Abstract

Pengawas Menelan Obat (PMO) pada pasien TB Paru adalah seseorang yang dipercaya untuk memantau penderita TB paru untuk minum obat secara teratur. Untuk mendukung strategi DOTS, dibutuhkan kedisiplinan dari penderita TB itu sendiri dalam pengobatannya. Penelitian ini bertujuan untuk menganalisis peran Pengawas Minum Obat (PMO) terhadap keberhasilan pengobatan di wilayah kerja Puskesmas Bulango Ulu. jenis penelitian ini kuantitatif dengan pendekatan Studi Retrospektif. juumlah sampel sebanyak 40 responden. Dari hasil analisis PMO yang berperan baik 21 orang (52,5%), berperan cukup 11 orang (27,5%) dan berperan kurang 8 orang (20%). Sedangkan pasien yang berhasil menjalani pengobatan 29 orang (72,5%) dan yang gagal/kabuh 11 orang (27,5%). Dari hail statistik menggunakan uji chi-square diketahui nilai p value sebesar 0,000 (< α 0,05) sehingga dapat di artikan terdapat hubungan signifikan Peran Pengawas Minum Obat (PMO) terhadap Keberhasilan Pengobatan TB paru di Wilayah Kerja Puskesmas Bulango Ulu. Diharapkan kepada petugas kesehatan untuk sering melakukan kunjungan rumah pasien TBC secara berkala sebagai dukungan dan pengawasan terhadap pengobatan pasien.

Abstract

Bencana adalah peristiwa atau rangkaian peristiwa yang terjadi secara mendadak tidak terencana secara perlahan tetapi berlanjut yang menimbulkan dampak terhadap pola khidupan normal atau kerusakan sehingga diperlukan tindakan darurat dan luar biasa untuk menolong dan menyelematkan korban baik manusia maupun lingkungannya. Tujuan Penelitian untuk Mengetahui Gambaran Pengetahuan Perawat Tentang Tanggap Bencana Banjir. Desain penelitian kuantitatif descriptif dengan pendekatan Cross Sectional. Pengambilan sampel menggunakan teknik Purposive Sampling dengan 40 responden. Pengumpulan data menggunakan lembar kuesioner. Hasil penelitian diperoleh bahwa yang memiliki pengetahuan baik sebanyak 22 responden (55,0%), dan pengetahuan cukup sebanyak 18 responden (45,5%). Kesimpulannya pengetahuan perawat tentang tanggap bencana banjir di RSUD Toto Kabila berada pada tingkat yang baik.

