Abstract

Asthma is a heterogeneous disease which is also one of the major health problems in the world. Uncontrolled asthma symptoms will exacerbate asthma, which is the biggest cause of the patient into the ER. Salbutamol is a bronchodilator that belonged to the short-acting beta-2 agonist (SABA), which is an obligatory choice in the management of asthma exacerbations. Unlike the aminophylline group in Indonesia which is still often used widely and rarely appear in the side effects of its use. The main objective of this study was to evaluate the cost-effectiveness of nebulized salbutamol versus intravenous aminophylline for reduction in asthma symptoms, improvement in lung function, and a long hospital stay. The study involved 57 adults asthma exacerbation patients without complication who received treatment in the ER, were studied by using quasi-experimental methods in this study cost-effectiveness analysis to compare intravenous aminophylline (n:27) versus nebulized salbutamol (n:30) therapy to determine the most cost-effective. Intravenous aminophylline therapy was more cost-effective than nebulized salbutamol in reducing the symptoms of asthma and PEF value improvement. Clinical outcome of the length of hospital stay outcome both had the same effective, and cost analysis results showed intravenous aminophylline (US$5.38) cheaper than nebulized salbutamol (US$5.71). intravenous aminophylline was more effective in reducing asthma symptoms than nebulized salbutamol (Pvalue=0.001). Meanwhile, the average decrease in lung function intravenous aminophylline than nebulized salbutamol, although not significantly different (Pvalue=0.507). Aminophylline therapy was more cost-effective in reducing asthma symptoms and improvement in lung function compared nebulized salbutamol in exacerbations asthma patients. But cost-effectiveness researchers need to measure substance abuse outcomes in terms of Quality-Adjusted Life Years (QALY), as this will make their findings more relevant to the development of treatment policy.

Abstract

Asthma is a major global problem. The annual economic burden of asthma is very large. These cost increases are caused by poor asthma management and management and an imbalance between the large number of health workers. This study aims to analyze the cost and effectiveness of treating asthma attacks at X Hospital Gresik by involving 23 respondents. This research is a non-experimental study which is retrospective in nature. As research material is medical record data of patients with asthma at the Petrokimia Hospital, during the 2020-2022 period. The location of the research was Pertokimia Hospital. The independent variable in this study is the therapy used. The dependent variable in this study is the total cost of treatment for treatment outcomes (hospitalization and improvement in asthma symptoms). Data collection was carried out through observation of patient medical records. Data analysis with Shapiro Wilk. The results of the difference test showed that there was no significant difference between classroom costs and total costs (P=0.513) and there was no significant difference between severity costs and total costs (P=0.275) in asthma patients. The number of cases of irrationality of treatment in all subjects (100%) with a total of 178 cases. There is no significant difference between real costs and INA-CBGs rates, total costs based on room class and severity level. Pembengkakan biaya tersebut disebabkan oleh penanganan dan pengendalian asma yang buruk dan tidak seimbang antara tenaga kesehatan yang cukup besar. Penelitian ini bertujuan untuk menganalisis biaya dan efektivitas pengobatan serangan asma di Rumah Sakit X Gresik dengan melibatkan 23 responden. Penelitian ini termasuk dalam penelitian non eksperimental yang bersifat retrospektif. Sebagai bahan penelitian adalah data rekam medik pasien dengan penyakit asma di Rumah Sakit Petrokimia, selama periode 2020-2022. Lokasi penelitian dilakukan di Rumah Sakit Pertokimia. Variabel bebas dalam penelitian ini adalah terapi yang digunakan. Variabel tergantung dalam penelitian ini yaitu total biaya penggobatan outcome pengobatan (rawat inap dan perbaikan gejala asma). pengumpulan data dilakukan dengan melalui pengamatan terhadap rekam medis pasien. Analisa data dengan Shapiro Wilk. Hasil uji beda menunjukkan tidak terdapat perbedaan yang signifikan antara biaya ruang kelas dengan biaya total (P=0,513) dan tidak terdapat perbedaan yang signifikan antara biaya keparahan dengan biaya total (P=0,275) pada pasien asma. Jumlah kasus ketidakrasionalitas pengbatan pada semua subjek (100%) dengan total kasus 178. Tidak ada perbedaan signifikan antara biaya riil dengan tarif INA-CBGs, total biaya berdasarkan kelas kamar dan tingkat keparahan. Abstract

