Abstract

Kombinasi glimepiride-metformin merupakan lini pertama pengobatan diabetes mellitus tipe 2 (DMT 2) dan kombinasi glimepiride-pioglitazon saat ini masuk dalam daftar pedoman pelayanan kefarmasian pada pasien diabetes mellitus. Penelitian ini bertujuan untuk mengetahui kombinasi terapi manakah yang paling cost-effective antara kombinasi glimepiride-pioglitazon dibandingkan kombinasi glimepiride-metformin dalam menurunkan gula darah puasa dan HbA1c. Pada penelitian ini didapatkan 60 sampel pada kelompok kombinasi obat glimepiride-pioglitazon dan 40 sampel pada kelompok kombinasi glimepiride-metformin. Desain penelitian yang dilakukan dengan metode observasional dan alur pengumpulan data secara retrospektif. Analisa data menggunakan perhitungan ACER (Average Cost-Effectiveness Ratio) dan ICER (Incremental Cost-Effectiveness Ratio) dan uji Mann Whitney. Hasil analisis pada kombinasi glimepiride-pioglitazon, yang memenuhi target GDP (gula darah puasa) sebanyak 21,67%, GDA sebanyak 45,00% dan HbA1c sebanyak 36,67% dengan rata-rata biaya pengobatan Rp 2.184.810,00. Sedangkan pada kelompok kombinasi glimepiride-metformin, GDP yang tercapai 2,50%, GDA yang tercapai 20,00% dan HbA1c yang tercapai sebanyak 20,00% dengan rata-rata biaya pengobatan Rp 2.198.060,00. Hasil analisis efektifitas biaya dalam menurunkan kadar GDP, GDA dan HbA1c menunjukan perbedaan efektifitas biaya untuk pengurangan per unit pada pencapaian glukosa darah dan HbA1c yang signifikan. Kombinasi glimepiride-pioglitazon merupakan terapi cost-effective yang signifikan sebagai terapi DMT2 dalam menurunkan GDP (ICER=Rp -387,767), GDA (ICER=Rp -935,074), dan HbA1c (ICER=Rp -794,841). The glimepiride-metformin combination is the first line of treatment for T2DM and the glimepiride-pioglitazone combination is currently included in the list of pharmaceutical service guidelines for diabetes mellitus patients. This study aims to find out which combination of therapy is the most cost-effective, the glimepiride-pioglitazone combination compared to the glimepiride-metformin combination in reducing fasting blood sugar and HbA1c. In this study, 60 samples were obtained in the glimepiride-pioglitazone drug combination group and 40 samples in the glimepiride-metformin combination group. The research design was carried out using an observational method and a retrospective data collection flow. Data analysis used ACER (Average Cost-Effectiveness Ratio) and ICER (Incremental Cost-Effectiveness Ratio) calculations and the Mann Whitney test. The results of the analysis on the glimepiride-pioglitazone combination, which met the GDP target (fasting blood sugar) of 21.67%, GDA of 45.00% and HbA1c of 36.67% with an average treatment cost of IDR 2,184,810.00. Meanwhile, in the glimepiride-metformin combination group, GDP was achieved at 2.50%, GDA was achieved at 20.00% and HbA1c was achieved at 20.00% with an average treatment cost of IDR 2,198,060.00. The results of the cost-effectiveness analysis in reducing GDP, GDA and HbA1c levels show that the difference in cost-effectiveness for the reduction per unit in achieving blood glucose and HbA1c is significant and the combination of glimepiride-pioglitazone is a significant cost-effective therapy as DMT2 therapy in reducing GDP (ICER=IDR -387,767), GDA (ICER=IDR -935,074), and HbA1c (ICER=IDR -794,841).

