Anti Mullerian Hormone Serum Level Indicates Ovarian Response in Controlled Ovarian Hyperstimulation of IVF Cycles Budi Wiweko

Abstract

Objective: To evaluate the clinical value of Anti Mullerian Hormone serum (AMH) level as one of ovarian response indicator in controlled ovarian hyperstimulation in IVF cycles. Method: This cohort-prospective study was conducted in Dr. Cipto Mangunkusumo General Hospital. The subjects of this study were infertile couples who underwent controlled ovarian hyperstimulation in IVF cycles. The measurement of FSH level, estradiol level, AMH level, and antral follicles count was done in the beginning of IVF cycles. The cycles were divided into two groups, good responder group and poor responder group. Good responder group had three or more mature oocytes, while the poor responder group had two or less mature oocytes. Statistical analysis was done using T-Test and Receiver Operator Characteristic area under curve (ROCAUC) to measure the predictive value of AMH, FSH, estradiol, age, and antral follicle count as ovarian response predictors. Results and Discussion: From 92 IVF cycles, there were 15 poor responder cycles (16.3%) and 77 good responder cycles. AMH serum level was 3.75 ± 2.77 μg/ml in good responder cycles and 1.04 ± 1.39 μg/ml in poor responder cycles (p < 0.0001). AMH serum level was more superior than other ovarian response predictors (AUC 0.846) with cut-off value of 1.40 μg/ml. AMH serum level ≥ 1.40 μg/ml had good predictive value as ovarian reserve or ovarian reserve parameter with 81% sensitivity and 87% specificity. Conclusion: AMH serum level was more superior ovarian reserve and ovarian response predictor compared to other parameters. Abstract

Objective: To improve diagnostic method to predict ovulation in cycle with clomiphene citrate. Method: This diagnostic research was done in RSUPN Dr. Cipto Mangunkusumo between January 2011 - October 2011 with 31 women who taking CC 50 mg/day and 30 women with normal cycle. Result: Affecting with ovulation in the two groups, it was found that in groups with CC, the follicle diameter is 24.33 ± 3.87 mm with LH 14.21 ± 7.95 IU/l, meanwhile in normal group, follicular diameter is 17.62 ± 3.45 mm with LH 14.42 ± 5.91 IU/l. Cut-off point for follicular diameter simultaneously with ovulation determined by ROC curve was found in 24.33mm (AUC 0.67, sensitivity 0.64, specificity 0.56), meanwhile the cut-off for LH is 14.40 IU/l (AUC 0.61, sensitivity 0.57, specificity 0.43). Conclusion: In group with CC, ovulation occurred in bigger follicular diameter than normal cycle, while LH does not different significantly. Abstract

Objective: To determine predictive factors for pregnancy after IVF. Method: The subject of this study were three hundred and forty eight IVF cycles in 266 couples who underwent controlled ovarian hyperstimulation in IVF cycles between January 2005 and March 2010. Categorical variables were compared using Chi Square test and continuous variables were analyzed using Independent t-test, p < 0.05 was considered statistically significant. Multivariate logistic regression analysis was done to test correlations between clinical variables and the occurrence of pregnancy. Results: The women’s age significantly influenced pregnancy rate since women under 35 years old has the best chance for pregnancy (56.4%). Endometrial thickness on the day of hCG administration also significantly influenced pregnancy in IVF (p < 0.001) because 64.1% of pregnancy occurred if endometrial thickness ≥ 10.95 mm. Serum FSH on 3rd day of period that can predict ovarian reserve also has significance on pregnancy. On the other hand, 61.5% pregnancy occurred if more than 6 mature oocytes were retrieved (p < 0.001). Among 92 patients of 348 cycles we found strong correlation between AMH level with number of mature oocytes retrieved (p < 0.001; r 0.659). Logistic regression done revealed the couple with best chance of pregnancy can be described as follows: women with endometrial thickness ≥ 10.95 mm, number of mature oocytes > 6 and age under 35 years old. Conclusion: This study enabled the characterization of many prognostic factors for pregnancy. Abstract

Objective: To understand the outcome on conservative surgical treatment of adenomyosis. Methods: A retrospective cohort study followed for 2 years from 2010 to 2012 of women with adenomyosis were diagnosed by transvaginal sonography and confirmed histologically. Subjects divided into women who were treated by adenomyosis resection (with/without Osada’s technique) and who were underwent hysterectomy. Results: After the surgery, as many as 40 patients (81.63%) did not feel any pain (VAS 0), and 9 patients (18.37%) still felt pain. For the fertility outcome, we had 8 patients (20.51%) getting pregnant naturally without any fertility intervention. Two patients (5.13%) had successfully conceived by IVF. According to the type of surgery, from 8 natural pregnancy, 7 patients (87.50%) was underwent conventional resection of adenomyosis and 1 patients (12.50%) underwent Osada’s procedures. Two patients who were conceived by IVF, both of them were underwent Osada’s resection. Conclusion: Adenomyosis resection both conservative or Osada’s procedures actually has a better outcome for relieving pain; therefore, some patients can still have a child. Abstract

