Abstract

Undang-Undang Nomor 17 Tahun 2023 tentang Kesehatan menjelaskan bahwa salah satu cara dalam penyelenggaraan upaya kesehatan adalah melalui upaya kesehatan sekolah. Pada tahun 2024, Kemendikbudristek berkolaborasi dengan Kemenkes dan lembaga lainnya meluncurkan program Gerakan Sekolah Sehat. Ketidakikutsertaan SDN 198 Kota Jambi dalam Lomba Sekolah Sehat tingkat Kota Jambi tahun 2024 menyebabkan tidak terdapat bukti konkrit atas upaya peningkatan kesehatan siswa di sekolah. Tujuan penelitian untuk menganalisis implementasi program Gerakan Sekolah Sehat di SDN 198 Kota Jambi. Jenis penelitian adalah kualitatif dengan desain studi kasus, mengacu pada teori implementasi kebijakan yang terdiri dari aspek komunikasi, sumber daya, struktur birokrasi, dan disposisi. Subjek penelitian dipilih menggunakan teknik purposive sampling dengan total delapan informan. Hasil penelitian menunjukkan struktural dan manajerial berjalan dengan baik, namun dihadapkan pada tantangan perilaku kesehatan peserta didik. Pada aspek komunikasi dan birokrasi, sistem berjalan terstruktur, intensif, dan tanpa tumpang tindih, didukung oleh alur pelaporan berjenjang dan dukungan penuh dari Kepala Sekolah hingga guru pelaksana, dengan Dana BOS sebagai sumber pendanaan utama. Sumber daya manusia (SDM) dinilai kompeten dan dibekali secara rutin. Terdapat kekosongan pada kegiatan kesehatan jiwa, dan belum tersedianya fasilitas ruang Bimbingan Konseling (BK). Tantangan terbesar adalah disposisi peserta didik. Kesadaran kebersihan belum optimal dan perilaku tidak sehat cenderung berulang, memerlukan pengawasan yang ketat dan berkelanjutan.

Abstract

The development of information technology in the health sector encourages primary healthcare facilities, such as community health centers, to transition from manual recording systems to Electronic Medical Records (EMR). However, the success of EMR implementation is not only determined by the availability of technology but also by the level of user acceptance of the system. The PMK Regulation 24 of 2022 states that every healthcare facility is required to implement electronic medical records. The purpose of this research is to analyze the implementation of Electronic Medical Records at Puskesmas Putri Ayu in Jambi City using the Technology Acceptance Model (TAM) approach. The research method used is descriptive qualitative, with data collection techniques thru in-depth interviews and direct observation. The subjects of this research are the Head of the Community Health Center, medical record officers, doctors, dentists, nurses, midwives, laboratory officers, pharmacists, and the Head of Administration. The object of this research is the Electronic Medical Record (EMR) using the Technology Acceptance Model (TAM) method. This research is reviewed from three aspects, namely perceived ease of use, perceived usefulness, and behavioral intention to use. According to the research findings, the implementation of Electronic Medical Records (EMR) at Puskesmas Putri Ayu shows positive results, as it is considered easy to use, beneficial in improving efficiency and service quality, and receives strong support from healthcare workers for sustainable use despite some technical challenges. Overall, it can be concluded that the implementation of EHR is very important to ensure it runs smoothly and as planned, including minimizing existing obstacles or issues and also improving the system to be better.

Abstract

The information quality is an important factor in influencing someone's decision, including online drivers. In Jambi City, some people affected by the Covid-19 pandemic chose to change professions to become online drivers due to disruption to the household economy. Undertaking this profession, online drivers are at risk of contracting diseases and safety and health hazards while working, while there are still those who do not have health insurance to face these risks. The aim of this research is to analyze the influence of information quality on the decision to participate in BPJS Health insurance for online drivers in Jambi City. A quantitative study approach with a cross-sectional design was carried out in Jambi City in 2023. There were 96 respondents selected based on accidental sampling techniques. The study instrument is a questionnaire. Data analysis used the chi-square test. There is a significant influence between the factors accuracy (p=0.001), timeliness (0.000), relevance (p=0.001), and completeness of information on the decision to participate in BPJS Health insurance for online drivers. It is recommended that BPJS Health increase its educational campaign about the benefits of health insurance and improving the quality of timely and relevant information.