Abstract

Asthma in children may affect children's efficiency at school because frequent attacks occur which disrupt children's activities, so that asthma in children is a serious problem that requires appropriate treatment. The aim was to know the difference in real costs and INA-CBGs rates and to analyze Drug-Related Problems (DRP) based on the severity of asthma and the real costs for childhood asthma at Anwar Medika Sidoarjo Hospital. The design of this study was retrospective using patient medical record data collection, for January 2020-December 2022. Data collection containing real cost data and BPJS claim cost data. Analysis of DRP based on PCNE (Pharmaceutical Care Network Europe) includes problems and causes. There were 77 subjects. the real costs for the severity of mild asthma class I, class III, and moderate asthma class II were high when compared with the INA-CBGS rates, while the severity level of severe asthma class III it was found that the real costs were lower than the INA-CBGS rates. There is no difference between total real costs and INA-CBGS rates for asthma cases in BPJS participating children. The average real cost is Rp. 2,557,453 and the average INA-CBGs tariff is Rp. 2,792,873. There was no significant difference between real costs and INA-CBGS rates (P=0.162). All respondents experienced DRP (100%) and the highest incidence of DRP was M3.1 and P1.2 in 44 people (57.14%) with 180 cases (60.82%). In 77 patients with a total number of cases of 296, the real costs were mostly incurred in class III mild asthma patients (58 people), namely DRP type M3.1 with P1.2 with an average cost of Rp. 2,640,221.

Abstract

Pharmacists are one of the health workers who play a role in providing information related to explanations of how to use inhaler preparations to asthma patients. This study aims to determine the effectiveness of providing educational videos on how to use several types of inhalers (MDI, MDI with spacers, Diskus®, and Bodyaler® given to pharmacist students. This research is a pre-post design (experimental) study. Sampling was conducted using the method non random sampling using purposive technique, normality test using Shapiro-Wilk and continued.From the results of the data obtained, each assessment verbally (as explained by the subject) and motoric assessment (as demonstrated by the subject) is seen from the pre- and post-intervention Comparing the completeness of explanations both verbally and motorically, showing significant differences compared to before education, with P values ​​for MDI (P (0,000) ≤0.05), MDI with spacers (P (0,000) ≤0.05) Diskus® (P (0,000) ≤0.05), and Turbuhaler® (P (0,000) ,00.05), therefore video education has proven to be effective in increasing student proficiency. self-pharmacist to explain how to use inhalers.

Abstract

BACKGROUND: Acute decompensated heart failure (ADHF) is a prevalent and complex condition that significantly burdens healthcare systems, requiring intensive care and leading to high treatment costs. OBJECTIVES: This study aims to identify factors influencing cost discrepancies in hospitalized ADHF patients. METHODS: This retrospective study was conducted at Universitas Brawijaya Hospital from July to August 2024. Data were collected from 86 ADHF patients who were hospitalized between January 2021 and December 2023. Information regarding the patients' clinical conditions, comorbidities, and medical procedures was extracted from their case histories. Statistical analyses included t-tests and Mann-Whitney tests. RESULTS: In this study of 86 individuals with ADHF, 58.1% were over 65 years old, 31.4% were between 45 and 64 years old, and 10.5% were between 18 and 44 years old. By classification of care, 58.1% were admitted for Class 1 care, 30.2% for Class 2, and 11.6% for Class 3. Our findings indicated that the costs of treatment for patients with moderate and severe diseases were higher as compared to those of mild severity. Patients who had a length of stay over 7 days had higher costs than the ones whose length of stay was 1 to 3 days. Furthermore, Class 2 care was associated with higher costs than Class 3 care. The analysis also revealed that an increase in the number of comorbidities and medical procedures corresponded with higher treatment costs. CONCLUSION: This study identified factors that increase the cost of treatment for patients with ADHF.

Abstract

Vitamin D deficiency or vitamin D deficiency can increase the incidence of respiratory diseases. The of individuals have less knowledge and attitudes towards health problems. The objective was determine relationship vitamin D status and knowledge-attitudes health Behaviour about vitamin D. This type of research was a cross-sectional research design. The research location used in this study was around the Surabaya area starting in March-July 2022. The research location was carried out in Rungkut District, Surabaya, Indonesia. The variables were vitamin D status and knowledge and attitudes on smoking health Behaviour about vitamin D. The relationship between vitamin D status and knowledge-attitudes on smoking health Behaviour about vitamin D using a contingency coefficient. There was no significant relationship with vitamin D status and knowledge on smoking health Behaviour about vitamin D (p=0.685), but there was a significant relationship with vitamin D status and attitude on smoking health Behaviour about vitamin D, but the relationship was low or weak (p=0.000). A positive attitude will reduce the risk of vitamin D deficiency which can boost the immune system during the COVID-19 pandemic.