Abstract

ABSTRACT Head injury is a traumatic disturbance of brain function accompanied or without interstitial bleeding which includes trauma to the scalp, skull and brain. Complications that occur in head injuries are increased intracranial pressure, namely the pressure that occurs in the cerebral space due to an increase in brain volume that exceeds the tolerance threshold in the cranial space. Head injury treatment can be done by controlling intracranial pressure. Surgical intervention by providing a 30 degree head position can be given to head injury patients. The aim of the research is to analyze the 30 degree head up position on the prevention of ICT. The research method used pre-experimental design with one group pre and post test. The sample in this study consisted of 10 respondents. The results of the research showed that the highest pre-intervention intracranial pressure was obtained by GCS, namely Mild Head Injury, 5 respondents, the highest blood pressure, namely Stage 1 Hypertension, 4 respondents, the highest pulse category, namely bradycardia, 5 respondents, and the highest pain scale, namely mild, 5 respondents. Meanwhile, post-intervention intracranial pressure experienced the most changes obtained for GCS, namely Mild Head Injury, 7 respondents, the highest blood pressure, namely Pre Hypertension, 5 respondents, the highest pulse category, namely normal pulse, 5 respondents, and the lightest pain scale, namely 6 respondents. Based on the results of parametric and non-parametric statistics, it was found that all components of intracranial pressure had an influence after intervention, namely GCS with a p-value of 0.003, blood pressure with a p-value of 0.000, pulse with a p-value of 0.001, headache with a p-value of 0.000 where < 0.05, which means it has a significant effect after being given the intervention. For this reason, applying a 30 degree head up position can increase blood supply to the brain thereby reducing the risk of increasing intracranial pressure due to thrombolytic stroke as well as clinical complications for the patient. Keywords: Head Injury, Head Up 30 Degrees, ICT  ABSTRAK Cedera kepala adalah gangguan traumatik dari fungsi otak yang disertai atau tanpa perdarahan intersisial yang meliputi trauma kulit kepala, tengkorak, dan otak. Komplikasi yang terjadi pada cedera kepala adalah peningkatan tekanan intrakranial, yaitu tekanan yang terjadi pada ruang serebral akibat bertambahnya volume otak melebihi ambang toleransi dalam ruang cranium. Perawatan cedera kepala dapat dilakukan dengan mengontrol tekanan intrakranial. Intervensi keperawatan dengan memberikan posisi kepala 30 derajat dapat diberikan pada pasien cedera kepala. Tujuan penelitian yaitu untuk menganalisis posisi head up 30 derajat terhadap pencegahan TIK. Metode penelitian menggunakan pre-experimental dengan desain one grup pre dan post test. Sampel pada penelitian ini berjumlah 10 respoden. Hasil penelitian menunjukkan tekanan intrakranial pre intervensi didapatkan untuk GCS terbanyak yaitu Cedera Kepala Ringan sebanyak 5 responden, tekanan darah terbanyak yaitu Hipertensi Tahap 1 sebanyak 4 responden, kategori nadi terbanyak yaitu nadi bradikardi sebanyak 5  responden, serta skala nyeri terbanyak yaitu ringan sebanyak 5 responden. Sedangkan tekanan intrakranial post intervensi mengalami perubahan didapatkan untuk GCS terbanyak yaitu Cedera Kepala Ringan sebanyak 7 responden, tekanan darah terbanyak yaitu Pre Hipertensi sebanyak 5 responden, kategori nadi terbanyak yaitu nadi normal sebanyak 5  responden, serta skala nyeri terbanyak yaitu ringan sebanyak 6 responden. Berdasarkan hasil statistik parametrik dan non parametrik didapatkan bahwa semua komponen tekanan intrakranial memiliki pengaruh setelah dilakukan intervensi yaitu GCS dengan p-value 0,003, tekanan darah dengan p-value 0,000, nadi dengan p-value 0,001, nyeri kepala dengan p-value 0,000 dimana < 0,05 yang artinya memiliki pengaruh secara signifikan setelah diberikan intervensi. Untuk itu pada penerapan posisi head up 30 derajat dapat meningkatkan suplay darah ke otak sehingga menurunkan resiko peningkatan tekanan intracranial akibat stroke trombolitik maupun komplikasi klinis pasien. Kata Kunci: Cedera Kepala, Head Up 30 Derajat, TIK

Abstract

Objective: Patients who are on a mechanical ventilator can increase the risk of nosocomial infections because this is a potential complication that can occur, including patients experiencing Ventilator Associated Pneumonia (VAP). The aim of this study was to analyze the effect of VAP bundle intervention in preventing VAP in patients on mechanical ventilators in the ICU Room at Prof. Hospital. Dr. H. Aloei Saboe Gorontalo City. Methods: The research method used is pre-experimental research with one group pre-post test design to carry out CPIS (Clinical Pulmonary Infection Score) measurements to diagnose VAP. Results: The results of this study were obtained before the VAP bundle intervention in patients on mechanical ventilators in the ICU, the average CPIS score was 4.75. After the VAP bundle intervention, the patient was placed on a mechanical ventilator in the ICU Room at Prof. Hospital. Dr. H. Aloei Saboe, Gorontalo City, the average CPIS score is 3.42. The results of the Wilcoxon statistical test obtained a p-value of 0.001 (<α 0.05), meaning that there is an influence of VAP bundle intervention in preventing VAP in patients on mechanical ventilators in the ICU room at the Regional Hospital Prof. Dr. H. Aloei Saboe Gorontalo City. Conclusion: From this research data, the implementation of VAP prevention measures and the patient's disease condition is supported by the provision of pharmacological therapy according to the dose.Elevation of the head 30-45° can reduce the occurrence of oropharyngeal and gastrointestinal aspiration and increase the patient's lung volume and lung ventilation. Oral hygiene with antiseptic chlorhexidine 0.02% as an inhibitor of dental plaque formation and gingivitis.

