Abstract

Background: Intradialytic muscle cramps are common complications experienced by patients with Chronic Kidney Disease (CKD) undergoing hemodialysis and may interfere with patient comfort and dialysis effectiveness. Intradialytic stretching exercise is a non-pharmacological nursing intervention that may help reduce these symptoms. Case Presentation: Two young adult patients with CKD undergoing routine hemodialysis at Jakarta Islamic Hospital, Cempaka Putih, experienced recurrent lower-extremity muscle cramps during hemodialysis sessions, particularly during the final hours of treatment. The first patient was 21 years old with an initial pain score of 4, while the second patient was 22 years old with an initial pain score of 5 based on the Numeric Rating Scale (NRS). Intervention and Outcomes: Both patients received intradialytic stretching exercises during two hemodialysis sessions, including ankle dorsiflexion movements and stretching of the gastrocnemius, soleus, hamstring, and quadriceps muscles. Following the intervention, both patients demonstrated reduced pain intensity and fewer muscle cramp complaints. The patients also appeared more relaxed and comfortable during hemodialysis sessions. Conclusion: Intradialytic stretching exercise may serve as an effective non-pharmacological nursing intervention to help reduce muscle cramp pain in patients with CKD undergoing hemodialysis. This intervention is simple to implement and may be considered as part of routine nursing care in hemodialysis units. Abstract

The enthusiastic engagement of Indonesian society in democratic processes, particularly during presidential elections and representation selections, highlights a notable manifestation of fanaticism across various domains. This phenomenon, while not novel, often generates conflicts, tensions, and mental health challenges. Methods: This article provides an overview of the impact of fanaticism on mental health using a literature synthesis approach. A systematic search was conducted across three reputable databases: Google Scholar, PubMed, and Taylor and Francis Online. The search strategy included keywords such as "fanaticism," "mental health," "Indonesia," and related terms. Articles were selected based on their relevance to the topic and inclusion of empirical evidence or theoretical insights. After the initial search, articles were screened based on their titles and abstracts to identify relevant literature. Full-text articles were then retrieved and thoroughly examined to assess their suitability for inclusion in the synthesis. Articles that met the inclusion criteria were further analyzed. Data synthesis involved identifying common themes, patterns, and trends across the selected articles, facilitating a comprehensive understanding of the impact of fanaticism on mental health in the Indonesian context.Results: Findings depict fanaticism as more than mere admiration, evolving into a profound commitment that shapes individuals' self-perception. This internalization fosters a belief in absolute righteousness, intolerance towards dissenting viewpoints, and alienation of those outside the established narrative. Consequently, individuals may experience psychological repercussions such as anxiety, anger, and frustration. Moreover, fanaticism can culminate in severe outcomes including antisocial behavior and self-harm. Discussion: Exploring the connection between fanaticism and mental health underscores the importance of understanding and addressing this complex phenomenon in society. By acknowledging the psychological impacts of fanaticism, interventions and support mechanisms can be developed to mitigate its adverse effects and promote societal well-being

Abstract

Coronary artery disease (CAD) patients undergoing preventive coronary artery bypass graft (CABG) surgery commonly experience psychological distress in the form of anxiety, stress, and depression, which can hinder the recovery process. Mindfulness-Based Stress Reduction (MBSR) programs are known to be effective in reducing psychological burden in various chronic disease conditions, but research in the CHD population after preventive CABG is limited. Objective to determine the effect of a mindfulness-based stress reduction (MBSR) intervention on psychological distress in CHD patients undergoing preventive CABG. The study used a true-experimental design. The sample consisted of CHD patients who met the inclusion criteria, divided into intervention and control groups. The intervention group participated in the MBSR program for 21 days, while the control group received standard care. A total of 12 respondents participated. Stress, anxiety, and depression levels were measured using standardized instruments such as the Hospital Anxiety and Depression Scale (HADS) instrument, the level of stress using the Perceived Stress Scale (PSS) instrument. Data analysis was performed using bivariate paired t-tests and multivariate. The MBSR program demonstrated a significant reduction in stress, anxiety, and depression levels in the intervention group compared to the control group (p < 0.05). The difference in mean psychological scores before and after the intervention indicated a greater improvement in mental well-being in MBSR participants. The MBSR intervention was effective in reducing psychological distress in coronary artery disease patients after CABG prevention.

Abstract

Cardiac catheterization is an invasive procedure that involves the insertion of one or more catheters into the heart and certain blood vessels to visualize the heart chambers, valves, great vessels, and coronary arteries. Prolonged immobilization in patients after cardiac catheterization can cause complications, one of them is back pain. The nursing intervention in this study was back and hand massage. This study aims to determine the effectiveness of back and hand massage interventions on pain in patients after cardiac catheterization. This study used a quasi-experimental method with a pre-test and post-test design, with an intervention group and a control group design. The sampling technique used purposive sampling. The sample size was 36 patients who had cardiac catheterization, calculated using the Federer formula. The intervention group received a total of 20 minutes of massage therapy: 10 minutes of back massage, 10 minutes of hand massage, the first period was carried out after the sheath was removed, and the second period was carried out after the elastic bandage was removed. The data analysis technique used multivariate with the Generalized Linear Model (GLM) Repeated Measures (RM) method. The results showed that back and hand massage had an effect on pain in post-cardiac catheterization patients, with a P value of 0.000. This indicates decreasing a pain scores after the intervention. Multivariate testing indicated that previous experience with catheterization pain influenced pain in post-catheterization patients. Back and hand massage intervention is expected to be an alternative supportive intervention, especially in post-catheterization patients, in reducing pain.

