Abstract

Introduction: Bathing of neonate is a  part of the birth process by cleaning the baby's body, watering or soaking. However, neonates may experience heat loss more rapidly and show signs of vital instability. Objectives: The purpose of this study was to compare the stability of the vital signs of neonates who immediately took a bath and who delayed bathing. Methods: the research method as a comprehensive search strategy, namely articles in the database of research journals through the Scopus database, Proquest, Clinical Key, and Science Direct with full text criteria and published in years. 2016-2020. Results: The results obtained in this study indicate that it is advisable to delayed bathing in newborns so that the stability of vital signs is better

Abstract

Diastasis rectus abdominis (DRA) in postpartum mothers has a high prevalence, with 85% experiencing this condition, leading to various complaints. However, clinical management remains limited due to the lack of consensus on symptom boundaries and clinical impacts. This study aims to analyze the concept of discomfort due to DRA in postpartum mothers using Walker and Avant’s approach. A descriptive method with concept analysis was applied. Literature search using international electronic databases with a global scope including Scopus, ScienceDirect, MEDLINE, EBSCO, ProQuest, and SAGE, using keywords such as "diastasis rectus abdominis," "interrectus abdominis," "postpartum," and "impact." Inclusion criteria were open-access, including literature review, and full-text articles published from 2018 to 2024. Articles that did not address antecedents, attributes, or consequences of discomfort were excluded. A total of 28 out of 88 identified articles were analyzed, covering various fields such as obstetrics, gynecology, surgery, physiotherapy, radiology, nursing, and public health. The study concludes that discomfort due to DRA consists of primary and secondary symptoms. Primary symptoms include a distended abdomen, abdominal pain, kinesiophobia, impaired self-concept, disturbed social interaction, and reduced quality of life. Secondary discomfort includes hunched posture, back and pelvic pain, spinal and pelvic instability, pelvic floor disorders, and gastrointestinal issues. This categorization of discomfort is expected to establish clearer symptom boundaries. Defining these symptoms can help develop standardized clinical practice guidelines, ensuring more effective diagnosis and treatment for postpartum mothers with DRA.

Abstract

Newborns, especially premature infants, are at high risk of ineffective thermoregulation due to their less complex skin structure. Impaired thermoregulation experienced by premature infants can lead to hypothermia. Premature infants who experience hypothermia can cause complications that have the potential to cause disability and death. One of the efforts to prevent hypothermia is to place premature infants in an incubator that gives moisture so that moisture regulation is needed to help regulate the baby's temperature, especially in premature infants who experience ineffective thermoregulation. Humidity regulation is required for the incubator to prevent fluid loss through evaporation. This study aims to determine the effectiveness of providing incubator humidity education on nurses' knowledge, attitudes, and skills in the Perinatology room. The research design used a pre-experimental group without control with an accidental sampling technique; namely, there was a pre-test before and a post-test after treatment. Thus, the treatment results can be more accurate because they can be compared with the situation before treatment. This study involved 37 respondents in the perinatology room in one of the state hospitals using pre-and post-test questionnaires conducted from April to May 2022. The results showed that there were significant differences before and after education on knowledge (p=0.001), attitudes (p=0.001), and skills (p=0.001). The results of this study can used as a reference in developing standard procedures for incubator humidity to prevent ineffective thermoregulation in premature infants through cognitive improvement of nurses about incubators.

Abstract

Thalassemia is a chronic disease that requires lifelong treatment. Children with thalassemia are highly dependent on their parents for daily care and management. This study aims to identify key themes reflecting the challenges, needs, and expectations of parents caring for children with thalassemia through a qualitative meta-synthesis approach based on Roy’s Adaptation Model (RAM). A literature search was conducted in the PubMed, MEDLINE, Scopus, and ScienceDirect databases for English-language articles published between 2014 and 2024. Twelve studies from six countries were analyzed using the Critical Appraisal Skills Programme (CASP). The synthesis identified four meta-themes describing the parents’ adaptation process: (1) physical fatigue and psychosomatic symptoms, (2) psychological stress and parental identity crisis, (3) disruption of family roles and functions, and (4) lack of systemic and social support. These findings emphasize the importance of a multidimensional support approach encompassing emotional, educational, and health policy aspects to improve family quality of life. This study provides a theoretical foundation for family-centered nursing interventions and supports parental resilience.

