Abstract

Abstract — CV. 777 is a spice import company that has problems in mitigating events that are quite detrimental to the company. Starting from late arrival until over stock in the warehouse. The method used is the House Of Risk and SWOT methods, where with these methods the highest risk can be known and mitigation actions will be taken. This research resulted in 14 Risk Events and 27 Risk Agents where the Risk Agent with the highest ARP value will be followed up using the SWOT method. In the study, one Risk Agent with the highest ARP value or the one with the strongest influence was found, namely Risk Agent A12 with an ARP value of 255 (fluctuations in demand for goods by customers), while the Risk Agent with the weakest influence was A4 with an ARP value of 3 (imported goods). excessive) and A26 with an ARP value of 3 (miscommunication between employees). The risk agent with the highest ARP then continued using the SWOT method and resulted in four strategies, namely SO strategy, ST strategy, WO strategy, and WT strategy. With the mitigation strategy produced by the company, it will be able to minimize the risks that can occur to the company.

Abstract

Abstract

With the development of the times, people are increasingly aware that food is safe for consumption. Throughout the food supply chain process, there are many risks of food safety from being safe to being unsafe. This study aims to analyze food safety and halal risk mitigation in the supply chain, which consists of identifying food safety risks throughout the supply chain process, measuring food safety risks in the supply chain, determining the most critical value for risks that arise in food safety, and determining food safety risk mitigation measures. The object used is the supply chain of fish cracker companies in the Buduran area, Sidoarjo. Data processing was performed using FMECA and AHP methods. The results showed that the most critical risk lies in the management of the meat grinding system with a value of 0.60494 where the influencing factors are the absence of halal certification on the product and not implementing food safety standards, so that mitigation measures are carried out, namely evaluating and fostering manpower planning and job analytic.

Abstract

Astrans Putra Logistik Ltd. is a logistics service company with shipping routes in Sidoarjo, Denpasar, Lombok and NTB. Some of the problems that occur, such as delays in the pick-up process of goods to the load that large but not enough for departure, cause the quality of the supply chain system to be poor. So this research needs to be done with the aim of knowing the point of waste in the supply chain, identifying the sigma value, and determining the priority for its improvement. Quality improvement is carried out using the Lean Six Sigma method with AHP integration as a priority selection for improvement. From the research results, it was found that there was waste that caused 3 (three) CTQ, namely delivery was delayed, goods were damaged or leaked, and there was a difference in the number of goods. The company's DPMO value is 34272 with a sigma value of 3.34. The priority for improvements that can be made based on AHP weight, namely check the package packages before they are sent (0,353), make SOP for the preparation of goods (0,167), and apply SOP for goods checking documents (0,142).

Abstract

Pt X is a company that produces steel pipe of various shapes and size. In the production procces not only using machines but also using humans as operators. So in this case humans play an important role in maintaining the quality of production. The purpose of this study is to analyze the human error probability with Human Error Assessment and Reduction Technique method (HEART). HEART is a method designed as a fast and simple human reliability assessment in quantifying the risk of human error. From this research, it was found that 3 tasks had a high HEP value which caused the decline in pipe quality during the production process, namely task 3.2, 4.1 and task 3.4. Which has the highest HEP value on task 3.2, namely setting the machine with a value of 0,7680. The cause is due to the lack of operator expertice and the center roll was not carried out when installing it, so that training is needed to increase operator expertice.