Abstract

Smoking is one of the biggest threats to public health in the world, so pharmacists should have a role in a multidisciplinary team to provide pharmaceutical care in smoking cessation in pharmacy. The research purpose was to determine the level of knowledge and experience of pharmacist pharmacy services on smoking cessation programs at pharmacists in East Surabaya. This research was mixed-method research, using the explanatory sequential design strategy. Data collection was carried out from October 2021 to January 2022 in Surabaya, East Java, Indonesia. The research variable were knowledge and experience related to pharmaceutical care in smoking cessation. The sample were pharmacists who carried out clinical pharmacy services at pharmacies in East Surabaya. Respondents were interviewed in depth until the data reached the saturation level (saturation so that no new answers/explanations are found) in order to obtain good and usable qualitative data. This study involved 40 respondents. Most of the respondents had sufficient knowledge level (57.50%). Many participants attended seminars on smoking cessation (55%), but none of the respondents had experience in pharmaceutical services. Some respondents have experience in giving advice on smoking cessation to patients during drug delivery at pharmacies (62.50%). Most of the respondents knew dangers of smoking and its relation to health, but most of them cannot explain the theoretical mechanism of the disease pathophysiologically. Pharmacists in pharmacies had sufficient knowledge about smoking cessation, but the practice of pharmaceutical services was still lacking. Abstract

Chronic obstructive pulmonary disease (COPD) is a major global health problem and has an impact on increasing healthcare costs and decreasing productivity. Patients with COPD have a higher prevalence of coronary ischemia and other factors that put them at higher risk for COVID-19-related complications. A strong immune system can help increase the body's resistance so as to reduce the risk in the midst of the COVID-19 pandemic. The use of natural therapy in the treatment of chronic diseases is an opportunity for Indonesia as a tropical country that has many variations of plants that have the potential to become herbal medicines. Red ginger (Zingiber officinale var. rubrum) is one of the most widely consumed medicinal plants in Indonesia. Red ginger has greater anti-inflammatory and antioxidant content than other types of ginger. The antioxidant and anti-inflammatory effects of red ginger in addition to helping reduce the effects of reducing the risk of COVID-19 severity, can also help improve lung function. The preparation of red ginger that has been used in the community is steeping. Therefore, red ginger has the potential to effectively reduce the risk of COVID-19 severity in COPD patients with anti-inflammatory and antioxidant effects in reducing NF-B and the effectiveness of improving lung function.

Abstract

Cigarettes are one of the causes of health problems in the world. Smoking has been known to have a direct impact on reducing lung function. Smoking increases the risk of vitamin D deficiency. Vitamin D is a vital nutrient as a lung immunomodulator. Deficient levels will cause problems with lung health, especially in active smokers. This cross-sectional research using multivariate path analysis and the SEM-PLS method has three objectives. The direct influence of three independent variables, including obesity, smoking, and demographics. Regarding vitamin D status and lung function, analyzing the direct effect of vitamin D on lung function, and thirdly, analyzing the indirect one of the three independent variables on lung function through vitamin D levels. This research took time from October 2023 to January 2024 and involved 47 active smoker respondents whose vitamin D level status and lung function were measured. There was a significant direct effect of obesity level on vitamin D levels in the active smoker population (p<0.05;f-square=2.889). While demographic factors (p>0.05;f-square=0.030) and smoking frequency (p>0.05;f-square=0.003) did not have a direct significant effect. Demographic factors, obesity, and smoking frequency don’t have significant direct effect on lung function in active smokers (p>0.05).

