Abstract

HIV/AIDS remains a significant public health concern in Indonesia, with reported cases reaching 519,158 in 2022, including 1,188 children. The Indonesian government prioritizes HIV/AIDS control within its Minimum Service Standards, yet effective implementation varies by region. This study evaluates the performance of the Regional AIDS Control Commission (KPAD) in Bima City, focusing on their role in detecting new HIV cases. Using a qualitative design, data were gathered through interviews with seven key informants, providing insights into KPAD’s operational and strategic challenges. Findings indicate that while Bima City KPAD holds primary responsibility for HIV/AIDS control, its impact has been limited. Operational setbacks, such as inadequate coordination with health services and restricted funding, have hindered effective case detection and public health outreach. By the end of 2022, Bima City recorded 31 new HIV cases, yet KPAD's involvement in early detection and case management remains suboptimal. Despite well-formulated plans within technical units, KPAD lacks proactive engagement, impeding its ability to fulfill its role as the leading agency for HIV/AIDS control. The study highlights the need for enhanced coordination, clearer role delineation, and robust advocacy to strengthen KPAD’s effectiveness. Greater collaboration across health services and community organizations could amplify early detection efforts and improve treatment adherence, ultimately supporting Indonesia’s HIV/AIDS control objectives. This research underscores the critical role of inter-agency synergy in advancing HIV/AIDS prevention and management in Bima City.

Abstract

Latar Belakang : Gizi kurang adalah salah satu bentuk malgizi pada anak yang akan berisiko mengalami keterlambatan perkembangan jangka panjang. Suplementasi gizi atau Pemberian makanan tambahan (PMT) pada anak merupakan salah satu kegiatan intervensi spesifik untuk membantu penurunan prevalensi balita gizi kurang.Tujuan  : mengetahui efektivitas PMT bahan makanan lokal pada balita gizi kurang terhadap peningkatan berat badan balita di Desa Kuta. Metode : peneltian ini adalah Quasi Ekperimen dengan desain One Group Pretest and Posttest yang dilaksanakan di Desa Kuta Wilayah kerja Puskesmas Kuta Kabupaten Lombok Tengah. Sampel sebanyak 20 balita berdasarkan indicator status gizi BB/TB dengan status gizi kurang dengan teknik total Sampling. Sampel diberikan intervensi berupa pemberian makanan tambahan dari bahan makanan lokal setiap hari sebanyak 1 porsi dengan kandungan kalori 300-450 kalori, protein 6-18 gram dan lemak sebesar 7,5-29 gram dalam bentuk makanan lengkap dan kudapan serta diberikan selama 90 hari berturut-turut. Untuk mengetahui perbedaan pengaruh intervensi PMT dengan bahan makanan lokal terhadap Status Gizi anak usia 24-59 bulan digunakan Uji statistik Paired Sample T-Test. Hasil : Balita yang mengalami penambahan berat badan sesudah mendapat Makanan Tambahan sebagian besar mengalami kenaikan berat badan yaitu sebanyak 18 balita (90%). Ada perubahan status gizi balita sebelum dan sesudah intervensi pemberian PMT bahan makanan lokal yaitu dari status gizi kurang menjadi status gizi baik sebanyak 10 balita (50%). penelitian ini menyatakan ada pengaruh kenaikan berat badan anak gizi kurang terhadap pemberian bahan makanan lokal (p = 0,002). Kesimpulan : ada pengaruh pemberian PMT makanan lokal terhadap peningkatan berat badan.