Objective: To evaluate whether Anti Müllerian Hormone (AMH) can be used as a predictor of metabolic syndrome in Polycystic Ovarian Syndrome (PCOS).Methods: This cross-sectional study was conducted in Yasmin Clinic, Dr. Cipto Mangunkusumo General Hospital Jakarta between June and December 2012. Forty-one patients diagnosed with PCOSbased on Rotterdam Criteria were enrolled. Secondary were was taken from medical record..Results: A total of 22 subjects were involved in this study. Mean AMH level in the metabolic syndrome group is compared to the non-metabolic syndrome group (10.72 ± 6.23 ng/ml vs 7.97 ± 4.50ng/ml, p=0.12). AMH was strongly associated with HDL, triglyceride and insulin resistance (r-value of -0.29, 0.23, and 0.21 respectively, p < 0.05).Conclusion: AMH can be used as a predictor of metabolic syndrome in PCOS.[Indones J Obstet Gynecol 2017; 5-2: 83-86]Keywords: anti müllerian hormone, metabolic syndrome, polycystic ovarian syndrome

Abstract

Objective: To determine whether visceral adipose tissue or serum RBP-4 were related with the risk increment of insulin resistance in normal BMI PCOS patients. Methods: This was a cross-sectional study conducted in normal body mass index PCOS patients at Yasmin Clinic, RSCM, Jakarta from July 2014 until March 2015. Diagnosis of PCOS was established using Rotterrdam (2003) criteria. Insulin resistance was confirmed by using ratio of HOMA-IR >1.4. Results: Among 40 subjects, 20 subjects (50%) belong insulin resistance group. Serum RBP-4 level was higher in insulin resistance group (p=0.06). After ROC analysis was conducted, area under curve for of serum RBP-4 was 69.9% (CI 95% -3754.77 - (186.60-7696.14, p=0.061)). Cut-off level of serum RBP-4 was 23814.5 ng/mL yielded sensitivity and specificity to a level of 60% and 60%, respectively. After logistic regression were analyzed, visceral adipose tissue demonstrated substantial association with the risk increment of insulin resistance in normal BMI PCOS patients. Conclusions: Visceral adipose tissue demonstrated substantial association with the risk increment of insulin resistance in normal BMI PCOS patients, independent with serum RBP-4 levels. Key words: body mass index, diagnosis, insulin resistance, PCOS, retinol binding protein-4 Abstrak Tujuan: Untuk menentukan apakah jaringan adiposa viseral atau serum RBP-4 berhubungan dengan peningkatan risiko resistensi insulin pada Sindrom Ovarium Polikistik dengan indeks masa tubuh normal. Metode: Studi potong lintang dilakukan pada subjek SOPK dengan IMT normal di Klinik Yasmin, RSCM, Jakarta sejak Juli 2014 sampai dengan Maret 2015. Penegakan diagnosis SOPK dilakukan dengan kriteria Rotterdam (2003). Resistensi insulin dikonfirmasi dengan pemeriksaan rasio HOMA-IR > 1.4 Hasil: Diantara 40 subjek, sebanyak 20 subjek (50%) mengalami resistensi insulin. Kadar serum RBP-4 lebih tinggi pada kelompok resistensi insulin (p=0.06). Setelah dilakukan analisis Receiver Operating Curve (ROC), serum RBP-4 memiliki Area Under the Curve (AUC) sebesar 69.9% (IK 95% -3754.77 - (186.60-7696.14, p=0,061)). Titik potong kadar serum RBP-4 adalah 23814.5 ng/mL dengan sensitivitas dan spesifisitas masing-masing 60% dan 60%. Setelah dilakukan analisis regresi logistik, jaringan adiposa viseral menunjukan asosiasi yang kuat dengan terjadinya resistensi insulin pada pasien SOPK dengan IMT normal. Kesimpulan: Jaringan adiposa viseral menunjukan asosiasi yang kuat dengan terjadinya resistensi insulin pada SOPK dengan IMT normal, independen terhadap kadar serum RBP-4. Abstract