Abstract

Smoking can affect vitamin D levels in the blood. Decrease in vitamin D levels are also influenced by a person's lifestyle habits, such as smoking habits, sun exposure, food intake and physical activity. This study aimed to determine relationship between vitamin D with lifestyle factors in active smoker. This research used cross-sectional design. The subjects were adult male active smokers in Rungkut Subdistrict, Surabaya City, from October 2023 to February 2024. The sampling method was purposive sampling technique. Data collection by measurement of smoking severity (Brinkman Index (BI)), vitamin D levels in blood (serum 25(OH)D), food intake (by SQ-FFQ (semi-quantitative food frequency questionnaire) and 24 hour food recall), and physical activity (by IPAQ (International Physical Activity Questionnaire)). The relationship using Spearman method (ordinal scale). There were 47 respondents. The majority of research respondents were moderate smokers, namely 38 people (80.85%). The respondents had an average 25(OH)D level of 23.19±8.86 ng/mL. The risk of vitamin D deficiency due to lack of sun exposure were 26 people (55.32%). The average consumption of foods containing vitamin D by SQ-FFQ was 5.75±5.68 mcg/day, and 24 hour food recall was 5.77±5.72 mcg/day. Most respondents had high physical activity (70.21%) and total mean was 8210.23±4955.89. The Spearman correlation coefficient was 0.742 (smoking severity), 0.117 (sun exposure), and 0.726 (food intake by FFQ), 0.742 (food intake by 24 hours recall), and 0.824 (physical activity). Vitamin D levels had significant association on severity smoking, food intake and physical activity. However, were not related to sun exposure.

Abstract

Diabetes melitus adalah penyakit yang menyebabkan morbiditas dan mortalitas di dunia. Hubungan antara diabetes melitus dengan obesitas telah diketahui, tetapi peran distribusi pada daerah abdominal belum sepenuhnya dijelaskan. Tujuan penelitian ini untuk mengetahui pengaruh pemberian edukasi kesehatan terhadap perubahan lingkar pinggang pada pasien diabetes melitus tipe 2. Penelitian ini merupakan studi eksperimental dengan satu kelompok desain pretest-posttest di Surabaya pada bulan Juni - Juli 2013, dan melibatkan 31 subjek yang diberi program edukasi kesehatan selama 1 bulan yang dibagi dalam 4 pertemuan. Data dari pretest dan posttest dikumpulkan dan dianalisis dengan metode wilcoxon signed rank test pada data yang tidak normal (p > 0,05). Hasil penelitian menunjukkan peningkatan pengetahuan dan perilaku pasien serta mengurangi besar lingkar pinggang pasien secara signifikan (p < 0,05). Lingkar pinggang mulai menunjukan perbedaan signifikan pada minggu ketiga edukasi (Z = 12,93; P = 0,003) dibandingkan pretest. Penurunan lingkar pinggang antara posttest dan pretest sebesar 0,94 cm (minggu ke-4) dan 1,68 (4 bulan kemudian). Oleh karena itu, edukasi kesehatan memiliki manfaat dalam meningkatkan self-monitoring diabetes melitus yang dapat mengurangi lingkar pinggang yang berguna untuk mengurangi risiko komplikasi. Diabetes mellitus is a disease that causes morbidity and mortality in the world. Association of diabetes mellitus with obesity are well known, but the role of distribution in the abdominal area has not been fully elucidated. The aim of this study was to determine the relationship of giving education to change waist circumference in type 2 diabetes mellitus. This study is an experimental with one group pretest-posttest design in Surabaya, from June July 2013, and involving 31 subjects whom were given health education program for 1 month which are divided in 4 meetings in Surabaya University. Data from the pretest and posttest were collected and analyzed with the Wilcoxon signed rank test method on the data that isn’t normal (p > 0.05). The results showed an increase in knowledge and behavior of patients and reduced significantly the size of the waist circumference (p <0.05). Waist circumference began to show a significant difference in the third week of education (Z = 12.93, P = 0.003) compared to the pretest. The decrease in waist circumferences between the pretest and posttest were 0.94 cm (week 4) and 1.68 (4 months later). Therefore, health education has benefits in increasing self-monitoring of diabetes mellitus that can reduce waist circumference are useful for reducing the risk of complications.