Abstract

The focus of the global problem that is of concern both in the world and in Indonesia is the HIV/AIDS case, that reported HIV/AIDS cases in districts/cities in Indonesia were 22,331 HIV cases and 4,534 AIDS cases. Meanwhile in NTB in 2022, there were 1471 accumulated AIDS cases and the addition of 192 new cases, in Central Lombok Regency there were 378 HIV cases and 220 AIDS cases as of September 2022, as well as 211 HIV cases in the MSM group. The strategy determined was to carry out in-depth interviews using the Snow Ball method with 4 informants, namely the VCT/KTH Mandalika Team at Praya Regional Hospital, Management at Praya Regional Hospital, MSM Clients at KTH at Praya Regional Hospital and the HIV Program Holder of the Central Lombok District Health Service. Test the validity of the data with Credibility, Continuous Observation, Triangulation, Transferability, Depreciability, Confirmability. This research has received ethical approval from the Ethics Committee of Qamarul Huda Badaruddin Bagu University with Number: 022/EC/FKES-UNIQHBA/YPPQH/VIII/2023. Management of HIV/AIDS Prevention Services in the MSM Group at KTH Mandalika RSUD Praya still needs to improve the effectiveness of HIV/AIDS prevention, more intensive strategic planning is needed to overcome the increase in HIV prevalence, involving an approach through a TEAM of Field Officers in providing more effective education and social support through VCT services and hotline counseling. Coordination and a stable organizational structure in KTH Mandalika RSUD Praya cannot be ignored, so that response efforts can run more focused. Staff training and development, especially those related to handling PLWHA, needs to be based on objective selection and unit needs, while the use of information technology, such as the SIHA 2.1 application and RS SIM, can be improved to ensure accurate recording and reporting. Management of BHP and ARV availability needs to be optimized through efficient coordination with the District and Provincial Health Services, and regular evaluations need to be carried out to increase distribution efficiency. Implementing the recommendations from 4 informants can have a significant positive impact in managing the management of HIV/AIDS prevention services at KTH Mandalika RSUD Praya, so as to reduce HIV transmission, and provide better support to PLWHA in the MSM community..

Abstract

In West Nusa Tenggara the prevalence of stunting is placed seventh with a frequency of 33.49%, whereas the prevalence of stunting in Indonesia is 30.8%, according to Riskesdas 2018. A useful strategy for raising awareness and preventing stunting problems is to use KIA book media to facilitate brainstorming sessions for 1000 HPK. To ascertain the impact of counseling on 1000 HPK (First Day of Life) participants' knowledge of reducing stunting at the Narmada Community Health Center Working Area through the use of the KIA book media and the brainstorming approach. Nonequivalent Control Group Design is the research method utilized in quasi-experimental design studies. There were 112 research targets as the research subjects. A questionnaire was the tool utilized. The independent sample t-test and the paired sample t-test were used to test the data. Findings from Research. There were four people (7.14%), eighteen people (32.14%), and thirty-four people (60.71%) who knew about pregnant women before treatment for the poor category. Following the intervention, it rose to fall into one of three categories: less than none, moderate (consisting of 11 individuals/19.64%), and good (consisting of 45 individuals (80.36%). The results of statistical tests utilizing the paired sample t-test show that there is a significant influence between the usage of KIA book media at 1000 HPK and brainstorming approaches, with a p value of 0.000 < 0.05. Pregnant women's knowledge increased after counseling with the brainstorming method using KIA book media, according to the analysis's results, which also indicate a substantial difference in the women's knowledge before and after counseling.

Abstract

This study aims to analyze the relationship between healthcare service capacity and referral rates at Puskesmas within BPJS Kesehatan Mataram Branch in 2024. This study used a correlation design with a cross-sectional approach. This design aims to evaluate the relationship between the service capabilities of health centers and referral rates at a certain time. The results of the study showed that Puskesmas with inpatient services tend to have safer referral rates compared to non-inpatient Puskesmas. Non-inpatient Puskesmas often refer more cases due to their limited capacity to manage complex cases at the primary level. The results emphasize the need for increased resources and medical training, especially in non-inpatient Puskesmas, to reduce unnecessary referrals to higher-level hospitals. Strengthening service capacity at Puskesmas is essential to ensure effective primary care and reduce the burden on referral hospitals.