Smoking habits have been shown to impact omega-3 food intake, and cigarette smoke can increase lipid peroxidation through the induction of oxidative stress.  Omega-3, an essential fatty acid, can be obtained through the consumption of fish, milk, eggs, and supplements. This cross-sectional research aimed to assess the relationship between omega-3 food intake and smoking habits. The research was conducted from March to June 2022 in Rungkut District, Surabaya, Indonesia. Data collected included the respondents’ quantities and frequency of consuming foods containing omega-3, namely fish, milk, and eggs. Additionally, the level of nicotine dependence among smokers was evaluated using the Fagerstrom Test for Nicotine Dependence (FTND). The method for data analysis was the chi-square test. The total respondents in this study were 116 people, consisting of 41 smokers and 75 non-smokers. No significant differences were observed in the consumption patterns of omega-3-rich foods, including eggs, fish, and milk, between smokers and non-smokers (p > 0.01 for each food item). However, a significant difference was observed in the consumption of supplements containing omega-3 between the smoker and non-smoker groups (p < 0.005). However, the overall omega-3 intake did not exhibit a significant difference between smokers and non-smokers. Notably, a higher proportion of respondents who smoked reported consuming omega-3 supplements compared to the non-smoker group. Consequently, there is a need to conduct further research to identify the underlying factors that influence the habit of smokers toward consuming omega-3.

Abstract

Illness perceptions can affect patient’s self-management behavior, thus affecting asthma therapy outcomes. This study aims to test the correlation between asthma perception and the outcomes of the therapy. The design was a cross-sectional with purposive sampling technique. Data was obtained by direct interview using questionnaire and was presented descriptively, from 22 asthma patients. Results reveal significant differences between consequences, timeline, emotional response, understanding, and compliance. Illness perceptions of patients regarding the identity, cause of illness, and cure or control were appropriate, while concerning the conse- quences and timeline were still lacking. These shortcomings should be alleviated to support an optimal pharmaceutical care and improving the quality of life. Persepsi terhadap penyakit asma dapat memengaruhi perilaku manajemen-diri pasien, sehingga memengaruhi hasil terapinya. Studi ini bertujuan melihat korelasi antara persepsi terhadap penyakit asma dan hasil terapi. Metode yang dipakai adalah belah silang dan teknik sampling bertujuan. Data diperoleh dari 22 pasien asma melalui wawancara langsung dan pengisian kuesioner. Terdapat perbedaan yang bermakna antara konsekuensi, perjalanan penyakit, respons emosional, pengertian dan kepatuhan pada perawatan farmasetis. Persepsi pasien terhadap penyakitnya menyangkut identitas, penyebab penyakit, dan penyembuhan atau pengendalian sesuai, sedangkan masih kurang terhadap konsekuensi dan perjalanan penyakit. Hal-hal yang masih kurang inilah yang perlu diatasi sehingga rawatan farmasetikal bisa optimal dalam memperbaiki kualitas hidupnya.

Abstract

Asthma is a non-communicable respiratory disease that can pose a risk of mortality and morbidity. Asthma is a long-term disease that carries the risk of drug-related problems (DRP). This study aims to analyze drug-related problem (MTP) in outpatient asthma treatment. This study used a cross-sectional analysis method using purposive sampling technique, carried out at a Pratama pharmacy in Wonokromo district, Surabaya City. The research sample was adult outpatient asthma patients who underwent treatment at the pharmacy from November 2018 to January 2019. Data were analyzed using GINA and Classification for Drug related problems. The results showed that of the 40 respondents the DRP was 34 respondents (85.00%). The domain related to the effectiveness is that the drug effect is not optimal (88.24%) and other drugs are not needed (11.76%) and the cause of drug selection is the improper combination of drugs (2.87%). And the associated therapy was most involved in DRP events. Outpatient asthma drug use was the most associated therapy for the short acting beta 2 agonist class of drugs. Therefore, inpatient asthma treatment requires monitoring by a pharmacist to achieve optimal treatment.

Abstract

Background: Smokers are at risk of Chronic obstructive pulmonary disease (COPD)which has a risk of decreasing in quality of life because of lung function decline and smoking habits. Decreased lung function due to an increase in free radicals. Intake of foods that contain antioxidants can help reduce free radicals.Objectives: The research aimed to determine the effect of antioxidant intake (vitamins A, C, and E) on the development of COPD risk in active smokers.Methods: This study was conducted in May-July 2023 in Surabaya. This study used a cross-sectional design, with purposive sampling and snowball sampling methods. The subjects were adult active smokers. Antioxidant intake consisted of the intake consumed by subjects daily containing vitamins A, vitamin C, and vitamin E using the Food Frequency Questionnaire (FFQ) method to describe nutritional intake in a certain period. COPD risk assessment was measured using a pulmonary function test with spirometry, with an assessment of the ratio of forced expiratory volume in one second (FEV1) / forced vital capacity (FVC) below 0.70 defined as airflow limitation. The data were tested using the Spearman test to see the relationship between antioxidant intake from food and lung function.Results: This study involved 49 active smokers with COPD. Most subjects experienced deficiencies in vitamin A (77.55%), and vitamin C (87.76%), and all subjects experienced vitamin E deficiencies. The most consumed food containing vitamin A was palm oil, the most consumed food containing vitamin C was papaya, and the most consumed food containing vitamin E was chicken. There was no relationship between vitamin A and the development of COPD risk (Sig.=0.187; r=-0.192), but there was a significant relationship (weak negative) between vitamin C and the risk of developing COPD (Sig.=0.031; r=-0.309). All subjects had a deficiency of vitamin E.Conclusions: The lower the intake of vitamin C, the greater the risk of COPD which is characterized by decreased lung function.

