Abstract

Pharmacy is a pharmaceutical service facility carried out by pharmacists by providing services directly and responsibly to patients related to pharmaceutical preparations in order to achieve definite results to improve the quality of life of patients. However, currently irrational treatment still often occurs, one of which is with inappropriate prescribing. This will have a negative impact on society. The purpose of this study was to evaluate drug prescribing in one of the pharmacies in the city of Medan. This is a descriptive evaluative research. The data was collected retrospectively and concurrently. Then the raw data was categorized into quantitative data and it was represented in tables to compare the data visually. Meanwhile, qualitative data was collected by observation and interview with the interviewees. The results showed that some indicators of prescribing drugs to patients in one of the pharmacies in Medan city still did not meet the standards, namely the average number of drug items per prescription sheet in 2021 was 3.63 and the percentage of prescribing drugs with generic names in 2021 was 80.9 %. While the prescribing indicators that are in accordance with the standards are the percentage of prescribing antibiotics for 2021 at 6.47%, the percentage of drugs that can be delivered is 90%, and the percentage of drugs that are completely labeled 100%. Based on the results of the study, it was concluded that there were still indicators of prescribing drugs carried out in one of the pharmacies in the city of Medan that did not meet the standards.

Abstract

Pharmacy is a place where pharmaceutical services are held by pharmacists who are responsible for patients. Pharmaceutical services in pharmacies are related to drugs with the aim of achieving definite results to improve the quality of life of patients. So the management of drugs in pharmacies needs to be done properly and precisely to ensure the availability of sufficient and quality drugs in order to achieve optimal service goals by implementing a storage system that is in accordance with pharmaceutical service standards no.73 of 2016. Therefore the purpose of this research is to evaluate the drug storage system in one of the Medan city pharmacies based on pharmaceutical service standards. This research is descriptive with retrospective data collection. The data obtained were grouped into quantitative data presented in tabular form to see visual comparisons, and qualitative data obtained through observation and interviews with informants. The results showed that in several indicators of drug storage in one of the Medan city pharmacies based on Permenkes No. 73 of 2016 there are still those that have not complied with, namely in the preparation of LASA drugs and writing document numbers on drug stock cards. Meanwhile, the standard drug storage indicators are the columns on the stock card consisting of receipt or dispensing date, receipt or disbursement document number, source of origin of the drug, no. batch/no. Lot, expiry date, receipt and disbursement amount, remaining stock, and officer's initials, as well as suitability for drug storage based on dosage form, alphabetical order, and FEFO and FIFO storage methods. Based on the results of the study it was concluded that the drug storage system carried out in one of the pharmacies in the city of Medan did not meet pharmaceutical service standards based on Permenkes No. 73 of 2016.

Abstract

Pemenuhan gizi seimbang untuk masyarakat adalah upaya untuk memastikan bahwa setiap individu dalam masyarakat mendapatkan asupan gizi yang cukup dan proporsional untuk menjaga kesehatan dan fungsi tubuh yang optimal. Pemenuhan gizi seimbang bertujuan untuk meningkatkan kesehatan dan kualitas hidup masyarakat dengan menyediakan akses terhadap makanan yang bergizi dan seimbang.Kegiatan pengabdian masyarakat bertujuan meningkatkan pengetahuan masyarakat mengenai gizi. Kegiatan ini dimulai menilai pengetahuan tentang gizi seimbang dengan memberikan soal pre-test sebelum edukasi, kemudian dilakukan penyuluhan dan diakhiri dengan post-test.Hasilnya dari pengetahuan kader tentang gizi seimbang menunjukkan kenaikan skor rata-rata pengetahuan yang signifikan sebelum dan sesudah dilakukan edukasi gizi seimbang. Pemberian edukasi gizi seimbang oleh kader yang rutin akan meningkatkan pengetahuan tentang gizi secara signifikan.