Abstract

Smoking has very negative health consequences. The dangers of smoking must be recognized from school age. The snakes and ladders game is an interesting type of educational media that can be used to increase students' knowledge about the dangers of smoking. The aim of this research is to find out how the use of the snakes and ladders game impacts junior high school students' knowledge about the dangers of smoking.This research is considered quasi-experimental with a non-randomized control group pretest and posttest approach. The number of respondents in this study was 90 students, where respondents were divided into 2 groups, namely the treatment group who were given health education using comic-based Photonovella media and the comparison group using conventional methods (posters). Data were collected using a questionnaire and analyzed using the Wilcoxon Test and Mann Whitney U Test. The results of the Wilcoxon test showed a significant increase in the post test scores for the knowledge and behavior variables in both the treatment and comparison groups, but there was no significant increase in the post test scores for the attitude variables. Then the results of the Mann Whitney U Test (sig.2-tailed) on knowledge (0.007 < 0.05), attitudes (0.217 > 0.05), and behavior (0.024 < 0.05). This indicates that there is a significant difference between the scores of the treatment group and the comparison group on the knowledge and behavior variables, but there is no significant difference on the attitude variable.Comic-based photonovella can be used in the learning process because they are very effective in increasing children's knowledge, attitudes and behavior about the dangers of smoking. Therefore, this can be used as an alternative learning media to increase students' knowledge of health by increasing their knowledge about health.

Abstract

Background: Maternal health services are one of the Minimum Service Standards (MSS) indicators in the health sector that must achieve a 100% target. However, the achievement of maternal health services in Muaro Jambi Regency in 2025 reached only 61.96%, while the achievement was 90.93% (496 of 546 targeted pregnant women). In addition, antenatal care coverage remained below the expected target, with K1 coverage at 67.40%, K4 at 61.36%, and K6 at 50.37%. These findings indicate that maternal health service implementation has not been optimal. Therefore, an evaluation using the Logic Model framework, consisting of input, activities, and output, was conducted. Purpose: To evaluate the achievement of minimum service standard health performance indicators in maternal health services. Method: This study employed a qualitative research design using the Logic Model evaluation framework. Informants consisted of the Head of the Community health center, the Maternal and Child Health (MCH) Program Coordinator, midwives, village midwives, and pregnant women selected through purposive sampling. Data were collected through in-depth interviews, observations, and document reviews. Data were analyzed using the Miles and Huberman interactive analysis model, including data reduction, data display, and conclusion drawing. Results: The input component generally supported the implementation of maternal health services. However, several challenges were identified, including suboptimal workload distribution among healthcare personnel , limited MCH clinic facilities, and delays in the distribution of Maternal and Child Health (MCH Handbooks). The activities component showed that all maternal health service activities had been implemented according to established standards. Nevertheless, delays in antenatal visits, low continuity of K1–K6  antenatal care visits, inadequate monitoring of iron tablet compliance, and delayed MCH Handbooks distribution affected service documentation and health education. The output component indicated that the achievement of maternal health service MSS indicators had not yet reached the national target of 100%. Conclusion: The achievement of maternal health service MSS indicators has not yet met the expected target. This was influenced by suboptimal workload distribution among healthcare personnel , delays in MCH Handbooks distribution, and inadequate continuity of  antenatal care visits through K6. Suggestion: Strengthening the community health center had not, optimizing resource management, and improving follow-up and monitoring of pregnant women are necessary to improve the achievement of maternal health service MSS indicators.

Abstract

Background: Minimum Service Standards are mandatory benchmarks for local governments and primary healthcare facilities to ensure the provision of essential maternal health services. Their achievement is important for improving antenatal care and reducing maternal morbidity and mortality. Purpose: To evaluate the achievement of Minimum Service Standards for maternal health services. Method: This qualitative evaluative study used a descriptive approach and the logic model framework, covering input, process, and output components. The study was conducted from November 2025 to August 2026. Fourteen informants were selected purposively, including the head of the primary health center, the maternal and child health coordinating midwife, a physician responsible for the maternal and child health cluster, six implementing midwives, and five pregnant women. Data were collected through in-depth interviews, observation, and document review and were analyzed thematically. Data credibility was supported through source and method triangulation. Results: Human resources were generally adequate in number and educational qualifications, although access to technical training was uneven. Maternal health facilities and basic equipment were available, but ultrasound services were underutilized because of the reassignment of trained physicians and limited service-room capacity. Antenatal care procedures and dual recording through manual registers and the e-Cohort system were generally implemented according to standard operating procedures. However, funding constraints affected outreach activities. Coverage of the first antenatal visit was 81.8%, while coverage of the sixth antenatal visit was 71.9%, both below the national target of 100%. Delayed first visits, refusal of triple elimination screening, sociocultural beliefs, and incomplete continuity of care contributed to the low achievement. Conclusion: Maternal health Minimum Service Standards at Penyengat Olak Primary Health Center have not yet reached the national target despite generally adequate human resources and service procedures. Improving technical capacity, continuity of antenatal care, community education, outreach financing, and cross-sectoral collaboration is necessary to improve maternal health service coverage.

