Abstract

The rise in non-communicable diseases (NCDs) is driven by risk factors such as smoking, physical inactivity, low consumption of fruits and vegetables, and alcohol use. Community-based surveillance of these risk factors is essential for NCD prevention; however, the availability and use of accurate data remain limited in many regions. Therefore, research is needed to enhance both the quality and utilization of surveillance data in NCD prevention efforts. This study aims to examine community-based risk factor surveillance activities and their role in preventing NCDs in Padang. A qualitative approach using a phenomenological method was employed. Research informants included individuals responsible for NCD programs at community health centers, health cadres, and members of the productive-age population in Padang in 2024. Informants were selected through purposive sampling. Data were primarily collected via in-depth interviews guided by a structured interview protocol. Interview transcripts were analyzed using an interactive analysis model, which included data reduction, data presentation, and conclusion drawing/verification. Findings indicate that risk factor data collection at integrated health posts was conducted both manually and electronically, with data entered into the Ministry of Health’s Asik application. Data processing and analysis were performed using the Asik application, with descriptive interpretations presented in tables and diagrams. Dissemination of risk factor information occurred through mini-workshops, although not all stakeholders were involved. Surveillance results were used to inform NCD prevention programs through counseling and community outreach. Overall, community-based risk factor surveillance data were collected manually and electronically via the Asik application, analyzed descriptively, and disseminated through workshops and follow-up activities supporting NCD prevention. The study highlights the need to improve the quality and use of community-based surveillance data, increase stakeholder engagement, evaluate intervention effectiveness, and assess the impact of surveillance applications on data processing and ongoing monitoring of NCD risk factors. Keyword: Surveillance, Risk Factors, Prevention, Non-Communicable Diseases.

Abstract

Upper Respiratory Infection (URI) is a major cause of morbidity and mortality of infants and toddlers in developing countries. The high infant morbidity and mortality rates in Indonesia are associated with the low exclusive breastfeeding ability. Breast milk is a natural drink for newborns in the first month of life that is beneficial not only for the babies, but also for mothers. The aim of study was to determine exclusive breastfeeding and decrease in incidence of URI among infants aged 6-12 months. This study was conducted by using case control design. Samples were taken by using cluster random sampling. Subject of study consisted of 162 cases and 162 control with infants aged 6-12 years. Cases were 162 infants aged 6-12 months suffering from URI within one last month and taken by mothers to primary health care that was selected location of study, while control was mothers who took their infants aged 6-12 months who did not suffer from URI within one last month to the selected primary health care. Data analysis included univariate, bivariate, stratification, and multivariate analysis with logistic regression. Results of study found that infants who were not exclusively breastfed were 1.69 times (95% CI: 1.02-2.80) more at risk of increasing URI incidence compared to infants who were breastfed exclusively after controlled by smoker in house and immunization variables. Health promotion of 6-month exclusive breastfeeding, provision of immunization, and anti-smoking program need to be implemented continuously to decrease the rates of morbidity and mortality due to URI disease.

Abstract

Susceptibility to infection and increasing antibiotic resistance put burn patients at risk of developing infections caused by multidrug-resistant organisms (MDRO). This condition can progress to sepsis, increasing morbidity and mortality. This retrospective cohort study employed the medical record data of patients treated at Public Hospital X in Jakarta, Indonesia, from January 2020 to June 2022. Of 160 subjects, most were aged <60 years (82.5%) and had comorbidities (16.88%). The most common cause of burns was fire (86.25%). The use of medical devices was 90.63%, with a 14-day median length of stay. The most common gram-negative MDRO pathogens were K. pneumoniae (29.91%), Enterobacter sp (22.32%), and Acinetobacter (20.54%); 45% of patients infected with MDRO died. The bivariate analysis found an increased risk of death due to MDRO infection in burn patients (RR 1.103; 95%CI 1.004-1.211, p-value = 0.046). After adjusting for role variables (age, comorbidities, total body surface area, use of medical devices, length of stay) and from multivariate analysis, the confounding variables for MDRO infection and mortality were length of stay and age. MDRO infection increases the mortality rate in burn patients. Mortality in burn patients due to MDRO infection is greater than non-MDRO.

Abstract

Upper Respiratory Tract Infection (URI) is an acute infection caused by viruses, fungi, and bacteria. Smoking habit at home is a risk factor for URI in infants. This study aimed to determine a relationship between smoking at home and the incidence of URI in infants. This study was conducted the fourth week of February 2010 until the third week of April 2010 in Kampar District, Indonesia. This analytical study used a case-control design and cluster random sampling. The samples comprised 162 cases and 162 controls, infants aged 6-12 months. The cases were 162 infants aged 6-12 months suffering from URI within the last month. The controls included mothers visiting primary health care with infants aged 6-12 months and not suffering from URI within the last month. Data analysis took a logistic regression. This study showed that smoking habit at home had an Odds Ratio of 2.68 times (95%CI: 1.51-4.81) for experiencing URI compared to infants whose families did not have such habit. Health promotion of the dangers of smoking at home through a family approach and anti-smoking campaigns must be more intensively carried out among all Indonesian families to control URI.

Abstract

Health workers play a key role in handling outbreaks, with performance influenced by factors such as ability, discipline, experience, perception, motivation, leadership support, and appreciation. Despite efforts, basic immunization coverage in recent years has not met the national target of 95%, underscoring the need for optimal health worker performance to achieve coverage goals, control rubella outbreaks, and protect vulnerable groups. This study, conducted at Sentajo and Perhentian Luas Health Centers from January to May 2024, analyzed factors affecting health worker performance in managing rubella outbreaks using a sample of 85 respondents and multivariate regression analysis. The findings indicated that awards (r = 0.377, P = 0.000), motivation (r = 0.336, P = 0.002), ability (r = 0.345, P = 0.000), and work experience (r = 0.302, P = 0.005) significantly influenced performance, with awards having the strongest impact. Conversely, leadership support, discipline, and perception had no significant effect on performance. Based on these results, the Kuantan Singingi District Health Office is advised to implement effective reward policies, such as performance bonuses and certificates of achievement, to boost health worker motivation and performance in handling rubella outbreaks. Enhancing health worker skills and rewarding good performance at the health centers can also help improve immunization rates and community protection against rubella. This approach could serve as a model for other health institutions aiming to increase immunization coverage and disease prevention efforts.
