expression warrants further investigation as a potential prognostic biomarker and therapeutic target in EEC.

Abstract

Pendahuluan: Depresi postpartum merupakan gangguan emosional yang diakibatkan oleh mekanisme koping yang tidak memadai selama periode postpartum. Jika tidak ditangani, depresi dapat meningkatkan risiko suicide maupun infanticide. Kejadian depresi postpartum sering kali dikaitkan dengan rendahnya dukungan dari suami dan adanya Kekerasan Dalam Rumah Tangga (KDRT), yang keduanya dapat memperburuk kondisi psikologis ibu. Tujuan: Penelitian ini bertujuan untuk menganalisis terjadinya depresi postpartum dan hubungannya dengan dukungan suami dan KDRT. Metode: Desain penelitian kuantitatif dengan pendekatan cross sectional yang dilakukan di Wilayah Kerja Puskesmas Andalas Kota Padang dari Juni 2023 hingga Mei 2024. Sampel penelitian terdiri dari 152 ibu dengan bayi antara 4 minggu dan 6 bulan. Data dikumpulkan menggunakan kuesioner terstruktur, dan analisis statistik dilakukan menggunakan uji chi-square. Hasil: Hasil penelitian menunjukkan bahwa kejadian depresi postpartum di Wilayah Kerja Puskesmas Andalas Kota Padang sebesar 24,3%. Selain itu, ditemukan hubungan yang signifikan antara depresi postpartum dengan dukungan suami (nilai p 0,000) dan juga ditemukan hubungan yang signifikan antara depresi postpartum dengan kekerasan dalam rumah tangga (nilai p 0,000). Kesimpulan: Temuan ini menggarisbawahi hubungan yang signifikan antara depresi postpartum dan faktor-faktor seperti dukungan suami dan KDRT. Temuan ini menyoroti pentingnya hubungan suami istri yang suportif dan pencegahan kekerasan dalam rumah tangga dalam mengurangi kejadian depresi postpartum

Abstract

Introduction : Ovarian cancer is the 8th most common cancer in women. High grade serous carcinoma (HGSC) is a subtype of ovarian cancer with the highest case and death rate compared to other types. Therefore, a preventive approach is needed by reviewing the underlying risk factors of this cancer. Body mass index (BMI) and parity are modifiable risk factors for ovarian cancer. This study aims to determine the relationship BMI and parity on high grade serous subtype ovarian cancer at RSUP Dr. M. Djamil Padang. Method: observational analytic study with a cross sectional approach. This research was conducted at RSUP Dr. M. Djamil Padang using medical record data of epithelial ovarian cancer patients at RSUP Dr. M. Djamil Padang from 2021 to 2022 with a sample of 129 patients. The data analysis method used in this research is the Chi-square test. Result : Epithelial ovarian cancer patients are most found at >50 years old (55%), menopausal woman (65.9%), non obese (77.5%) and  parous women (76%). Conclusion : There was no relationship between body mass index and parity with the incidence of ovarian cancer, subtype high grade serous, at RSUP Dr. M. Djamil Padang.

Abstract

Introduction: Ovarian cancer is a significant cause of death in women. One of the management of advanced ovarian cancer patients is chemotherapy. The effects of administering Platinum-Taxane-Based chemotherapy to ovarian cancer patients show side effects of myelosuppression, one of which is leukopenia. Methods: This type of study is a correlation analytical study with a cross-sectional design from October 2023 to May 2024 at Dr. M. Djamil Central General Hospital Padang. The patients included were ovarian cancer patients who underwent Platinum-Taxane-Based chemotherapy with laboratory results of leukopenia. Results: From 70 patients, it was found that 37 patients (52.9%) had grade I leukopenia, 27 patients (38.6%) had grade II, and 6 patients (8.6%) had grade III. Based on the results obtained, in grade I leukopenia, the average vitamin D value is 13.162 ng/mL, grade II has an average vitamin D value of 11.851 ng/mL, grade III has an average vitamin D value of 14.333 ng/mL. The study results showed a relationship between leukopenia and vitamin D levels p = 0.034 (p < 0.05). Conclusion: There was a relationship between leukopenia and vitamin D levels in ovarian cancer patients undergoing Platinum-Taxane-Based Chemotherapy at Dr. M. Djamil Central General Hospital Padang.