Abstract

Pelayanan prima ialah suatu usaha untuk melayani kebutuhan orang lain dengan memberikan yang terbaik. Penelitian ini bertujuan untuk mengetahui hubungan pelayanan prima dengan tingkat kepuasan pasien. Metode penelitian ini yaitu deskriptif dengan rancangan cross sectional. Pengambilan sampel menggunakan non probability sampling. Hasil penelitian ini adalah responden yang menilai pelayanan prima kategori prima sebanyak (45,5%) responden, kategori cukup prima sebanyak (54,5%) responden. Kepuasan pasien kategori puas (45,5%) responden  dan kategori cukup puas sebanyak (54,5%) responden. Hasil menunjukan ada hubungan antara pelayanan prima dengan tingkat kepuasan pasien di ruang rawat inap interna RSUD dr. Hasri Ainun Habibie Provinsi Gorontalo

Abstract

Flood disasters are one of the major threats in Gorontalo Province, with significant impacts on communities and the health service system. Nurses, as frontline health workers, play a strategic role in community-based emergency response, particularly in providing first aid, triage, and health education. This study aims to analyze the preparedness level of community-based nurses in responding to flood disasters in Limboto Barat District, Gorontalo Regency. A descriptive quantitative design with a cross-sectional approach was applied. The sample consisted of all 30 nurses working at Limboto Barat Public Health Center, selected through a total sampling technique. Data were collected using questionnaires and analyzed univariately. The results showed that most respondents were aged 31-40 years (36.7%), female (73.3%), had a bachelor’s degree in nursing (56.7%), and had more than five years of work experience (60%). Nurses’ preparedness was still low in terms of knowledge, with 73.3% categorized as poor. Regarding attitudes, 60% of respondents showed poor preparedness, while skills were mainly in the fair (46.7%) and poor (46.7%) categories. These findings indicate that community-based nurse preparedness is still not optimal, especially in the dimensions of knowledge, attitude, and skills. In conclusion, structured interventions through continuous training, disaster simulations, and cross-sectoral collaboration are required to strengthen nurses’ preparedness capacity in responding to flood disasters.

Abstract

Bencana menjadi salah satu isu kontemporer dalam ranah pergaulan internasional yang membutuhkan perhatian khusus, karena dampak yang ditimbulkan mengakibatkan penderitaan bagi masyarakat baik berupa  korban  jiwa  manusia, kerugian  harta benda maupun kerusakan lingkungan. Tujuan dalam pnelitian ini untuk menguraikan faktor kesiapsiagaan tenaga kesehatan dalam tanggap kegawatdaruratan bencana banjir di Puskesmas Tibawa Kabupaten Gorontalo. Penelitian ini dilakukan bulan mei–September 2020 diwilayah kerja puskesmas tibawa. Desain penelitian ini menggunakan tipe penelitian survey untuk mendiskripsikan informasi informasi dengan pendekatan penelitian analitik kesehatan. Sampel dalam penelitian ini sebanyak 69 responden dengan tehnik total sampling dengan menggunakan kusioner pegetahuan, sikap dan kesiapsiagaan. Hasil analisis didapatkan pengetahuan baik dengan kesiapsiagaan tidak siap sebanyak 2 orang dan siap sebanyak 27 orang dan pengetahuan kurang dengan kesiapsiagaan tidak siap sebanyak 39 orang dan siap sebanyak 1 orang sedangkan sikap baik dengan kesiapsiagaan tidak siap sebanyak 60 orang dan siap sebanyak 3 orang dan siakp kurang dengan kesiapsiagaan tidak siap sebanyak 4 orang dan siap sebanyak 2 orang. Kesiapsiagaan tenaga kesehatan sangat perlu dilakukan untuk dapat meningkatkan kemampuan dalam penanganan bencana banjir. Kata Kunci: Kesiapsiagaan, Tenaga Kesehatan, Bencana Banjir

