Abstract

Self-regulated learning is a process of learning how to manage and regulate oneself to obtain skills and knowledge without depending on others. It is expected that diabetes patients can manage their diabetes by themselves. This study aimed to analyze the effect of self-regulated learning-based educational intervention on quality of life for patients with type-2 diabetes mellitus (DM) at Dr. Ramelan Naval Hospital, Surabaya. This was a quasi-experimental study in the form of pretest and posttest with a control group design. A total of 20 patients with DM were randomly divided into intervention and control groups to undergo a 4-week intervention. The intervention group received dietary education with self-regulated learning, while the control group received standard education. The results revealed differences in variables such as dietary obedience, food intake at home, fasting blood sugar, diastolic blood pressure, and quality of life. The intervention group experienced a positive impact on the quality of life and resulted in better dietary obedience than that of the control group. In conclusion, the implementation of self-regulated learning-based dietary education can improve dietary obedience, knowledge, intention, family support, social support, physical activity, and quality of life for patients with type 2 diabetes.

Abstract

The coverage of postnatal care program in Surabaya City had declined since 2011–2013 and could not reach the target. This analytic observational study used a cross-sectional design and was conducted during April–May 2015 to analyze the effect of role stress, personality type, and burnout on midwives’ performance towards postnatal care program achievement in Surabaya City. A total sample of 45 midwives was collected from eight primary health care centers in Surabaya City with basic emergency obstetric and neonatal care services. Accidental sampling was used for sample selection. This study indicated that most of the midwives were aged 20–30 years and had an diploma level of education in midwifery. Majority of them were married and were predominantly contract workers with However, majority of them had mild burnout and showed medium performance level. There was no influence of demographic characteristics, role stress, and personality type on burnout, whereas the performance was affected by role stress and personality type. The midwives’ performance was more influenced by personality type than by role stress.

Abstract

COVID19 pandemic has significantly changed the world and human's normal life into a new normal life. World Health Organization had recommended early in the pandemic for the people to wear a face mask, keep a distance at a minimum of 1 meter between individuals and practice frequent handwashing with soap. Compliance with health protocols in the COVID-19 pandemic may be influenced by external factors and internal factors i.e., individual perception, knowledge, attitude, beliefs and intentions. This study aimed to compare Milgram's compliance theory and HBM concerning the community’s compliance to the health protocols of COVID-19 prevention. An online survey for a cross-sectional study had been conducted among the community in East Java province in 2020 which had obtained a total of   350 respondents. The results showed some internal factors in Milgram Theory such as region status, personal responsibility, support between communities, and relationship of heads of villages with the community had significantly influenced compliance to health protocols. Meanwhile, internal factors in HBM Theory which include perceived benefits, perceived barriers, self-efficacy, and cues to action had also a significant influence on the community’s compliance with health protocols. The internal factors in HBM Theory were more influential on the community's compliance compared to the external factors in Milgram Theory. Based on this, the government and other related parties who are responsible for improving community awareness and understanding on the need to comply with health protocols could apply the internal factors as stipulated in the HBM. The community awareness on the internal factors could also be a success factor in declining the case fatality rate (CFR) from Covid 19 infection.

