Abstract

Role of local health managers in innovating district level public health programs in Yogyakarta ProvincePurpose: The study aimed to describe the strategic decision making by the heads of the DIY health departments concerning the issue of maternal and child health and nutrition.Methods: This qualitative research was an exploratory study.Results: The district health office for Yogyakarta city in the Special Region of Yogyakarta does not have many strategic decisions to solve problems about maternal and child health and nutrition. However, the decisions are always hampered by human resources. Only the Gunung Kidul District Health Office stated that there was no political involvement in decisions by the head of the health department.Conclusions: The decisions made by heads of health department are still limited to the strategic plan and local medium term development plan (RPJMD). Not many innovations are developed to overcome the problems of maternal and child health and nutrition.

Abstract

Social support for people with mental disorders in poor areas: a case study in GunungkidulPurposeThe purpose of this study was to describe the role of mental health cadres in the effort of community based mental health service at Wonosari II Health Center Gunungkidul.MethodsQualitative research was done by case study approach. The cadres were chosen purposively with the criteria of: having attended training or socialization of mental health, having at least 2 years work experience related to community mental health service, and still active. Data collection was done through in-depth interviews and document utilization.ResultsCadres play an important role in providing social support. First, the cadre can show empathy to the family of people with mental disorders by building close relationships and facilitating the social acceptance of the community. Secondly, the cadres provide socialization related to mental disorders and mental health services. Third, approaches through home visits, referral assistance to health services, and health insurance and social assistance suggest that cadres facilitate access to care for people with mental disorders.ConclusionThere was a high social awareness of cadres to families with mental disorders in poor neighborhoods. We found that poverty does not limit people to share with others, and social support helps prevent mental illness from getting worse.

Abstract

PurposeThe aim is to examine the suitability of the educational process in place with the expected professional development.MethodWe conducted in-depth interviews with (a) 16 third-year student who recently completed 2-month clinical rotation in the hospital and (b) four teachers.ResultsEvidence from this study indicate weak learning environment. Clinical instructor only has a limited time to meet with students, so that students rarely get the guidance and direct supervision. Students more often obtain “lessons” of the young staff, which many of them indifferent in providing guidance. Students feel insecure and fear in communicating with senior staff from both the nursing and medical professions. Academic lecturers provide minimum learning support. Their supervision does not allow students to reflect on their experiences.ConclusionProfessional development as reflected in clinical rotations in hospitals is still very limited. The government must monitor more strictly, especially for the medical school located in the district, where professional educators are very limited.

Abstract

What do MPH student theses tell us? Experience from Universitas Gadjah MadaPuposeThis study aimed to evaluate MPH professional performance through thesis writing. MethodWe review 103 MPH student theses in health policy and management tract from data library of Faculty of Medicine Universitas Gadjah Mada since 2013 to 2015. We extract topic and reference from each thesis with a questionnaire.ResultsSeventy percent of students choose subject relevant to health policy and management tract. Sixty percent of theses are located in quadrant 3 because they have relevant topic but weak on use of appropriate reference. Only several journals in health policy and management are cited in theses such as health policy and planning, health policy, BMC health services research and human resources for health. More references related to other multidisciplines and published over 5 years. ConclusionMPH students are in need of clear understanding of the domain and scope of work they are interested in. Their theses showed limited evidence about references focus that has practical relevance to their future work or to the job they currently have.

Abstract

Purpose: Water refill depots have increased in all areas of Indonesia. Many studies have reported concerns about the bacteriological quality of drinking water at refill depots. However, limited studies have focused on ensuring the safety and quality of drinking water in refill depots, particularly for small-scale depots. This paper examines water microbiological quality at the refill depots and their depot production inspections from the local public health authority.Method: This survey used  60% of depots (47 out of 77 depots in the district) from April 4th to May 31st, 2016, in the North Luwu District of Central Sulawesi. Water samples were taken for E. coli and coliform examination. In-depth interviews were conducted with environmental health officers at the local health authority, depot owners, and workers.Results: Almost half of the total depots (49%) failed to meet the hygiene standard. Site observation found that depots were with poor sanitation and poor water handling. Depot owners found the cost of two times a year of the bacteriological examination was too expensive, including the transportation cost of water samples to the available closest laboratory in the provincial capital.Conclusion: Depots with non-standard bacteriological quality were still high. Supervision of the depot by the district health office was very loose. Efforts to advocate the district health office to take a more assertive position are critical. Community organizations and refill water depot associations should be involved so that public health interests get attention from the local government.

Abstract

Health center manager time spending for outside administrative versus field work in East Indonesian region: an analysis of community facility of 2012 IFLS East PurposeThis study evaluates time spent on administrative and field public health program of physician or non-physician community health center managers in East Indonesian region.  MethodThe study used 85 community health center managers from 7 provinces of East Nusa Tenggara, East Kalimantan, Southeast Sulawesi, Maluku, North Maluku, West Papua, and Papua from which the data was available in the community facility section of Indonesian Family Life Survey East (IFLS East) 2012.ResultsPhysician managers spent more hours than non-physician managers for administrative matters. In addition, non-physician managers had more hours per week for field program activities as compared to physician managers.ConclusionResearch shows that physician managers have potential lost of their time in performing their medical service functions, which they are trained for. Policy should be directed to improve the management skills of non physician health workers so that physicians can maximize their time to solve medical problems in health centers.

