Abstract

Introduction: Health services at PKU Muhammadiyah Sampangan Hospital require nurses to have good nursing management skills, but initial survey results show that there are still limitations in knowledge and skills in the areas of case rounds, conferences, patient classification, leadership, and communication using the ISBAR format. Objective: to improve nurses' knowledge and skills through a nursing management training and mentoring program. Methods: The program was conducted in 14 sessions using lectures, interactive discussions, case simulations, role plays, and live demonstrations in the treatment room. Evaluation was conducted using pre-tests and post-tests to measure changes in participants' abilities. Results: The program showed an increase in average knowledge in all aspects, including case rounds (59.77 to 69.09), ISBAR (81.43 to 90.00), conferences (95.91 to 98.18), leadership (73.68 to 85.53), and patient classification (82.63 to 91.42). Conclusion: This activity demonstrated that training and mentoring are effective in improving the nursing management competencies of nurses. Therefore, it is recommended that this activity be carried out continuously to support the quality of service and professionalism of nursing practice in hospitals

Abstract

Schizophrenia is a chronic mental disorder characterized by delusions, a symptom of which is a disturbance in the patient's thought processes that impacts the patient's behavior. The purpose of this study was to describe a case of a schizophrenic patient with delusions as the primary symptom and comorbid hyperthyroidism who was given psychoreligious therapy. This qualitative case study involved a single patient diagnosed with paranoid schizophrenia and comorbid hyperthyroidism, selected using purposive sampling. Data were collected through interviews, behavioral observations, physical examinations, and laboratory data. Management included pharmacological therapy in the form of antipsychotics and hyperthyroid medications, and non-pharmacological interventions in the form of reality orientation and psychoreligious therapy. The results of the therapy were obtained through daily observations, interviews, and assessment of reality orientation and behavioral responses, and were analyzed descriptively by comparing the patient’s condition before and after the intervention. The findings showed a decrease in the intensity of delusions, stabilization of thought patterns, and improvement in the patient’s behavioral functioning, including an increased ability to distinguish reality and more regular religious activities. The conclusion of this study is that hyperthyroidism is suspected to trigger symptoms of anxiety and physical complaints such as heart palpitations and abdominal pain due to irregular eating patterns. A holistic approach combining medical and psychosocial therapy was effective in managing schizophrenic symptoms and comorbid conditions, as well as improving the patient's quality of life.

Abstract

Latar Belakang: Skizofrenia masih menjadi permasalahan di Indonesia. Salah satu gejala positif dari skizofrenia adalah halusinasi pendengaran. Pasien dengan halusinasi pendengaran biasanya akan mengalami penurunan kemampuan individu dalam kehidupan sehari-harinya. Beberapa dampak yang muncul pada pasien halusinasi adalah cemas, depresi, dan isolasi sosial. Salah satu terapi yang bisa mengurangi gejala tersebut adalah mengekspresikan perasaan pada orang lain. Tujuan: Mengaplikasikan intervensi ekspresi perasaan pada kasus halusinasi pendengaran melalui asuhan keperawatan. Gambaran Kasus: Seorang pria berusia 30 tahun mengalami trauma sebagai korban buli. Akibat koping yang tidak efektif dan kurangnya dukungan, muncullah suara-suara yang berisi menyalahkan pasien sehingga pasien tidak memiliki keberanian untuk melakukan hal baru karena takut dianggap salah. Intervensi: Studi kasus desain observasi partisipasif menggunakan proses keperawatan berdasarkan Evidence-Based Practice distraksi mengekspresikan perasaan dengan bercakap-cakap yang dilakukan sekali sehari selama 3 hari dengan mengajarkan strategi pelaksanaan 1-4. Hasil: Pasien dapat mengidentifikasi isi halusinasinya, yaitu suara-suara yang selalu menyalahkan dirinya; waktu terjadinya, yaitu di pagi hari atau di tengah malam; situasi yang memicu halusinasi, yaitu saat pasien berdiam diri; manfaat distraksi perasaan, yaitu pasien merasakan suara halusinasi terdengar samar saat sedang bercakap-cakap; dan pasien sadar bahwa halusinasi yang ia alami adalah penyakit dan bukanlah hal yang salah. Kesimpulan: Distraksi ekspresi perasaan bercakap-cakap dinilai efektif dalam mengendalikan halusinasi pendengaran karena pasien mengetahui jenis halusinasi, waktunya, situasi yang menyebabkannya, dan manfaat bercakap-cakap. Saran: Diharapkan teknik distraksi untuk mengungkapkan perasaan melalui percakapan dengan pasien halusinasi pendengaran dapat diterapkan secara konsisten dan diajarkan kepada keluarga untuk mempersiapkan pelaksanaannya saat pasien kembali bersama keluarganya.

Abstract

Background: Knowing the important relation between knowledge and skill is important because it reflexes the action that nurses do to save people life. Objective: This research aimed to determine the relationship between the nurse's knowledge level and their performance on cardiopulmonary resuscitation in critical and emergency care unit. Method: The study was quantitative descriptive with the cross-sectional design used sequential sampling with 30 nurses in critical and emergency care unit. The data were analyzed using the Fisher Exact Test. Data was taken from questioner and observational assessment using a standard operating procedure which was developed by a hospital. Results: The results of this study found that there were 63.3% respondents which all of them had poor knowledge also poor performance on doing cardiopulmonary resuscitation. Conclusion: There is a relation between nurse’s knowledge level and the nurse’s performance on undertaking cardiopulmonary resuscitation. The more knowledge they have the better performance they have on cardiopulmonary resuscitation. Routine training and evaluation may be useful to increase nurses’ knowledge and skills.