Abstract

Smoking is responsible for a high proportion of morbidity and mortality in Indonesia. Because of the loss in terms of health and economy, the smoking cessation program is the main treatment option for smokers. The information-motivation-behavioral skill (IMB) explains that knowledge is not the only determinant of behavior change, but also the motivation and behavioral skills of individuals. The purpose of the study was to develop a health education strategy by exploring the conditions of knowledge that could affect motivation to quit smoking. This research employed interpretive phenomenological analysis (IPA) on male young active smokers as students in Surabaya, East Java, from April to July 2021. The collection of samples employed snowball and purposive sampling, utilizing the in-depth interview method until data saturation was achieved. The data analysis process involved coding and verbatim transcription of the interview results, which described the interview process and provided research reflection on the interview results (self-reflection). The study was conducted on 23 students. Most respondents fell within the 20–23-year age range. On the topic of the dangers of cigarettes, they showed they only knew that they were dangerous, but didn't know to what extent nicotine and tar were hazardous to health. On the dangers of smoking to health topic, respondents knew the impact of tobacco on health, but didn't care about the effects on health. In conclusion, the level of knowledge related to smoking cessation can inform therapeutic options tailored to individual conditions.

Abstract

Background: Physical activity among women of productive age plays an important role in maintaining physical and mental health as well as preventing non-communicable diseases. Objective: This study aimed to analyze physical activity patterns and perceptions regarding the health benefits of physical activity among women of productive age. Methods: This study employed a mixed-methods approach using a convergent parallel design. The quantitative phase used a cross-sectional approach, while the qualitative phase applied interpretative phenomenological analysis (IPA). The study involved women aged 18–45 years living in Rungkut District, Surabaya. A total of 130 respondents participated in the quantitative phase using purposive sampling, while 20 respondents were involved in in-depth interviews during the qualitative phase. Physical activity was measured using a modified International Physical Activity Questionnaire (IPAQ). The questionnaire validity and reliability tests showed valid items with Cronbach’s alpha value of 0.84. Results: The results showed that most respondents frequently engaged in physical activity (73.18%) with predominantly light-intensity activities (56.92%). Respondents generally understood the health benefits of physical activity, including maintaining muscle health, controlling blood pressure, improving sleep quality, and maintaining mood stability. Chi-square analysis showed no significant relationship between the frequency of physical activity and respondents’ perceptions of health benefits (p>0.05). Qualitative findings revealed that respondents perceived daily activities such as house cleaning, walking, exercise, and yoga as forms of physical activity. Time constraints, fatigue, and busy daily schedules were identified as barriers to regular exercise. Conclusion: Most women of productive age demonstrated good awareness regarding the health benefits of physical activity, although this awareness was not significantly associated with the frequency of physical activity performed.

Abstract

AAsma adalah penyakit yang ditandai dengan peradangan saluran napas yang mengakibatkan obstruksi saluran napas yang bersifat reversible. Penatalaksanaan asma bertujuan untuk meningkatkan dan mempertahankan kualitas hidup pasien agar pasien dapat menghilangkan dan mengendalikan gejala asma, pasien dapat mencegah eksaserbasi akut serta memelihara fungsi paru agar pasien tidak mengalami kematian karena asma. Tujuan penelitian ini adalah untuk mengetahui masalah terkait obat (MTO) dan terapi yang paling banyak terlibat pada kejadian MTO pada pasien asma yang menjalani rawat inap di suatu rumah sakit umum di Sidoarjo. Desain penelitian adalah retrospektif dengan menggunakan data rekam medik sampel asma pada tahun 2017-2018. Dari total 100 data rekam medik pasien, sebanyak 143 (97,28%) kasus terkait masalah obat saat pengobatan di IGD dan sebanyak 202 (98,06%) kasus terkait masalah obat saat menjalani rawat inap. Terapi yang paling banyak terlibat kejadian MTO adalah ceftriaxone dengan jumlah 86 (60,14%) kasus saat pengobatan di IGD dan saat menjalani rawat inap adalah ceftriaxone dengan jumlah kasus sebesar 112 (55,44%) kasus. Oleh karena itu pengobatan asma di rumah sakit harus mendapatkan perhatian agar dapat mencapai hasil yang optimal dalam efektifitas dan keamanan.
