Abstract

Methicillin-resistant Staphylococcus aureus (MRSA) is a significant healthcare-associated pathogen, capable of spreading through contaminated aerosols generated by medical equipment such as nebulizers. The increasing prevalence of MRSA infections, coupled with challenges in effective disinfection, highlights the urgency of identifying optimal disinfection methods. This study aims to assess the effectiveness of chlorhexidine in reducing MRSA contamination in nebulizer equipment, focusing on determining the optimal concentration for disinfecting nebulizers to minimize airborne transmission. This study employed an in vitro experimental design. The study population consisted of MRSA isolates obtained from clinical sputum specimens. The samples included nebulizer equipment inoculated with MRSA suspensions standardized to 10⁷ CFU/mL. Disinfection was carried out using chlorhexidine at concentrations of 2.5%, 4%, and 5%, each diluted in 70% alcohol. Control groups were rinsed with sterile saline and 70% alcohol. Post-disinfection, the nebulizer surfaces were swabbed and cultured on Mueller-Hinton agar to quantify colony-forming units (CFUs). Data were analyzed using paired t-tests and one-way ANOVA followed by Tukey’s post-hoc test, with a significance level set at p 0.05. The results show that chlorhexidine 2.5% reduced bacterial counts to an average of 1 CFU/plate, and 4% chlorhexidine reduced it to 49 CFU/plate. Chlorhexidine 5% resulted in an average of 1,430.6625 CFU/plate. Statistical analysis revealed significant reductions with 2.5% and 4% chlorhexidine compared to controls (p0.01), whereas 5% Chlorhexidine did not show a significant difference. These findings suggest that lower concentrations of chlorhexidine may be sufficient for effective disinfection in nebulizer equipment, addressing the risk of MRSA spread through airborne particles.

Abstract

Drug planning, a key responsibility of the hospital’s pharmacy unit, plays a crucial role in ensuring that drug management is safe, effective, and efficient. Inadequate drug planning can lead to stockouts and dead stock which negatively affect the quality of patient care and resource utilization. The purpose of this study was to identify the obstacles hindering the optimal implementation of the drug planning process in the pharmacy. This study employed a qualitative approach in the form of a case study. Data collection was carried out from September to October 2024 through in-depth interviews with informants and direct observation through document review. Informants were selected using purposive sampling and included the Assistant Manager of Medical Support, the Head of the Pharmacy Unit, and the Person in Charge of the Pharmacy Warehouse. The data analysis was conducted using triangulation to check the validity of the data. The results of the study revealed that there were five main problems that caused obstacles in planning so pharmaceutical management was not optimal. These obstacles are the imbalance between the workload and the number of officers, lack of officer skills, the absence of a drug planning team, inappropriate drug planning calculation and methods, and inadequate hospital information system (HIS) related to drug planning. In conclusion, although the management of drug planning and procurement in the pharmacy unit of Hospital X, Malang City, East Java, Indonesia, has been carried out, its implementation remains suboptimal because of various managerial and operational barriers.

Abstract

Avoidant restrictive food intake disorder (ARFID) is an eating problem in children, causing nutritional deficiency, obstructing growth, and development. This study investigates the correlation between demographic characteristics and parenting alliance to nutritional status in children with ARFID in Malang Regency. This analytic observational study used a cross-sectional approach from December 2018 to March 2019. There were 245 respondents with multistage sampling. Parenting alliance was quantified using questionnaires, and the children's nutritional status was quantified using anthropometry, categorized based on Z-score weight-for-age. Data analysis used Pearson correlation coefficients and Multiple linear regression with α=0.05. The result showed no correlation between parenting alliance and children's nutritional status (p>0.05). The multiple linear regression analysis demonstrated that children's nutritional status was influenced by a child's age (β = -0.156, p=0.020) and the number of children in a family (β = -0.263, p=0.029). In conclusion, parenting alliance is not directly related to children's nutritional status in children with ARFID. Nevertheless, the child's age and the number of children in a family affect their nutritional status.

Abstract

Background: Women diagnosed with breast cancer often experience emotional distress, leading to a decrease in psychological well-being. Online self-management training during the COVID-19 pandemic is crucial for maintaining psychological well-being, as it is closely related to stress hormone cortisol levels, which play a vital role in boosting the body’s immune system. However, research on self-management strategies for breast cancer patients, particularly regarding psychological aspect and its relationship to cortisol levels during the COVID-19 pandemic, remains limited.Purpose: The purpose of this study was to measure the effectiveness of self-management training on psychological well-being and salivary cortisol levels in breast cancer patients. Methods: This study used a pre-post-test quasi-experimental design with a control group. The participants included 70 breast cancer patients undergoing chemotherapy at a hospital in Malang City, East Java, Indonesia, recruited using simple random sampling. Self-management training using the EDUDARA (Edukasi Kanker Payudara) was administered to the participants in the intervention group for six weeks. Data on wellbeing were collected using the Ryff Psychological Wellbeing Questionnaire, while salivary cortisol levels were measured with the DBD cortisol kit using the competitive ELISA method. SPSS with t-tests was used to analyze the data. Results: The average post-test psychological well-being score in the intervention group was 90.3, while in the control group, it was 82.69. The results of statistical tests showed a significant difference in psychological well-being between the two groups, with the intervention group showing greater improvement after the training. Additionally, the mean salivary cortisol level post-training was 4.531 in the intervention group and 6.169 in the control group (p=0.001). This indicates a greater reduction in cortisol levels in the intervention group after training, showing that participants’ psychological condition following the “EDUDARA” training was better than that of the control group. Conclusion: Self-management training had a positive effect on increasing psychological well-being and decreasing salivary cortisol levels among breast cancer patients. Therefore, structured self-management training for these patients can be used for nursing intervention
