Abstract

Objective: This literature review aims to explain the role of nephrectomy in the treatment of localized RCC and its effects on postoperative renal function, as well as the role of the new baseline glomerular filtration rate (NBGFR) estimation method in the determination of the RCC method. Material & Methods: Articles were obtained through online searches using Pubmed, ScienceDirect, Web of Science, and Wiley Online Library using keywords NBGFR, renal function, nephrectomy, or localized renal cell carcinoma. Results: The development of several diagnostic factors as well as operative factors has made partial nephrectomy (PN) replace radical nephrectomy (RN) which for decades has been the main therapy in localized RCC. Postoperative determination of renal function is a crucial factor in the selection of therapies and several methods of determining NBGFR have been proposed in several previous studies. Conclusion: The NBGFR measurement method can be an accurate guide in the determination of nephrectomy methods in the treatment of localized RCC while use of computer software can help improve accuracy in the implementation of radical nephrectomy in RCC.

Abstract

Traumatic brain injury (TBI) is the largest contributor to morbidity and mortality in various parts of the world. Skull base fracture (SBF) is one of the many manifestations that can occur in cases of mild to severe TBI. With varying patterns of TBI, it was necessary to review the characteristics of SBF, clinical manifestations, cerebrospinal fluid leakage, and complications. The data were taken from the medical records of SBF patients who were treated at RSUD (Regional Public Hospital) Dr. Soetomo in the period January 2014 - July 2019. Then, the data obtained were written on the collection sheet and analyzed descriptively using RKward. It was found that SBF most often occurs due to severe TBI (60.14%). 77.7% of SBF patients were male and 35.1% of all patients aged 15-24 years. The most frequent cause was traffic accidents (86.5%). The anterior cranial fossa (ACF) was the most frequently fractured part of the skull base (30.4%). There was a significant relationship between the severity of TBI with the occurrence of CSF leakage and complications. About 33 patients (22.3%) had complications such as pneumocephalus and 9 patients (6.1%) had meningitis. Complications in the form of brain abscess and hydrocephalus in 1 (0.7%) patient each. SBF often occurred in men of productive age 15-24 years. The ACF was the most frequently fractured part. The majority were caused by traffic accidents accompanied by severe brain injuries. The most common complications were pneumocephalus, meningitis, brain abscess, and hydrocephalus.

Abstract

The extended-spectrum b-lactamase (ESBL) producer bacteria until now were mostly identified in hospital environment. The aim of this study was to analyze the prevalence of ESBL-producing gut flora and distribution of ESBL encoding genes between hospitalized patient in Tropical Wards of Dr. Soetomo Hospital and patient from a primary health centre (PHC) as community environment in Surabaya. Thiry rectal swab samples from hospital of Dr. Soetomo patients and from PHC (60 samples in total) were collected for this study. Samples were screened in MacConkey agar supplemented with 2 mg/L of cefotaxim, incubated at 37ºC for 24 hours. Then the growing colony were confirmed with Disk Diffusion Synergy test (DDST) for diagnosis of ESBL producer. The identified ESBL producers were then identified the bacteria species by biochemical method. ESBL gene were detected by PCR with specific primers. The results showed that there was not difference of positif nuber of ESBL-producing bacteria gut floral between patients of Dr.Soetomo Hospital, 25/30 (83.3%) and PHC, 11/30 (36.7%) (p=1). The pattern of ESBL gene distributions among samples from hospital showed that SHV was 12%, TEM was 36%, and CTX-M was 80%, and from PHC were SHV 18.2%, TEM 27,3% and CTX-M 81,8%. Statistical analysis showed that the pattern was not significantly different among hospitals and PHC samples as shown by SHV gene (p=0,631), TEM (p= 0.715), and CTX -M (p=1). From each ESBL gene, the dominant genes that found producing ESBL were the CTX-M genes followed by TEM and SHV genes. The prevalence of ESBL producersin intestinal flora of both the hospital (83,3%) and the PHC (36,7%) was very high. There was not significant difference between the prevalence of ESBL producer in gut flora of hospitalized patients compared to PHC. There was found other patterns of ESBL gene combinations in the hospital of SHV+CTX-M genes, TEM+CTX-M, SHV+TEM+CTX-M genes and PHC, the combination pattern of SHV+CTX-M, TEM+CTX-M.

Abstract

Hightlight: • The effect of orlistat on glycemic control and weight loss in overweight or obese type 2 diabetes mellitus patients was determined • HbA1c and FPG level in overweight and obese type 2 diabetes mellitus patient can improved by orlistat as an adjuvant therapy. Abstract: Risk of Type 2 diabetes mellitus (T2DM) increases steadily with increasing overweight and obesity, and these two-health problems are emerging epidemics worldwide. Orlistat, a lipase inhibitor for weight loss drug, is often used in T2DM medication as adjuvant therapy, but effectiveness of the drug for improving glycemic control on T2DM patients is unclear. This study was to determine the effect of orlistat on glycemic control and weight loss in overweight or obese patients with T2DM. Term "Orlistat” AND "(obesity OR overweight)” AND "(HbA1c OR A1C)” AND "diabetes” were systematically searched in Pubmed and Science Direct web databases up to March 2021. Only randomized controlled study (RCT) methods studies were included in this study. Collected final samples were presented in a table with narrative review. There were 9 RCT studies with a total 2,175 subjects that met inclusion criteria. Of the sample, 360 mg/day orlistat as an adjuvant therapy, was administered to overweight or obese T2DM patients together with hypocaloric intake (8 studies) or without hypocaloric intake (1 study) intervention. They were examined for 12-52 weeks. From 2 short-term (12 weeks) studies, one study revealed that orlistat improved HbA1c and fasting plasma glucose (FPG) level significantly, while one study showed no significant effect compared to placebo. Seven other studies (long term observation) had found that orlistat significantly improved HbA1c and FPG level. All studies found that orlistat significantly reduced body weight. As an adjuvant therapy, Orlistat improved HbA1c and FPG level in overweight and obese T2DM patients.

Abstract

Uncontrolled hypertension can cause complications such as heart disease, brain disease, kidney disease, and other diseases. Medication adherence is a primary factor in controlling blood pressure (BP) in hypertensive patients. This study aims to provide an overview of the factors associated with medication adherence of hypertensive patients treated at Taman Public Health Center in Sidoarjo, Indonesia. This study used a cross-sectional approach involving hypertensive patients. This study employed consecutive sampling, with the number of samples comprised of patients undergoing outpatient care from January 2, 2023, to February 25, 2023, totaling 103 respondents. The study employed the Morisky Medication Adherence Scale 8-item (MMAS-8) questionnaire to evaluate medication adherence. Fifty-nine (57.3%) patients had poor adherence, and 83 (80.6%) patients had poor BP control. The significant factors associated with medication adherence were employment status (p = 0.021), drug regimen and prescription refill time (p = 0.001), and experiencing forgetfulness/negligence related to their medication (p = 0.011). Significant factors associated with systolic and diastolic BP are having health insurance (p = 0.030 and p = 0.006) and drug regimen-prescription refill time (p = 0.029 and p = 0.041). These factors can be used as considerations during policymaking to improve medication adherence and BP control of hypertensive patients.