Abstract

Cosmetics that have no Marketing Authorization Number (MAN) can harm people's health. This study aimed to analyze the factors that affect the obedience of cosmetics distribution facilities in distributing cosmetics that have MAN in East Java-based on Niven's theory (understanding the instructions, quality of interaction, beliefs, attitudes, personality, social isolation) and Milgram's theory (the status of the facilities, personal responsibility, the legitimacy of authority figures, the status of authority figures, peer support, the proximity of authority figures). This study was a cross-sectional study, using a validated questionnaire. 179 participants were chosen using simple random sampling. The data were analyzed using Partial Least Square (PLS) (p= 0.05). The factor based on Niven's theory that has a significant relationship with obedience is personality (p=0.001). The factors based on Milgram's theory that have a significant relationship with obedience are the legitimacy of authority figures (p=0.025) and peer support (p=0.000). Whereas, understanding the instructions, quality of interaction, beliefs, attitudes, social isolation, the status of facilities, personal responsibility, the status of authority figures, and the proximity of authority figures have no correlation. Milgram’s theory more affects obedience than Niven’s theory. Various kinds of education and monitoring evaluation of distribution facilities were implemented to improve obedience.  Abstrak: Kosmetik merupakan salah satu kebutuhan penting tetapi juga dapat membahayakan kesehatan apabila kosmetik tersebut tidak memiliki nomor izin edar. Penelitian ini bertujuan untuk menganalisis faktor kepatuhan sarana distribusi dalam mengedarkan kosmetika yang memiliki nomor izin edar di Jawa Timur berdasarkan Teori Niven (pemahaman tentang instruksi, kualitas interaksi, keyakinan, sikap, kepribadian, isolasi sosial) dan Teori Milgram (status sarana, tanggungjawab pribadi, legitimasi figur otoritas, status figur otoritas, kedekatan figur otoritas). Jenis penelitian observasional dengan desain studi cross sectional. Jumlah sampel sebanyak 179 responden dipilih secara simple random sampling. Instrumen penelitian adalah kuesioner yang sudah divalidasi. Analisis data statistik menggunakan Partial Least Square (PLS) (p= 0.05). Faktor berdasarkan Teori Niven yang mempengaruhi kepatuhan adalah kepribadian (p=0,001). Faktor berdasarkan Teori Milgram yang mempengaruhi kepatuhan adalah legitimasi figur otoritas (p=0,025) dan dukungan rekan (p= 0,000). Sedangkan pemahaman tentang instruksi (p=0,513), kualitas interaksi (p=0,860), keyakinan (p=0,064), sikap (p=0,597), isolasi sosial (p= 0,333), status sarana (p=0,504), tanggungjawab pribadi (p=0,707), status figur otoritas (p=0,060), dan kedekatan figur otoritas (p=0,072) tidak memiliki hubungan  signifikan terhadap kepatuhan. Teori Milgram lebih mempengaruhi kepatuhan daripada Teori Niven. Upaya untuk meningkatkan kepatuhan dilakukan melalui berbagai macam edukasi dan evaluasi monitoring sarana distribusi kosmetika.

Abstract

Adopting the WHO conceptual framework on stunting in children, the causes of stunting in growth and development are due to household and family factors, including maternal factors during pregnancy or before pregnancy, such as teenage pregnancy. This study uses the 2021 Family Data Collection (PK21) which has been verified and validated in 2022 (PK22), a census conducted by the Indonesian Population and Family Planning Agency . The prevalence of stunting uses data from the 2022 SSGI (Indonesian Nutrition Status Survey). To determine the relationship between age at first marriage and the prevalence of stunting, this study used 38 districts in East Java Province as the unit of analysis. Based on PK21 and verval data, families at risk of stunting are defined as families at risk of giving birth to stunted children. While the dependent variable is selected from data on the number of couples who meet the requirements who are married under age (under 19 years). This research is an observational research that is analytic in nature. Bivariate analysis was performed using Spearman's correlation analysis. From the results of the analysis it was found that there was a relationship between the prevalence of marriage age and the prevalence of stunting in East Java with a p value = 0.005. This means that the higher the prevalence of young marriages in a district, the higher the prevalence of stunting. Therefore, increasing the age of first marriage is a very important measure to end child marriage.

Abstract

Introduction: The government ratifies and establishes the Healthcare and Social Security Agency that manages health insurance for all people in Indonesia. However, this agency, through the National Health Insurance program, can still not pay all claims submitted by hospitals. Several claims were rejected because the claimant did not meet the requirements for the benefits covered. This research analyzes national health insurance claims' constraints to reduce unworthy claims at Undata Hospital, Central Sulawesi Province. Method: This research is a Constraint Analysis with an analytical observational method and a cross-sectional approach where the researcher wants to identify constraints in each BPJS claim flow process, which consists of verification, data coding process, and file entry process at the National Health Insurance Unit of Undata Hospital, Central Sulawesi Province. This research was conducted from November 2020 to September 2021. There were 17 subjects in this study. Sampling using a purposive sampling technique. Results: The cause of the constraint in the BPJS Health verification claim process at Undata Hospital is that the health service verification process was not carried out before being it to Agency. The cause of the constraint in coding BPJS Health claim files at Undata Hospital is that, based on knowledge constraints, it is known that there is no training for coders regarding the coding process. Conclusion: Implementing the BPJS Health claim procedure at Undata Regional Hospital, Central Sulawesi Province, still needs improvement because the existing SOP still lacks a detailed explanation of the stages of the existing claim procedure.Keywords: Constraints, Health Guarantee, Claims