Abstract

Background: Tuberculosis is a global health problem, especially in children. Handling child TB cases in Mataram City, in 2022 there will be 184 TB cases in children and in 2023 there will be an increase of 310 cases. Reporting records experienced treatment failure of 43.7%, this condition indicates the phenomenon of implementing the DOTS program in the treatment of TB patients in children. Purpose: To analyse the implementation of the Directly Observed Treatment Shortcourse (DOTS) programme in the treatment of children with pulmonary tuberculosis. Method: Qualitative research with an exploratory descriptive design conducted on children with pulmonary tuberculosis in Mataram City. The sources of data and information were 13 informants consisting of: Persons in charge of the TB program at the Health Service, Hospitals, namely pediatricians, Community Health Center TB program holders, Medicine Drinking Supervisors and families of TB sufferers. In-depth interview methods and data analysis through reduction and triangulation, as well as document searches to record and report the implementation of DOTS program activities, including identification and diagnosis, classification and staging of TB, treatment of pediatric TB, treatment, monitoring and evaluation, prevention and education, handling side effects, and drug-resistant TB (TB-RO). Results: The implementation of the DOTS program in handling TB in children is still not in accordance with the SOP. Active screening of new case detection rates is still low, the readiness of professionals in identification and diagnosis, classification and staging of pediatric TB have not run optimally, the implementation process is carried out with a tiered referral system, the logistics of tools and drugs often occur out of stock, TB-RO and drug allergies are found to not be handled properly. Public compliance in treatment is still low and stigmatization is high in society. The government's commitment to handling pulmonary TB patients in children has not been implemented properly. Conclusion: The implementation of the Directly Observed Treatment Short-course (DOTS) program has not been optimal. The government's commitment to handling TB cases in children has many obstacles and challenges, limited professional human resources, running out of medicines, high absenteeism of TB patients, and high stigmatization of TB disease in society.

Abstract

Background: Self-medication is an act of self-medication without medical consultation, the practice of self-medication in the people of West Nusa Tenggara reaches 66.62% and the community in Bima City is indicated to be 60.44%. This practice risks causing serious side effects and drug resistance. Purpose: To explore why the practice of hard drug self-medication is high in the community of Bima City. Method: Employing a descriptive qualitative approach with an emphasis on exploratory research. Conducted in Bima City starting in August 2024, it involved a total sampling method, selecting 10 participants who served as key informants from the National Agency of Drug and Food Control. Data collection was carried out through triangulation, integrating multiple sources, methods, and theories to enhance result validity. In-depth interviews were used as the primary data collection technique, and the data were analyzed using thematic analysis to extract key insights and interpret the underlying meaning of the findings. Results: Self-medication practices, such as buying antibiotics without a prescription, risk causing drug resistance and side effects. Monitoring drug distribution, especially online purchases, is a challenge for the Health Office and National Agency of Drug and Food Control. People choose practical and cost-effective ways, while pharmacists face pressure in selling drugs without a prescription. Collaboration and education are needed to ensure safe and rational drug use. Conclusion: The high cost of medicine and queues at health facilities encourage self-medication, exacerbated by poor Social Security Administration services. 24-hour pharmacies and online purchasing make it easier to access drugs without a prescription, with weak distribution oversight. Lack of accurate information and distrust of formal health services increase the risk of side effects and drug abuse.