Abstract

Puskesmas Paal Merah I Kota Jambi telah menerapkan sistem Rekam Medis Elektronik (RME) sejak tahun 2024 dalam rangka digitalisasi pelayanan kesehatan. Namun, dalam implementasinya masih menghadapi hambatan. Penelitian ini bertujuan untuk menganalisis secara mendalam implementasi sistem RME di Puskesmas Paal Merah I Kota Jambi menggunakan pendekatan model HOT-FIT. Penelitian ini menggunakan pendekatan kualitatif dengan desain studi kasus. Informan dipilih menggunakan teknik snowball sampling, dengan estimasi 8 informan. Data dikumpulkan melalui wawancara semi terstruktur dan observasi, kemudian dianalisis menggunakan analisis tematik dengan bantuan perangkat lunak NVivo. Hasil penelitian menunjukkan bahwa implementasi RME di Puskesmas Paal Merah I masih belum optimal. Pada aspek manusia, terdapat ketidaksesuaian latar belakang pendidikan, pelatihan yang belum merata, serta kompetensi pengguna yang belum seragam. Pada aspek organisasi, dukungan organisasi belum optimal yang ditandai keterbatasan sarana prasarana, SOP RME belum tersosialisasikan, serta penggunaan satu akun bersama. Pada aspek teknologi, terdapat fitur yang belum terintegrasi dengan P-Care dan masih terdapat gangguan sistem. Namun, pada aspek manfaat, RME telah meningkatkan efisiensi pelayanan dan mengurangi kesalahan pencatatan. Secara keseluruhan, implementasi RME masih belum optimal meskipun telah memberikan manfaat. Diperlukan peningkatan pelatihan, sosialisasi SOP, penataan hak akses, pemenuhan sarana prasarana, penambahan tenaga PMIK, serta peningkatan integrasi sistem dan dukungan teknis.

Abstract

BPJS Health experienced losses due to stopped payment of contributions for participants who had utilized delivery services of more than 200 billion in a period of 2 years. It was recorded that 64.7% of mothers who were about to give birth registered as participants a month before giving birth and then stopped (43%) or behaved in adverse selection in payments after delivery. The largest percentage comes from independent participants or Non-Wage Recipient Participants (PBPU). In Jambi Province, non-compliance in paying BPJS Health contributions is highest in Jambi City. It was recorded that 77,489 participants were in arrears in 2021, resulting in losses of 60.1 billion. Non-compliance with paying contributions is adverse selection behavior. The aim of this study is to analyze the factors causing adverse selection behavior among independent participants in paying post-natal JKN contributions in Jambi City. Quantitative study approach with a cross-sectional design carried out in Jambi City. Respondents totaling 96 people were selected based on accidental sampling technique. The study instrument is a questionnaire. Data analysis consists of univariate analysis stages and bivariate analysis using the chi-square test. The study results found that as many as 33.3% of independent participants behaved in adverse selection when paying JKN contributions after giving birth. There is a significant relationship between the factors number of family members (p=0.001), knowledge (0.000), perception of illness (p=0.001), clinical assessment (p=0.000), and willingness to pay (WTP) with adverse selection behavior in independent JKN participants postpartum. It is recommended that BPJS Health consider implementing a waiting period method for participants who will utilize maternity services. As well as increasing promotive activities for the Jambi City Government in educating and increasing public awareness about the importance of health insurance in protecting household finances.

Abstract

Community Health Centers are first-level health service facilities that have a strategic role in providing healthcare services to the community. In the implementation of the National Health Insurance (JKN) program and the Financial Management Pattern of Regional Public Service Agencies (PPK-BLUD), Community Health Centers are required to implement efficient, transparent, and accountable financial management. One of the efforts that can be undertaken is the implementation of healthcare cost accounting to determine unit costs and to serve as a basis for determining rational service tariffs. This study aims to analyze the implementation of healthcare cost accounting in Community Health Centers based on the results of previous studies. The method used in this research is a systematic review with a PRISMA approach through the stages of identification, screening, eligibility, and inclusion of articles. Out of 150 identified articles, 7 articles met the inclusion criteria and were analyzed in this study. The results show that the Activity Based Costing (ABC) method is the most commonly used method and is considered more accurate than conventional methods in calculating the unit cost of healthcare services. The implementation of the ABC method is able to provide more detailed cost information, assist in evaluating service tariffs, and support cost control as well as the sustainability of healthcare services at Community Health Centers.

Abstract

Drug logistics planning in primary healthcare centers remains a major challenge in ensuring medicine availability and continuity of health services. Inadequate planning often leads to stockouts, overstock, and mismatches between drug supply and patient needs. This study aimed to analyze the implementation of drug logistics planning management at Pakuan Baru Public Health Center, Jambi City. This research used a qualitative observational design with a case study approach. The population consisted of all personnel involved in drug logistics planning, while seven informants were selected using purposive sampling. Data were collected through semi-structured interviews, observation checklists, and document review of LPLPO, stock cards, and planning reports. Data analysis employed the Miles and Huberman model through data reduction, data display, and conclusion drawing with triangulation techniques. The results showed that drug planning was conducted through stages of drug selection, consumption compilation, calculation of drug needs, projection, and procurement adjustment based on LPLPO data, National Formulary, and consumption methods. However, several obstacles were identified, including distribution delays, mismatches between proposed and received drugs, limited budgets, and recurring stock shortages. In conclusion, drug logistics planning management at the health center has generally followed national guidelines, but improvements in data based planning, distribution systems, and monitoring are still required to optimize drug availability and service continuity.