Abstract

Diabetes mellitus is a chronic disease that requires continuous treatment. This can result in an economic burden on sufferers. One method that can be used in analyzing the economic burden due to a disease is the Cost of Illness method by calculating the direct and indirect costs of sufferers and program costs that have been incurred by the government. The purpose of this study was to analyze the Cost of Illness in type 2 diabetes mellitus patients. This research was a quantitative descriptive cross-sectional approach. The sampling technique used multistage random sampling and obtained a sample size of 144 type 2 diabetes mellitus patients from hospitals and health centers in Bulungan Regency, North Kalimantan. The results of the analysis of the Cost of Illness for type 2 diabetes mellitus patients in Bulungan Regency amounted to Rp.7,350,237 per patient each year. The results of the calculation of the Cost of Illness for type 2 diabetes mellitus patients can be used to advocate that promotive and preventive programs are the main focus of disease control, and promote a social security system to offset the direct costs of individuals, so that the entire population can be protected from the high Cost of Illness. Keywords: Cost of illness, Diabetes Mellitus, promotive, preventive

Abstract

Background: The rate of visual disorders due to cataracts in Indonesia is the highest in Southeast Asia region and got the third place in the world at 1.47%. The prevalence of cataracts in East Java Province based on Riskesdas 2013 was 1.6%. Every year as many as 38 thousand residents of East Java Province are threatened with cataracts. Cataracts require a very large cost for medication. Aims: This study aims to conduct a Health Expenditure Analysis of cataract patients at Public and Private Hospital in Surabaya. Methods: Observational analytical research using quantitative methods. The time approach used for this research is a prospective design. The sample in this study were 174 patients. Results: Based on the results of the study, there was a significant difference (p-value = 0.001) between the Health Expenditure of cataract patients in private hospitals and government hospitals. The average health expenditure at private eye hospitals was higher than that of government eye hospitals, with an average difference -an average of IDR 4,240,030 per patient. Even so, patients from private and government hospitals experienced catastrophic health expenditure. Based on the chi-square test, there was a significant relationship between cataract severity (p-value¬= 0.001) and the incidence of catastrophic health expenditure. Conclusion: Catasthropic health expenditure can be experienced by patients in both private and public hospitals due to health costs incurred for cataract treatment for a period of 3 months.

Abstract

Program Imunisasi Dasar Lengkap merupakan suatu upaya yang dilakukan oleh Kementerian Kesehatan Republik Indonesia dalam rangka pencegahan dan pemutusan mata rantai penularan penyakit yang dapat dicegah dengan imunisasi (PD3I). Salah satu indikator keberhasilan dari program tersebut adalah Universal Child Immunization (UCI). Pada tahun 2016-2018, Puskesmas Sukorejo Kota Blitar mengalami penurunan capaian kelurahan UCI sebesar 28,56%. Penelitian ini bertujuan untuk mengidentifikasi perbedaan faktor input dan faktor proses Program Imunisasi Dasar Lengkap antara Puskesmas Sukorejo dengan Puskesmas Sananwetan guna menyusun upaya peningkatan cakupan kelurahan UCI di wilayah kerja Puskesmas Sukorejo. Penelitian ini termasuk penelitian observasional deskriptif dengan metode benchmarking. Data diperoleh melalui observasi dan wawancara dengan Penanggung jawab Program Imunisasi Dasar Lengkap dari Puskesmas Sukorejo dan Puskesmas Sananwetan. Variabel yang menjadi bahan studi benchmarking antara lain faktor input meliputi petugas imunisasi; dana; sarana dan prasarana; dan kebijakan atau SOP serta faktor proses yang terdiri dari perencanaan; pelaksanaan pelayanan; serta pemantauan dan evaluasi. Hasil penelitian menunjukkan bahwa variabel kebijakan atau SOP, tahap perencanaan, serta tahap pemantauan dan evaluasi yang ada di Puskesmas Sananwetan memiliki kondisi dan pelaksanaan yang lebih baik dibandingkan dengan Puskesmas Sukorejo. Masalah yang ditemukan antara lain tingginya surviving infant, masih adanya penolakan imunisasi dari beberapa orangtua bayi, mobilitas masyarakat yang tinggi, serta adanya kawasan elite yang sulit dijangkau oleh petugas. Kesimpulan dari penelitian adalah perlunya Puskesmas Sukorejo untuk melakukan benchmark khususnya pada variabel kebijakan atau SOP, tahap perencanaan, serta tahap pemantauan dan evaluasi.
