Abstract

Introduction: Low birth weight (LBW) infants deal with various problems during transitional period from intra-uterine and extra-uterine because of immature organs’ functions. This leads to LBW as the second death cause in Indonesia, particularly in the fi rst seventh days of infants’ lifes. The problem continues to occur at home when the infants have discharged. This research was aimed to develop the nursing care services model for LBW infants and to test the model.Method: The research design was an action research using quantitative and qualitative approach. This design was chosen as it facilitated improvement in health care system, which was involving nurses and other health providers.Results: Nursing care services provided by the nursing team are hindered by several factors, such as various level of nurses’ knowledge, not optimal health education activities, incomplete standard operational procedure, ethical dilemma, paramedic functions, and documentation system. This model was developed based on conservation and becoming a mother/maternal role attainment theory, family-centered care principles, and input from the experts through focus group discussion.Conclusion: The result of this research is going to increase the quality of nursing care for LBW infants by achieving nurses’ and parents’ satisfaction in giving care for their infants which can lead to lower infant death rate.

Abstract

Introduction: Low birth weight (LBW) infants who have been hospitalized are exposed to various conditions that provoke discomfort, which could trigger a stress response and growth disorders. This study aimed to identify the effects of auditory stimuli from Murottal Quran when it comes to the comfort and weight of LBW infants.Methods: This study used a clinical randomized controlled trial design involving 52 low birth weight infants. The infants were randomly assigned to either the intervention group (n=26) or the control (n=26). The intervention of Murottal Quran was given using a speaker 4 times a day for 20 minutes every 3 hours. This intervention was conducted for three days.Results: The results of this study show that there is a significant difference in the comfort of  low birth weight infants after Murottal Qur’an on the first, second, and third days between the intervention and control groups (p=0.014; <0.001; <0.001). There was no significant difference in the weight between the intervention and control groups in the first, second, and third days (p=0.481; 0.481; 0.464).Conclusion: The auditory stimuli of Murottal Quran can help to improve the comfort level of low birth weight infants during hospitalization. It can therefore be used as a supportive therapy to facilitate low birth weight infant and premature infant development.

Abstract

Introduction: A good comprehension of the clinical profile and nursing diagnosis of newborn in Special Care Nursery (SCN) unit guides decision-making by nurses. In addition, it can become an initial basis for making plans for improving quality of care, management and nursing research. The purpose of this study was to identify clinical profile and nursing diagnosis of newborns in an SCN unit.Methods: It is a quantitative, cross-sectional, descriptive study in SCN (Level II and III) with a sample of 77 medical records of newborns less than 31 days old in a national referral hospital. Descriptive analysis was used to identify demographic characteristic, the medical diagnosis and nursing diagnosis of newborns at birth and hospitalization.Results: Newborns treated in SCN unit are dominated by preterm with low birth weight (74%). Respiratory system disorders (55.8%) and infections (35.1%) dominate newborn problems both at birth or coming. Most   nursing diagnoses when the baby is born or coming are the risk of infection (90.9%) and ineffective breathing patterns (76.6%). As for the treatment, most diagnoses show the risk of infection and hypovolemia.Conclusion: A good clinical profile and nursing diagnosis of newborns can guide nurses to decision-making. Continuous update on nursing diagnosis determination, improvement of nurses’ knowledge about the signs of deterioration, and future research that prioritizes issues in neonates are required to optimize nurses’ role in SCN units.

Abstract

Pain in a baby is difficult to detect is because the method for detecting pain is self-reporting even though babies themselves still cannot describe the pain verbally, then by observing changes in behavior in the form of facial expressions. Statistically, it is also recorded that about 80% of the world's population pays less attention to pain assessment, especially for children, even though this pain gives children a bad experience so that it can interfere with pain responses in the future or psychological trauma. Based on these problems, a prototype system was made using the NVIDIA Jetson Nano Developer kit to help detect pain, especially in infants 0-12 months by using the Convolutional Neural Network (CNN) model with the PyTorch framework and the You Only Look Once (YOLO) algorithm with three detection classification is sad, neutral and sick. From the results of the study, it was found that the YOLO algorithm was able to detect the three classifications with a sad mAP value of 77.8%, neutral 76.7%, in pain 68.9%. With a precision value of 71.4%, recall 62.5% and f1-score 66.6%. The average value of Confidence is 53.57%.

Abstract

Background Kangaroo mother care (KMC) was introduced toIndonesia in the 1990s. Since then, KMC has not been widelyimplemented and has not received national policy support.Objective The objectives of this case study were to implementKMC by an intervention that would ultimately benefit tenhospitals in Java, Indonesia, as well as identify supporting factorsand barriers to KMC implementation.Methods An intervention with four phases was conducted inten hospitals. Two teaching hospitals were supported to serve astraining centers, six hospitals were supported to implement KMCand two other hospitals were supported to strengthen existingKMC practices. The four phases were comprised of a baselineassessment, a five-day training workshop, two supervisory visitsto each hospital, and an end-line assessment.Results A total of 344 low birth weight infants received KMCduring the intervention period. Good progress with regards toimplementation was observed in most hospitals between the firstand second supervisory visits. Supporting factors for KMC were thefollowing: support received from hospital management, positiveattitudes ofhealthcare providers, patients, families and communities,as well as the availability of resources. The most common challengeswere record keeping and data collection, human resources and staffissues, infrastructure and budgets, discharge and follow-up, as wellas family issues. Challenges related to the family were the inabilityof the mother or family to visit the infant frequently to provideKMC, and the affordability of hospital user fees for the infant tostay in the hospital for a sufficient period of rime.Conclusion KM C appeared to be well accepted in most hospitals.For an intervention to have maximum impact, it is importantto integrate services and maintain a complex network ofcommunication systems. Abstract

