Abstract

Anemia is the most common hematologic abnormality in patients with human immunodeficiency virus (HIV) infection. This abnormality is associated with HIV infection itself, HIV-related opportunities infections or drug use. Zidovudine (AZT) is the most common cause of anemia in HIV patients. Recent study showed anemia in HIV patients is also associated with CD4 cell counts. Aim of this study was to evaluate the risk of anemia on HIV patients with different CD4 cell counts after AZT-based antiretroviral therapy (ART).This retrospective cohort study was conducted using medical record of HIV patients in Dr. Soetomo General Hospital, Surabaya. Subjects who fulfilled the inclusion and exclusion criteria were divided into two group i.e. HIV patients with CD4 cell counts 200-350 cell/mm3 and those with CD4cell counts ≥350 cell/mm3. All available demographics, clinical and laboratory data of subjects before and after AZT-based ART were then recorded and evaluated. Ninety-seven HIV patients (50 male and 47 female) were involved in this study. The result showed that the anemia incidence significantly increased after AZT-based ART (p<0.05), however no significantly different in anemia incidence, mean Hb level reduction and Hb level time reduction were observed between HIV patients with CD4 cell counts 200-350 cell/mm3 and those with CD4cell counts ≥350 cell/mm3(p>0.05). Gender, age, weight and clinical stage were not associated with anemia incidence (p>0.05). In contrast, anemia incidence is associated with Hb level before AZT therapy (p<0.05). In conclusion, the anemia incidence in HIV patients after AZT based ART is not associated with the level of CD4 cell counts, however it is associated with Hb levels before AZT therapy.

Abstract

Anti Retroviral Therapy (ART) adherence infl uenced HIV/AIDS patient’s quality of life and otherwise quality of life influenced ART adherence positively, where the better quality of life of People Living With HIV/AIDS (PLWHA), the more ability of patient to adhere on their ART regiment. The purpose of this study was to analyze association between ART adherence and outpatient HIV/AIDS patient’s quality of life in four domains at RSUD Dr. Soetomo Surabaya. This study was an observational study with cross sectional design. Sample was 94 HIVAIDS patients’s outpatient and selected by using accidental sampling technique. The data was analyzed using Spearman correlation test. The result of study showed that most of HIVAIDS patients were 36–55 years old (53.2%), male (59.6%), normal nutritional status (42.6%), having high ART adherence in the last one month (79.8%), and having quality of life’s mean score in all domains ranged at 50–60 (30–40%). This study showed that there was an association between ART adherence in the last one month and quality of life in physical health (r =0.212, P = 0.040) and environment’s domain (r = 0.258, P = 0.012). It is suggested a comprehensive AIDS care program primarily with considering physical health and environment factors that could increase HIV/AIDS patient’s quality of life. It is suggested a longitudinal study with greater numbers of respondent and combine the use of measurement instrument to understand factors related to ARTadherence and quality of life.Keywords: sex, ART adherence, quality of life, nutritional status, age

Abstract

BACKGROUND: Human Immunodeficiency Virus (HIV) is an infectious disease that targets the human immune system by attacking cluster of differentiation (CD)4 cells. The use of propolis in HIV patients is expected to be safe and beneficial in terms of increasing endurance and immunity by its role in increasing CD4 level. This study aimed to analyze the influence of propolis supplementation in increasing the CD4 level in anti-retroviral (ARV)-treated HIV patients.METHODS: Double-blind randomized controlled clinical trial was conducted in 50 HIV patients who took regular ARV therapy. The subjects were divided into two groups, one group was treated with ARV and propolis, while another one was given ARV and placebo. The CD4 cell count was measured during pre-treatment, in the 3rd month, in the 6th month after treatment. The level of hemoglobin, leukocyte, and platelets were also measured. The SF-12 questionnaire was used to evaluate quality of life of the subject.RESULTS: Out of 50 subjects, 43 subjects completed the study, which were 19 subjects from the propolis group and 24 subjects from the placebo group. After 3-month of treatment, there was a statistically significant difference in the increase of CD 44 level in propolis group, while the increment was not significant in the placebo group. After 6-month treatment, the increase of CD4 level was occurred in both groups, propolis and placebo, however the increment was not statistically significant. The levels of hemoglobin, leukocyte, and platelets were not altered by the treatment and remained normal throughout the study. The quality of life was improved during the study; however, it was also not statistically significant. Mild adverse events occurred in 3 subjects which were relieved after the treatment stopped.CONCLUSION: Based on the result of this study, the administration of propolis on HIV patients receiving ARV bring significant difference in the increase of CD4 in propolis group from baseline to 3 month after the treatment. While in placebo group, this increment was not significant. At the end of study, CD4 count continued to rise up, however the increase was not statistically significant. There are no hemoglobin, leukocyte, platelets, and quality of life abnormalities. Therefore, it is necesary to do further research with a spesific CD4 count. However, it may be beneficial in relieving the clinical symptoms and quality of life of patient living with HIV.KEYWORDS: CD4, ARV, HIV, propolis