Abstract

The Glasgow Coma Scale (GCS) is included in the primary survey carried out when a patient enters the emergency room which is usually assessed by the emergency room nurse and can prevent complications so that it requires a proper GCS examination. The purpose of this study was to determine the relationship between the level of knowledge of nurses and the accuracy of the assessment of the Glasgow Coma Scale in patients with head injuries in the emergency room of Prof. Dr. H. Aloei Saboe Hospital, Gorontalo City. Quantitative research method with a cross sectional study approach, the population in this study was all implementing nurses in the emergency room of Prof. Dr. H. Aloei Saboe Hospital, Gorontalo City, a research sample of 29 nurses with a total sampling technique, research instruments using knowledge questionnaires and GCS assessment observation sheets, data were analyzed by Chi-square test. The results of the study obtained the level of knowledge of nurses about the GCS assessment which was good knowledge as many as 19 nurses (65.5%) and sufficient knowledge as many as 10 nurses (34.5%), then the accuracy of the right GCS assessment was 21 nurses (72.4%) and inappropriate as many as 8 nurses (27.6%), and a p-value of 0.000 (? ? 0.05) was obtained. It can be concluded that there is a relationship between the level of nurse knowledge and the accuracy of GCS assessment in head injury patients in the emergency room of Prof. Dr. H. Aloei Saboe Hospital, Gorontalo City.

Abstract

BBLR is a health problem that is still a major concern in the world, including Indonesia, because BBLR babies have a high risk of experiencing disturbances of physiological functions including the respiratory system, cardiovascular and body temperature regulation which if not above has a serious impact on the baby's condition. The purpose of the study was to determine the effect of tactile touch on changes in vital signs in low-birth weight babies (BBLR) in the NICU room of dr. MM. Dunda Limboto. Quantitative research method with a pre-experiment design with a one-group pre-test and post-test design approach, the population of this study was all BBLR babies, a research sample of 20 infants with purposive sampling technique, the research instrument used a vital sign observation sheet, the data was analyzed by the Wilcoxon test. The results of the study were obtained before being given tactile touch, the majority of hypothermic body temperature was 11 infants (55%) and an average of 36.295OC, normal heart rate was 11 infants (55%) and an average of 153x/minute, and normal oxygen saturation was 15 infants (75%) and an average of 90.90%. After being given tactile touch, the majority of normal body temperature changes in 17 infants (85%) and an average of 36.580OC, normal heart rate of 15 infants (75%) and an average of 148.27x/minute, and oxygen saturation was completely normal in 20 infants (100%) with an average of 94.44%. Then body temperature p=0.036, heart rate p=0.006 and oxygen saturation p=0.000 (? ? 0.05) were obtained. It can be concluded that there is an effect of tactile touch on changes in vital signs in BBLR babies in the NICU room of dr. MM. Dunda Limboto. Therefore, nurses can apply tactile touch in the nursing care of BBLR patients so as to improve nursing services.

Abstract

Background: Adequate knowledge of flood disaster management among nurses is essential to support preparedness and effective response in primary healthcare settings. This study examined the effect of flood disaster management education on nurses’ knowledge at the Limboto Barat Community Health Center. Methods: A quasi-experimental one-group pretest–posttest design was conducted among 20 nurses recruited through purposive sampling. The intervention comprised two 90-minute educational sessions delivered over two consecutive days using lectures, group discussions, and audiovisual media. Knowledge was measured using a validated 20-item multiple-choice questionnaire (Cronbach’s α = 0.82). Pre- and post-intervention scores were compared using a paired-sample t-test with p < 0.05. Results: Mean knowledge scores increased significantly after the intervention (pretest 11.10 ± 2.10 vs posttest 13.55 ± 2.00; mean difference 2.45; p < 0.001). The proportion of nurses categorized as having good knowledge increased from 20.0% to 50.0%, while poor knowledge decreased from 35.0% to 15.0%. Conclusion: Flood disaster management education significantly improved nurses’ knowledge at the Limboto Barat Community Health Center. Further studies with larger samples and controlled designs are recommended to strengthen generalizability and assess longer-term retention.