Abstract

Kegiatan pengabdian kepada masyarakat ini bertujuan untuk meningkatkan pengetahuan dan keterampilan perawat hemodialisis dalam menerapkan pendekatan perawatan holistik guna memperbaiki kualitas pelayanan di unit hemodialisis. Pelatihan dilakukan melalui metode workshop dan pendampingan yang mencakup empat aspek utama, yaitu pelayanan prima, komunikasi terapeutik, serta manajemen stres pada pasien, keluarga, dan perawat. Pengukuran dilakukan dengan menggunakan pre-test dan post-test terhadap 17 perawat peserta pelatihan. Hasil kegiatan menunjukkan adanya peningkatan pengetahuan perawat sebesar 20% setelah pelatihan, yang mengindikasikan efektivitas program edukasi dalam memperluas wawasan dan kesadaran perawat mengenai pentingnya pendekatan bio-psiko-sosial-spiritual dalam asuhan keperawatan. Selain peningkatan kognitif, kegiatan ini juga berkontribusi pada peningkatan kemampuan psikomotor dan afektif, khususnya dalam pelaksanaannya melalui komunikasi terapeutik serta manajemen stres. Dampak kegiatan ini terlihat pada meningkatnya motivasi kerja perawat, penurunan stres perawat, serta meningkatnya hubungan positif antara perawat, pasien, dan keluarga. Kegiatan ini memberikan kontribusi nyata terhadap peningkatan kompetensi profesional perawat dan diharapkan dapat menjadi model berkelanjutan dalam pengembangan pelayanan keperawatan holistik di unit hemodialisis.

Abstract

ABSTRAK Pasien yang menjalani hemodialisis membutuhkan dukungan keluarga yang optimal. Namun, keluarga sering menghadapi beban perawatan dan keterbatasan dalam memahami kondisi pasien serta berkomunikasi secara efektif, sehingga dapat memengaruhi kualitas dukungan yang diberikan. Menguatkan kapasitas keluarga melalui edukasi untuk meningkatkan pemahaman penyakit, keterampilan komunikasi terapeutik, manajemen stres, serta mengurangi beban perawatan. Metode: Kegiatan pengabdian kepada masyarakat ini dilaksanakan melalui edukasi terstruktur mengenai penyakit dan perawatan hemodialisis, pelatihan komunikasi efektif, latihan teknik manajemen stres, dan diskusi kelompok. Sebanyak tiga puluh empat keluarga pasien berpartisipasi. Evaluasi dilakukan menggunakan pre-test dan post-test pengetahuan serta pengukuran beban perawatan menggunakan instrumen beban keluarga. Terdapat peningkatan pengetahuan keluarga sebesar 20% setelah intervensi. Selain itu, terjadi penurunan beban perawatan yang ditunjukkan oleh penurunan rerata skor dan pergeseran kategori beban menuju tingkat yang lebih ringan. Kesimpulan: Edukasi komprehensif efektif dalam meningkatkan kapasitas keluarga, memperbaiki kualitas komunikasi keluarga–pasien, dan mengurangi beban perawatan. Program ini memiliki dampak positif dan berpotensi menjadi model pendampingan keluarga di unit hemodialisis. Kata Kunci: Edukasi Keluarga, Hemodialisis, Beban Perawatan, Komunikasi Terapeutik, Manajemen Stres, Dukungan Keluarga.  ABSTRACT Patients undergoing hemodialysis require strong family support, yet family caregivers often experience a heavy caregiving burden and limitations in understanding the patient’s condition and communicating effectively. To strengthen family capacity through education aimed at improving disease understanding, therapeutic communication skills, stress management, and reducing caregiving burden. Methods: This community engagement program consisted of structured educational sessions on hemodialysis care, effective communication training, stress-management exercises, and group discussions. Thirty-four family caregivers participated. Evaluation included pre- and post-test knowledge assessments and caregiving burden measurements using a standardized family burden instrument. Results: Family knowledge increased by 20% following the intervention. A reduction in caregiving burden was observed, reflected by decreased mean burden scores and a shift toward lighter burden categories. Conclusion: Comprehensive education effectively enhances family capacity, improves patient–family communication, and reduces caregiving burden. This program demonstrates positive outcomes and may serve as a model for family-support interventions in hemodialysis units. Keywords: Family Education, Hemodialysis, Caregiving Burden, Therapeutic Communication, Stress Management, Family Support.