Abstract

Thalassemia is a hereditary blood disorder that, while incurable, is entirely preventable through early detection. In Indonesia, an estimated 6–10% of the population carries the thalassemia gene but screening is not a nationwide mandatory. Existing initiatives remain fragmented across regions and target only specific groups, such as prospective brides and grooms in Jakarta or pilot programs in Bekasi Regency. Lack of early screening leads to the birth of thousands of children with thalassemia major, requiring lifelong transfusions and iron chelation therapy that costs the government IDR 800 billion per year. To achieve long-term sustainability, Indonesia must shift from curative to preventive strategies by integrating genetic screening into the national health insurance and reproductive health systems. Policy Ask: Develop a joint regulation between the Minister of Health and the Minister of Religious Affairs, or Minister of Health Regulation (Permenkes), mandating premarital and school-based thalassemia screening as part of national health and marriage registration programs.

Abstract

Background: Health services have increasingly shifted their focus from treating skin diseases and skin injuries to emphasizing preventive efforts through routine assessments. Various treatment equipment and neonatal conditions such as infant weight, gestational age, clinical status, and underlying diseases have been shown to have a strong association with the risk of skin injury. Early detection using valid assessment instruments is therefore essential in neonate care. Purpose: To determine differences in skin injury risk levels in neonates using the SRAMT and NSRAS Plus instruments, and to examine the relationship between gestational age and birth weight with the risk of skin injury. Method: This short cohort design involving a total of 66 neonates, divided into an exposed group and an unexposed group (historical control). Skin injury risk assessments were conducted using the SRAMT and NSRAS Plus instruments. Data were analyzed by comparing the first and last assessments within the exposed group, comparing exposed and unexposed groups, and testing correlations between gestational age, birth weight, and skin injury risk. Results: The findings showed significant differences in skin injury risk levels between the first and last assessments in the exposed group (p = 0.001). There were also significant differences in skin injury risk levels between the exposed and unexposed groups (p = 0.001). However, no significant correlation was found between gestational age (p = 0.446) or birth weight (p = 0.821) and the level of skin injury risk. Conclusion: The study concludes that preventive monitoring using the SRAMT and NSRAS Plus instruments effectively identifies changes in skin injury risk in neonates. Gestational age and birth weight were not found to have a significant relationship with the level of skin injury risk. Suggestion: Researchers and health professionals are encouraged to integrate nurses’ knowledge regarding factors contributing to skin injury and to consistently re-monitor neonates using the SRAMT instrument. Modification of the instrument is also suggested to align it with current neonatal service conditions in Indonesia, enabling more effective prevention of skin injury, especially among preterm infants.

