Abstract

Introduction: Human error is a major safety concern in upstream oil & gas operations because small performance failures may lead to near-misses, incidents, illness, and production loss. This study examined the determinants of human error among Indonesian upstream oil and gas workers, with emphasis on sleep, psychosocial strain, psychosocial strain, physical activity, work characteristics and recovery outside work. Method: A cross-sectional online survey was conducted among oil and gas workers in Indonesia. Of 1,650 respondents, 1573 valid responses were included in the analysis. Human errors was assessed using a composite score based on self reported errors and near-misses. Independent variables include age, sleep duration, sleep quality, physical activity, work entity, rotation type, shift type, distress, and secondary employment. Binary logistic regression was used to identify determinant factors associated with high human error. Result: High human errors was significantly associated with poor sleep quality (OR 5.08), high distress (OR 5.71), moderate distress (OR 2.83), maintenance and production work (OR 2.05), off-duty secondary employment (OR 1.69), short sleep durations of les than 7 hours (OR 1.55), and low physical activity (OR.155). Workers age 50-54 years had lower odds of high human error (OR 0.39). Rotational type, shift type, and on-duty secondary employment were not significant. Conclusion: Human error in this population appears to be driven more by sleep problems, psychosocial strain, inadequate recovery, and operational job demands. Prevention strategies should therefore extend beyond roster design and include sleep health, psychosocial risk control and recovery protection program and policy.

Abstract

Industri kelapa sawit merupakan salah satu sektor dengan tingkat risiko keselamatan dan kesehatan kerja (K3) yang tinggi, sehingga kualitas pengungkapan K3 dalam laporan keberlanjutan menjadi aspek penting dalam mendukung transparansi dan akuntabilitas perusahaan. Penelitian ini bertujuan menganalisis kualitas pengungkapan K3 pada perusahaan kelapa sawit yang terdaftar di Bursa Efek Indonesia (BEI) serta menganalisis faktor-faktor karakteristik perusahaan dan tata kelola yang berhubungan dengan kualitas pengungkapan tersebut. Penelitian menggunakan pendekatan kuantitatif dengan metode content analysis terhadap laporan keberlanjutan, laporan tahunan, dan laporan keuangan dari 28 emiten kelapa sawit selama periode 2022–2024. Kualitas pengungkapan diukur menggunakan indeks yang dikembangkan berdasarkan GRI 403:2018 dan POJK No. 51/POJK.03/2017. Analisis data dilakukan secara univariat, bivariat, dan multivariat terhadap variabel ukuran perusahaan, profitabilitas, leverage, independensi dewan komisaris, kepemilikan institusional, external assurance, dan kerangka pelaporan. Hasil penelitian menunjukkan bahwa rata-rata kualitas pengungkapan K3 mencapai 61,25, yang mengindikasikan tingkat pengungkapan yang relatif memadai, meskipun masih terdapat variasi yang cukup lebar antarperusahaan. Pengungkapan K3 juga cenderung didominasi oleh informasi mengenai kebijakan, sistem, dan program, sementara informasi yang merefleksikan hasil aktual pelaksanaan K3 masih relatif terbatas. Analisis multivariat menunjukkan bahwa kerangka pelaporan, external assurance, dan leverage berhubungan positif dan signifikan dengan kualitas pengungkapan K3, sedangkan ukuran perusahaan, profitabilitas, kepemilikan institusional, dan independensi dewan komisaris tidak menunjukkan hubungan yang signifikan. Temuan ini mengindikasikan bahwa kualitas pengungkapan K3 lebih dipengaruhi oleh faktor-faktor yang berkaitan dengan tata kelola pelaporan, kredibilitas informasi, dan akuntabilitas eksternal dibandingkan karakteristik ekonomi maupun struktur tata kelola formal perusahaan.

Abstract

Manufacturing and construction companies are significant contributors to most accidents in Indonesia. Accident frequency rate (AFR) can be reduced by adopting an appropriate management system, technical approach, and safety culture at the workplace. Employees' perception of Work Safety culture is the key to success in minimizing AFR. A survey of employees' perception of safety can identify their reality concerning the Safety Climate at their workplace. The study aimed to observe Safety Climate through a survey of employees' perception on the dimensions of management commitment, safety communication, safety rules and procedures, supportive environment, personal safety involvement and needs, and safety training. The study discusses possible ways to improve the safety climate in their organization and provides feedback to maintain and improve it. A mixed-methods descriptive design was used for this study. The sample size was determined using the Slovin formula and the sample selection using the Proportionate Stratified Random Sampling method. The survey was conducted on 156 employees using a questionnaire model with a personal approach to all levels of employees in a feminine care products manufacturing company. The survey result was analyzed quantitatively through assessments on questionnaires and qualitatively through employees' comments. The study found employees' perception of safety climate to be at a reasonable level. The management commitment dimension in this study has the best assessment result. The analysis results showed that the active role of the leadership, strong safety values to build employee interaction, employee involvement and investment in training programs for employees were critical keys to the successful implementation of Occupational Safety.

Abstract

Jakarta is the capital city of Indonesia, with a dense population of more than 10 million population. DKI Jakarta Province is divided into five city areas, including South Jakarta. South Jakarta has an area of 141,17 km2, with more than three million population. The scope for this study focused on two sub-districts in South Jakarta, which are Pasar Minggu and Jagakarsa. The objective of this study was to provide fire risk situations in two sub-districts of South Jakarta. The study used a descriptive method with non-probabilistic sampling. Data collection was performed from June 2021 to July 2021 through focus group discussions (FGDs) and filling out questionnaires. FGDs were performed to the Local Government Fire Brigade, and questionnaires were filled out by the head of the RW. The FGD guideline and questionnaire were developed based on the fire risk assessment checklist. The checklist instrument was developed by utilizing a disaster fire risk approach, including fire hazard, vulnerability, and fire protection, with twenty-two questions. The data was then analyzed using univariate analysis, bar, and spider chart. The results showed three RWs with severe-risk classification. The factors that increase the risk of fire are the use of combustible building materials and the high density of population and buildings. There were several recommendations contributed such as firefighters must have a list of the high-risk areas and develop a rescue evacuation plan, doing a routine inspection in each target area, and raising public awareness for the community to increase participation in fire risk management.
