Abstract

Vitamin D is reported to affect immune system and prevent autoimmunity. Some studies show that low vitamin D levels in patients with type 1 diabetes are associated with glycemic control and inflammatory status. The study was cross sectional design with subjects T1DM patients aged 1-18 years, and healthy subjects with similar age. Plasma levels of vitamin D (25(OH)D3) was measured using ELISA. Glycemic control measured by hemoglobin A1c (HbA1c). Inflammatory status measured by examination of IL-10 using ELISA method. Comparison of vitamin D levels, HbA1c levels and IL-10 levels were analyzed by independent samples t-test. The relationship between vitamin D levels, HbA1c, and IL-10 were analyzed by Pearson's correlation. Our subjects were 20 T1DM patients and 20 healthy controls. Our study showed that vitamin D (25(OH)D3) levels was significantly lower than in healthy controls (20.82±5.53 ng/ml vs 33.14±2.17 ng/ml; p=0.000), HbA1c levels was significantly higher than in healthy controls (10.08±5.02% vs 5.02±0.18%; p=0.000), IL-10 levels was significantly lower than in healthy controls (16.50±4.57 ng/ml vs 73.52±7.11 ng/ml; p=0.000). Vitamin D levels were correlated with HbA1c (p= 0.000; r= -0.871), correlated with IL-10 (p= 0.000; r= 0.853). HbA1c levels were correlated with IL-10 levels (p= 0.000; r= -0.878). Low vitamin D levels are common in T1DM patients. There were significant differences in vitamin D levels, IL-10, and HbA1c among T1DM patients compared to normal subjects. Vitamin D levels were associated with HbA1c levels and IL-10 levels in T1DM.

Abstract

Ferritin is an acute-phase reactant that is elevated in autoimmune disorders, including systemic lupus erythematosus (SLE). However, their correlation with disease activity scores has not been confirmed. Pandemic Covid-19 makes children visitation to hospital to get the treatment of SLE were delayed. This study aimed to evaluate correlation between serum ferritin and disease activity and its role in screening for flare in pediatric SLE (pSLE) patients during pandemic Covid-19. This is a cross-sectional study conducted in Saiful Anwar General Hospital Malang. Sampling was carried out sequentially on pediatric patients who met the criteria for Systemic Lupus International Collaborating Clinics (SLICC) and were recorded between July 2021-May 2022. All patients were interviewed and assessed for disease activity using SLE Disease Activity Index 2000 (SLEDAI-2k). A score <4 was categorized as inactive disease. Biochemical, serological tests including markers of disease activity and ferritin level were measured by standard laboratory procedure. Comparison, correlation and ROC curve analyses were performed with SPSS software. There were 38 females pSLE participated in this study. The mean age of the patients were 12.6 ± 3.02 years. Serum ferritin significantly higher in active disease compared to inactive disease 84.50 ng/mL (68.00-151.75 ng/mL) ng/mL and 815.00 ng/mL (451.25-1570.00 ng/mL), a value of p<0.05 was determined to be statistically significant. A significant correlation was found between serum ferritin with SLEDAI 2K (r = 0.890, p = 0.000). Correlation was also found between serum ferritin and IgM anti-double stranded-DNA (r = 0.325, p = 0.046), but not with other laboratory and serological parameters. In ROC curve analysis, we found that Area Under The Curve (AUC) 0.989, 95%CI 0.964-1.014, p value 0,000, with cut off value 297.50 with sensitivity 85% and specificity 94.4%. Ferritin was increased in active disease   as compared to inactive disease and correlated with SLEDAI score and IgM-dsDNA. Thus, ferritin may be potential as an affordable and available marker of disease activity in pSLE during pandemic Covid-19.

Abstract

NPSLE diagnosis is still challenging because of many SLE-related and non-SLE-related processes that can be presented in patient. The report of NPSLE in Indonesia is still limited. This study aim to describe the clinical features, nutrition status, and laboratory characteristics of Neuropsychiatric Systemic Lupus Erythematosus (NPSLE) and compared to non NPSLE case in Indonesian children. The study is a retrospective cohort study. Data were collected from the complete medical record of Juvenile Systemic lupus Erythematosus (jSLE) patients 2016 - 2020 at the Allergy Immunology Outpatient clinic at Dr. Soetomo General Academic Hospital. We include all patients with ages ranging from age 0-18 years old with a diagnosis of Systemic lupus Erythematosus (SLE). The diagnosis fo SLE based on American College of Rheumatology (ACR) criteria 1997 and Neuropsychiatric (NP) manifestations were classified using the standardized nomenclature and case definitions for the 19 NP manifestations linked to SLE developed in 1999 by the ACR ad hoc Committee. Disease activity SLE was defined according to the American Mexican-Systemic Lupus Erythematosus Disease Activity Index (Mex-SLEDAI) criteria. Statistical analysis conducted in this study was descriptive analysis, paired T-test (NPSLE vs. non-NPSLE as the dependent variable), Fischer exact test, and Pearson Chi-square test using SPSS ver. 21. A total of 90 patients with juvenile SLE were enrolled, but only 71 patients were eligible as participants with complete medical records obtained. Mex-SLEDAI score was significantly higher on NPSLE compared to non-NPSLE (p=0.001).