Abstract

Permasalahan Stunting di desa sokong merupakan permasalahan yang telah menjadi perhatian pemerintah Kabupaten Lombok Utara dalam waktu 1 tahun terakhir, banyak terjadi kehamilan dan melahirkan. Begitupula jumlah balita dan stunting semakin banyak, di perkirakan jumlah balita stunting sebanyak 91 orang di desa sokong. Banyak upaya yang sudah dilakukan pejabat desa setempat. Akan tetapi, kejadian balita stunting tetap saja belum turun di desa tersebut. Oleh karena itu kami tertarik memberika edukasi pemberdayaan masyarakat terkait dengan sadar stunting di Desa sokong Kecamatan Tanjung dengan melakukan penyuluhan kepada masyarakat setempat terutama pada ibu hamil, orang tua beserta kader dengan tujuan dapat menurunkan angka kejadian stunting serta meningkatkan derajat Kesehatan masyarakat, meningkatkan pengetahuan, dan kesadaran masyarakat tentang stunting di desa sokong. Stunting berpotensi memperlambat perkembangan otak, dengan dampak jangka panjang berupa keterbelakangan mental, rendahnya kemampuan belajar, dan risiko serangan penyakit kronis seperti diabetes, hipertensi, hingga obesitas. Tim pengabdian masyarakat Melakukan identifikasi masalah pada tanggal 04 februari 2025 di desa dengan melibatkan bidan dan perwakilan kader posyandu dan merumuskan masalah utama berdasarkan tingkat kegawatan, keseriusan dan besarnya masalah sehingga di putuskan masalah yaitu masih kurangnya pemahaman masyarakat terhadap stunting. Solusi terhadap masalah sesuai dengan kearifan budaya dan kemampuan desa setempat berupa edukasi tentang pemberdayaan masyarakat sadar stunting melalui penyuluhan. Melakukan identifikasi atau mendaftar ibu hamil, balita yang ada di desa sokong. Pelaksanaan pengabdian kepada masyarkat di laksanakan 1 hari secara langsung pada tanggal 08 februari 2025 di desa sokong oleh dosen dan mahasiswa dalam memberikan penyuluhan edukasi tentang sadar stunting yaitu mempersiapkan ruangan atau tempat penyuluhan yang berkoordinasi dengan pihak puskesmas. Peserta yang hadir diberikan penyuluhan oleh tim pelaksana menggunakan Bahasa dan komunikasi yang komunikatif secra dua arah serta di bantu menggunakan lembar balik. Berdasarkan hasil yang di dapatkan dari diberikannya penyuluhan tentang edukasi sadar stunting menunjukan adanya peningkatan pengetahuan masyarakat tentang Stanting lebih baik serta kader dapat menerapkan pemberian PMT sesuai scaling up nutrition (SUN) pada Posyandu khususnya pada balita yang ada di Desa Sokong.

Abstract

Abstract Basic Trauma Cardiac Life Support (BTCLS) training has played an important role in improving the preparedness of health workers in dealing with emergency conditions. The blended learning method as a solution to limited resources and time, as well as to increase the effectiveness of training. Purpose: To find out the level of satisfaction and competency improvement of participants with BTCLS training based on blended learning. Methods: This study was a quantitative study using a cross-sectional survey of one group pre-post test design, with a sample of 73 trainees. Data were collected through standardized questionnaires and statistically analyzed using fisher exact test and paired t-test, with a significance level of p ≤ 0.005. Results: Showed that instructor quality, material accuracy, and organizer facilities were significantly related to participant satisfaction (Sig ≤ 0.01). The paired t-test also showed a significant increase in pre-test and post-test scores in all professional groups (p ≤ 0.05), so that the training provided proved to be effective in improving the competence of participants and needed to be continuously developed. Conclusion: The blended learning method has been shown to be effective in increasing trainee satisfaction and competence. The quality of the instructor and the relevance of the material are the main factors that affect participant satisfaction. Strengthening of practice aspects and curriculum adjustments is recommended to ensure the sustainability of BTCLS training. This research will be carried out at the Mataram Health Agency in 2025. Abstract

Maksud ditetapkannya Indikator Nasional Mutu (INM) adalah untuk memantau dan menilai apakah upaya mutu yang telah dilakukan fasilitas pelayanan kesehatan dapat meningkatkan mutu pelayanan secara signif. Penelitian kuantitatif yang menggunakan observasional analitik dengan desain cross sectional approach ini bertujuan untuk menganalisis determinan implementasi kebijakan tentang INM di Puskesmas Batunyala Kecamatan Praya Tengah Kabupaten Lombok Tengah-Nusa Tenggara Barat. Terdapat 86 orang sampel dari 114 populasi yang ditentukan dengan teknik simple random sampling. Data hasil penelitian diperoleh melalui angket dan dianalisis melalui tiga tahapan yaitu analisis univariat, bivariat, dan multivariat. Hasil penelitian menunjukkan bahwa variabel komunikasi, sumber daya, sikap, dan struktur birokrasi memiliki hubungan dan berpengaruh kuat serta signifikan terhadap implementasi kebijakan tentang INM.