Abstract

Background: The success of individual antiretroviral drug (ARV) treatment in patients with human immunodeficiency virus (HIV) and acquired immune deficiency syndrome (AIDS) was determined by conducting a routine evaluation of the patients’ Cluster of Differentiation 4 (CD4) count. The indicators used to measure the success of the HIV and AIDS treatment were mortality, mobility, and quality of life (QoL). Purpose: The purpose of this research was to analyze the relationship between clinical status (smoking status, duration of ARV therapy, the CD4 count, and body mass index [BMI]) and the QoL of patients with HIV and AIDS who were stable during treatment. Methods: This type of research was quantitative analytical research with a cross-sectional design. This research was conducted at Dr. Soetomo Hospital, Surabaya, from September to November 2017. The study population was patients with HIV and AIDS in Dr. Soetomo Hospital, Surabaya. The research sample was taken by purposive sampling with the inclusion criteria being patients with HIV and AIDS who had been treated for ≥6 months with adherence ≥95% and who came directly to the hospital. Results: The majority of respondents were female (53.36%), junior/senior high school graduates (66.67%), married (62.22%), non-smoking (75.56%), had undergone ARV therapy for ±10 years (77.78%), and had a QoL in the adequate category (62.22%). The basic clinical status with a significant relationship with the respondents’ QoL were the CD4 count (p = 0.00) and BMI (p = 0.00). Conclusion: There was a relationship of the CD4 count and BMI with the QoL of the patients with HIV and AIDS.

Abstract

Background: Oral candidiasis is the most common clinical manifestation in a person infected with HIV which has been linked with the severity of the disease. Candida albicans responsible for most oral candidiasis. The increased resistance to the available antifungal drugs can reduce the efficacy of therapy. Therefore, it will be crucial to discover novel antifungal agents. One of them is tea tree oil (TTO) or Melaleuca alternifolia which has a potent antifungal effect to inhibit the growth of Candida sp.  Purpose: To evaluate the antifungal susceptibility of TTO 5% compared with nystatin against Candida sp. taken from HIV/AIDS patients. Methods: The study assessed the antifungal effect of TTO 5% and nystatin against 33 isolates of Candida sp. that was isolated from the oral cavity of 30 HIV/AIDS patients in Dr. Soetomo General Academic Teaching Hospital Surabaya, Indonesia. Antifungal susceptibility was evaluated by the disc diffusion method. The diameter of the inhibition zone was determined as the result of this study. Result: The most strains found in this study was Candida albicans (84.8%). The mean inhibition zone of nystatin against all strains was 23.24 mm whether it was 17.55 mm for TTO 5%. The inhibition zone of both TTO 5% and nystatin in Candida non-albicans were tended to be higher than in Candida albicans. It revealed that the inhibition zone of TTO 5% significantly lower than nystatin (P = 0.00; P < 0.05).Conclusion: Therefore, the antifungal susceptibility of nystatin was significantly higher than TTO 5% against Candida albicans and non-albicans.

Abstract

Introduction: Dengue fever is a tropical disease caused by Dengue virus that is still becoming one of the main health problem in Indonesia. Dengue fever patients are usually children with age ranging between 5-14 years old, that indicates a higher likelihood of infection at school compared to infection at home.The purpose of this study is to analyze the difference between knowledge and practice of DHF prevention and also the existence of larvae before and after education to the elementary school’s guard in MojokertoMethods:  The research method used in this study is pre-experimental design method, which is a one group pretest and posttest study.The number of samples was 25 school guards from all elementary school in the urban villages with DHF cases in 2017.Results: The average correct answer at pretest was 58.9% while in posttest it was 80.9%. From the results of Wilcoxon test also obtained p-value 0.00 (<0.05). From observation before the education, one school was found positive with larvae, and after the education was given, all schools were found negative.Conclusion: There are differences in knowledge and practice prevention of dengue fever between before and after education in primary school guards in Mojokerto

Abstract

Although the prevalence of intestinal cryptosporidiosis in Indonesian HIV and AIDS patients were high, however the prevalence of pulmonary cryptosporidiosis have not been previously reported. The objective of this study was to know the determination of the pulmonary cryptosporidiosis prevalence in HIV and AIDS patients with pulmonary symptom that was treated in Dr. Soetomo General Hospital Surabaya. The detection of Cryptosporidium in sputum samples used modified versien acid fast stain of Ziehl Neelsen technique. In this study was found that three (3) of the eight (8) sputum samples (37.5%) of ≥55 years old and CD4≤70 HIV and AIDS patients were Cryptosporidium positive. Based on this study the HIV and AIDS patients with pulmonary symptoms should be suspect having the possibility of pulmonary cryptosporidiosis beside suffered tuberculosis.