Abstract

Background: The process of becoming a mother requires mothers to adapt during the transition that occurs immediately after childbirth. Nursing interventions based on Mercer's theory of becoming a mother have been developed in several countries, but their form and scope have not been comprehensively mapped. Purpose: To explore and map the scientific evidence for nursing interventions based on Mercer's theory of becoming a mother. Method: A scoping review was conducted based on Joanna Briggs Institute (JBI) guidelines. Articles were searched in PubMed, Scopus, Proquest, and ScienceDirect databases using a search strategy based on the Population-Concept-Context (PCC) framework. Keywords and synonyms relevant to nursing interventions, the process of becoming a mother, and the postpartum period were identified. A combination of free keywords and standard terms (Medical Subject Headings/MeSH) were used in the search using Boolean operators (AND and OR). Analysis included the population, intervention characteristics, implementation methods, and reported outcomes. Results: Five articles met the inclusion criteria for further analysis. Nursing interventions based on Mercer's theory were generally developed in the form of structured education, training, counseling, and technology-based multimodal approaches. Interventions delivered from the third trimester of pregnancy through the postpartum period have been shown to be effective in improving maternal role adaptation, self-efficacy, and mother-infant bonding. However, the duration of the interventions and study designs varied across the five articles. Conclusion: Nursing interventions based on Mercer's theory effectively support the parenting process by strengthening knowledge, parenting skills, and emotional adjustment. Further studies are needed for standardization and long-term evaluation. Suggestion: In order to improve the quality of nursing services, it is recommended that nurses develop a theory-based standardized nursing intervention model.   Keywords: Mothering; Mercer's Theory; Nursing Interventions.   Pendahuluan: Proses menjadi ibu memerlukan kemampuan adaptasi maternal pada masa transisi yang terjadi segera setelah melahirkan. Intervensi keperawatan berdasarkan teori Becoming a Mother dari Mercer telah dikembangkan di beberapa negara, namun bentuk dan cakupannya belum dipetakan secara komprehensif. Tujuan: Untuk mengeksplorasi dan memetakan bukti ilmiah dari intervensi keperawatan berdasarkan teori Becoming a Mother dari Mercer. Metode: Scoping review dilakukan dengan berpedoman pada Joanna Briggs Institute (JBI). Pencarian artikel dilakukan pada basis data PubMed, Scopus, Proquest, dan ScienceDirect dengan strategi pencarian berdasarkan kerangka Populasi-Concept-Context (PCC) dengan mengidentifikasi istilah utama dan sinonim yang relevan dengan intervensi keperawatan, proses menjadi ibu (becoming a mother), serta periode postpartum. Kombinasi kata kunci bebas dan istilah baku (Medical Subject Headings/MeSH) digunakan dalam pencarian dengan operator Boolean (AND dan OR). Analisis mencakup populasi, karakteristik intervensi, metode implementasi, dan luaran yang dilaporkan. Hasil: Didapatkan lima artikel memenuhi kriteria inklusi untuk dianalisis lebih lanjut. Intervensi keperawatan berdasarkan teori Mercer umumnya dikembangkan dalam bentuk edukasi terstruktur, pelatihan, konseling, dan pendekatan multimodal berbasis teknologi. Intervensi yang diberikan sejak trimester ketiga kehamilan hingga periode postpartum menunjukkan efektifitas dalam emningkatkan adaptasi peran maternal, efikasi diri, serta ikatan ibu dan bayi. Namun durasi intervensi dan desain penelitian menunjukkan variasi pada kelima artikel. Simpulan: Intervensi keperawatan berbasis teori Mercer efektif mendukung proses menjadi ibu melalui penguatan pada aspek pengetahuan, kemampuan pengasuhan, dan penyelarasan emosional. Studi lanjutan diperlukan untuk standardisasi dan evaluasi jangka panjang. Saran: Guna meningkatkan kualitas layanan keperawatan, disarankan perawat mengembangkan model intervensi keperawatan terstandar berbasis teori.   Kata Kunci: Becoming a Mother; Intervensi Keperawatan; Teori Mercer.

Abstract

Background: Impairment of nutritional fulfillment is one of the problems that is often encountered in premature infants. Purpose: This study aims to determine the effect of the type of nutrition on the weight increase of premature babies.Methods: A retrospective study was conducted through a medical record review in a top national referral hospital, Indonesia. Total of 75 preterm infants divided into groups of 15 infants by nutrition type (100% BM, 75% BM, 50% BM, 25% BM, and 0% BM). Weight gain was assessed for 12 days through the patients’ medical records. Weighing observation was devided into four times: birth weight in day-1, day-4, day-8, and day-12. Subjects were recruited using consecutive sampling with the following criteria: preterm infants with chronological age > 7 days and weight > 1000 grams. Meanwhile, the exclusion criteria were preterm infants treated in the neonatal intensive care unit (NICU), preterm infants with medical diagnosis of hydrocephalus, sepsis, and congenital anomalies, conjoined twin babies. The general linear model-repeated measure was used to quantify the contrast of increases in infants’ body weight among groups for days 1-4; 4-8; and 8-12. Results: There were no significant differences between the five types of nutrients in terms of increase in the infants’ body weight in day 1-4 (p > 0,68); day 4-8 (p > 0,65); day 8-12 (p > 0,12). Conclusion: Although it not likely to accelerate the infant’s weight, mother’s own milk is still the first recommendation due to its immunity components, nutritional elements, and safety.