Abstrak Hukum yang mengatur pengelolaan teknologi reproduksi terbantu (TRB) belum komprehensif. TRB merupakan kemajuan teknologi kesehatan yang dapat membantu pasangan memiliki anak di luar cara alamiah. Mekanisme dilakukan dengan membekukan embrio dengan nitrogen cair sehingga embrio tetap viabel dan sewaktuwaktu dapat ditanamkan ke dalam rahim. Sudah banyak peraturan yang mengikat tentang pemanfaatan TRB, di antaranya UU no. 36 tahun 2009, PP no. 61 tahun 2014, dan Permenkes no. 43 tahun 2015. Ditinjau dari segi etik, salah satu lafal sumpah dokter yang berbunyi “Saya akan menghormati setiap hidup insani mulai saat pembuahan” menjadi landasan etik pengelolaan TRB. MUI juga telah mengeluarkan fatwa bahwa TRB merupakan usaha yang dibolehkan untuk memiliki anak, namun dengan batasan-batasan tertentu. Namun, hingga kini, pemusnahan sisa embrio yang sudah tidak digunakan lagi masih belum memiliki payung hukum yang jelas sehingga diperlukan revisi KUHP dan UU lex spesialis untuk memperjelas status bahwa tindakan medis tanpa niat jahat tidak termasuk hukum pidana.Abstract Regulation which manage the use of In Vitro Fertilization (IVF) is not comprehensive yet. IVF is an advanced in health reproduction technology which facilitates couples to concieve in a way other than the natural one. IVF was done by freezing the embryo with liquid nitrogen so the embryo stay viable and can be implanted in the uterus at any time. There are many regulations regarding the use of IVF, including Constitution no. 36 of 2009, Government Regulations no. 61 of 2014, and Ministry of Health Regulations no. 43 of 2015. From an ethical point of view, in one of Hippocratic Oath for physician is said that “I will maintain the utmost respect for human life from the time of conception” has become the ethical basis for IVF. Indonesian Ulema Council (MUI) has also issued a fatwa which stated that IVF is an effort that is allowed to have children, but with certain limitations. However, until now, the extermination of the remaining embryos that are no longer needed still has no legal clarity, so that an amandement of the criminal code (KUHP) and the Lex Specialist Law is needed to clarify the status that medical actions without malicious intent are not included in the criminal law.

Abstract

Anti-Mullerian Hormone (AMH) adalah anggota dari kelompok Transforming Growth Factor-β yang berperan penting dalam regulasi folikulogenesis reproduksi wanita. Peningkatan kadar AMH 2-3 kali lipat ditemukan pada pasien SOPK (Sindrom Ovarium Polikistik) dibandingkan dengan wanita dengan ovulasi normal. Tujuan dari penelitian ini adalah untuk mengetahui hubungan kadar serum AMH dengan jumlah mutasi pada gen promoter AMH (Anti-Mullerian Hormone) pada pasien SOPK (Sindrom Ovarium Polikistik). Besar sampel adalah 114 pasien yang terdiri dari 60 pasien SOPK dan 54 pasien bukan SOPK sebagai kontrol. Kadar AMH didapatkan dari rekam medis pasien di Klinik Yasmin IVF RSCM Kencana Hospital, Jakarta. Analisis molekuler dan genotipe dilakukan dengan PCR dan sekuensing dilanjutkan dengan analisis bioinformatika. Terdapat 60 mutasi titik pada varian promotor gen AMH. Jenis mutasi varian tertinggi yang ditemukan adalah -674 G/A (100%), diikuti oleh -245 C/CT (88,2%), dan -444 A/G (17,9%) di seluruh sampel. Berdasarkan hasil uji Wilcoxon Signed Rank test, jumlah mutasi pada kelompok SOPK berpengaruh nyata terhadap AMH serum (p<0,05). Pada kelompok kontrol, jumlah mutasi tidak berpengaruh nyata terhadap kadar AMH (p>0,05). Jumlah mutasi pada promotor gen mempengaruhi kadar serum AMH pada PCOS.

