Abstract

Patients hemodialysis with experience complaints of anemia, fatigue, sleep disturbances, and increased urea levels. Fatigue and sleep disorders are still a high problem in Indonesia. Increased urea levels can cause decreased appetite, poor nutritional intake and complaints of fatigue. The aim of the research was to analyze the effect of deep breathing exercise therapy combined with Orem self-care based aromatherapy on fatigue levels and sleep quality. The design in this research is quasi-experimental. Population of all hemodialysis patients at RSI Jemursari Surabaya. The sample size was 40 people, divided into 2 groups, namely the intervention group and the control group. The sampling technique was taken using a total sampling technique. Data were analyzed using the independent t-test a < 0.05 and looking at the difference in values using the delta test. The results of research conducted at RSI Jemursari Surabaya, showed that the average fatigue score in the intervention group before giving deep breathing exercise therapy, a combination of aromatherapy based on self-care Orem, was 36.90 and after being given the intervention, it was found to decrease by 15.90, while the control group did not. given any therapy, according to hospital standards of 34.65 and experienced a decrease of 19.65. Based on the independent t - test, it shows p = 0.003 and sleep quality shows p = 0.001, which means that deep breathing exercise therapy combined with aromatherapy based on Orem self-care has an effect on fatigue and sleep quality. Deep breathing exercise therapy a combination of aromatherapy based on Self-Care Orem according to standards, can reduce fatigue levels and improve sleep quality.

Abstract

Stroke often causes long-term disabilities, significantly affecting motor and functional abilities. Rehabilitation interventions such as aquatic therapy, which utilize the properties of water to enhance movement and reduce pain, have gained increasing attention as innovative rehabilitation strategies. This systematic literature review aims to analyze and summarize the effectiveness of aquatic therapy as a rehabilitation innovation for improving functional recovery in stroke patients. A systematic search was conducted in three databases—PubMed, ScienceDirect, and Google Scholar—for articles published between January 2020 and January 2025. The keywords used in English included “aquatic therapy,” “stroke rehabilitation,” and “water-based therapy.” From the initial search, a total of 45 articles were identified. After screening titles and abstracts based on inclusion criteria (language, publication year, and open-access availability), 20 full-text articles were reviewed in detail. Following critical appraisal and relevance assessment to the research objectives, 10 articles were selected for final analysis. These articles were then synthesized to evaluate the effects of aquatic therapy on functional recovery, particularly focusing on gait and balance improvement in post-stroke patients. This review found 45 studies after identification. Most studies showed that aquatic therapy significantly improved balance, gait, muscle strength, and quality of life among stroke survivors. Additionally, resistance offered by water supports muscle strengthening without overstraining. However, variations in session duration, water temperature, and exercise intensity across studies suggest a need for standardized protocols. Aquatic therapy is an effective and innovative rehabilitation approach for stroke patients, particularly in enhancing motor recovery and quality of life. Future research should focus on standardized intervention protocols and long-term outcomes.

Abstract

Stroke survivors are at high risk of hospital readmission due to recurrent stroke, comorbidities, and inadequate discharge preparation. Readiness for hospital discharge plays a pivotal role in ensuring a safe transition of care and improving post-stroke outcomes. Evaluating patient and caregiver readiness for hospital discharge is essential to reduce the likelihood of readmission and optimize home care outcomes. This review aimed to identify, describe, and evaluate existing tools used to assess discharge readiness in post-stroke patients, and examine their effectiveness in supporting care transitions and preventing hospital readmissions. Methods: A systematic review was conducted in accordance with PRISMA guidelines. Four databases-PubMed, Science Direct, Clinical Key, and Google Scholar—were searched for relevant articles published between 2021 and 2025 using predefined keywords related to stroke, discharge readiness, transition of care, and readmission. After screening 202 initial records, 34 articles were selected for full-text review. Ten studies met the inclusion criteria and were then critically appraised using the Joanna Briggs Institute (JBI) checklist to assess methodological quality and risk of bias. The included studies were synthesized narratively based on study design, assessment tools used, psychometric properties, and their impact on discharge readiness and hospital readmission outcomes. The review identified several validated tools, including the Readiness for Hospital Discharge Scale (RHDS), the Preparedness Assessment for the Transition Home After Stroke (PATH-s), and the Caregiver Contribution to Self-Care Inventory (CC-SC-CII-v2). These tools varied in focus, population, and application context. While some showed strong psychometric properties and predictive power for readmission, others highlighted the need for caregiver inclusion and context-specific adaptation. However, inconsistencies in implementation, limited integration into clinical workflows, and lack of long-term outcome evaluations remain significant barriers. This review enhances understanding of discharge readiness assessment tools in post-stroke care and underscores their value in reducing readmissions and optimizing care transitions.

Abstract

Non-hemorrhagic stroke occurs when blood flow to the brain is stopped due to atherosclerosis or a blood clot that has blocked a blood vessel to the brain. As a result of brain disorders, it causes hemiplegia and hemiparesis. Many types of interventions are available for physical problems of hemiparesis, one of which is ROM therapy and Warm Water Compress. The purpose of this study was to analyze the effectiveness of Range Of Motion (ROM) therapy and Warm Water Compress with the Orem Self Care theory approach to Increasing Muscle Strength in Non-Hemorrhagic Stroke Patients. The design in this study was a Quasy Experiment with Pretest t and Posttest . The sample size was 44 with a Simple Random Sampling technique with 22 intervention groups and 22 controls. The intervention group was given ROM Exercises and Warm Water Compresses. Analysis used the Wilcoxon and Mann-Whitney tests to identify differences between the intervention and control groups with a significance level of a <0.05. The results obtained from the Wilcoxon Signed Rank Test analysis of Muscle Strength in Non-Hemorrhagic Stroke Patients in the intervention group p value = 0.000 and control p value = 0.000. The results show that there is an effect of ROM exercises and warm water compresses on increasing muscle strength in non-hemorrhagic stroke patients. ROM exercises and warm water compresses can be used as non-pharmacological therapy in the rehabilitation process for non-hemorrhagic stroke patients.

Abstract

Complaints that often arise in hemodialysis patients are nausea, vomiting, anemia, fatigue, sleep disorders, and increased urea levels. Anxiety is caused by by innate and physiological factors, namely those that influence or manifest in physical symptoms, especially in the function of the central nervous system, both from within the patient and from outside the patient. This study aims to provide scientific evidence to support the use of these interventions as a comfortable and effective prevention strategy for patients with chronic kidney failure. Comforting intervention needed in nursing intervention, the comfort level of Kolkaba is at the transcendence comfort level. The method used is quasy experimental with a pretest-posttest control group design approach, the sample size is 40 people, namely 20 interventions groups and 20 controls groups. The sampling technique is Purposive Sampling Technique. The research instrument used the Fatigue Severity Scale (FSS) questionnaire and DASS 42. FSS shows high reliability with Cronbach's Alpha value >0.9, such as 0.968 as well as good validity based on Pearson correlation (r >0.623, Sig <0.05). The DASS-42 has excellent reliability (Cronbach's Alpha >0.9 for each dimension: depression 0.954, anxiety 0.903, stress 0.917) and validity 0.782. The data were analyzed using the Pariet t- test with significance at fatigue p = 0.000 (<0.05). While anxiety shows the Pariet t- test with significance p = 0.015 (<0.05), indicating that hand and foot massage therapy has an effect on reducing fatigue and anxiety values. The conclusion of this study is that there is an effect of hand and foot massage based on the theory of comfort on reducing fatigue and anxiety values in chronic kidney failure patients undergoing hemodialysis.