Abstract

Background: Pain assessment scale in neonates is the cornerstones of pain management so that the impact of pain can be prevented to maximize neonatal growth and development.Purpose: This study aimed to identify the most appropriate pain assessment scale used for neonates in Indonesia.Methods: A cross sectional study design was used in 30 neonates hospitalized using Neonatal Infant Pain Scale (NIPS), Neonatal infant Acute Pain Assessment Scale (NIAPAS) and Pain Assessment Tool (PAT). Pain assessment was performed by nurses (n=30) and expert nurses (n=5) participated in the validation of scales. Statistical analysis using validity (content, construct and concurrent validity) and reliability (inter-rater reliability and internal consistency) test.Results: NIPS instrument have excellent validity, reliability, and feasibility value compared with NIAPAS and PAT.Conclusion: NIPS was shown a valid, reliable, and practical scale for assessing pain in neonates. It allows nurses to identifying pain and help to provide of appropriate pain management

Abstract

Latar Belakang Masalah: Kelahiran bayi prematur berdampak terhadap kondisi psikologis ibu. Ibu merasa ketakutan membutuhkan dukungan informasi tentang kondisi dan cara merawat bayi prematur. Dukungan ini dapat diperoleh melalui program discharge planning yang telah banyak diteliti dan hasil penelitian menunjukan bahwa berbagai model discharge planning berdampak pada ibu dan bayi. Dengan demikian diperlukan suatu kajian terhadap hasil penelitian berbagai model discharge planing tersebut.Tujuan : Penelitian ini bertujuan mereview hasil penelitian tentang dampak discharge planning terhadap self efficacy ibu.Metode: Pencarian Study menggunakan data base Proquest, EbscoHost, JSTOR, SAGE, ScientDirect, Scopus dan Google Shoolar menggunakan key words “Self Efficacy” AND (Premature OR Preterm) AND “Discharge Planning”. Seleksi artikel melalui dua tahap yaitu tahap pertama seleksi judul dan abstrak dilanjutkan tahap kedua yaitu seleksi full text. Artikel yang tidak relevan, tidak full text, bukan hasil penelitian kuasi atau true ekperimental dikeluarkan sehingga mendapatkan artikel yang masuk ke dalam study primer. Materi review study primer meliputi karakteristik study primer, karakteristik intervensi, efek study terhadap self efficacy ibu dan efek study terhadap bayi prematur.Hasil: Kami mendapatkan 10 study primer dari hasil seleksi terhadap 1316 artikel. Karakteristi study primer: 2 study quasi eksperintal, 8 study true eksperimental dengan desain RCT, hasil penelitian dipublikasikan dalam Bahasa Inggris pada periode tahun 2014-2020. Karakteristik intervensi: melibatkan 22 – 300 pasangan ibu dan bayi prematur yang memenuhi kriteria inklusi dan eksklusi, 8 nama program discharge planning yang berdampak pada ibu dan bayi. Dampak Intervensi pada ibu terdiri dari kesiapan emosi, kesiapan pulang (n=2), Self efficacy ibu (n=5), dan performa laktasi (n=1). Dampak intervensi pada bayi terdiri dari LOS (n=5), saturasi oksigen (n=1), dan berat badan bayi (n=1).Simpulan: Sebagian besar hasil pernelitian menunjukan bahwa Disharge Planing berdampak positif pada ibu dan bayi. Berbagai macam model discharge planning perlu dikaji lebih lanjut untuk mendapatkan satu model discharge planning yang efektif