Abstract

Introduction : Endometrial carcinoma is a malignant epithelial tumor that forms in the uterus's inner lining, or endometrium. The average age at diagnosis is 61 years, with cases diagnosed after 50 years more than 90%. Endometrial carcinoma is rare in young women, usually defined as occurring under age 50 or menopause, with rare cases occurring under age 40. Case Report : A 35-year-old woman nulliparous complaints of abdominal pain since 3 months ago and complained of weight loss. Based on the anamnesis, the patient is not married. On physical examination, revealed an area of firmness in the suprapubic region associated with pain on palpation. On ultrasound examination, the uterus was found to be anteflexed, with an inhomogeneous appearance, and a solid cystic mass along the uterus. Adnexa measuring 10.02 x 10.20 x 12.02 cm, vascular scale 4, not clearly defined. The patient was diagnosed with suspected uterine corpus carcinoma. Result : In this patient, a total hysterectomy and bilateral salpingo-oophorectomy were performed because the patient did not want fertility. Conclussion : In young women who want to preserve fertility, conservative management is often implemented. But, when fertility-sparing treatment is not considered, the rate of recurrence and progression is considerably low in this case. Ultimately, although fertility-sparing treatments are an attractive alternative to surgery resulting in permanent loss of fertility, unfortunately, they can only be applied in a subset of cases. The standard surgical procedure recommended is a total extra-fascial hysterectomy with bilateral salpingo-oophorectomy. Keywords: Uterine Corpus Endometrial Carcinoma, Young Women

Abstract

Background : Inferior caval syndrom is a rare case. It can caused by compresssion of the tumor mass and paraaortic metastastasis lession of ovarian dysgerminoma. Dysgerminoma is one of the most common germ cell tumor, approximately 3-5% of all ovarian cancers.1 Rate of dysgerminoma metastasis to lymph node is around 28%. In all type of germ cell, dysgerminoma has higher insidens to lymph node metastasis compare to the others. The first level of lymph node metastsis is paraaortic lymph node. However, the insidens is unknown because there is no research about it yet.2,3 Dysgerminoma frequently in young age, less then 30 years. The exact etiology of dysgerminomas has not been determined, although recent molecular studies have implicated loss function of potential tumor suppressor gene TRC8/RNF139, abnormality of Y chromosome and gonadal dysgenesis.4 The problem arises when dysgerminoma is diagnosed in pregnancy woman as clinically has inferior vena cava syndrom sign and symptoms which acquired efective and fast management.Objective : Discuss inferior caval syndrom as effect of compression by tumor mass, metastasis tumor lesion and chemotherapy management in dysgerminoma with pregnancyCase Report : Patient 32 years old admitted to M. Djamil central general hospital referred from Batusangkar hospital with diagnosis G3P2A0L2 25-26 weeks of preterm pregnancy + dysgerminoma + obs. Dyspnue. Patient has symptoms dispnue, exercise intolerance, oedem extremity. Physical examination : blood pressure was 100/70 mmHg, HR 120 dpm, RR 35 bpm, T 37 C. Abdomen : uterus fundal was palpated 2 fingers above umbilical, ballotement (+), FHR 150-158 bpm. Genitalia : I V/U normal, vaginal bleeding (-), oedem extremity +/+. From ultrasonography found dysgerminoma ovary with compression to inferior vena cava and paraaortic metastatic. Patient was diagnosed with dispnue ec. inferior vena cava syndrom caused by compression of tumor mass + paraaortic lymph node metastasis lession of ovarian dysgerminoma on G3P2A0L2 25-26 weeks of preterm pregnancy. Patient was managed by BEP chemotherapy and symptom was dissapeared. Patient was admitted to M. Djamil again with 34-35 weeks of preterm pregnancy in active phase of first stage. Because obtructed of labor patient was decide to performed LSCS. Female baby was born 1800 gram, baby’s length was 45 cm, A/S 7/8, there is no congenital anomaly. Patient was followed up 1 month after surgery, there is no symptoms, but fromUltrasonograhy found corpus metastasis and CT scan impressed paraaortic lymph node metastasis + copus metastasis. Chemotherapy was continue one month after caesarean section. She was completed 4 cycles chemothrapy and the disease was cureable.Conclusion : Caval syndrom is a rare case and fatal, caused by direct compression of dysgerminoma mass and paraaortic lymph node metastasis. Diagnosis of dysgerminoma is anatomy pathology diagnose. Management of dysgerminoma is surgical staging and 3-4 cycle of chemoterapy with BEP regimen. Chemoterapy with BEP (Bleomisin-Etopuside-Cisplatin) is safe to performed at second trimester with inferior vena cava syndrom ec paraaorta lymph nodes metastasis.          Dysgerminoma has a good response to chemotherapy with survival rate 96%Keywords: Caval syndrom, lymph node paraaortic metastatic