Abstract

A widely used scoring system to assess the severity of sepsis is Acute Physiology, Age, and Chronic Health Evaluation (APACHE) II scoring system, however there are some disadvantages in using this. Other parameters are needed to predict severity and outcome of sepsis. Proinflammatory cytokines and Fas receptors are increased in sepsis and their concentration elevations are correlated with disease severity. An increase of soluble Fas level will follow increasing Fas receptors. This study aimed to prove any correlation between the level of soluble Fas and degree of sepsis severity based on APACHE II score. A cross-sectional observational study was conducted in January-June 2015 on 30 septic patients. APACHE II scores were calculated from the patients’physiological data, age, and chronic health problem status. Levels of soluble Fas were measured using the ELISA method (Human FAS/ CD95 (Factor-Related Apoptosis) ELISA Kit, Elabscience Biotechnology). Levels of soluble Fas ranged between 1,049-2,783 pg/mL (1,855.7 ± 477.27 pg/mL). APACHE II scores varied between 4-29 (17.2 ± 5.82). Significant positive correlations between levels of soluble Fas and APACHE II score (r=0.347, p=0.03) were found. A prediction model of soluble Fas levels based on APACHE II score was made. Linear regression analysis produced a prediction model of soluble Fas levels based on APACHE II score, in which soluble Fas level= 1,365.8 + 28.485 x APACHE II score.

Abstract

The incidence of HIV is increasing in Indonesia and Asia as well, Indonesia is considered as the most rapid. One of the diagnostic toolsfor diagnosing HIV is by viral load. Lymphocyte T-CD8+ secreted IFN-γ will inhibit replication of HIV virus through the induction of antiviralprotein and the host immune response, which kills infected cells. An examination of plasma IFN-γ and viral load will be more convincingfor the treatment and/or to know the progressiveness of HIV & AIDS. The aim of this study is to know the correlation between plasma IFN-γand viral load in HIV patients. Forty two samples from HIV patients were collected at the Intermediate Care and Infectious Disease Unit ofDr. Soetomo Hospital, Surabaya from April to June 2011. The concentration of plasma IFN-γ was measured by ELISA (eBioscience) methodand the amount of viral load was measured using PCR Cobas Amplicor (Roche Diagnostics). The level of plasma IFN-γ in this study wasfound 11.4 pg/mL up to 576 pg/mL and the level of viral load was 589 copies/mL up to 510.000 copies/mL. The statistical analysis showedno significant correlation (p>0.05) between plasma IFN-γ level and viral load in HIV patients, and no correlation was found between IFN-γplasma and viral load in HIV patients.

Abstract

Aktivasi sistem imun memegang peran yang sangat penting di infeksi HIV. Hal ini dapat diketahui dengan salah satu pengukuranneopterin dalam cairan tubuh manusia sebagai pemeriksaan untuk pemantauan aktivitas imun seluler, yang dapat dikerjakan denganmudah dan peka. Neopterin merupakan hasil katabolik guanosine triphosphate (GTP), yaitu nukleotida purin tertentu, yang memilikigolongan kimiawi yang dikenal sebagai pteridin. Tujuan penelitian ini adalah untuk mengetahui keberadaan kenasaban antara kadarneopterin dan jumlah limfosit T-CD4+ darah di pasien HIV. Penelitian bersifat analitik pengamatan dengan rancangan potong lintang.Sampel terdiri dari 32 pasien yang terinfeksi HIV stadium I yang datang di Unit Perawatan Intermediit Penyakit Infeksi RSUD Dr.Soetomo Surabaya antara bulan Juli−September 2014. Pemeriksaan neopterin dengan metode ELISA dan memeriksa jumlah limfositT-CD4+ menggunakan metode flowcytometry (BD FACSCaliburTM). Hasil menganalisis secara statistik menggunakan uji kenasaban dariPearson dan dilanjutkan dengan uji regresi. Kadar neopterin penderita yang terinfeksi HIV cenderung meningkat dengan rerata 14,74nmol/L sedangkan jumlah limfosit T-CD4+ menurun dengan rerata 231,81 sel/μL. Keberadaan kenasaban negatif antara neopterindan limfosit T-CD4+ darah di infeksi HIV stadium I. Penurunan limfosit T-CD4+ disertai peningkatan kadar neopterin di pasien yangterinfeksi HIV stadium I.