Abstract

Objective: To determine the potential of examining embryo morphology, and morphokinetic parameters in predicting the chromosomal status of embryos. Methods: This study is a cross-sectional that requires patients undergoing IVF followed by chromosome examination with NGS that was conducted at the IVF Center at Pondok Indah Hospital and Morula IVF Center at Bunda Hospital from December 2021 to December 2022. Each embryo that reaches the blastocyst stage on day 5 or 6 will be washed and put into a PCR tube for a week; then, embryologists annotate them to determine morphological assessment and morphokinetic parameters using Time-Lapse Microscopy. The chi-square test was used to analyse bivariate variables. Results: One hundred twenty-four samples were collected on day 5 of patients undergoing the IVF procedure. 50.8% of the samples were aneuploid chromosomes, and 49.2% were euploid. The morphokinetic characteristics median was 3.86 fold. It was found that expansion grade, time to pro-nuclear fading, and time to the synchrony of the third cell cycle were significantly associated with euploid status (p = 0.000; 0.041 and 0.036). Conclusion: The expansion grade has been proven as the most influential component for accurately predicting the ploidy status of embryos. Metode: Penelitian ini merupakan penelitian potong lintang yang memerlukan pasien yang sedang menjalani program bayi tabung yang dilanjutkan dengan pemeriksaan kromosom dengan NGS yang dilakukan di Pusat IVF RS Pondok Indah dan Pusat IVF Morula RS Bunda pada bulan Desember 2021 hingga Desember 2022. Setiap embrio yang mencapai tahap blastokista pada hari ke 5 atau 6 akan dicuci dan dimasukkan ke dalam tabung PCR selama seminggu; kemudian, ahli embriologi membuat anotasi untuk menentukan penilaian morfologi dan parameter morfokinetik menggunakan Mikroskop Time-Lapse. Uji chi-square digunakan untuk menganalisis variabel bivariat. Hasil: Seratus dua puluh empat sampel dikumpulkan pada hari ke 5 dari pasien yang menjalani prosedur IVF. 50,8% sampel adalah kromosom aneuploid, dan 49,2% adalah euploid. Median karakteristik morfokinetik sebesar 3,86 kali lipat. Ditemukan bahwa tingkat ekspansi, waktu menuju pemudaran pro-nuklir, dan waktu hingga sinkronisasi siklus sel ketiga berhubungan secara signifikan dengan status euploid (p = 0,000; 0,041 dan 0,036). Kesimpulan: Tingkat ekspansi telah terbukti sebagai komponen yang paling berpengaruh dalam memprediksi status ploidi embrio secara akurat. Abstract

A decrease in serum anti-müllerian hormone (AMH) concentration suggests the reduction of antral follicle number. AMH serum level was correlated with ovarian stimulation outcomes, oocyte quantity and quality, embryo quality and in vitro fertilization (IVF) outcomes. However, the use of AMH to predict pregnancy rates in IVF is still debatable. This study aimed to determine the role of AMH in predicting ovarian response and pregnancy rates in IVF programs. This was a retrospective cohort study involving patients who underwent a short protocol of ovarian stimulation for IVF at the Yasmin clinic, dr Cipto Mangunkusumo General Hospital. from March 2013 to March 2021. A total of 1,527 out of 3,880 IVF cycles were included. Poor ovarian response was determined as a collection of fewer than four oocytes following ovarian stimulation. The data was then analyzed using the Mann Whitney test if the data distribution was not normal or the unpaired t-test if the data distribution was normal. Median AMH levels were higher in the normal (2.5(0.01-32)) compared to poor responder (0.68(0.01-6.86)) group (p<0.05; Mann Whitney). Serum AMH level and age had predictive value for pregnancy rate (p<0.05; Mann Whitney). Keywords: AMH, ovarian response, pregnancy rate, IVF.   Kadar Hormon Anti-Müllerian sebagai Prediktor Luaran Fertilisasi in Vitro Abstrak Penurunan konsentrasi serum hormon anti-müllerian (HAM) menunjukkan penurunan jumlah folikel antral. HAM serum berkorelasi dengan luaran stimulasi ovarium kuantitas dan kualitas oosit serta kualitas embrio dan luaran fertilisasi in vitro (FIV). Namun, penggunaan HAM untuk memprediksi angka kehamilan dengan stimulasi ovarium pada FIV masih diperdebatkan. Tujuan penelitian ini untuk mengetahui peran serum HAM dalam memprediksi respons ovarium dan laju kehamilan pada program FIV. Penelitian ini adalah studi kohort retrospektif yang melibatkan pasien yang menjalani protokol singkat stimulasi ovarium pada program FIV di klinik Yasmin rumah sakit dr Cipto Mangunkusumo.dari bulan Maret 2013 sampai Maret 2021. Sebanyak 1.527 dari 3880 siklus IVF dimasukkan ke dalam penelitian. Dikatakan perespon buruk apabila diperoleh kurang dari empat oosit setelah stimulasi ovarium. Data kemudian dianalisis dengan menggunakan uji Mann Whitney bila sebaran data tidak normal atau uji t tidak berpasangan bila sebaran data normal. Median kadar HAM lebih tinggi pada kelompok perespons baik (2.5 (0.01-32)) dibandingkan perespons buruk (0.68 (0.01-6.86)) (p<0,05; Mann Whitney). Kadar HAM serum dan usia memiliki nilai prediktif terhadap angka kehamilan (p<0,05; Mann Whitney). Kata kunci: hormon anti-Müllerian, respons ovarium, angka kehamilan, FIV.