Background Childhood community-acquired pneumonia (CAP) is one of the leading causes of morbidity and mortality in children under five years of age. Objective To explore the unmet needs of family caregivers of children with community-acquired pneumonia. Methods A qualitative study using in-depth interviews was conducted at one referral hospital located in Jakarta, Indonesia. Convenience sampling was used to select participants. Data were analyzed using qualitative content analysis. Results Ten family caregivers participated in our study. The results of this research are shown in five main themes: (1) the burden of persistent symptoms; (2) exposure to cigarette smoke at home; (3) home remedies as initial treatment; (4) fulfilling the children’s nutritional needs; and (5) health promotion needs. The care of children with CAP involves more than antibiotic treatment and vaccines. Conclusion This study found several unmeet needs of family caregivers in caring for children with CAP, particulary pertaining to health promotion and the fulfilment of nutritional needs.

Abstract

The period of 1000 days of life plays an important role in determining the quality of children's growth and development. Accessibility of quality health services through the MCH handbook is an important strategy for improving maternal and child health. The purpose of this review is to present an overview of the use and usefulness of the possession of a specific MCH handbook on maternal, infant and child outcomes, as well as the potential for further research development. The method of systematic study and study search through the ProQuest, EBSCOhost, and Scopus databases, with the keywords maternal and child health handbook, home based record, mother, newborn, infant, and child, published from January 2008 to October 2018. Furthermore, filtering of articles and data abstraction. A total of 56 articles on the use of the MCH handbook that met the eligibility/inclusion criteria were screened, and 16 articles were selected for a systematic review. The variety of existing studies, has proven the benefits, functions, and effectiveness of the MCH handbook on the health of mothers, infants, and children. The categories of use and use of MCH handbook are integrated with home-based records, namely as an effective tool to facilitate improvement in health-seeking behavior; continuous care; health communication, information and education; as well as home based health records. In order to fill the gap, further research can be directed at evaluating especially premature babies/LBW as part of a population of newborns with high risk. The use of digital technology in improving the implementation of interventions and the accessibility of MCH handbook is also important to be developed.

Abstract

Application of developmental care by nurses are essential to facilitate the growth and development of infants during care in the hospital, however not all nurses understand the benefits and practice of developmental care. This study identifies the influence of providing developmental care training using videos,booklets and practices. The study design used a quasi-experiment pre and post tests equivalent group involving 34 NICU nurses divided into 2 groups (control and intervention), chosen using total sampling method. Data were analyzed using SPSS software with dependent t-test. The results showed there is a significant difference in knowledge and skills between the intervention and control groups (p = 0.000), and there are no relationship between nurses’ characteristics (age, education, length of work, training history) against skills (p> 0.05).trained skills namely: nesting creation, families visitation facilities and positioning. Developmental care training for nurses is important to optimize developmental care application.

Abstract

Care development appliation by nurses is essential to minimize pain and stress, however not al nurses have yet to understand about care development. This study identifies the influence of providing developmental education through videos and pocket books. Study layout applied pre-test post-test equivalent group quasi experiment involving 34 NICU nurses divided into 2 groups (control and intervention) chosen from total sampling method. D ata analysis applied SPSS software with dependant test t-test. Study result showed there was a significant knowledge and skill difference between te control and intervention groups (p=0,000), and not found the relation between nurses’ characteristics (age, education, lenth of employment,training history) with knowlede and skill (p>0,05). Education on care development is essential for nurses and for care development application to be optimized.

Abstract

Premature infants had been experience of pain in neonatal ward that occur each day during treatment. Nurse need to do an intervention for reduce the scale of pain on premature infants. Pain management have to do at birth because of the repeated painful procedures in early life can affect the development of central nerve system permanently. The objective of this study was to describe the application of Kolcaba Comfort’s theory through facilitated tucking accompanied with ‘being with-talking to’ techniques in premature’s infant at high risk infant care. The method that used was case study by applying the four contexts of comfort’s experience associated with the three types of comfort based on Kolcaba Comfort’s theory in providing nursing care on five premature infants who have some painful procedures. The nursing intervention through facilitated tucking accompanied by ‘being with-talking to’ based on the principles of Kolcaba Comfort’s theory gave a positive result against premature infants’s comfort level. Four from five premature infants are in the level of transcendence which is a type of supreme comfort after the ease and relief. Kolcaba Comfort’s theory can be applied within the scope of neonatal care due in accordance with the developmental care of the infants and could reduce the scale of pain.