Abstract

Hiperlipidemia merupakan gangguan metabolisme lipid yang berisiko menyebabkan penyakit kardiovaskular jika tidak ditangani dengan baik. Keberhasilan pengelolaan kondisi ini sangat dipengaruhi oleh tingkat kepatuhan pasien dalam minum obat. Penelitian ini bertujuan untuk mengetahui tingkat kepatuhan minum obat pada pasien hiperlipidemia serta menganalisis hubungan keduanya terhadap outcome klinis. Desain penelitian dengan pendekatan cross-sectional yang melibatkan 110 responden di UPT Puskesmas Teladan Kota Medan. Analisis statistik yang digunakan yaitu uji spearman rho untuk menganalisis hubungan antara kepatuhan minum obat dengan outcome klinis pasien hiperlipidemia. Hasil penelitian menunjukkan bahwa tingkat kepatuhan minum obat dari pasien hiperlipidemia sebanyak 10 pasien (9,1%) memiliki tingkat kepatuhan minum obat dengan kategori tinggi. Kepatuhan minum obat sebagian besar berada pada kategori sedang yaitu sebanyak 74 pasien (67,3%), sedangkan 26 pasien (23,6%) lainnya termasuk dalam kategori rendah. Setelah dilakukan uji analisis menggunakan uji spearman rho didapatkan hubungan kepatuhan minum obat dengan kolesterol total tidak signifikan (p=0,777);hubungan kepatuhan minum obat dengan HDL juga tidak signifikan (p=0,883);hubungan kepatuhan minum obat dengan trigliserida tidak signifikan (p=0,278);hubungan kepatuhan minum obat dengan LDL tidak signifikan (p=0,597). Berdasarkan penelitian ini dapat disimpulkan bahwa tidak terdapat hubungan yang signifikan antara kepatuhan minum obat dengan outcome klinis.

Abstract

Chronic Obstructive Pulmonary Disease (COPD) remains a major global health burden, ranking among the leading causes of morbidity and mortality. In Indonesia, the increasing prevalence of COPD is closely linked to smoking behavior, air pollution, and low treatment adherence, which collectively diminish patients’ quality of life and treatment success. Despite various pharmacological advancements, non-adherence to prescribed medication regimens continues to impede optimal therapeutic outcomes. This study aims to analyze the relationship between medication adherence and therapeutic outcomes among outpatient COPD patients at Pirngadi Hospital Medan, as well as to identify factors contributing to non-adherence. The research adopts a quantitative correlational design with a cross-sectional approach, involving 70 respondents selected through quota sampling. Primary data were collected using structured questionnaires, including the General Medication Adherence Scale (GMAS) and the St. George’s Respiratory Questionnaire (SGRQ). Data were processed using IBM SPSS Statistics version 25 and analyzed through univariate and bivariate (Chi-square) tests. The results revealed a significant relationship between medication adherence and therapeutic outcomes (p < 0.05). Patients with higher adherence levels demonstrated better treatment responses and higher health-related quality of life scores. The findings also indicate that smoking status, education level, and duration of illness significantly influence adherence behavior. In conclusion, strengthening patient adherence is essential to improving COPD management outcomes. The study contributes to clinical practice by emphasizing the integration of behavioral and educational interventions into pharmacological therapy. Its novelty lies in combining adherence measurement with quality-of-life assessment in an Indonesian hospital context, offering empirical evidence that supports the development of patient-centered, technology-assisted adherence monitoring systems.

Abstract

The rapid expansion of nanomaterial-based rheumatoid arthritis (RA) research has produced a fragmented body of literature, limiting clear recognition of major journals, influential contributors, collaboration patterns, and shifting research priorities. This study presents a two-decade bibliometric mapping of nanomaterials in RA with an inflammation-focused perspective. English-language publications from 2005 to 2025 were collected from Scopus using a nanomaterial–RA–inflammation search strategy, yielding 5,110 initial records. Data were refined in OpenRefine through deduplication, author-name harmonization, and keyword normalization. Following eligibility screening, 720 articles were retained for core bibliometric analysis. Biblioshiny, supported by the Bibliometrix R package, was used to measure annual publication trends, productive sources, leading authors, institutional output, and collaborative networks. Conceptual development was assessed through keyword co-occurrence analysis and thematic mapping, with network visualizations generated in VOSviewer. Results indicate accelerated publication growth in the recent period, with outputs concentrated in drug delivery and biomaterials-related journals. Scientific productivity was driven by a limited group of countries and institutions, reflecting hub-based knowledge production. Keyword and thematic analyses identified targeted drug delivery and macrophage polarization as motor themes, while PI3K–AKT and Hedgehog signaling emerged as developing topics. Overall, the field shows rapid growth and organized thematic evolution.