Abstract

Tujuan: Penelitian ini bertujuan untuk mengidentifikasi hubungan antara status pekerjaan ibu dan pemberian Air Susu Ibu (ASI) pada ibu bekerja yang tinggal di wilayah perkotaan. Metode: Penelitian ini merupakan penelitian kuantitatif dengan desain penelitian cross-sectional menggunakan data Indonesia Family Life Survey 5. Sampel dalam penelitian ini adalah ibu berusia 15-49 yang bekerja, memiliki anak berusia 2 tahun dan tinggal di perkotaan. Besar sampel dalam penelitian ini adalah 340 responden. Status pekerjaan dibagi menjadi wirausaha, pegawai pemerintah, pegawai swasta dan pekerja bebas, sedangkan pemberian ASI dibagi menjadi kurang dari 2 tahun dan selama 2 tahun. Analisis menggunakan chi-square dan regresi logistik dengan tingkat kemaknaan 95%. Hasil: Pada ibu bekerja yang tinggal di perkotaan, persentase ibu yang memberikan ASI kurang dari 2 tahun sebesar 68,82% sedangkan ibu yang memberikan ASI selama 2 tahun sebesar 31,18%. Status pekerjaan ibu yang tinggal di perkotaan paling banyak yaitu sebagai pegawai swasta (42,06%) dan wirausaha (34,41%). Status pekerjaan yang memiliki persentase pemberian ASI kurang dari 2 tahun adalah pegawai pemerintah (77,78%) dan pegawai swasta (75,52%). Terdapat hubungan antara status pekerjaan ibu dan pemberian ASI pada ibu bekerja yang tinggal di wilayah perkotaan. Ibu yang memiliki status pekerjaan sebagai wirausaha memiliki kemungkinan lebih besar 1,85 kali untuk memberikan ASI selama 2 tahun dibandingkan dengan ibu bekerja sebagai pegawai swasta. Kesimpulan: Ibu pegawai swasta memiliki kemungkinan untuk memberikan ASI selama 2 tahun yang rendah. Hak menyusui bagi pekerja wanita harus dilindungi dengan menyediakan ruang laktasi atau tempat penitipan anak.

Abstract

Tujuan: Maldistribusi dari dokter yang bekerja di kota dan desa, termasuk daerah terpencil dapat diprediksi dari faktor individual calon dokter. Jika ketika mahasiswa mereka sudah tidak berkeinginan, besar kemungkinan hal itu tidak berubah. Karena itu pemerintah tetap penting memikirkan sistem yang mendorong mahasiswa berkeinginan bekerja di daerah terpencil dan sulit. Faktor individu mahasiswa akan besar sekali tertarik bekerja di kota di tempat pasar bagi layanan kedokteran sangat tinggi.Metode: Sejumlah 205 mahasiswa kedokteran tahun keempat. 181 (dari 209) mahasiswa reguler dan 24 (dari 94) mahasiswa internasional). Self reported questionnaire melalui google form. Pertanyaan ya atau tidak untuk keinginan bekerja di daerah pedesaan dan daerah terpencil. Variabel prediktor mencakup jenis kelamin, besar di desa dan di kota, karir sebagai pekerja di puskesmas, dan suka mengelola program.Hasil: Penelitian ini membutikan temuan yang sejalan dengan prinsip pasar. Mahasiswa lebih tertarik bekerja di kota daripada di daerah pinggiran. Meski demikian, laki-laki lebih besar kemungkinan berkeinginan pergi ke daerah dibanding perempuan (57% versus 45%). Mahasiswa bermobil pribadi jauh lebih kecil berkeinginan pergi ke daerah dibanding mahasiswa pengendara motor (37% versus 56%). Di antara mereka yang ingin menjadi spesialis, yang belum bisa memutuskan karir ke depan, dan yang memiliki karir yang non spesialis, pilihan ke daerah konsisten lebih rendah daripada pilihan ke kota (kurang dari 45%). Hanya untuk yang berkeinginan memiliki karir di puskesmas, mereka lebih besar persentase yang ingin ke daerah (56% versus rata-rata 45%)Kesimpulan: Motivasi mahasiswa kedokteran berkarir mengikuti pola pasar. Agar distribusi dokter terpenuhi di daerah terpencil, pemerintah harus membangun kebijakan khusus agar mahasiswa tertarik bekerja di daerah terpencil.

Praktik Perilaku Pemberian ASI Bagi Wanita Studiatau Bekerja Di UniversitasBreastfeeding Practice of Students and Workers Women in The UniversityNoviana Nur Sari1, Mubasysyir Hasanbasri2, Citra Widya Kusuma3          AbstractIntroduction: Breast milk plays a role in the growth and development of infants and prevent stunting. Mothers who work or study have support and obstacles in the breastfeeding process.Purpose: This research aims to explore practical experience resistance strategies to keep giving breastfeeding for working or studying mothers in the university.Method: This research is a qualitative case study design study. The subjects are working or studying mothers who have breastfeeding experience while working or studying for a maximum duration last two years including lecturers, employees, and students.Results: Working or studying mothers have various resistance strategies such as keep milk stock during three months leave, pumping before going to work and come home and pumping in the office. The various strategies are used by students moms to continue breastfeeding such as go home every breaking time of lectures to express milk, find the empty private spaces to express milk, and choose courses that have breaking time, pumping in the car when women go back to home. Working mothers feel a lot of support and there are no obstacles compared to student moms. They have strong self-efficacy due to the mother's determination to continue breastfeeding even though returning to work or study and the mothers want to give the best rights of the children successfully.Conclusion: Self-efficacy working or studying mothers is caused by a strong determination to succeed give breast milk and provide the best rights for their children. It is a motivating factor for mothers to carry out various resistance strategies successfully despite several obstacles.Keywords: breastfeeding, strategies, student mom, university, working women.
