Abstract

This study aims to implement a digital signature system for filling out Integrated Patient Progress Notes (CPPT) using QR-Code technology. The research method employed is the waterfall development model. The stages of the study include requirements analysis, data collection from various sources, system design, implementation according to the design, testing, and reporting. The study was conducted at XYZ Hospital in Bandung, where it was found that the CPPT signing process is still manually done using a pen, which is considered inefficient as it takes a long time for authorization and is prone to manipulation, thereby reducing the document's validity. The implementation of a digital signature aims to simplify the document signing process quickly without the need for printing, sending, or waiting for physical documents, allowing it to be done digitally and automatically, thereby increasing speed and efficiency. This system also provides authentication assurance by the rightful owner, ensures document security, reduces the risk of damage or loss, and minimizes the possibility of manipulation, thus providing convenience for staff and doctors in terms of time and data accuracy.

Abstract

The use of the SIMRS application is necessary because it is related to information management. A good information system results in good information. Nevertheless, from a practical standpoint, medical recorders who serve as outpatient registration officers have challenges while attempting to use the SIMRS application. The level of enjoyment that medical recorders have as users of the SIMRS application is impacted because of these challenges. Therefore, it is very important to conduct usability testing procedures to assess the extent to which the SIMRS application is user friendly. This study's objectives are to measure the usability of the SIMRS application and identify any issues with it by evaluating three usability factors: effectiveness, efficiency, and errors. The researchers employed a combination of research methods for this investigation. Because of this study, six issues were discovered throughout the usability testing process. The findings of the measurement of usability aspects were as follows: 87% for effectiveness aspects, 0.0005 goals per second for efficiency aspects, and 13% for error aspects. The need for adding barcodes for new patient identity data, recommended SIMRS application to do Bridging System and the need for adding fingerprint features to the SIMRS application

Abstract

Medical records are important documents that record patient information, including examinations, treatments, actions, and services. Article 46 paragraph (1) of the Medical Practice Law defines medical records as files containing patient identity, examination results, treatments, actions, and other services. Regulation of the Minister of Health No. 21 of 2020 concerning the Integration of Health Information Systems, in the Ministry of Health's Strategic Plan 2020-2024, emphasizes the importance of a fast, accurate, integrated, and electronic-based health information system. This study aims to develop an electronic medical record information system for creating web-based medical resumes, using Visual Studio Code, PHP, MySQL, and Laravel. The development method used is agile. The results of the study show that the application of the agile method in developing web-based medical resumes increases effectiveness and efficiency. The problem identified is manual medical resume writing, which is often illegible, making it less efficient. To overcome this, an electronic medical resume design was developed as part of an electronic medical record system, which allows for faster, timelier, and clearer medical resume creation. This medical resume management system is expected to increase efficiency and readability

Abstract

Digital transformation is still an ongoing process in health service delivery to improve operational performance and service quality. However, BPJS Kesehatan patient referrals are still trapped in administrative bottlenecks. At Mulia Medika Clinic, staff used to handle BPJS patient referrals manually, so that records were prone to errors and delays in obtaining information. We designed, implemented, and tested a desktop-based BPJS referral information system using Python and Tkinter for clinic operations. The development process followed the Waterfall methodology, which consisted of requirements analysis, system architecture design using Context Diagrams, Data Flow Diagrams, Entity-Relationship Diagrams as well as Flowcharts followed by implementation and black-box testing validation. The system will manage patient records, referral processing as well as user administration. Automated features include generating referral letters and producing reports. Testing has proven that this system is accurate and efficient—the manual workload has reduced, data traceability has improved, and continuity in the referral service has been maintained. Results prove operational readiness for clinic deployment to enhance administrative efficiency and precision of the referral data. Currently, it runs standalone without real-time database synchronization; hence workflow integration cannot be achieved. Future versions should have direct connections with both clinic management and BPJS databases to allow seamless data exchange without manual synchronization

Abstract

Medical action consent forms are known to be legal documents in the delivery of healthcare services. However, when they are not well completed, it affects the quality of services and the accountability of an institution. Recurring problems in documentation processes include incomplete data entry, late submissions, and substandard recording; all these increase risks for medical errors and administrative complications. A web-based information system was developed to address these systematic deficiencies using Rapid Application Development (RAD) methodology. Field observations and structured interviews with healthcare personnel were used as research tools in the design of this system. The development process continued through four iterative phases: requirements planning, user-centered design, system construction, and deployment evaluation. This platform comprises automated validation protocols that prevent incomplete submissions from being accepted into the database until all required fields have been completed by the user; real-time alert mechanisms for missing data fields within a submission; and streamlined interfaces optimized for clinical workflows. Functional testing shows statistically significant improvements in documentation efficiency and completion rates among medical staff documented by the system compared to those not documented by it. The system permits real-time documentation of procedures with lower error margins compared to existing methods plus shorter processing times than existing methods. This study found that RAD-based solutions present feasible approaches toward improving accuracy and speed in medical consent documentation while simultaneously achieving legal compliance and patient safety goals within hospitals.