Abstract

Infeksi Saluran Pernapasan Akut (ISPA) masih menjadi masalah kesehatan masyarakat yang signifikan, dengan tingginya angka kejadian yang tidak hanya dipengaruhi oleh rendahnya literasi kesehatan, tetapi juga oleh praktik penggunaan obat yang kurang rasional di masyarakat. Dalam konteks farmasi klinis, peran farmasis menjadi krusial dalam memberikan edukasi terkait deteksi dini, pencegahan, serta penggunaan obat yang tepat untuk menghindari komplikasi dan resistensi, khususnya akibat swamedikasi yang tidak rasional. Temuan awal di Puskesmas Teladan Kota Medan menunjukkan bahwa masyarakat belum mampu mengenali gejala awal ISPA secara tepat dan cenderung melakukan swamedikasi tanpa informasi yang memadai. Kegiatan pengabdian kepada masyarakat ini bertujuan untuk mengoptimalkan edukasi berbasis farmasi klinis sebagai intervensi promotif dan preventif dalam meningkatkan pengetahuan masyarakat terkait deteksi dini, pencegahan, dan penggunaan obat yang rasional pada ISPA. Metode yang digunakan adalah penyuluhan dan edukasi interaktif dengan desain quasi-eksperimental one group pre-test–post-test terhadap 17 responden. Materi edukasi mencakup pengenalan ISPA, tanda dan gejala, faktor risiko, pencegahan berbasis Perilaku Hidup Bersih dan Sehat (PHBS), serta prinsip penggunaan obat yang rasional. Hasil menunjukkan peningkatan signifikan tingkat pengetahuan (p<0,05), dengan peningkatan kategori pengetahuan baik dari 11,76% menjadi 52,94% dan penurunan kategori kurang dari 58,82% menjadi 11,76%. Temuan ini mengindikasikan bahwa edukasi berbasis farmasi klinis efektif dalam menunjang upaya promotif dan preventif sekaligus meningkatkan rasionalitas penggunaan obat dalam pengendalian ISPA di masyarakat.

Abstract

Warfarin remains the primary oral anticoagulant for patients with mechanical heart valves, but considerable interindividual variability in stable dose requirements poses a clinical challenge. Although VKORC1 gene polymorphisms are well-established determinants of dose variability, the impact of objectively measured medication adherence is less clear. This study aimed to evaluate the independent and combined effects of VKORC1 polymorphisms and objectively measured medication adherence on stable warfarin dose requirements in patients with mechanical heart valves. Genotyping of VKORC1 −1639G>A (rs9923231) and 1173C>T (rs9934438) was performed using PCR–RFLP. Adherence over 90 days was expressed as the percentage of prescribed doses taken. The −1639A allele frequency was 81.7%, and patients with the AA genotype required significantly lower daily doses than GA or GG genotypes (p<0.001). Mean adherence was 89.4±11.2%. Multivariate regression showed that VKORC1 genotype, age, and adherence independently predicted stable dose,explaining 45.2% of variability. Stable warfarin dose was defined as an unchanged maintenance dose with a therapeutic INR (2.0–3.5) for ≥3 months.Adherence below 80% was associated with increased INR fluctuations and dose adjustments, highlighting adherence as a key clinical modifier alongside genetic factors.

Abstract

The complexity of treatment with the disease in patients with chronic kidney disease (CKD) can increase the incidence of Drug Related Problems (DRPs). Analysis of DRPs is very useful to reduce the duration of illness, death, and treatment costs. The condition of patients who require hemodialysis therapy can cause changes such as physical, psychological, lifestyle and social that have an impact on the quality of life. This study aims to determine correlation between DRPs and the quality of life of CKD patients undergoing hemodialysis at Dr. Pirngadi Hospital, Medan, Indonesia. This study uses an observational method with a retrospective and prospective cross sectional research design on 72 patients who met the inclusion criteria in February-March 2024. DRPs were analyzed applying Cipolle and quality of life was obtained using the EQ-5D-5L questionnaire. The data was analyzed using the Spearman Rho test in the SPSS program. The results showed that there were DRPs in 64 patients with the categories of needing additional drugs (3.38%), unnecessary therapy (15.54%), drug interactions (35.13%) and ineffective drugs (45.95%). Quality of life with categories of good (36.1%), adequate (41.7%), and poor (22.2%). The patient's average quality of life score was 0.657. There was a correlation between the number of DRPs and the patient's quality of life (r=-0.802) with a significance value of p=0.032 (p<0.05), which shows that the higher the incidence rate of DRPs, the lower the patient's quality of life. Based on the results of the study, it was concluded that there was a significant relationship between the number of DRPs events and the quality-of-life value of CKD patients undergoing hemodialysis.
